Objective To explore the automated identification and diameter measurement methods for inferior vena cava (IVC) based on clinical ultrasound images of IVC. Methods An automated identification and localization method based on topology and automatic tracking algorithm was proposed. Tracking algorithm was used for identifying and continuously locating to improve the efficiency and accuracy of measurement. Tests were conducted on 18 sets of ultrasound data collected from 18 patients in intensive care unit (ICU),with clinicians' measurements as the gold standard. Results The recognition accuracy of the automated method was 94.44% (17/18),and the measurement error of IVC diameter was within the range of±1.96s (s was the standard deviation). The automated method could replace the manual method. Conclusion The proposed IVC automated identification and localization algorithm based on topology and automatic tracking algorithm has high recognition success rate and IVC diameter measurement accuracy. It can assist clinicians in identifying and locating IVC,so as to improve the accuracy of IVC measurement.
3.Clinical value of compression rate combined with Young's modulus value in assessing median nerve injury of patients with dialysis

Objective:To investigate the clinical value of compression rate(CR)of median nerve combined with Young's modulus value(E)in quantitatively evaluating median nerve injury of patients with dialysis-related amyloidosis(DRA).Methods:A retrospective analysis was conducted on 53 maintenance hemodialysis(HD)patients who admitted to department of blood purification of Beijing Chao-Yang Hospital of Capital Medical University from November 2019 to October 2020,and they were divided into case group,and 60 volunteers who underwent physical examination at the outpatient of the department of ultrasound medicine were selected as healthy control group.Basic clinical data of the patients were collected.Based on the results of physical examination,the patients with dialysis were divided into a sign group(n=18)and a non-sign group(n=35).High-frequency ultrasound was used to measure the anteroposterior diameter of the median nerve at the entrance of the carpal tunnel and the protrusion of the distal radius in all subjects.The CR values of anteroposterior diameters were calculated.Shear Wave Velocity elastography(SWE)technique was employed to measure E values of the median nerves of compressed position.The differences in CR and E values between case group and healthy control group,as well as between sign group and non-sign group were respectively compared and analyzed.Furthermore,the diagnostic efficacies of CR alone,E value alone and their combination for median nerve injury were evaluated.Results:The CR and E values of median nerve of cases group were respectively(0.16±0.01)and(104.49±49.42)kPa,and they were respectively(0.07±0.05)and(42.47±15.64)kPa in health control group.The CR and E values of median nerve in case group were significantly higher than those in health control group(t=4.92,6.39,P<0.001),respectively.The CR and E values of the median nerve in the sign group of cases group were respectively(0.31±0.09)and(150.58±47.71)kPa,and those in the non-sign group of case group were respectively(0.10±0.09)and(87.95±38.73)kPa.The CR and E values of median nerve of the sign group were significantly higher than those in the non-sign group(t=7.78,4.89,P<0.001),respectively.The duration of the disease was positive correlation with E value and CR value of median nerve in HD patients who underwent dialysis(r=0.598,0.459,P<0.001),respectively.The area under curve (AUC) value of receiver operating characteristic (ROC) curve showed that the best CR cut-off value was 21% and the E value was 76.22kpa in diagnosing median nerve injury,.and the AUC values of CR and E value were respectively 0.844 and 0.817,and the AUC value,sensitivity and specificity of the combine of them reached respectively 0.908,100% and 89%. Conclusion:Both CR and E values of the median nerve can be used to evaluate the median nerve injury in patients with DRA,and the combination of them has a higher diagnostic efficiency. Conventional ultrasound and SWE elastography can be the preferred imaging methods for the evaluation of median nerve injury.
4.Analysis of occurrence time and risk factors of acute cerebral infarction complicated with deep venous thrombosis

Objective:To explore the occurrence time and risk factors of deep vein thrombosis (DVT) in patients with acute cerebral infarction, so as to guide clinical prevention and treatment.Methods:1 129 patients with acute cerebral infarction treated in Beijing Tiantan Hospital from May 2014 to May 2016 were selected as the research objects. According to whether DVT occurred, the patients were divided into DVT group ( n=22) and non DVT group ( n=1 107); The information was analyzed retrospectively and the occurrence time of DVT was counted. The independent risk factors of acute cerebral infarction complicated with DVT were analyzed by univariate and multivariate logistic regression. Results:The time of DVT in patients with acute cerebral infarction was 10.5 (4-14) days. Univariate analysis showed that there were significant differences in age, gender, atrial fibrillation, smoking, drinking, chronic obstructive pulmonary disease, peripheral artery disease, renal failure, anticoagulants, BMI, white blood cell, blood glucose at admission and length of stay between the DVT group and the non DVT group ( P<0.05). Multiple factors further confirmed that renal failure [odds ratio ( OR)=57.421; 95% confidence interval ( CI), 5.792-569.314)] and length of hospital stay ( OR=1.148; 95% CI: 1.071-1.232) were independent risk factors for DVT. Conclusions:The median time of DVT in patients with acute cerebral infarction was 10.5 days. Renal failure and hospital stay were independent influencing factors of DVT in patients with acute cerebral infarction. This is helpful to determine the best prevention and treatment duration of DVT in patients with acute cerebral infarction, make rational use of medical resources and formulate personalized prevention and treatment strategies.
5.Effect of inter-fractional positional uncertainty on cumulative dose of target volume in intensity-modulated radiotherapy of cervical cancer

Objective:To obtain the inter-fractional set-up errors of intensity-modulated radiotherapy (IMRT) of cervical cancer by cone-beam CT (CBCT), and to analyze the variations of the set-up errors on the cumulative dose deviation of the target volume.Methods:A total of 48 patients with cervical cancer who underwent IMRT were enrolled in this study, and the set-up errors of 696 CBCTs were obtained. The set-up errors were input into the treatment planning system, and the cumulative set-up error dose was obtained by superposing the set-up errors dose. The deviation percentage was calculated by the deviation formula and the standard planning dose.Results:The set-up errors caused the offset of isocenter distance by 0.58(0.36, 0.80) cm. Different statistical differences were noted between the cumulative set-up error dose and the standard planning dose by WilCoxon test. All the dose deviations in the target volume were reduced, and the differential dose volume histogram (DVH) appeared negatively skewed, and the peak value was declined. The DVH diagram shifted to the left with an inverse S-curve and the slope was increased. The HI deviation of the target volume from small to large were: CTV 1, CTV 2, GTV/CTV, CTV 3, CTV n, CTV all, and GTV nd; The HI deviation of the target volume were increased. Conclusions:The effect of set-up errors in IMRT of cervical cancer upon the cumulative doses of the target volume significantly differs. The cumulative dose of the target volume is reduced, and the uniformity of the target volume dose becomes worse. The uncertainty of the inter-fractional position leads to an increase or decrease in the the fractional doses of the target volume. The biological effect on tumor cells and the tumor recurrence remains to be investigated. In IMRT of cervical cancer, the CBCT position calibration is required before each treatment to ensure the dose accuracy of each structure in the target volume.
6.Clinical characteristics and correlation of adult primary nephrotic syndrome with thyroid dysfunction

Objective To explore the clinical characteristics and correlation of adult primary nephrotic syndrome (PNS) with thyroid dysfunction,and early identify high-risk adult PNS patients with abnormal thyroid function by clinical data.Methods The clinical data of 101 adult PNS patients in Heji Hospital Affiliated to Changzhi Medical College from March 2015 to December 2017 were retrospectively analyzed.According to the thyroid function,the patients were divided into 2 groups:normal thyroid function group (67 cases) and thyroid dysfunction group (34 cases),including 9 cases with low triiodothyronine (T3) syndrome and 25 cases with subclinical hypothyroidism.The clinical data were compared,and the correlation between thyroid-stimulating hormone (TSH) and 24 h urinary protein,blood albumin and systolic blood pressure were analyzed.Results The incidence of thyroid dysfunction in adult PNS patients was 33.66% (34/101),including 21 cases of membranous nephropathy,8 cases of minimal change disease,4 cases of IgA nephropathy and 1 case of membranoproliferative nephritis.The 24 h urinary protein in thyroid dysfunction group was significantly higher than that in normal thyroid function group:(8.76 ± 3.62) g vs.(6.96 ± 3.43) g,the albumin was significantly lower than that in normal thyroid function group:(21.82 ± 4.89) g/L vs.(24.49 ± 4.14) g/L,and there were statistical differences (P<0.05 or <0.01);there was no significant difference in gender composition,age,course of disease,systolic blood pressure,diastolic blood pressure,body mass index,hemoglobin,platelet,creatinine,cystatin C,fasting blood glucose,total cholesterol,triacylglycerol,low-density lipoprotein cholesterol (LDL-C),fibrinogen,complement C3,IgG,IgM,IgA,PNS types and comorbidities between 2 groups (P>0.05).The results of subgroup analysis results showed that the systolic blood pressure in subclinical hypothyroidism patients of thyroid dysfunction group was significantly higher than that in normal thyroid function group and the low T3 syndrome patients of thyroid dysfunction group:(148.16 ± 18.09) mmHg (1 mmHg =0.133 kPa) vs.(139.55 ± 18.77) and (127.78 ± 16.81) mmHg,the 24 h urinary protein was significantly higher than that in normal thyroid function group:(9.00 ± 3.64) g vs.(6.96 ± 3.43) g,the albumin was significantly lower than that in normal thyroid function group:(21.71 ± 5.26) g/L vs.(24.49 ± 4.14) g/L,and there were statistical differences (P<0.05).Pearson correlation analysis result showed that TSH had no correlation with 24 h urinary protein and systolic blood pressure (r =0.193 and 0.072,P =0.053 and 0.472);however TSH was negatively correlated with albumin (r =-0.340,P =0.001).Conelusions In adult PNS patients with thyroid dysfunction,membranous nephropathy is the most common,followed by minimal change disease.The systolic blood pressure in PNS patients with subclinical hypothyroidism is significantly higher than that in patients with normal thyroid function and low T3 syndrome.In adult PNS patients,the lower the blood albumin is,the more likely they have thyroid dysfunction.

8.Effects of Dendrobium officinale Polysaccharides on Gene Expression Profile of HUVEC

OBJECTIVE: To investigate the effects of Dendrobium officinale polysaccharides on gene expression profile of HUVEC. METHODS: HUVEC was selected as objects. MTS method was used to detect the effects of different doses of D. officinale polysaccharides (50, 100, 200, 400, 800 μg/mL) on the proliferation activity of HUVEC. The growth inhibitory concentration of 30% cells (IC30) was calculated to screen the dose of follow-up tests. cDNA microarray assay was used to detect the changes of gene expression profile for HUVEC after treated with D. officinale polysaccharides for 24 h, so as to screen differentially expressed genes. GO enrichment analysis and KEGG pathway enrichment analysis were performed for top 5 differentially expressed genes by using DAVID bioinformatics resource database. Real-time fluorescence quantitative polymerase chain reaction (qRT-PCR) was used to validate the results of microarray detection with immunity-related differentially expressed genes as objects. RESULTS: After treated with 100, 200, 400, 800 μg/mL D. officinale polysaccharides, survival rate of HUVEC were decreased significantly (P<0.05 or P<0.01). IC30 value was 408 μg/mL. After treated with 400 μg/mL (by IC30) D. officinale polysaccharides, there were 91 differentially expressed genes in HUVEC cells, of which 84 were up-regulated and 7 were down-regulated. Top 5 genes of up-regulated and down- regulated expression were SELE, CCL2, CXCL6, IL8, ICAM1 as well as VWCE, CPT1A, CLU, CCL14, CINS4, which may be mainly associated with immune conditions and inflammatory responses. The differentially expressed genes mainly distributed in extracellular domain, and were enriched in biological processes such as production and response of cytokines and stimulus response, and played molecular functions such as chemokine and its receptor activity. The up-regulated genes as SELE, ICAM1 and CXCL2 were mainly enriched in TNF signaling pathway, influenza A (H1N1), herpes simplex virus infection and other pathways. The down-regulated gene CCL14 was mainly enriched in chemokine signaling pathway. Results of qRT-PCR validation tests showed that relative expression of ICAM1 was increased significantly, while that of CCL14 was decreased significantly (P<0.05), which was in agreement with microarray detection results. CONCLUSIONS: After treated with D. officinale polysaccharides, the expression of 91 genes in HUVEC cells are different significantly, mainly being up-regulated. The differentially expressed genes may participate in immune regulation through TNF signaling pathway, influenza A (H1N1) and herpes simplex virus infection.
9.The application of superb micro-vascular imaging in predicting pregnancy outcome in patients with in vitro fertilization-embryo transfer

Objective To investigate the role of endometrial thickness,uterine artery and spiral artery blood flow parameters,endometrial and subendometrial blood flow on pregnancy outcome in vitro fertilization-embryo transfer (IVF-ET) cycle,compare the difference of the endometrial and subendometrial blood flow typing between the superb micro-vascular imaging (SMI) and power doppler ultrasound (PDUS). Methods Forty-one patients undergoing IVF-ET cycles in our hospital were recruited in this study. Transvaginal color Doppler ultrasound was performed to determine and record endometrial thickness,peak systolic velocity(PSV),end diastolic velocity(EDV),pulse index(PI),resistance index(RI), systolic diastolic ratio(S/D) of uterine artery and spiral artery,and degree endometrial and subendometrial blood flow signals were showed under the two blood flow modes of PDUS and SMI respectively on the day of human chorionic gonadotropin(HCG) injection.The degree of the endometrial and subendometrial blood flow was scored using Applehau classification method.According to the pregnancy outcome,patients were divided into pregnancy group and nonpregnancy group,the difference of the above indicators between the two groups were compared,and the degree of endometrial and subendometrial blood flow under two kinds of blood flow mode were observed.Results ①On the day of HCG injection,patients in pregnancy group had lower RI,PI,S/D of spiral artery than those in the nonpregnancy group (P< 0.05). There were no significant difference between the pregnant group and nonpregnant group in endometrial thickness and the blood flow parameters of uterine artery ( P >0.05). ②On the day of HCG injection,there was significant difference between the pregnant group and nonpregnant group in endometrial and subendometrial blood flow degree with SMI mode ( P =0.005).The patients with type Ⅲ in pregnancy group were most (50% ). ③The endometrial and subendometrial blood flow typing (β= 1.085,OR= 1.085,95% CI=1.040 -8.421, P=0.042) of pregnancy group under SMI mode have an independent predictive value.With increased blood flow typing under endometrial and subendometrial blood flow,the higher the possibility of pregnancy in patients.The ROC curve analysis showed that the area under ROC endometrial and subendometrial blood flow typing SMI mode on the HCG day was 0.746( P =0.008).Conclusions SMI is a new and effective type of Doppler ultrasound technique to evaluate the endometrial and subendometrial blood flow in IVF-ET cycle.On the day of HCG injection,the endometrial and subendometrial blood flow typing in SMI mode may predict pregnancy outcomes in IVF-ET cycles,and guide to choose the best embryo transfer time.
10.The value of superb microvascular imaging in the evaluation of papillary thyroid carcinoma

Objective To compare the difference between color Doppler flow imaging (CDFI) and superb microvascular imaging (SMI) in the evaluation of vascular grading and the display rate of vascular in papillary thyroid carcinoma (PTC).Methods Between June 2015 and December 2015 in Beijing Chaoyang Hosptial,50 patients with 61 thyroid nodules were detected by ultrasonography,and pathologic types were confirmed as PTC.CDFI and SMI were used to evaluation the same nodule respectively according to Adler's grading criteria.The different flow characteristics were analyzed by the x2 test.Results In CDFI,the vascular grading of 33 patients were 0 ~ Ⅰ and these of 28 patients were Ⅱ ~Ⅲ by CDFI.In SMI,the vascular grading of 6 patients were 0 ~ Ⅰ and these of 55 patients were Ⅱ ~Ⅲ by SMI.There was a significant difference between CDFI and SMI for vascular grading (x2=27.475,P < 0.01).In CDFI,50 patients with PTC showed punctiform and rod-like flow signals,and 5 patients showed more detail internal small branches by CDFI.In SMI,42 patients with PTC showed punctiform and rod-like flow signals,and 17 patients showed more detail internal small branches.There was a significant difference between CDFI and SMI about the display rate of vascular (x2=7.011,P < 0.01).Conclusions Compared with CDFI,SMI is highly improved blood flow sensitivity.It can provide more detail internal small branches in PTC.The characteristics of vascular distribution are similar to strengthening mode of contrast-enhanced ultrasound in PTC,therefore,SMI is worth to further research.
