1.Prevalence of Epstein-Barr virus infection in individuals with nasopharyngeal carcinoma in Malaysia: The first systematic review and meta-analysis

Epstein-Barr virus (EBV) was the first herpesvirus associated to human malignancies. Despite
the well-known association between EBV and malignancies, the prevalence of EBV infection
in Malaysians with malignancies is unknown. Preferred Reporting Items for Systematic
Reviews and Meta-Analysis (PRISMA) was used to conduct a systematic review and metaanalysis of published data in this study. Studies reporting the occurrence of EBV infection in
Malaysian malignancy patients were searched in electronic databases like PubMed, Scopus,
ScienceDirect, and Google Scholar without year or language constraints. The study protocol
was filed in PROSPERO (CRD42021273769). A total of 21 studies were included, with 1,036 EBV
infection cases among 2,078 malignancy patients. The random-effects model was used to
produce summary estimates. The pooled prevalence of EBV infection in Malaysians with
malignancy was 36.3% (95% CI, 20.3 – 56.2). When the prevalence estimates were stratified by
malignancy type, nasopharyngeal carcinoma has the highest prevalence (90.5%), followed
by lymphoma (23.4%), and gastric carcinoma (10.0%). Male patients had a higher cases
prevalence and most patients were above the age of 40. In Malaysia, many malignancies are
increasingly linked to EBV infection. Screening for EBV infection in malignancy patients is
therefore important to determine disease recurrence and metastases.
2.Maternal Obesity and Its Associated Factors and Outcomes in Klang Valley, Malaysia: Finding from National Obstetric Registry

Introduction: Maternal obesity presents significant health risks to mothers and their fetuses. This study aimed to determine the proportion, associated factors and outcomes of maternal obesity among pregnant women in Klang Valley, Malaysia.
Methods: A retrospective cross-sectional study was conducted between January 2018 and March 2018 using secondary data from the Malaysian National Obstetric Registry (NOR) for the year 2015. All pregnant women with first-trimester booking at 12 weeks and below that were registered with the NOR and met the inclusion and exclusion criteria were included in the study. Descriptive statistics and multiple logistic regression analysis were used. Data were analysed using SPSS version 22.0. A total of 2113 respondents were included in this study to determine the proportion, associated factors and outcomes of maternal obesity. Regarding the univariate and multivariate analyses, respondents were classified into two groups: normal and obese. The obese group comprised overweight and obese mothers. The underweight group was excluded in the subsequent analysis.
Results: Out of the 2113 respondents, 7.1% were underweight, 41.7% were of normal weight, 28.6% were overweight, 15.9% were in obese class I, 4.6% were in obese class II, and 2.1% were in obese class III according to the WHO (1995) reference. However, when the MOH (2003) cutoff point was used, there was a marked increase in the proportion of respondents in the overweight categories by 2.7% and obesity class I by 12.8%. The Indian (AdjOR 2.06, 95% CI: 1.11, 3.83, p=0.021) and Malay (AdjOR 1.75, 95% CI: 1.02, 3.00, p=0.040) ethnicities, as well as both multiparity (AdjOR 1.46, 95% CI: 1.23, 1.73, p <0.001) and grand multiparity (AdjOR 2.41, 95% CI: 1.78, 3.26, p <0.001), were significantly associated with maternal obesity. There were significant association between maternal obesity with hypertensive disorder in pregnancy (p=0.025), caesarean section delivery (p=0.002) and macrosomic infant (p <0.001).
Conclusion: The identification of risk factors for maternal obesity is important to facilitate intervention programmes focused on improving the pregnancy outcomes for a high-risk group of women.
3.Coordinating a Postgraduate Orthopaedic Exam During the COVID-19 Pandemic

The Coronavirus (COVID-19) pandemic and institution of the
Movement Control Order (MCO) had resulted in the
cancellation of a major orthopaedic exam in April 2020. The
exam is known as the Malaysian Orthopaedic Specialist
Committee (OSC) Part I Examinations. It is similar to the
British Royal Colleges of Surgeons Membership (MRCS)
exams and held twice annually in April and October. There are
up to 200 candidates involved. With implementation of new
guidelines and standard operating procedures (SOP), the OSC
Part I exam was successfully held by Universiti Kebangsaan
Malaysia (UKM) from 5th-9th October 2020. Here we
highlight the challenges we faced whilst coordinating a major
exam at a national level during the COVID-19 pandemic with
recommendations for future exams.
4.Getting patient blood management Pillar 1 right in the Asia-Pacific: a call for action.

Preoperative anaemia is common in the Asia-Pacific. Iron deficiency anaemia (IDA) is a risk factor that can be addressed under patient blood management (PBM) Pillar 1, leading to reduced morbidity and mortality. We examined PBM implementation under four different healthcare systems, identified challenges and proposed several measures: (a) Test for anaemia once patients are scheduled for surgery. (b) Inform patients about risks of preoperative anaemia and benefits of treatment. (c) Treat IDA and replenish iron stores before surgery, using intravenous iron when oral treatment is ineffective, not tolerated or when rapid iron replenishment is needed; transfusion should not be the default management. (d) Harness support from multiple medical disciplines and relevant bodies to promote PBM implementation. (e) Demonstrate better outcomes and cost savings from reduced mortality and morbidity. Although PBM implementation may seem complex and daunting, it is feasible to start small. Implementing PBM Pillar 1, particularly in preoperative patients, is a sensible first step regardless of the healthcare setting.

Some filarial nematodes, such as Wuchereria bancrofti, Brugia malayi, and Brugia
timori, cause lymphatic diseases in humans in the tropics, whereas other filarial parasites
from wild animals cause zoonotic diseases in humans worldwide. To elucidate the prevalence
and diversity of filarial parasites in Malaysia, we investigated the filarial parasites from wild
animals in Gemas, Negeri Sembilan. To find adult filarial parasites, we dissected 26 animals,
which included five frogs, one skink, one snake, two birds, six common treeshrews, and 11
rats. Then, we examined microfilariae in the blood smears and skin snips obtained from each
animal. We found two types of microfilariae in the blood smears of common treeshrews: one
was very similar to Malayfilaria sofiani and the other closely resembled Brugia tupaiae.
These findings indicate an additional distribution of these filarial parasites in Gemas.
6.Dosimetric Study of Rhizophora Spp. Particle Board Using Gafchromic XRQA2 Film

Introduction: Various phantom with varied materials has been proposed to replace the human body. Besides, there is always a demand to use the local material as a phantom material, which is readily available and inexpensive. Wood is usually preferred because it is multifunction, environmentally friendly, low in toxic, inexpensive, as well as easy to use and prepare. Previous studies have found that Rhizophora spp. is a suitable natural source material and has been suggested due to its comparable dosimetric properties to commercial phantom. Methods: In this study, fabricated Rhizophora spp. particleboards phantom was opted as a solid-equivalent phantom medium at low energy photon beams using Gafchromic film x-ray quality assurance 2 (XRQA2). Additionally, the characteristics of XRQA2 film in the diagnostic energy range were generated. Results: Interestingly, the density of the fabricated Rhizophora spp particleboards was observed to have the same density with the water equivalent material (ρ= 1.00 g.cm-3) and has shown to have loosened agreement with PDD of water phantom at approximately 25% of the dose error. Also, further analysis using XRQA 2 film showed that energy was independent at different ranges. Conclusion: The analysis of fabricated Rhizophora spp particleboards undertaken here has extended our knowledge of the possibility of manufacturing cost-effective water equivalent phantom by using binder-less particleboard from Rhizophora spp. Therefore, a definite need for smaller interspacing particles should be considered to elevate the potential of Rhizophora spp particle boards as water equivalent materials.
8.RISK OF ACUTE KIDNEY INJURY AND HYPONATREMIA IN LONG-DISTANCE RUNNERS

Long-distance running has gathered some momentum among health-conscious participants. However, some studies have revealed association between long-distance running and development of acute kidney injury. Although the impact usually lasts only for a few days after the event, some participants have been admitted for severe acute kidney injury, the minority of which require dialysis treatment. The mechanisms underlying the injury may include dehydration, development of rhabdomyolysis, heat stroke and concomitant use of NSAIDS. Unfortunately, there is no long-term follow-up study to determine the long-term effect on kidney function. Acute hyponatremia may develop in a significant proportion of long-distance runners. Majority of them were asymptomatic but a few fatal cases which were supposedly due to cerebral oedema have been reported. Excessive intake of hypotonic drinks, excessive sweating and secretion of non-osmotic antidiuretic hormone have been postulated to be the causes of hyponatremia. This mini review will discuss the pathophysiology of the development of acute kidney injury and hyponatremia. It will also discuss the prevention and treatment of both conditions
9.Metallic hair pin aspiration into the left tertiary bronchus

Foreign body aspiration is commonly described in infants and children. However, recently, a newhigh-risk group was identified among young women, especially those from the Muslim populationwho wear the traditional hair scarf. This is due to the habit of holding the scarf pin in between thelips to free hands to adjust the scarf more easily. Talking, laughing, or coughing while fixing thescarf may result in inadvertent inhalation of the pin into the tracheobronchial tree. We present acase of scarf pin inhalation and the challenges encountered in managing this patient during thesuccessful removal of the pin via flexible bronchoscopy under fluoroscopy guidance. This particularcase was technically challenging for us as the sharp tip of the needle was pointing upward andpiercing the bronchial mucosa.
10.Prolong Placement of Tracheostomy Tube Causing Unusual Complication

Tracheostomy can be associated with numerous complications. Here, we present
a case of large hypertrophic skin of tracheostoma with tracheal granulation tissue
causing malpositioning of tracheostomy tube and airway obstruction, secondary to
prolong placement of a cuff tracheostomy tube due to improper tracheostomy care.
This case illustrates that awareness and knowledge of proper tracheostomy tube
care among medical personnel are very important to avoid any mismanagement
that can cause life-threatening complications.
