1.Finite element model establishment and stress analysis of lumbar-sacral intervertebral disc in ankylosing spondylitis

BACKGROUND:Ankylosing spondylitis is a chronic inflammatory disease with chronic rheumatic immunity.Soft tissue ossification and fusion and spinal stiffness can cause biomechanical changes. OBJECTIVE:To reconstruct the lumbar-sacral intervertebral disc in ankylosing spondylitis patients with lumbar kyphosis by finite element analysis,and to study the range of motion of each segment of T11-S1 and the biomechanical characteristics of annulus fibrosus and nucleus pulposus. METHODS:The imaging data were obtained from an ankylosing spondylitis patient with lumbar kyphosis.The original CT image data of continuously scanned spine were imported into Mimics 21.0 in DICOM format,and T11-S1 was reconstructed respectively.The established model was imported into 3-Matic software in the format of"Stl"to reconstruct the intervertebral disc,and the fibrous intervertebral disc model was obtained.The improved model was further imported into Hypermesh software,and the vertebra,nucleus pulposus,annulus fibrosus and ligament were mesh-divided.After the material properties were given,the model was imported into ABAQUS software to observe the range of motion of each vertebral body in seven different working conditions of T11-S1,and analyze the biomechanical characteristics of each segment of annulus fibrosus and nucleus pulposus. RESULTS AND CONCLUSION:(1)The range of motion of L1 vertebrae was higher than that of other vertebrae under six different working conditions:extension,forward flexion,rotation(left and right),and lateral flexion(left and right).The maximum range of motion was 2.18° during L1 vertebral flexion,and the minimum range of motion was 0.12° during L5 vertebral extension.(2)The annular fiber flexion at L2-L3 segments was greater than the extension(P<0.05),and the annular fiber flexion at L3-L4 and L4-L5 segments was less than the extension(P<0.05).The left rotation of L1-L2 annular fibers was greater than the right rotation(P<0.05).The left flexion of the annulus was greater than the right flexion in L1-L2,L2-L3,L3-L4,L4-L5 and L5-S1 segments(P<0.05).(3)The nucleus pulposus stresses of T11-L12,L1-L2,L2-L3,L3-L4 and L4-L5 segments in forward flexion were greater than in extension(P<0.05).The left rotation of T12-L1 and L3-L4 segments was smaller than the right rotation(P<0.05),and that of T11-T12,L1-L2,and L2-L3 segments was larger than the right rotation(P<0.05).The left flexion was larger than the right flexion in the T11-S1 segment.(4)It is concluded that in ankylosing spondylitis patients with lumbar kyphosis,the minimum range of motion of the vertebral body is located at the L5 vertebral body in extension.To prevent fractures,it is recommended to avoid exercise in the extension position.During the onset of lumbar kyphosis in patients with ankylosing spondylitis,the maximum stress of the annulus fibrosus and nucleus pulposus is located in the L1-L2 segment,which is fixed and will not alter with the change of body position.The late surgical treatment and correction of deformity should focus on releasing the pressure of the annulus fibrosus and nucleus pulposus in this segment to avoid the rupture of the annulus fibrosus and the injury of the nucleus pulposus.

Objective To evaluate the relationship between diabetic nephropathy(DN)and diabetic retinopathy(DR)in patients with type 2 diabetes mellitus(T2DM)based on imaging and clinical testing data.Methods Totally 600 T2DM patients who visited the First People's Hospital of Ziyang from March 2021 to December 2022 were included.The fundus photography and fundus fluorescein angiography were performed on all these patients and their age,gender,T2DM duration,cardiovascular diseases,cerebrovascular disease,hypertension,smoking history,drinking history,body mass in-dex,systolic blood pressure,diastolic blood pressure and other clinical data were collected.The levels of fasting blood glu-cose(FPG),triglyceride(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),low-density lipo-protein cholesterol(LDL-C),glycosylated hemoglobin(HbA1c),24 h urinary albumin(UAlb),urinary albumin to creati-nine ratio(ACR),serum creatinine(Scr)and blood urea nitrogen(BUN)were measured.Logistic regression was used to analyze the risk factors associated with DR.DR staging was performed according to fundus images,and the convolutional neural network(CNN)algorithm was used as an image analysis method to explore the correlation between DR and DN based on emission computed tomography(ECT)and clinical testing data.Results The average lesion area rates of DR and DN detected by the CNN in the non-DR,mild-non-proliferative DR(NPDR),moderate-NPDR,severe-NPDR and pro-liferative DR(PDR)groups were higher than those obtained by the traditional algorithm(TCM).As DR worsened,the Scr,BUN,24 h UAlb and ACR gradually increased.Besides,the incidence of DN in the non-DR,mild-NPDR,moderate-NPDR,severe-NPDR and PDR groups was 1.67%,8.83%,16.16%,22.16%and 30.83%,respectively.Logistic regression analysis showed that the duration of T2DM,smoking history,HbA1c,TC,TG,HDL-C,LDL-C,24 h UAlb,Scr,BUN,ACR and glomerular filtration rate(GFR)were independent risk factors for DR.Renal dynamic ECT analysis demonstrated that with the aggravation of DR,renal blood flow perfusion gradually decreased,resulting in diminished renal filtration.Conclusion The application of CCN in the early stage DR and DN image analysis of T2DM patients will improve the diag-nosis accuracy of DR and DN lesion area.The DN is worsening as the aggravation of DR.
3.Correlation of the emm genotyping and virulence genes with the isolation sites of Group A Streptococcus strains from children with impetigo

Objective:To investigate the correlation of the emm genotypes and virulence genes with the isolation sites of Group A Streptococcus (GAS). Methods:It was a retrospective study.The specimens were collected from children with impetigo in Beijing Children′s Hospital, Capital Medical University from 2006 to 2008 for GAS isolation and identification.A total of 24 GAS strains were isolated from 16 children with impetigo, among which 7 pairs of strains were isolated from the throat and skin of 7 children, and 1 pair of strains was isolated from the vulva and skin of one child, and the remaining 8 GAS strains were isolated from the skin pus samples of 8 children.Polymerase chain reaction was applied to detect the emm genotypes and 13 virulence genes ( speA, speB, speC, speF, speG, speH, speI, speJ, speK, speL, speM, smeZ and ssa). The correlation of the emm genotypes and virulence genes with the isolation sites of GAS strains was analyzed. Results:In this study, four emm genotypes were detected, including emm1.0 (15/24), emm12.0 (4/24), emm22.0 (2/24) and emm160.0 (1/24), and one subtype emm12.19 (2/24) was detected as well.The carrying rates of 13 virulence genes speA, speB, speC, speF, speG, speH, speI, speJ, speK, speL, speM, smeZ and ssa were 58.3%, 100%, 91.7%, 100%, 50.0%, 12.5%, 54.2%, 66.7%, 16.7%, 25.0%, 12.5%, 100% and 91.7%, respectively.All strains carried 5 to 11 virulence genes and they all carried speB, speF and smeZ.There were significant differences in the carrying rate of speA and speJ among the strains with different emm genotypes (all P<0.05). There was no significant difference in the distribution of virulence genes between skin isolates and pharyngeal isolates, including the 5 pairs of strains carrying the emm1.0 genotype (all P>0.05). Conclusions:The distribution of virulence gene of GAS in children with impetigo is significantly correlated with the emm genotype, rather than the isolation site.
4.Visualization analysis on research literature about TCM formula granules based on CiteSpace

Objective:To analyze the research hotspots and development trends of TCM formula granules.Methods:Literature about TCM formula granules was retrieved from CNKI from the establishment of the database to August 31, 2022. CiteSpace 5.7.R5 software was used to visually analyze the information such as literature volume, journals, research authors, research institutions, and keywords, and to draw the knowledge map.Results:Totally 1 135 articles were finally included. The number of articles published on TCM formula granules research showed a year-on-year increase. The main journals were the Chinese Journal of Experimental Traditional Medical Formulation and the Asia-Pacific Traditional Medicine. The research teams represented by Sun Dongmei and Li Junshan and the research institutions of Guangdong Yifang Pharmaceutical Co.,Ltd. and Shenwei Pharmaceutical Group Co., Ltd. were formed. The research contents mainly covered characteristic chromatogram, content determination, quality standards, standard decoction, extraction processes and so on. Conclusions:The determination of the active components of TCM formula granules, their consistency with the chemical composition of tonics and their equivalence with clinical efficacy remain hotspots for future research. At present, the research on TCM formula granules is at a developmental stage and should be further developed and cooperation between institutions and authors should be strengthened.
5.Introduction to Guidelines for public health protection against high temperature and heatwaves

In recent years, the high temperature and heatwaves have seriously affected the health of Chinese residents, and there is an important need for public health protection guidelines for high temperature and heatwaves in China. The National Bureau of Disease Control and Prevention has organized experts to fully investigate the evidence from epidemiological research on the health of populations in high temperature and heatwaves globally and in China, analyze the health hazards and protection needs of different populations, and put forward practical and effective individual protection measures and health recommendations. For this reason, the "Guideline for Public Health Protection against High Temperature and Heatwaves" (referred to as the "Guideline") was officially issued in June 2023. This article interprets the background and significance of the Guideline, the principles of compilation, the main considerations, the main contents, the implementations and promotions and other aspects, to improve the understanding of the content of the Guideline and strengthen the publicity and implementations.
6.Clinical and molecular genetic analysis of a child with comorbid 16p11.2 microdeletion syndrome and Rett syndrome

Objective:To explore the genetic characteristics of a child with comorbid 16p11.2 microdeletion syndrome and Rett syndrome (RTT).Methods:A male infant who was admitted to Gansu Provincial Maternity and Child Health Care Hospital in May 2020 was selected as the study subject. Clinical data of the infant was collected. Genomic DNA was extracted from peripheral blood samples from the infant and his parents, and subjected to whole exome sequencing (WES). Candidate variant was verified by Sanger sequencing.Result:The patient, a 4-day-old male infant, had presented with poor response, poor intake, feeding difficulties, and deceased at 8 months after birth. WES revealed that he has harbored a 0.643 Mb deletion in the 16p11.2 region, which encompassed key genes of the 16p11.2 microdeletion syndrome such as ALDOA, CORO1A, KIFF22, PRRT2 and TBX6. His father has carried the same deletion, but was phenotypically normal. The deletion was predicted to be pathogenic. The child was also found to harbor a maternally derived c. 763C>T (p.R255X) hemizygous variant of the MECP2 gene, which was also predicted to be pathogenic (PVS1+ PS4+ PM2_Supporting). Conclusion:The 16p11.2 deletion and the MECP2: c.763C>T (p.R255X) variant probably underlay the pathogenesis in this infant.

8.Based on network pharmacology to explore mechanism of Dabuyuanjian against AD and verification of AMPK/SIRT1 signaling pathway

Objective:To explore the mechanism of Dabuyuanjian in Against alzheimer's disease(AD)through network phar-macology and molecular docking technology,and to verify the molecular mechanism discovered by animal experiments.Methods:Net-work pharmacology was used to analyze the active ingredients and targets of AD in the treatment of large supplementary yuan decoc-tion.The core components of the drug were verified by molecular docking with the core protein by using AutoDock and PyMOL soft-ware.AD model mice were treated with Dabuyuanjian,and the core pathways which discovered were verified.Results:A total of 80 active ingredients and 107 disease targets were screened out.Dabuyuanjian had 95 targets in the treatment of AD,of which 35 were core targets.GO enrichment found that it mainly involved in programmed cell death process,apoptosis process and signal transduction regulation,etc.KEGG signaling pathway enrichment found that it mainly involved PI3K/Akt signaling pathway,Wnt signaling pathway,AMPK signaling pathway,etc.Morris water maze experiment showed that Dabuyuanjian could reduce the escape latency of AD mice,and increase the number of crossing platform and time's target quadrant.Immunohistochemistry(IHC)showed that Dabuyuanjian could increase the number of positive labeled-NeuN cells in the hippocampal CA3 region of AD mice.Immunofluores-cence(IF)showed that Dabuyuanjian could inhibit the expression levels of(GFAP)and ionized calcium-binding protein 1(IBA1)in the hippocampal CA3 region of AD mice.Western blot experiments showed that Dabuyuanjian could increase the expression levels of phosphorylated adenylate-activated protein kinase α(AMPKα)and silent information regulator 1(SIRT1)in the hippocampus of AD mice.Conclusion:This study explores the mechanism of Dabuyuanjian against AD,and find that Dabuyuanjian can improve cognitive impairment,neuron loss and neuroinflammation via activating AMPK/SIRT1 signaling pathway of AD.
9.Study on Tongue Image and Formation of Mucus Plug in Airway of Mycoplasma Pneumoniae Pneumonia in Children

Objective To investigate the role of tongue diagnosis in the evaluation of airway mucus plug formation in mycoplasma pneumoniae pneumonia(MPP).Methods Totally 418 MPP children with MPP were included in this study.According to the formation of mucus plug under the microscope,the children with MPP were divided into mucus plug group(n=108)and non-mucus plug group(n=310).The purpose of this study was to compare the tongue features(tongue color,coating color,coating quality,tongue fluid)and levels of related inflammatory factors C-reactive protein(CRP),lactate dehydrogenase(LDH),interleukin(IL)-6,etc between the two groups,and to explore the role of tongue examination in evaluating the formation of mucus plugs in MPP.Results The tongue color of MPP mucus plug group was mainly crimson tongue,the coating color was mainly yellow fur,the coating quality was mainly thick greasy coating,and the tongue fluid was mainly fluid.The tongue image score of MPP mucus plug group was significantly higher than that of non-mucus plug group(P<0.05).Multivariate logistic regression analysis showed that CRP,IL-6,IL-17,γ interferon(IFN-γ),LDH and tongue total score were independent risk factors for mucus plug formation in MPP.The results of receiver operating characteristic(ROC)curve showed that the area under the curve(AUC)of CRP,tongue image score,LDH,IL-6,IL-17 and IFN-γ to predict the formation of mucus plug in MPP were 0.893,0.830,0.783,0.794,0.627 and 0.602,respectively.The optimal cut-off values were 37.445(mg/L),4.5(pionts),358.673(U/L),27.174(pg/mL),4.855(pg/mL)and 26.427(pg/mL),respectively.The AUC of MPP mucous plug formation was 0.930,the sensitivity was 0.907,and the specificity was 0.827.Conclusion There are differences in tongue image characteristics in MPP children with mucus plug formation.Total tongue image score can better predict mucus plug formation in MPP,and combined analysis has greater value in predicting MPP mucus plug formation.
10.Preliminary application of virtual reality for pain management in patients undergoing peritoneal dialysis-related procedures

Objective:To investigate the application of virtual reality (VR) technology on intraoperative pain in patients undergoing peritoneal dialysis (PD)-related procedures with local infiltration anesthesia and the satisfaction.Methods:It was a single-center, prospective, concurrent controlled study. Patients were divided into two groups: VR group and control group. In the VR group, patients wore a VR headset to watch soothing audio and video content during surgery, while the control group underwent routine procedures. Intraoperative pain and satisfaction were assessed using the visual analog scale (VAS) and a 5-point satisfaction scale within 30 minutes of surgery. In addition, tolerance of the VR experience in the VR group was assessed using the VR sickness questionnaire.Results:A total of 43 patients were included in the study, including 25 males (58.1%). Chronic glomerulonephritis [17 cases (39.5%)] and diabetic nephropathy [6 cases (14.0%)] were the main primary diseases. There were 23 cases in the control group and 20 cases in the VR group. There were no significant differences between the two groups in age, sex ratio, proportion of primary disease, diabetes, hypertension, distribution of operation methods, preoperative vital signs and operation time (all P>0.05). VAS pain score was significantly lower in the VR group than that in the control group (5.90±2.38 vs. 7.43±1.67, t=2.469, P=0.018). The percentage of patients who were satisfied was 89.5% (17/19) in the VR group and 78.3% (18/23) in the control group, but there was no significant difference (chi-square test for continuity correction, χ2=0.308, P=0.579). Three patients in the VR group withdrew from the study due to severe discomfort, while the remaining participants found the VR experience to be tolerable. Common adverse effects included fatigue and blurred vision. Conclusions:The application of VR technology in PD-related procedures has been effective in reducing intraoperative pain when combined with local infiltration anesthesia. Furthermore, the utilization of VR technology in PD-related procedures is associated with a safe and tolerable outcome, despite the observation of some adverse effects.
