1.Intergenerational Effects on Metabolic Health: Perspectives on Maternal Nutrition and Exercise During Pregnancy

		                        			With the increasing prevalence of overweight and obesity among children and adolescents in China, pediatric metabolic syndrome has emerged as a significant public health challenge. The Developmental Origins of Health and Disease (DOHaD) theory underscores the critical influence of early environmental factors on lifelong metabolic health. Consequently, maternal nutritional status and physical activity during pregnancy have become key modifiable factors that have attracted considerable attention in recent years. Research indicates exposure to a maternal high-fat diet (HFD) during pregnancy has long-term effects on offspring health, which may be transmitted through placental transit disorder, inflammation, and oxidative stress. Similarly, a high-protein diet (HPD) during pregnancy exhibits a dose- and time-dependent biphasic effect: excessive intake may lead to fetal growth restriction and an increased risk of preterm birth, whereas moderate supplementation may instead reduce the susceptibility of offspring to obesity. Interestingly, caloric restriction (CR) during pregnancy presents a double-edged sword: while it may impair the development of metabolic organs in offspring, moderate CR in metabolically compromised mothers can ameliorate maternal metabolic dysfunction and reprogram oocyte DNA methylation, significantly lowering the risk of metabolic disorders in offspring. Notably, metabolic abnormalities induced by a low-protein diet (LPD) during pregnancy demonstrate lifecycle-accumulative effects and transgenerational inheritance, with offspring exhibiting obesity phenotypes during weaning, insulin resistance in adulthood, and hepatic decompensation in old age, mediated through oocyte epigenetic reprogramming. Additionally, maintaining an optimal micronutrient balance is crucial for the metabolic homeostasis of offspring, as both deficiency and excess can lead to detrimental outcomes. Maternal exercise has been established as a safe and effective non-pharmacological intervention that confers multigenerational metabolic benefits through diverse biological pathways. Maternal metabolic dysregulation represents a critical determinant of offspring metabolic disorders. Regular exercise during gestation exerts protective effects by attenuating maternal systemic inflammation and reducing the incidence of pregnancy-related complications, thereby effectively mitigating fetal overgrowth and metabolic dysfunction. This dual benefit for both mother and offspring underscores the pivotal role of gestational physical activity in promoting long-term metabolic health. The placenta, serving as the exclusive interface for maternal-fetal communication, mediates exercise-induced metabolic programming through enhanced secretion of key regulatory factors (including SOD3, Apelin, ADPN, and Irisin) and promotes the development of vascular networks, collectively optimizing nutrient transport efficiency. The intrauterine period represents a crucial window for epigenetic reprogramming, during which maternal exercise modulates DNA methylation patterns of critical metabolic genes (e.g., Ppargc-1α, Prdm16, Klf4, and Slc23a2) in offspring, thereby enhancing their capacity to resist metabolic disorders. Notably, the regulatory effects of maternal exercise extend beyond the gestational period. Postnatally, exercise-induced modifications in the bioactive components of breast milk and gut microbiota composition contribute to the sustained maintenance of metabolic homeostasis in offspring, establishing a continuum of metabolic protection from prenatal to postnatal stages. This review explores the potential of maternal combined nutrition-exercise interventions, suggesting that such strategies may synergistically enhance transgenerational health benefits through interactions within the metabolic-epigenetic network, thereby outperforming single interventions. Additionally, it examines current research limitations, including controversies surrounding transgenerational mechanisms, sex-specific responses, and undefined dynamic thresholds, while providing directions for future investigations. 2.Pharmacoeconomic evaluation of durvalumab combined with chemotherapy as first-line therapy for advanced biliary tract cancer

		                        			OBJECTIVE To assess the cost-effectiveness of durvalumab combined with chemotherapy as a first-line treatment for advanced biliary tract cancer from the perspective of the Chinese healthcare system. METHODS Using data from the TOPAZ-1 clinical trial, a three-state Markov model comprising progression-free survival (PFS), progressive disease (PD) and death was developed, with a cycle length of 21 days and a 10-year time horizon. Patients in the observation group received durvalumab in combination with gemcitabine and cisplatin, whereas those in the control group received placebo plus the same chemotherapy regimen. The evaluation indexes were quality-adjusted life year (QALY) and the incremental cost-effectiveness ratio (ICER). The willingness-to-pay (WTP) threshold was set at three times the 2024 Chinese per capita gross domestic product (GDP) (287 247 yuan/QALY). The sensitivity analyses, along with scenario analyses, were performed. RESULTS In the base-case analysis, the ICER of observation group compared to control group was 1 166 344.46 yuan/QALY, far exceeding the WTP threshold, indicating that the regimen was not cost-effective. One-way sensitivity analysis identified the PD state utility, discount rate, cost of durvalumab, and PFS state utility as the main drivers of ICER variation. Probabilistic sensitivity analysis showed that, at the above WTP threshold, the probability of the acceeptance of this regimen was 0, further supporting the robustness of the base-case findings. In the scenario analysis, inclusion of a patient assistance program reduced the ICER to 235 885.16 yuan/ QALY, below the above WTP threshold, suggesting cost-effectiveness under this assistance program. However, when applying a regional WTP threshold set at three times the per capita GDP (158 475 yuan/QALY) of Gansu Province (the province with the lowest GDP in China in 2024), the ICER remained above the threshold, indicating that the regimen was not cost-effective at the regional level. CONCLUSIONS At current pricing, durvalumab plus chemotherapy as a first-line treatment for advanced biliary tract cancer is not cost-effective in China. Although the introduction of a patient assistance program can substantially reduce the ICER and achieve cost-effectiveness at a WTP threshold set at three times the 2024 per capita GDP of China, due to limited affordability in low-income areas, the program remains not cost-effective.
		                        		
		                        		
		                        		
		                        	
3.Efficacy and safety of CDK4/6 inhibitors combined with endocrine therapy for HR+/HER2− advanced or metastatic breast cancer: A network meta-analysis

		                        			Objective  To compare the efficacy and safety of different cyclin-dependent kinase 4/6 inhibitors (CDK4/6i) combined with endocrine therapy (ET) for the treatment of hormone receptor-positive (HR+)/human epidermal growth factor receptor 2-negative (HER2−) advanced or metastatic breast cancer. Methods  Randomized controlled trials (RCTs) on CDK4/6i for the treatment of HR+/HER2− metastatic or advanced breast cancer were retrieved from databases including PubMed, EMbase, Web of Science, The Cochrane Library, CNKI, Wanfang, VIP, and SinoMed, with the search period ranging from database inception to August 2023. Bayesian network meta-analysis was conducted using R 4.2.0 software. Results  A total of 18 RCTs from 25 articles, involving 8 031 patients and 11 treatment regimens, were included. There was no significant difference in progression-free survival (PFS) or overall survival (OS) among different CDK4/6i+ET combinations. The highest cumulative probability for PFS was observed with dalpiciclib (DAL)+fulvestrant (FUL), while ribociclib (RIB)+FUL ranked first for OS. In terms of efficacy, abemaciclib (ABE)+aromatase inhibitors (AI) and ABE+FUL ranked first in objective response rate and clinical benefit rate, respectively. Regarding safety, statistically significant difference in grade 3-4 adverse events was observed among certain types of CDK4/6i (P<0.05). Conclusion  Current evidence suggests that CDK4/6i+ET is superior to ET alone for the treatment of HR+/HER2− advanced/metastatic breast cancer. Different CDK4/6i+ET combinations demonstrate comparable or similar efficacy; however, the incidence of adverse reactions is higher with combination therapy. 4.Analysis of Animal Model of Chronic Heart Failure Based on Combination of Diseases and Syndromes of Traditional Chinese Medicine and Western Medicine

		                        			Chronic heart failure (CHF) represents the terminal stage of cardiovascular diseases, and its prevalence remains high in China. In this study, existing animal models of CHF were retrieved and categorized. In combination with the characteristics of CHF from traditional Chinese medicine (TCM) and Western medicine perspectives, the models were weighted, and their clinical consistency was evaluated. The advantages and disadvantages of the models were assessed. Among them, models with higher TCM clinical consistency included the doxorubicin-induced model, the isoproterenol-induced model, and the left anterior descending coronary artery ligation model, each with a TCM consistency rate of 90%. The animal model established by the left anterior descending coronary artery ligation showed a high degree of clinical consistency with Western medicine, with a consistency rate of 82%. Each model exhibited its own advantages and disadvantages, with a general lack of modeling methods combining diseases and syndromes of TCM and Western medicine. At present, the inducement factors used for animal models are relatively singular, mainly reflecting the etiology and pathogenesis of Western medicine, with insufficient correlation to the pathogenesis of TCM. The characteristics of TCM syndromes are not fully represented, and the consistency between TCM and Western medicine is generally not high. TCM has the advantage of a multi-dimensional syndrome differentiation and treatment approach. It is necessary to integrate the characteristics of diseases and syndromes of TCM and Western medicine, adopt multi-factor modeling methods to reflect the pathological process of CHF, improve existing models, and establish animal models of CHF that better align with the characteristics of clinical diseases and syndromes of TCM and Western medicine, so as to provide a reliable reference for clinical prevention and treatment.

6.Noninvasive Diagnostic Technique for Nonalcoholic Fatty Liver Disease Based on Features of Tongue Images.

		                        			OBJECTIVE:
		                        			To investigate a new noninvasive diagnostic model for nonalcoholic fatty liver disease (NAFLD) based on features of tongue images.
		                        		
		                        			METHODS:
		                        			Healthy controls and volunteers confirmed to have NAFLD by liver ultrasound were recruited from China-Japan Friendship Hospital between September 2018 and May 2019, then the anthropometric indexes and sampled tongue images were measured. The tongue images were labeled by features, based on a brief protocol, without knowing any other clinical data, after a series of corrections and data cleaning. The algorithm was trained on images using labels and several anthropometric indexes for inputs, utilizing machine learning technology. Finally, a logistic regression algorithm and a decision tree model were constructed as 2 diagnostic models for NAFLD.
		                        		
		                        			RESULTS:
		                        			A total of 720 subjects were enrolled in this study, including 432 patients with NAFLD and 288 healthy volunteers. Of them, 482 were randomly allocated into the training set and 238 into the validation set. The diagnostic model based on logistic regression exhibited excellent performance: in validation set, it achieved an accuracy of 86.98%, sensitivity of 91.43%, and specificity of 80.61%; with an area under the curve (AUC) of 0.93 [95% confidence interval (CI) 0.68-0.98]. The decision tree model achieved an accuracy of 81.09%, sensitivity of 91.43%, and specificity of 66.33%; with an AUC of 0.89 (95% CI 0.66-0.92) in validation set.
		                        		
		                        			CONCLUSIONS
		                        			The features of tongue images were associated with NAFLD. Both the 2 diagnostic models, which would be convenient, noninvasive, lightweight, rapid, and inexpensive technical references for early screening, can accurately distinguish NAFLD and are worth further study.

7.Pathogenesis and Differentiated Treatment Strategies of Childhood Tic Disorders Based on WANG Xugao's “Thirty Methods of Treating the Liver”

		                        			Based on WANG Xugao's “thirty methods of treating the liver”, it is believed that the occurrence and development of childhood tic disorders follow the dynamic progression from liver qi disease to liver fire disease and then liver wind disease. The basic pathogenesis of three stages are characterized by binding constraint of liver qi, liver fire hyperactivity, and internal stirring of liver wind. Moreover, liver-blood deficiency and stagnation, and malnutrition of liver yin as the main point in terms of the imbalance of liver qi, blood, yin, and yang should be considered, as well as the imbalance relationship of the five zang organs such as the involvement of other organs and the gradually reach of the other organs. Guided by the principles of “thirty methods of treating the liver”, the treatment of tic disorders in liver qi stage should focus on soothing the liver and rectifying qi, soothing the liver and unblocking the collaterals, using Xiaochaihu Decoction (小柴胡汤) and Sini Powder (四逆散). The treatment of tic disorders in liver fire stage involves clearing, draining and resolving liver heat, using Longdan Xiegan Decoction (龙胆泻肝汤), Xieqing Pill (泻青丸), Danggui Longhui Pill (当归龙荟丸), and Huagan Decoction (化肝煎). The treatment of tic disorders in liver wind stage involves extinguishing wind and subduing yang, using Lingjiao Gouteng Decoction (羚角钩藤汤) and Liuwei Dihuang Pill (六味地黄丸). Throughout the treatment process, attention should be paid to harmonizing the liver's qi, blood, yin, and yang, as well as addressing the pathology of other organs. 
		                        		
		                        		
		                        		
		                        	
8.Image quality of CBCT under different exposure parameters

		                        			Objective:To investigate the imaging quality of CBCT under different exposure parameters and the relationship between subjective and objective evaluation.Methods:CBCT scanners of 6 different suppliers[3D Accuitomo(Morita),i-mCAT(Kavo),5G(NewTom),Smart3D(Beijing Langshi),DCT Pro(Vatech),Vgi(NewTom)]were used to scan the spatial resolution models and head simulators under typical exposure conditions(different device with different tube voltage and current for exposure).The visibility of SedentexCT IQ phantom and anatomical structures generated by different CBCT scanners were compared.For objective evaluation,7 professional evaluators scored CBCT imaging qualities.For subjective evaluation,the resolution(LP/mm)of the images were com-pared among the devices.Results:No significant difference was found by 7 evaluators either intra or inter of the teams.By objective e-valuation,the score of NewTom 5G was 2,that of i-CAT was 5 and that of the others was 4.By subjective evaluation,the LP/mm of i-CAT was 1.8,that of Smart3D was 2.0 and that of the others was 1.0-1.7.The subjective evaluation of image quality obtained un-der different tube voltages and tube currents were significantly different.Conclusion:The subjective and objective evaluations of image quality are approximately consistent.The difference of the subjective image quality may be produced by the different tube voltage and current for exposure.

		                        			Objective:To study the technological roadmap of integrated development of digestive tract endoscopy and artificial intelligence precision medicine,and to provide research directions and feasible technological paths for the"overtaking on a curve"of domestic gastrointestinal endoscopy.Methods:The Delphi method was used to investigate the needs and research directions for the refinement of gastrointestinal endoscopy from the perspective of medical professionals.An analysis of development directions of artificial intelligence precision medical technology based on technical documents on artificial intelligence precision medical technology was conducted.The application scenarios and technology roadmap of early gastric cancer and inflammatory bowel disease patients were designed from four main service directions of precise diagnosis,precise treatment,precise medication,and precise health management of artificial intelligence precision medicine.Results:Two refined application scenarios were designed for precise diagnosis of early gastric cancer and precise health management of inflammatory bowel disease patients,the layout direction and feasible path were planned for the development of the new gastrointestinal endoscopy industry,and a technology roadmap for the development of intelligent gastrointestinal endoscopy industrywas formed.Conclusion:The technology roadmap for the integrated development of gastrointestinal endoscopy with artificial intelligence precision medicine provides a sustainable development path for addressing the patent blockade of foreign gastrointestinal endoscopy companies on domestic products,uneven distribution of medical resources in the field of gastroenterology,and early diagnosis and treatment of digestive system diseases.
		                        		
		                        		
		                        		
		                        	
10.Discussion on Difficulties of Ethical Review of DCD Organ Donation and Transplant based on Cases

		                        			The ethics committee of organ transplantation technology and clinical application in a hospital has encountered some difficulties and typical cases in its review work and practice for many years. Sometimes, it is difficult to make a decision in these dilemmas. Based on the previous experience of the hospital in the ethical review of organ donation and transplantation, combined with two typical cases, this paper discussed and analyzed two review points of whether the voluntary unpaid donation and the principle of informed consent were met, and whether the risk-benefit ratio was reasonable, and put forward relevant ethical and legal countermeasure for further research by institutional ethics committees and other parties, in order to provide reference for discussing the practical problems and ethical confusion of ethical review of organ donation and transplantation.
