1.Changes in coordination of departments for major epidemic prevention and control in China before and after the outbreak of COVID-19: an analysis on official documents

		                        			ObjectiveTo analyze the changes in the degree of coordination of China's major epidemic prevention and control efforts before and after the outbreak of the Corona Virus Disease 2019 (COVID-19), so as to explore the impact of epidemic prevention and control measures on coordination dynamics. MethodsA total of 3 864 policy documents related to epidemic prevention and control from January 2000 to December 2020 across 31 provinces (autonomous regions, and municipalities) in China were systematically collected. Contents specific to collaborative and cooperative efforts were extracted, and the extent of interdepartmental coordination were quantified to assess the effectiveness of epidemic prevention and control efforts. Wilcoxon signed-rank test was adopted to statistically analyze the differences between the indicators before and after the epidemic. ResultsThe average overall coordination level for major epidemic prevention and control in 31 provinces (autonomous regions, and municipalities) increased from 43.06% to 97.62%, and the average coordination levels in the eastern, central, and western China soared from 42.29%, 37.50%, and 47.46%, to 98.81%, 96.20%, and 97.46%, respectively, with statistically significant differences (all P<0.05). In terms of department categorization, coordination levels in the professional departments and the key support departments peaked at 100.00%, while other support departments rose to 95.43%, with an increase of 77.15%, 181.85%, and 139.89%, respectively, exhibiting noteworthy statistically significant differences (all P<0.001). ConclusionThe scope of coordination departments of China’s major epidemic prevention and control exists a remarkable surge following the COVID-19 outbreak, notable heightened coordination is particularly observed among the key support departments. Future endeavors should prioritize the roles played by diverse departments in epidemic prevention and control, enhancing both the clarity of departmental responsibilities and the effectiveness of interdepartmental coordination. 
		                        		
		                        		
		                        		
		                        	
2.Characteristic volatile organic compounds in exhaled breath of coal workers' pneumoconiosis patients by thermal desorption gas chromatography-mass spectrometry

		                        			Background Coal workers' pneumoconiosis is a serious occupational disease in China. Exhaled volatile organic compounds (VOCs) can serve as the "breath fingerprint" of internal pathological processes, which provides a theoretical basis for exhaled VOCs to be used as potential non-invasive biomarkers for early diagnosis of coal workers' pneumoconiosis. Objective To screen out the characteristic VOCs and important characteristic VOCs of exhaled air in patients with coal workers' pneumoconiosis, and to explore the potential of these VOCs as biomarkers for early non-invasive diagnosis of the disease. Methods In this study, 27 VOCs in the exhaled breath of 22 patients with stage I coal workers' pneumoconiosis, 77 workers exposed to dust, and 92 healthy controls were quantitatively detected by thermal desorption gas chromatography-mass spectrometry (TD-GC-MS). Substances with P<0.05 in univariate analysis and variable importance projection (VIP) >1 in supervised orthogonal partial least squares discriminant analysis (OPLS-DA) model were selected as the characteristic VOCs for early diagnosis of coal workers' pneumoconiosis. Age was included in the LASSO regression model as a covariate to screen out important characteristic VOCs, and the diagnostic performance was evaluated by receiver operating characteristic (ROC) curve. Spearman correlation was further used to explore the correlation between important characteristic VOCs and clinical lung function indicators. Results Through univariate analysis and OPLS-DA modeling, 8 VOCs were selected, including 2-methylpentane, 3-methylpentane, n-hexane, methylcyclopentane, n-heptane, methylcyclohexane, 4-methyl-2-pentanone, and 2-hexanone, in exhaled breath of patients with coal workers' pneumoconiosis. The concentrations of 4 VOCs, including 3-methylpentane, n-hexane, 4-methyl-2-pentanone, and 2-hexanone, showed a decreasing trend with the increase of dust exposure years. By LASSO regression, the important characteristic VOCs of the coal workers' pneumoconiosis group and the dust exposure group were n-hexane, methylcyclohexane and 4-methyl-2-pentanone, and the important characteristic VOCs of the coal workers' pneumoconiosis group and the healthy group were 2-methyl-pentane and 4-methyl-2-pentanone. The ROC analysis showed that the area under the curve (AUC) of n-hexane, methylcyclohexane, and 4-methyl-2-pentanone were 0.969, 0.909, and 0.956, respectively, and the AUC of combined diagnosis was 0.988 and its Youden index was 0.961, suggesting that these results can serve as a valuable reference for further research on early diagnosis. The Correlation analysis found that there was a positive correlation between n-hexane and lung function indicators in the important characteristic VOCs, indicating that it could indirectly reflect the obstruction of lung function ventilation, further proving that important characteristic VOCs have the potential to monitor lung function decline. Conclusion Three important characteristic VOCs selected in this study have the potential to be used as non-invasive biomarkers for early diagnosis and disease monitoring of coal workers' pneumoconiosis, and are worthy of further study and verification.
		                        		
		                        		
		                        		
		                        	
3.Clinical analysis of pseudomonas aeruginosa injection in the treatment of postoperative chylous leakage in robotic thyroid cancer

		                        			Objective:To investigate the safety and efficacy of pseudomonas aeruginosa Injection (PAI) in the treatment of coeliac leakage after cervical lymph node dissection for robotic thyroid cancer.Methods:Retrospective analysis of 1262 patients who underwent robotic thyroid surgery at the 960th Hospital of the People’s Liberation Army from Jan. 2021 to Aug. 2023 was made. Postoperative celiac leakage happened in 28 patients. The control and injection groups were divided according to whether pseudomonas aeruginosa injection was used or not. In the control group, there were 4 males and 11 females out of 15 patients, with an average age of (46.20±9.02) years old, and the treatment methods of low-fat diet, negative pressure suction, and pressure bandage were used. In the injection group, there were 7 males and 6 females among 13 patients, with an average age of (41.00±8.87) years. They were treated with low-fat diet, negative pressure suction, pressure bandage, and PAI .The number of lymph node dissection, total drainage volume, peak drainage volume, days of hospitalization and the rate of decline in drainage volume within 24 h after the use of PAI in the injection group, post-injection temperature, number of injections, and post-injection extubation time were statistically analyzed in both groups. Independent samples t test, Mann-Whitney U test, χ2 test or Fisher’s exact test were used for comparison between groups. Results:There was no statistically significant difference in age, gender, extent of thyroidectomy, highest daily drainage volume, and total drainage volume between the two groups ( P>0.05). The difference in the number of lymph node dissection (49.15±23.05 vs. 30.80±11.76, P=0.012) and hospitalization time (11.77±4.64 vs. 16.40±6.42, P=0.041) between the injection group and the control group was statistically significant. After the use of pseudomonas aeruginosa injection, the draining fluid decreased from the previous day (69.56± 20.82) % in the injection group; Twelve patients were successfully extubated after one injection of pseudomonas aeruginosa injection, and one patient was successfully extubated after two injections; the mean time to extubation after injection was (3.85±1.28) days; 76.9% patients (10/13) had fever symptoms within 48 h with body temperature (38.05±0.89) ℃ after injection, body temperature returned to normal after symptomatic treatment,and no other adverse reactions occurred except fever. Conclusion:Pseudomonas aeruginosa injection is safe and effective in the treatment of celiac leakage after cervical lymph node dissection for robotic thyroid cancer, effectively reducing cervical drainage and shortening hospitalization days.
		                        		
		                        		
		                        		
		                        	
4.A multicenter clinical study on the incidence and influencing factors of cephalosporin-induced anaphylaxis

		                        			Objective:To investigate the incidence and influencing factors of allergic reactions to cephalosporins.Methods:A cross-sectional study of 29 medical institutions in Zhejiang Province was conducted from April 2021 to June 2021.The incidence of allergic reactions to cephalosporins was investigated,and the influencing factors of cephalosporin-induced allergic reactions were analyzed by Poisson regression.Results:A total of 56 155 patients were included in this study.The total incidence of allergic reactions to cephalosporin was 1.67‰,the highest incidence of anaphylaxis occurred for ceftizoxime(4.27‰),followed by ceftriaxone(3.49‰)and cefotaxime(2.40‰).There was no significant difference in the incidence of allergic reactions between patients with negative skin tests and those without skin tests(1.75%o vs.1.63‰,RR=1.07,95%CI:0.70-1.63,P>0.05).Poisson regression showed that body mass index(BMI)<18.5 kg/m2(RR=2.43,95%CI:1.23-4.82,P<0.05)and history of β-lactam antibiotics allergy(RR=33.88,95%CI:1.47-781.12,P<0.05)increased cephalosporin-induced anaphylaxis.Compared with cefuroxime,the risk of allergic reactions was increased for ceftriaxone(RR=3.08,95%CI:1.70-5.59,P<0.01),ceftazidime(RR=1.89,95%CI:1.03-3.47,P<0.05),and ceftizoxime(RR=3.74,95%CI:1.64-8.50,P<0.01).Conclusions:Lower BMI and history of β-lactam antibiotics allergy increase the risk of cephalosporin allergic reactions.The routine skin test may not reduce the occurrence of allergic reactions to cephalosporins. 5.Content determination of 6 components in Jinlian qingre granules by QAMS method based on a variety of internal reference substances

		                        			OBJECTIVE To establish a quantitative analysis of multi-components by single marker (QAMS) method based on a variety of internal reference substances for the content determination of 6 components in Jinlian qingre granules, such as mangiferin, 2″-O-β-L-galactopyranosylorientin, orientin, veratric acid, vitexin, harpagoside. METHODS The determination was performed on Agilent Eclipse Plus C18 column with mobile phase consisted of acetonitrile-0.1% phosphoric acid solution (gradient elution) at the flow rate of 1 mL/min. The column temperature was 30 ℃, and the detection wavelength was set at 270 nm. Taking orientin, vitexin and 2″-O-β-L-galactopyranosylorientin as internal references, the relative correction factors (RCF) of the other 5 components to be determined and internal substances were determined by QAMS. The contents of 6 components in 21 batches of Jinlian qingre granules were calculated and then compared with the results of the external standard method. RESULTS The contents of mangiferin, 2″-O-β-L-galactopyranosylorientin, orientin, veratric acid, vitexin and harpagoside in 21 batches of samples were determined by QAMS in the range of 0.234-0.516, 1.804-2.270, 2.143-2.606, 0.190-0.223, 0.594-0.782, 0.080-0.152 mg/g; the contents of them determined by external standard method were 0.235-0.523, 1.798-2.265, 2.137-2.599, 0.190-0.224, 0.597-0.786, 0.077-0.151 mg/g, respectively. The percentage difference between the results measured by the two methods should not exceed 4.00%. CONCLUSIONS QAMS has been constructed for the simultaneous determination of 6 components in Jinlian qingre granules based on a variety of internal reference substances. The results obtained by this method are not significantly different from those obtained by the external standard method, and can be used for the quality control of Jinlian qingre granules.
		                        		
		                        		
		                        		
		                        	
6.Targeting PFKL with penfluridol inhibits glycolysis and suppresses esophageal cancer tumorigenesis in an AMPK/FOXO3a/BIM-dependent manner.

		                        			As one of the hallmarks of cancer, metabolic reprogramming leads to cancer progression, and targeting glycolytic enzymes could be useful strategies for cancer therapy. By screening a small molecule library consisting of 1320 FDA-approved drugs, we found that penfluridol, an antipsychotic drug used to treat schizophrenia, could inhibit glycolysis and induce apoptosis in esophageal squamous cell carcinoma (ESCC). Gene profiling and Ingenuity Pathway Analysis suggested the important role of AMPK in action mechanism of penfluridol. By using drug affinity responsive target stability (DARTS) technology and proteomics, we identified phosphofructokinase, liver type (PFKL), a key enzyme in glycolysis, as a direct target of penfluridol. Penfluridol could not exhibit its anticancer property in PFKL-deficient cancer cells, illustrating that PFKL is essential for the bioactivity of penfluridol. High PFKL expression is correlated with advanced stages and poor survival of ESCC patients, and silencing of PFKL significantly suppressed tumor growth. Mechanistically, direct binding of penfluridol and PFKL inhibits glucose consumption, lactate and ATP production, leads to nuclear translocation of FOXO3a and subsequent transcriptional activation of BIM in an AMPK-dependent manner. Taken together, PFKL is a potential prognostic biomarker and therapeutic target in ESCC, and penfluridol may be a new therapeutic option for management of this lethal disease.
		                        		
		                        		
		                        		
		                        	
7.Analysis on pre-donation blood testing in blood services in Chongqing

		                        			【Objective】 To provide data reference for the implementation of the homogenization of pre-donation blood testing by investigating the relevant situation of pre-donation blood testing in various blood services in Chongqing and analyzing their differences. 【Methods】 A questionnaire covering the basic information of pre-donation blood testing items, quality control and the management of deferral donors was developed, and issued to 19 blood services in Chongqing through E-mails by Chongqing Society of Blood Transfusion. The data collected were sorted, revised and analyzed. 【Results】 A total of 19 questionnaires from 19 blood services(including 1 blood center, 1 sub-center, 6 central blood stations and 11 central blood banks) were collected. All of the pre-donation blood test items of 19 blood services met the Blood Donor Health Test Requirements. Hemoglobin, blood group, ALT and HBsAg testing were carried out by 19 blood services, anti-TP testing by 15, and lipid blood testing by 11, using different detection methods and reagents. Significant differences were found in the frequency and rules of internal quality control for quantitative testing items. In addition, the deferral time and re-recruitment strategy of deferral blood donors were also significantly different. 【Conclusion】 There were differences in the management of pre-donation blood testing and blood donor management after blood donation among blood services in Chongqing. 8.Studies on the correlation between FeNO and Pulmonary ventilation function in patients with mugwort pollen allergy

		                        			OBJECTIVE To invest igate the correlation between fractional exhaled nitric oxide(FeNO) and lung function in artemisia argyi allergy patients. Whether the course of disease and rhinitis score are correlated with FeNO and lung function. METHODS A total of 88 patients with mugwort pollen allergy, who visited the hospital between August and September 2017 and August and September 2018, the results of their FeNO and Pulmonary ventilation function, were included in this study. RESULTS Compared with patients with a duration of 9 years or more, FeNO and FEV1% of patients with a duration of 5 years were significantly different(P =0.004, P =0.032), and FeNO increased with the extension of the duration, while FEV1% decreased gradually. FeNO was negatively correlated with FEF25%-75%pred and FEV1%, while there was no correlation with FEV1%pred. Rhinitis score was positively correlated with FeNO. There was no correlation with FEV1%pred, FEV1%, FEF25%-75%pred. CONCLUSION With the development of mugwort pollen allergy, the probability of airflow limitation is also increased. To some extent, FeNO can predict the changes of airway function early, and can be used as a monitoring indicator for asthma prediction.FeNO value can reflect the degree of inflammation of the body, and can be used in the auxiliary diagnosis of the severity of allergic rhinitis patients.
		                        		
		                        		
		                        		
		                        	
9.Internal fixation with proximal femoral nail anti-rotation for intertrochanteric fracture involving lateral wall: a finite element analysis

		                        			Objective To study the biomechanical performance of proximal femoral nailing antirotation (PFNA) in the treatment of femoral intertrochanteric fracture involving the lateral wall using finite element analysis and the significance of the lateral wall.Methods A healthy senior volunteer,male,80 years of age,was recruited for this study.The CT data of his proximal femur were used to establish a three-dimensional finite element model of proximal femur by software Mimics17.0 and Geomagic Stusio.After the intertrochanteric fracture of AO-type 31-A1.2 was simulated together with coronal displacement of the lateral wall fracture,it was assembled with PFNA into the three-dimensional finite element model.Finite element analysis was performed on the femoral head,neck,fracture ends,lateral wall,and internal components of the model to observe the stress values of various parts of the model under the same load,with different degrees of fracture reduction (no displacement,anterior displacement of 5%,10%,15% and 20%,posterior displacement of 5%,10%,15% and 20%) and with or without the lateral wall.Results The stress values for internal components at the femoral head,neck,fracture ends,main nail with anatomical reduction of fracture ends and integrity of the lateral wall were 0.40 MPa,30.05 MPa,74.35 MPa and 121.68 MPa,respectively.The respective stress values of the above with lateral wall fracture were 0.82 MPa,47.32 MPa,151.92 MPa and 266.88 MPa,increased by about 100%.With every horizontal reduction loss increased by 5% forward or backward,the stress exerting on the corresponding spots would see a 10% progressive growth,plus 20% lateral wall fracture and reduction loss.The stress of the main nail (468.43 MPa) was thrice that (121.68 MPa) of the stress posing on the complete lateral wall with the fracture ends 100% reduced.Conclusions When PFNA is used in the treatment of femoral intertrochanteric fractures involving the lateral wall,stresses on the internal components may be doubled.Loss of coronal reduction may lead to the most significant increase in stress on the bonding site between the main nail and screw blade.
		                        		
		                        		
		                        		
		                        	
10.The differential diagnosis value of high frequency ultrasound in dorsal thyroid nodules

		                        			Objective To explore the value of high frequency ultrasound in differential diagnosis in dorsal thyroid (including gland dorsal or posterior) nodules.Methods Ultrasonographic features of 101 patients with ultrasonography suspected or misdiagnosed nodules in the dorsal area of the thyroid gland,which were then confirmed by pathology or hyaluronography/gastroscopy,were retrospectively analyzed.Ultrasonographic homogeneity (other nodules with similar ultrasonographic features were found in the ipsilateral gland),parenchyma homology (nodule parenchyma was continuous with glandular parenchyma) and blood homology (blood flow signals in nodules were continuous with those in glandular parenchyma) were applied as indicators for determinng thyroid nodules.Double-line sign or vascular arch sign at the edge of nodules served as an indicator for determining parathyroid nodules.Results Of the 101 cases,there were 46 thyroid nodules,35 parathyroid nodules,7 enlarged lymph nodes;6 esophageal diverticulum,6 esophageal cancer,1 cleft cyst.The determination of thyroid nodules by at least one of ultrasonographic homogeneity,parenchyma homology and blood homology showed sensitivity of 73.5 %,specificity of 96.5 % and accuracy of 85.8%.In the determination of parathyroid nodules by double-line sign or vascular arch sign at the edge of nodules,sensitivity,specificity and accuracy was 54.1%,85.5 % and 74.5 %,respectively.Conclusions Nodules in the dorsal area of the thyroid gland have different sources,and the understanding of corresponding characteristic ultrasonographic appearances is helpful for improving the differential diagnosis of these nodules.
