2.Association between cognitive function and anterior cingulate cortex gamma-amino-butyric acid concentrations in patients with depression before and after treatment

Objective:To explore the association between cognitive function and the level of gamma-amino-butyric acid(GABA)in anterior cingulate cortex(ACC)before and after treatment in patients with major depres-sion disorder.Methods:Totally 31 medication-naive patients with major depression disorder meeting the criteria of the Diagnostic and Statistical Manual of Mental Disorders,Fifth Edition(DSM-5)and 33 normal controls were col-lected.Each eligible patient received treatment with selective serotonin reuptake inhibitor agents for 8 weeks.The MATRICS Consensus Cognitive Battery(MCCB)was used to evaluate the cognitive function.By means of 1H magnetic resonance spectroscopy,anterior cingulate cortex GABA concentrations were measured.Results:At base-line,the concentration of ACC GABA relative to water(GABA+/W)was lower in the patient group than in the control group(P<0.05)and increased after treatment(P<0.05).ACC GABA+/W was negatively associated with verbal learning and visual memory score in patient group at baseline(correlation coefficient and P value were r=-0.40,P<0.05;r=-0.42,P<0.05,respectively).The ACC GABA+/W difference resulted of treatment in patient group was positively associated with the difference of working memory score and the difference of reasoning and problem-solving score(correlation coefficient and P value were r=0.58,P<0.05;r=0.66,P<0.05,respec-tively).Conclusion:The cognitive dysfunction of patients with major depression disorder may not be related to the degree of depression and anxiety.And improvement of cognitive function may be associated with increase of ACC GABA concentrations.
3.Syndrome Distribution and Characteristics of Endometriosis in 1 895 Cases

ObjectiveTo explore the distribution and characteristics of traditional Chinese medicine (TCM) syndromes in patients with endometriosis (EMS). MethodA cross-sectional survey was conducted to analyze the characteristics of TCM syndromes in 1 895 cases of EMS in hospitals of 19 provinces, autonomous regions, and municipalities. ResultAmong the 1 895 patients, Qistagnation and blood stasis syndrome accounted for the highest proportion, followed by Qi deficiency and blood stasis syndrome, cold coagulation and blood stasis syndrome, and kidney deficiency and blood stasis syndrome. The distribution of TCM syndrome types of patients with EMS in different regions, different treatment stages, and different disease types and with different therapeutic goals was different, and the differences were statistically significant. However, under different conditions, the Qi stagnation and blood stasis syndrome accounted for the highest proportion. Under the stratification of different regions, the cold coagulation and blood stasis syndrome in north China was relatively high, the kidney deficiency and blood stasis syndrome in south China was relatively high, and the combined phlegm and stasis syndrome was relatively high in southwest China. Different diagnosis and treatment goals corresponded to different clinical syndromes. With pelvic pain as the main manifestation, the proportion of cold coagulation and blood stasis syndrome was higher. The proportion of kidney deficiency and blood stasis syndrome was higher in married patients with infertility. Patients with the main diagnosis and treatment goals of controlling mass and inhibiting recurrence had a higher proportion of Qi deficiency and blood stasis syndrome. In different treatment stages, the proportion of Qi deficiency and blood stasis syndrome in postoperative patients and those with recurrent EMS was higher. Among different disease types, the Qi deficiency and blood stasis syndrome accounted for a higher proportion in patients with ovarian endometriosis (OEM). The cold coagulation and blood stasis syndrome accounted for a higher proportion in patients with deep invasive endometriosis (DIE). The kidney deficiency and blood stasis syndrome accounted for a higher proportion in patients with peritoneal EMS. There were significant differences in age, body mass index (BMI), and course of disease among patients with different syndromes. Patients with Qi deficiency and blood stasis syndrome was relatively older, and their course of the disease was relatively long. Patients with combined phlegm and stasis syndrome had relatively high BMI. There was no significant difference in CA125 and CA199 levels among all syndrome types. ConclusionThe distribution of TCM syndromes of EMS has a certain regularity, and there are differences in regional distribution, therapeutic goals, treatment stages, and disease types(P<0.05). However, the Qi stagnation and blood stasis syndrome accounts for a large proportion under different conditions, suggesting that Qi stagnation is the key link of EMS. In the early stage, the team took relieving depression and activating blood as the primary treatment, and created Huoxue Xiaoyi prescription, which was the core prescription for the treatment of EMS with Qi stagnation and blood stasis syndrome, achieving good clinical effect. At the same time, it is emphasized that EMS treatment should be integrated into the concept of chronic disease management and combined with health management. Through psychological counseling, cognitive behavior intervention, popular science lectures, and other methods, it is advised to adjust the emotion of patients with EMS, thereby increasing the curative effect. This study is expected to provide references for the clinical treatment of EMS.

Objective:To analyze the risk factors of bronchopulmonary dysplasia(BPD)in very preterm infants(VPI), and to provide scientific basis for the prevention and treatment of BPD in VPI.Methods:A prospective multicenter study was designed to collect the clinical data of VPI in department of neonatology of 28 hospitals in 7 regions from September 2019 to December 2020.According to the continuous oxygen dependence at 28 days after birth, VPI were divided into non BPD group and BPD group, and the risk factors of BPD in VPI were analyzed.Results:A total of 2 514 cases of VPI including 1 364 cases without BPD and 1 150 cases with BPD were enrolled.The incidence of BPD was 45.7%.The smaller the gestational age and weight, the higher the incidence of BPD( P<0.001). Compared with non BPD group, the average birth age, weight and cesarean section rate in BPD group were lower, and the incidence of male infants, small for gestational age and 5-minute apgar score≤7 were higher( P<0.01). In BPD group, the incidences of neonatal respiratory distress syndrome(NRDS), hemodynamically significant patent ductus arteriosus, retinopathy of prematurity, feeding intolerance, extrauterine growth restriction, grade Ⅲ~Ⅳ intracranial hemorrhage, anemia, early-onset and late-onset sepsis, nosocomial infection, parenteral nutrition-associated cholestasis were higher( P<0.05), the use of pulmonary surfactant(PS), postnatal hormone exposure, anemia and blood transfusion were also higher, and the time of invasive and non-invasive mechanical ventilation, oxygen use and total hospital stay were longer( P<0.001). The time of starting enteral nutrition, cumulative fasting days, days of reaching total enteral nutrition, days of continuous parenteral nutrition, days of reaching 110 kcal/(kg·d) total calorie, days of reaching 110 kcal/(kg·d) oral calorie were longer and the breastfeeding rate was lower in BPD group than those in non BPD group( P<0.001). The cumulative doses of amino acid and fat emulsion during the first week of hospitalization were higher in BPD group( P<0.001). Multivariate Logistic regression analysis showed that NRDS, invasive mechanical ventilation, age of reaching total enteral nutrition, anemia and blood transfusion were the independent risk factors for BPD in VPI, and older gestational age was the protective factor for BPD. Conclusion:Strengthening perinatal management, avoiding premature delivery and severe NRDS, shortening the time of invasive mechanical ventilation, paying attention to enteral nutrition management, reaching whole intestinal feeding as soon as possible, and strictly mastering the indications of blood transfusion are very important to reduce the incidence of BPD in VPI.
6.Multicenter retrospective study of 38 cases with fumarate hydratase deficiency uterine leiomyoma

Objective:To investigate the clinicopathological features of fumarate hydratase (FH) deficiency uterine leiomyoma.Methods:The data of 38 patients with FH deficiency uterine leiomyoma were screened and analyzed. The expressions of FH, S-(2-succino)-cysteine (2SC), desmin, p16, p53, CD 10 and cell proliferation associated nuclear antigen (Ki-67) proteins were detected by immunohistochemistry, and their clinicopathological features were analyzed retrospectively. Results:(1) Clinical features: the median age of the patients was (42.5±7.4) years old. Twenty-one cases (55%) of them were myomas found in physical examination, and the median maximum diameter of the tumor was 6.0 cm (range: 5.0-7.5 cm); myomectomy was performed in 23 cases (61%), total hysterectomy with or without bilateral appendages in 15 cases (39%); laparoscopic surgery in 27 cases (71%), open surgery in 11 cases (29%); none of the patients had renal cell carcinoma. (2) Histological features: atypical nuclear cells were distributed locally or diffusely, eosinophilic nucleoli and intranuclear inclusion bodies could be seen, glass like globules could be seen in the cytoplasm, nuclear division was 0-4/10 high power field (HPF), and antler like blood vessels and pulmonary edema-like changes could be seen in the stroma. Among 38 patients with FH deficiency uterine leiomyoma, FH was negative in 37 cases (97%), and positive in 1 case (3%); 2SC, desmin, p16, p53, CD 10 and Ki-67 showed focal positive expression in 38 cases (100%), including 35 cases (92%) with Ki-67 index<10% and 3 cases (8%) with Ki-67 index ≥10%. (3) Follow-up: 4 cases (11%) recurred, and there was no death. There were significant differences in age, family history, distribution of atypical nuclei and mitosis number between recurrent group and non-recurrent group (all P<0.05). Conclusions:FH deficiency uterine leiomyoma is a rare tumor, which needs pathological examination,immunohistochemical examination and clinical history. Patients younger than 43 years old, with family history, histologically atypical diffuse nuclear distribution and mitotic number ≥3/10 HPF should be alert to the risk of recurrence.
7.Effects of different feeding patterns on mother-to-child transmission of HBV infections in pregnant women with high viral loads after antiviral medication during pregnancy: A prospective cohort study

Objective:To study the influence of different feeding patterns on mother-to-child transmission (MTCT) of hepatitis B virus (HBV) in pregnant women with high viral loads who received antiviral medication during pregnancy to the day of delivery.Methods:This prospective cohort study was conducted in Beijing You'an Hospital. From January 1, 2019, to March 31, 2020, and 574 pregnant women with positive hepatitis B surface antigen (HBsAg) and HBV DNA>2×10 5 IU/ml were enrolled. All participants received tenofovir, telbivudine, lamivudine, or propofol tenofovir from 24-28 weeks of gestation and discontinued on the day of delivery, and their neonates were postnatally given routine passive-active immunoprophylaxis. Based on the feeding patterns, the subjects were divided into three groups: breastfeeding ( n=257), bottle-feeding ( n=241) and mixed feeding groups ( n=76). The follow-up data were obtained from liver functions and HBV DNA level of the mothers at 6-8 weeks postpartum and HBV serological markers of infants at 7-12 months. One-way ANOVA, Student-Newman-Keuls, Chi-square test or Fisher exact test, and repeated measures ANOVA were used to analyze the data. Results:The average maternal HBV DNA levels before antiviral treatment did not differ significantly between the three groups [(7.90±0.67), (7.82±0.70), (7.83±0.70) log 10 IU/ml, F=0.912, P>0.05]. HBV DNA level before delivery in the mixed feeding group was slightly lower than that in the breastfeeding and bottle-feeding group [(3.87 ±1.08) vs (4.21±1.17) and (4.30±1.28) log 10 IU/ml, q= 3.052 and 3.831, both P<0.05], while the comparison between the latter two groups showed no significant differences ( P>0.05). After delivery, HBV DNA level in the bottle-feeding group was slightly lower than that in the breastfeeding group [(7.42±0.93) vs (7.69±0.90) log 10 IU/ml, q=4.583, P<0.05]. Among 580 infants (including six pairs of twins), only one bottle-fed infant (0.4%, 1/243) was infected with HBV through MTCT, and none in the breastfeeding or mixed feeding group ( P=0.553). Conclusions:For pregnant women with high viral loads of HBV who have received antiviral medication during pregnancy, although HBV DNA level will rebound after discontinuation upon delivery, breastfeeding is recommended considering it does not increase the risk of MTCT.

Objective To understand the distribution of novel coronaviruses in the external environment of confirmed COVID-19 cases. Methods Environmental surface swab specimens such as bed rails, doorknob, closestool, hand washing sink, table, locker,ward pager, mobile phone, cup, clothes, were collected from the sentinel hospital of COVID-19, and samples were collected for the nucleic acid detection by RT-PCR. Results A total of 150 environmental samples were collected from 30 confirmed COVID-19 cases, 6 samples were determined to be novel coronaviruses postive (positive rate 4.00%). The total 14 mobile phone showed 3 novel coronaviruses positive.Among the 30 confirmed COVID-19 cases, 6 cases (positive rate 20.00%)were found novel coronaviruses in the external environment. Conclusions Novel coronaviruses exists in external environment of confirmed COVID-19 cases, which indicates the potential risk of COVID-19 infection.
9.Influence of the second stage of labor >2 hours of new birth process standards on preventing mother-to-child transmission and delivery outcome among chronic hepatitis B puerperas with antiviral therapy

Objective:To explore the influence of the second stage of labor>2 hours of new birth process standards on preventing mother-to-child transmission and delivery outcome among chronic hepatitis B puerperas.Methods:Clinical data of 100 puerperas with hepatitis B virus (HBV) were retrospectively analyzed in Beijing YouAn Hospital from January 1 2014 to December 1 2018. Puerpera with the second stage of labor >2 hours were in observation group, and puerperas with the second stage of labor<2 hours were in control group, 50 cases in each group. The follow-up of two groups was carried out until neonate aged 7 months. This study compared the main outcome measures (the rate of mother-to-child transmission of HBV) and the secondary outcome measures (surgical delivery, rate of lateral perineal incision, blood loss during delivery and 2 hours after delivery, incidence of neonatal asphyxia) between two groups.Results:There was no statistical difference in the number of neonate with positive HBsAg during delivery between two groups ( P>0.05) ; there was no neonate aged 7 months with positive HBsAg between two groups. Numbers of lateral perineal incision, surgical delivery and blood loss during delivery in observation group increased compared with those in control group with statistical differences ( P<0.05) . There were no statistical differences in the incidence of neonatal asphyxia and blood loss 2 hours after delivery between two groups ( P>0.05) . Conclusions:The second stage of labor >2 hours of new birth process standards is carried out without increasing the rate of mother-to-child transmission of hepatitis B, but it increases rates of surgical delivery, lateral perineal incision and blood loss during delivery. We should comprehensively consider and discuss when implementing the new birth process management in hepatitis B puerperas so as to assure the safety of months and infants.
10.Comparison of psychological anxiety between HBV-carrying pregnant women and healthy pregnant women and analysis of its influencing factors

Objective:To analyze the psychological anxiety and social support of HBV-carrying pregnant women and healthy pregnant women, and provide a reference for improving nursing interventions for hepatitis B virus (HBV) -carrying pregnant women.Methods:In this cross-sectional study, totally 314 pregnant women with HBV who attended outpatient services, established health files and had clear test results in a Beijing infectious disease specialist hospital from March to April 2019, and in two other ClassⅢ Grade A hospitals in the same period 420 healthy pregnant women were selected by convenient sampling. The Sociodemographic questionnaire, Self-Anxiety Scale (SAS) and Social Support Rating Scale (SSRS) were used to investigate the pregnant women.Results:There were statistically significant differences between HBV-carrying pregnant women and healthy pregnant women in education level, household registration status, family monthly income per capita, type of medical insurance, duration of pregnancy, status of pregnancy and subjective support scores ( P<0.05) . Multiple linear regression analysis controlling related factors found that whether the pregnant women carried the HBV had no effect on their psychological anxiety ( P>0.05) , and support utilization score, age, pregnancy days and education level affected the psychological anxiety of pregnant women ( P<0.05) .There was no statistically significant difference in SAS scores between the two groups of pregnant women ( t=-0.965, P>0.05) . Conclusions:The status of HBV carrying will not affect pregnant women's psychological anxiety, and factors such as social support utilization, education level, pregnancy days and age will have effect on pregnant women's psychological anxiety. It is recommended to take corresponding intervention measures to improve the utilization of social support, thereby alleviating the psychological anxiety of pregnant women.
