1.Clinical Efficacy of Tangning Tongluo Tablets for Nonproliferative Diabetic Retinopathy

		                        			ObjectiveTo observe the clinical efficacy and safety of Tangning Tongluo tablets in the treatment of nonproliferative diabetic retinopathy (DR). MethodsFourteen research centers participated in this study, which spanned a time interval from September 2021 to May 2023. A total of 240 patients with nonproliferative DR were included and randomly assigned into an observation group (120 cases) and a control group (120 cases). The observation group was treated with Tangning Tongluo tablets, and the control group with calcium dobesilate capsules. Both groups were treated for 24 consecutive weeks. The vision, DR progression rate, retinal microhemangioma, hemorrhage area, exudation area, glycosylated hemoglobin (HbA1c) level, and TCM syndrome score were assessed before and after treatment, and the safety was observed. ResultsThe vision changed in both groups after treatment (P<0.05), and the observation group showed higher best corrected visual acuity (BCVA) than the control group (P<0.05). The DR progression was slow with similar rates in the two groups. The fundus hemorrhage area and exudation area did not change significantly after treatment in both groups, while the observation group outperformed the control group in reducing the fundus hemorrhage area and exudation area. There was no significant difference in the number of microhemangiomas between the two groups before treatment. After treatment, the number of microhemangiomas decreased in both the observation group (Z=-1.437, P<0.05) and the control group (Z=-2.238, P<0.05), and it showed no significant difference between the two groups. As the treatment time prolonged, the number of microhemangiomas gradually decreased in both groups. There was no significant difference in the HbA1c level between the two groups before treatment. After treatment, the decline in the HbA1c level showed no significant difference between the two groups. The TCM syndrome score did not have a statistically significant difference between the two groups before treatment. After treatment, neither the TCM syndrome score nor the response rate had significant difference between the two groups. With the extension of the treatment time, both groups showed amelioration of TCM syndrome compared with the baseline. ConclusionTangning Tongluo tablets are safe and effective in the treatment of nonproliferative DR, being capable of improving vision and reducing hemorrhage and exudation in the fundus.

3.Preliminary effectiveness of the whole-life cycle management model for valvular heart disease at West China Hospital: A retrospective cohort study

		                        			Objective  To propose a whole-life cycle management model for valvular heart disease (VHD), systematically elucidate its underlying logic and implementation pathways, and concurrently review and analyze its preliminary application outcomes. Methods  Since 2020, West China Hospital of Sichuan University has established a management system encompassing "assessment-decision-intervention-follow-up", including: (1) a risk-stratified, tiered management pathway; (2) six core functions ("promotion, screening, prevention, diagnosis, treatment, and rehabilitation") coordinated by disease-specific managers; (3) an intelligent decision support information platform; and (4) a collaborative network of multidisciplinary teams and regional academic alliances. To evaluate the effectiveness of this management model, we retrospectively included three cohorts: (1) the population screened by echocardiography from 2020 to 2024, analyzing the detection rate of aortic valve disease and risk stratification; (2) patients enrolled in the whole-life cycle management from April 2021 to December 2024, assessing follow-up outcomes, hospital satisfaction, and changes in quality of life; (3) patients who underwent transcatheter aortic valve replacement (TAVR) from January 2022 to January 2024, evaluating the one-year all-cause mortality rate, perioperative complications, and improvements in New York Heart Association (NYHA) classification. Results  Between 2020 and 2024, a total of 583 874 individuals underwent echocardiographic screening. A total of 48 089 patients with aortic valve disease were identified, including 3 401 (7.1%) high-risk patients, 18 657 (38.8%) moderate-risk patients, and 26 031 (54.1%) low-risk patients. Among them, 2 417 patients were enrolled in whole-life cycle management. Patient satisfaction scores showed a yearly increase, rising from 73.89 points before 2020 to 93.74 points in 2024. The 1-year mortality rate in the TAVR cohort decreased to 5.3%, significantly lower than the 8.2% observed under early standard management between 2014 and 2019 (P<0.01). Conclusion  Through process optimization and resource integration, the VHD whole-life cycle management model has demonstrated significant effectiveness in standardizing diagnostic and follow-up procedures, enhancing patient satisfaction and quality of life, and reducing mortality. 4.Effects of Huoxue Bushen Formula Granules on endometrial receptivity during ovulation induction in infertility patients with polycystic ovary syndrome

		                        			Objective:To evaluate the effects of Huoxue Bushen Formula Granules on endometrial receptivity (ER) in patients with polycystic ovary syndrome (PCOS) in ovulation induction cycle.Methods:A randomized controlled trial was conducted. Totally 50 patients with PCOS infertility in Xiyuan Hospital of China Academy of Chinese Medical Sciences from January 2019 to January 2023 were selected as the observation objects, and were divided into observation group (29 cases) and control group (21 cases) according to the random number table method. Both groups were given oral clomiphene citrate on the 5th day of menstrual cycle or progesterone withdrawal. On this basis, the observation group was given Huoxue Bushen Formula Granules on the 5th day of menstruation, while the control group was given estradiol valerate tablets. 7 days after taking medication and injecting hCG from the 10th day of menstruation, ovulation was monitored and sexual intercourse was guided. Both groups were treated for 3 menstrual cycles and followed up for 3 months. B-ultrasound was performed on 6-8 days after ovulation to obtain uterine artery pulsatility index (PI) and blood flow resistance index (RI). TCM syndrome scores were evaluated before and after treatment, and the endometrial receptivity score (Salle score) was used to evaluate the level of ER. The pregnancy and adverse reactions during treatment were recorded in the two groups.Results:After treatment, TCM syndrome score in the observation group [14.0 (13.0, 15.0) vs. 16.0 (14.5, 19.5), Z=-3.23] was lower than that of the control group ( P<0.001). The Salle score of the observation group after treatment [12.0 (9.5.0, 13.5) vs. 10.0 (9.0, 11.0), Z=-3.84] was higher than that before treatment ( P<0.001). The PI (0.68±0.52 vs. 0.81±0.06, t=18.25), RI (1.65±0.36 vs. 2.24±0.45, t=22.78) after treatment in the observation group and the control group were lower than those before treatment ( P<0.001), without statistical significance ( t values were -0.39, -1.37, respectively, P>0.05). The total effective rate of the observation group was 75.86% (22/29), and that of the control group was 71.43% (15/21), without statistical significance ( χ2=0.12, P=0.724). The pregnancy rate of the observation group was 82.76% (24/29), and that of the control group was 57.14% (12/21), with statistical significance ( χ2=3.96, P=0.046). The incidence of adverse reactions of the observation group was 3.45% (1/29) and that of the control group was 9.52% (2/21), without statistical significance ( χ2=0.80, P=0.372). Conclusion:Huoxue Bushen Formula Granules can improve the ER of PCOS infertility patients with kidney deficiency and blood stasis syndrome, improve the pregnancy rate, reduce the TCM syndrome score, improve the quality of life of patients with high safety.
		                        		
		                        		
		                        		
		                        	
5.Multimodal analgesia in the treatment of a case of pediatric primary erythromelalgia with infection and pharma-ceutical care

		                        			OBJECTIVE To explore a multimodal analgesia regimen based on spinal cord electrical stimulation for children with primary erythromelalgia and the key points of pharmaceutical care. METHODS Clinical pharmacists participated in the treatment of a child with primary erythromelalgia complicated with skin infection. After reviewing domestic and foreign literature, multimodal analgesia was formulated and pharmaceutical care was carried out to address the difficulties in treating the patient’s illness. RESULTS The treatment team applied multimodal analgesia based on spinal cord electrical stimulation for the child, including a multi-drug combination involving different analgesic pharmacological targets, multiple administration routes (oral, intravenous, epidural, percutaneous), multiple technologies (spinal cord electrical stimulation, local nerve block, patient- controlled analgesia), individualized schemes of adjuvant therapy, and the child was monitored for the safety of drug use. The pain was controlled during the treatment and follow-up period, the wound was healed, and no serious adverse drug reactions occurred. CONCLUSIONS Multimodal analgesia based on spinal cord electrical stimulation is a safe and effective treatment for children with primary erythromelalgia.
		                        		
		                        		
		                        		
		                        	
6.PDCA-guided Nursing Management course design and reform

		                        			Objective:To explore Nursing Management course design and teaching management reform under the guidance of the concept of Plan-Do-Check-Act (PDCA).Methods:The PDCA cycle was implemented in four stages and eight steps throughout the course design and teaching management of Nursing Management. With each class as a small cycle and the whole course as a big cycle, the teaching objectives, teaching content, teaching methods, teaching implementation, classroom quality, and learning effects were dynamically controlled. The implementation effects were evaluated through assessments and feedbacks.Results:The qualitative analysis showed that the teaching design was reasonable, the teaching objectives were specific, and the teaching content was clear in hierarchy and appropriate in arrangement and organization; and peers and experts rated classroom performance excellent. The quantitative analysis showed that the students' process assessment score was (88.14±1.23), written test score was (80.21±6.25), and average overall score was (82.60±4.43), all with significant improvements in horizontal and vertical comparisons; and the students had improvements in management awareness, management ability, practical application ability, and self-confidence, with a high degree of satisfaction with the course.Conclusions:PDCA-guided whole-process management can optimize the course teaching design of Nursing Management, which is conducive to developing students' management practice abilities and improving teaching quality.
		                        		
		                        		
		                        		
		                        	
7.Nuclear Magnetic Resonance Analysis Implicates Sex-Specific Dysregulation of the Blood Lipids in Alzheimer’s Disease: A Retrospective Health-Controlled Study

		                        			The aging demographic landscape worldwide portends a heightened prevalence of neurodegenerative disorders. Foremost among these is Alzheimer’s disease (AD), the foremost cause of dementia in older adults. The shortage of efficacious therapies and early diagnostic indicators underscores the imperative to identify non-invasive biomarkers for early detection and disease monitoring. Recently, blood metabolites have emerged as promising candidates for AD biomarkers. 
		                        		
		                        			Methods:
		                        			Leveraging nuclear magnetic resonance (NMR) spectroscopy on plasma specimens, we conducted a cross-sectional study encompassing 35 AD patients and 35 age-matched healthy controls. Cognitive function was evaluated using the mini-mental state examination in all participants, followed by peripheral blood sample collection. We utilized univariate and multivariate analyses to perform targeted lipidomic profiling via NMR spectroscopy. 
		                        		
		                        			Results:
		                        			Our study revealed significant differences in the expression profiles of low-density lipoprotein-associated subfractions in females and high-density lipoprotein-associated subfractions in males between AD patients and healthy controls (all p<0.05). However, there was no significant metabolite overlap between males and females. Furthermore, receiver operating characteristic curve analysis demonstrated that the combination of lipid metabolites had good diagnostic values (all area under the curve>0.70; p<0.05). 
		                        		
		                        			Conclusion
		                        			Our findings suggest that the blood plasma samples using NMR hold promise in distinguishing between AD patients and healthy controls, with significant clinical implications for advancing AD diagnostic methodologies. 
		                        		
		                        		
		                        		
		                        	
8.Trend Analysis on Incidence and Age at Diagnosis of Lymphoma in Cancer Registration Areas of Jiangsu Province from 2009 to 2019

		                        			[Purpose]To analyze the trend of incidence and mean age at diagnosis of lymphoma in cancer registration areas of Jiangsu Province from 2009 to 2019.[Methods]Continuous and com-pleted data of cancer incidence from 2009 to 2019 were collected from 16 cancer registries in Jiangsu Province,and all indicators meeting the requirements.Joinpoint 4.7.0.0 software was used to analyze the trend of lymphoma incidence,the average annual percentage change(AAPC)and 95%confidence interval(CI)were calculated.A birth cohort was built from 1929 to 2019,to ana-lyze the change trend of lymphoma incidence rate among people born in different periods.[Re-sults]From 2009 to 2019,the AAPC of lymphoma incidence rate in cancer registration areas of Jiangsu Province was 5.74%,5.55%for men and 5.02%for women.The AAPC in urban areas was higher than that in rural areas(6.26%vs 4.90%).The age-standardized incidence rate(ASIR)of lymphoma increased by 3.40%annually on average.The rising amplitude of ASIR of women(3.16%)was higher than that of men(3.05%),and the rising amplitude of ASIR of lymphoma in urban areas(3.39%)was higher than that in rural areas(2.43%).The birth cohort analysis showed that the incidence rate of lymphoma fluctuated greatly before the age of 40,and there was no sig-nificant trend of change,while the incidence rate in age groups of 40 years old and above showed an overall upward trend.The average age at onset of lymphoma showed an upward trend from 2009 to 2019,with an average annual increase of 0.35 years old(P<0.001).The standardized average age at onset for lymphoma still showed a significant upward trend,with an average annual increase of 0.15 years old(P=0.016).Compared with 2009,the standardized age specific incidence of lym-phoma in 2019 showed an overall backward trend,while the standardized incidence in all age groups after 60 years old(except for the age group of 70~74 years old)increased.The proportion of lymphoma in people above 60 years old was 56.16%in 2019,which was higher than that in 2009(52.57%).[Conclusion]The incidence rate of lymphoma in Jiangsu Province was increasing year by year from 2009 to 2019.The age at onset tended to shift back,and the trend was more pronounced in male than female.
		                        		
		                        		
		                        		
		                        	
9.Radiosensitizing effect of quercetin-encapsulated manganese dioxide nanoparticles on breast cancer cells

		                        			Objective To investigate the radiosensitizating effect of quercetin(QU)loaded manganese dioxide nanoparticles[Mn(QU)]on breast cancer cell line 4T1 and tumour-bearing mice.Methods Mang anese dioxide(MnO2)nanoparticles were synthesized by oleic acid template method.The morphology and chemical composition of MnO2 nanoparticles were characterized by transmission electron microscopy(TEM),scanning electron microscopy(SEM)and X-ray photoelectron spectroscopy.Then QU nanomaterials were encapsulated by using physical adsorption.The composition was characterized by ultraviolet spectrophotometer,and the ability of Mn(QU)nanoparticles reacting with different doses of hydrogen peroxide to produce oxygen at different pH values was detected by dissolved oxygen analyzer.CCK-8 assay was employed to detect the effects of different concentrations of Mn(QU)nanoparticles on the viability of 4T1 cells.Colony formation,γ-H2AX fluorescence staining,ROS fluorescence staining,LIVE/DEAD cell viability assay and flow cytometry were used to evaluate the radiosensitizing and pro-apoptotic effects of Mn(QU)nanoparticles on 4T1 cells.Finally,the effect of Mn(QU)nanoparticles combined with radiotherapy on tumor growth inhibition was evaluated in mouse model of 4T1 cell transplanted tumor.Results MnO2 nanoparticles with particle size of about 120 nm were successfully synthesized and encapsulated with QU.The oxygen generation capacity of the prepared Mn(QU)nanoparticles reacting with hydrogen peroxide was negatively correlated with pH value and positively with hydrogen peroxide concentration.The results of cell experiments showed that Mn(QU)nanoparticles at a concentration of 50 μg/mL had no obvious toxicity to 4T1 cells,but could significantly enhance the X-ray-induced killing effect on 4T1 cells,at a radiotherapy sensitization ratio of 1.61,improve DNA double-strand breaks and ROS production,and induce apoptosis of 4T1 cells.The results of tumor xenograft model experiment indicated that the inhibition of tumor volume was Mn(QU)nanoparticles combined with radiotherapy>MnO2 nanoparticles combined radiotherapy>QU combined radiotherapy>Radiotherapy>Control.Conclusion Mn(QU)nanoparticles combined with radiotherapy can significantly inhibit the proliferation and show radiosensitization of breast cancer 4T1 cells,and also exert a significant inhibitory effect on the growth of the transplanted tumor.
		                        		
		                        		
		                        		
		                        	
10.Application of UHPLC-MS/MS-based nontargeted metabolomics in plasma metabolism in altitude-related hypertension among high-altitude migrants

		                        			Objective To investigate the differences in plasma metabolites between patients with altitude-related hypertension(ARH)and healthy individuals,and analyze the potential pathogenesis of ARH.Methods Convenient sampling was conducted on a unit of male healthy officers and soldiers who resident at altitude of<500 m and migrated to an altitude of 4 200 m in July 2020.Twenty of them diagnosed with ARH were assigned into the ARH group,and another 30 non-ARH individuals served as the control group.Their blood pressure,body mass index(BMI),blood oxygen saturation,and heart rate were measured and recorded,and fasting venous blood samples were harvested to screen and identify plasma metabolites with ultra-high performance liquid chromatography-tandem mass spectrometry(UHPLC-MS/MS).Metabolite fingerprinting was performed using unsupervised Principal Component Analysis(PCA)and supervised Orthogonal Partial Least Squares Discrimination Analysis(OPLS-DA)models in order to assist in biomarker screening.The quality of the OPLS-DA model was assessed and validated to guarantee the stability and reliability of the model.Differential plasma metabolites were screened using independent sample t test and fold change(FC)analysis,and volcano plots were drawn.Finally,Kyoto Encyclopedia of Genes and Genomes(KEGG)enrichment pathway analysis was used to perform functional pathway enrichment and topological analysis on the screened differential metabolites.Results Compared to the control group,the ARH group showed significantly higher systolic and diastolic blood pressure and heart rate,and lower arterial oxygen saturation(P<0.05).PCA analysis showed that 81.96%of the variance was explained in the positive ion mode and 79.25%in the negative ion mode,indicating significant metabolic differences between the 2 groups.OPLS-DA model analysis indicated that in the positive ion mode,PC1 explained 77.36%of the variance and PC2 explained 12.25%of the variance,with R2Y=0.96 and Q2Y=0.91;in the negative ion mode,PC1 explained 84.15%of the variance and PC2 explained 17.24%of the variance,with R2Y=0.99 and Q2Y=0.86.Inter-group difference exceeded 75%,and intra-group difference was less than 20%.The 7-fold cross-validation and 200 permutation test confirmed that the model was stable and reliable.In the positive ion mode,the Y-axis intercepts of the R2 and Q2 fitted lines were 0.58 and-0.48,respectively;in the negative ion mode,the Y-axis intercepts were 0.93 and-0.41,respectively.A total of 32 significantly different metabolites were screened out,including amino acids,nucleosides,fatty acids,and organic alkaloids.KEGG analysis revealed that among the 10 metabolic pathways,4 were amino acid metabolic pathways,with the aminoacyl-tRNA biosynthesis pathway having the most enriched metabolites.Conclusion Based on UHPLC-MS/MS technology,untargeted metabolomics analysis identifies 32 significantly different metabolites,which may serve as characteristic biomarkers for ARH,and the aminoacyl-tRNA biosynthesis pathway may be associated with the pathogenesis of ARH.
