1.Comparison of macular and choroidal capillary vessel density between untreated CSC and PCV and its significance

Objective:To quantitatively assess the macular and choroidal capillary vessel density and foveal avascular zone (FAZ) variables in untreated central serous chorioretinopathy (CSC), polypoidal choroidal vasculopathy (PCV) patients and healthy people using optical coherence tomography angiography (OCTA), and to investigate their correlation with retinal thickness and choroidal thickness.Methods:A cross-sectional study was adopted.Fifty-five eyes of 55 Chinese adults, including 20 eyes with untreated unilateral CSC, 16 eyes with untreated unilateral PCV and 19 eyes of healthy controls, were enrolled from January 2018 to June 2021 in Shenzhen Aier Eye Hospital Affiliated to Jinan University.The 6 mm×6 mm scanning of Optovue OCTA was carried out in all the subjects.The superficial retinal vessel density (SRVD), deep retinal vessel density (DRVD), outer retinal vessel density (ORVD) and choroidal capillary vessel density (CCVD), FAZ area, FAZ perimeter and vessel density in a 300-μm-wide annular region around FAZ were measured by ReVue software of Optovue OCTA.Central foveal thickness (CFT) and subfoveal choroidal thickness (SFCT) were assessed by Optovue OCTA HD scan.Correlation between CFT, SFCT and OCTA variable, FAZ variables were evaluated by Pearson liner correlation analysis.This study adhered to the Declaration of Helsinki.The study protocol was approved by the Ethics Committee of Shenzhen Aier Eye Hospital Affiliated to Jinan University (No.AIERKY6236-1). Written informed consent was obtained from each subject prior to entering the cohort.Results:There were significant differences in the foveal, parafoveal and perifoveal SRVD among the three groups ( F=6.344, 9.006, 5.617; all at P<0.01). Compared with healthy control group, the foveal, parafoveal and perifoveal SRVD in CSC group as well as the foveal SRVD in PCV group were significantly increased (all at P<0.01). Compared with CSC group, the parafoveal and perifoveal SRVD were significantly decreased in PCV group (both at P<0.01). There were significantly differences in the parafoveal DRVD among the three groups ( F=3.383, P<0.05). Compared with healthy control group, the parafoveal DRVD of CSC and PCV groups was increased significantly (both at P<0.05). There were significant differences in the ORVD and CCVD among the three groups ( F=16.931, 19.412; both at P<0.001). Compared with healthy control group, the ORVD and CCVD in CSC group and the CCVD in PCV group were decreased significantly (all at P<0.001). Compared with the PVC group, the ORVD was significantly decreased in CSC group ( P<0.001). A significantly difference was found in CFT among the three groups ( F=20.495, P<0.001). Compared with healthy control and PCV group, the CFT was significantly increased in CSC group (both at P<0.001). No significant difference was found in SFCT, FAZ area, FAZ perimeter and vessel density of the 300-μm-wide annular region around FAZ among the three groups ( F=1.083, 0.906, 0.819, 1.530; all at P>0.05). Negative correlations were found between parafoveal DRVD, ORVD and CFT in CSC eyes ( r=-0.555, -0.516; both at P<0.05), and a significant negative correlation was found between ORVD and CFT in PCV eyes ( r=-0.585, P<0.05). No significant correlation was found between OCTA variables (SRVD, DRVD, ORVD, CCVD), FAZ parameters (FAZ area, FAZ perimeter and vessel density of the 300-μm-wide annular region around FAZ) and SFCT in the three groups (all at P>0.05). Conclusions:Hyperaemia in the superficial capillary of macular fovea and atrophy of choroidal capillaries may be the same pathogenesis between CSC and PCV.The vessel density of outer retinal layer is a common independent influencing factor of CFT between the two diseases.
2.Antidepressant effects of extracts from Zanthoxylum bungeanum and Zanthoxylum schinifolium in mice exposed to chronic restraint stress

Objective:
To study the antidepressant effects of extracts fromZanthoxylum bungeanumandZanthoxylum schinifoliumin mice exposed in chronic restraint stress.
Methods:
45 mice of SPF male ICR were randomly divided into five groups: normal group, model group, sertraline hydrochloride group,Zanthoxylum bungeanumextract group andZanthoxylum schinifoliumextract group, with 9 mice in each group. Except for the normal group, mice in all groups were chronically restrained to establish the depressive model. The normal group and model group were given olive oil by intragastric administration, and other groups were given sertraline hydrochloride(10 mg/kg),Zanthoxylum bungeanumextract(200 mg/kg) andZanthoxylum schinifoliumextract(200 mg/kg), respectively. The behaviors of mice in each group were evaluated by open-field test and forced swimming test. The levels of brain-derived neurotrophic factor(BDNF), 5-hydroxytryptophan(5-HT), glutathione(GSH) and nitric oxide(NO) in serum and brain were determined by ELISA.
Results:
Compared with model group,Zanthoxylum bungeanumextract could reduce the percentage of immobility time in forced swimming test, increase the level of cortical BDNF, and improve the levels of 5-HT and GSH in cortex and serum, with statistical significance(P<0.05).Zanthoxylum schinifoliumextract could improve the behavior of mice in open field test and forced swimming test, increase the levels of cortex 5-HT and serum GSH, and decrease the contents of NO in serum and cortex.
Conclusion
The extracts ofZanthoxylum bungeanumandZanthoxylum schinifoliumcan improve the depression-like behaviors of mice exposed to chronic restraint stress.
3.Research progress on pachychoroid disease

Pachychoroid disease is a relatively novel concept proposed in ophthalmology in recent years, which describes a series of pathological conditions including chronic persistent choroidal thickening and choroidal vasodilatation, with choroid blood capillary layer capacity attenuation, choroidal vasodilation, focal or disseminated choroidal thickening, progressive retinal pigment epithelial dysfunction and choroid retinal neovascularization as characteristic symptoms.Nowadays, the definition of pachychoroid disease has gradually changed from the previous emphasis on the simple thickening of choroid to the morphological description of complex pathological conditions.The clinical manifestations of certain pachychoroid diseases have been clearly described including central serous chorioretinopathy (CSC), pachychoroid neovasculcularization (PNV), polypoid choroidal vasculopathy (PCV), focal choroidal excavation (FCE), peripapillary pachychoroid syndrome (PPS). These entities have certain characteristic choroidal vasculopathy, which may represent different clinical manifestations of the same type of pachychoroid diseaseor indicate that they have similarpathogenesis .In this article, the existing literature and recent findings about pachychoroid disease, as well as the classification and treatment of pachychoroid diseases spectrum were summarized and discussed.
4.Relationships between Islet-Specific Autoantibody Titers and the Clinical Characteristics of Patients with Diabetes Mellitus

Dysimmunity plays a key role in diabetes, especially type 1 diabetes mellitus. Islet-specific autoantibodies (ISAs) have been used as diagnostic markers for different phenotypic classifications of diabetes. This study was aimed to explore the relationships between ISA titers and the clinical characteristics of diabetic patients. A total of 509 diabetic patients admitted to Department of Endocrinology and Metabolism at the Affiliated Hospital of Nantong University were recruited. Anthropometric parameters, serum biochemical index, glycosylated hemoglobin, urinary microalbumin/creatinine ratio, ISAs, fat mass, and islet β-cell function were measured. Multiple linear regression analysis was performed to identify relationships between ISA titers and clinical characteristics. Compared with autoantibody negative group, blood pressure, weight, total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), visceral fat mass, fasting C-peptide (FCP), 120 minutes C-peptide (120minCP) and area under C-peptide curve (AUCCP) of patients in either autoantibody positive or glutamate decarboxylase antibody (GADA) positive group were lower. Body mass index (BMI), waist circumference, triglycerides (TGs), body fat mass of patients in either autoantibody positive group were lower than autoantibody negative group. GADA titer negatively correlated with TC, LDL-C, FCP, 120minCP, and AUCCP. The islet cell antibody and insulin autoantibody titers both negatively correlated with body weight, BMI, TC, TG, and LDL-C. After adjusting confounders, multiple linear regression analysis showed that LDL-C and FCP negatively correlated with GADA titer. Diabetic patients with a high ISA titer, especially GADA titer, have worse islet β-cell function, but less abdominal obesity and fewer features of the metabolic syndrome.

7. Mediating effect of filialpiety values on the relationship between anxiety and caregiving burden among caregivers of patients with chronic heart failure

Objective:
To understand the filialpiety values, anxiety and caregiving burden of caregivers of patients with chronic heart failure and to analyze the mediating effect of filialpiety values on the relationship between anxiety and caregiving burden.
Methods:
During April 2017 to April 2018, 191children caregivers of patients with chronic heart failure from 3 secondary and tertiary medical institutions in Suzhou, Nanjing and Kunshan were selected to complete Visual Analogue Scale for Anxiety, Chinese Filial Values Index scale (FVI) and Dutch Objective Burden Inventory (DOBI).
Results:
The total score of caregivers' anxiety, filialpiety values and caregiving burden were 3(0, 6), 96(84, 107) and 1(0, 9) respectively. Correlation analysis showed that the caregivers' anxiety was positively correlated with the caregiving burden (
8.Influence of filial piety values on care-giving burden among caregivers of patients with chronic heart failure

Objective? To investigate the status of filial piety values and care-giving burden of caregivers of patients with chronic heart failure(CHF) and to explore the influence of filial piety values on care-giving burden. Methods? By purposive sampling method, 190 CHF patients and their caregivers were selected from three hospitals in Nanjing, Kunshan and Suzhou from April 2017 to April 2018. Patients' General Information Questionnaire, caregivers' General Information Questionnaire, Chinese version of Filial Values Index(FVI), and Dutch Objective Burden Inventory(DOBI) for chronic heart failure patients' caregivers were used in the study. Results? The total scores of FVI and DOBI of caregivers of CHF patients were (94.88±11.29) and (7.01±11.25). Patient's course of disease, caregiver's gender, whether to live with patients, attitude to patients' disease and filial piety were the influencing factors of care burden (P< 0.05). Conclusions? It is suggested that more attention should be paid to the burden of caregivers of patients with chronic heart failure and interventions should be taken to improve their filial piety values so as to reduce their burden of care.
9.Reliability and validity analysis of Chinese Filial Values Index scale in caregivers of chronic heart failure patients

Objective To introduce the Filial Values Index (FVI) scale and form Chinese FVI scale and evaluate reliability and validity of the scale. Methods Formed the Chinese FVI to investigate 98 caregivers for chronic heart failure patients. SPSS18.0 software was adopted to analyze the reliability and validity of the scale. Results The content validity of Chinese FVI scale was 0.89,and two common factors were extracted by exploratory factor analysis which could explained 68.91% of the total variance (P <0.01). The Cronbach α coefficient of the overall scale and each dimension was between 0.89-0.94, and the retest reliability was 0.97. Conclusions Chinese FVI scale had good reliability and validity, which can be used for evaluation of domestic caregivers filial piety values for chronic heart failure patients.
10.Impact of diabetes dietary knowledge on hospital readmission rate among patients with type 2 diabetes mellitus

Objective To explore the effect of dietary knowledge of diabetes on the readmission rate on patients with type 2 diabetes mellitus. Methods Totally 334 patients of type 2 diabetes from Suzhou finished the general information and the questionnaires of knowledge of diabetes diet. The impacts of knowledge of diabetes dietary on their readmission rates were also analyzed. Results Both whether having hypoglycemia or not and the score of diabetes dietary knowledge were associated with readmission rate within half a year. Those having low blood sugar (B=1.276, P<0.01) and higher score of diabetes dietary knowledge (B = 0.049, P = 0.006) had higher readmission rate. Further analysis showed that patients with low blood sugar getting higher score of diabetes dietary knowledge had higher hospital admission rate (P = 0.01). Conclusions The readmission rate of type 2 diabetes mellitus patients was impacted by hypoglycemia occurrence and the familiarity degree of diabetes diet knowledge. So it′s important to further explore the causes and make the reasonable intervention to reduce the readmission rate.
