1.A Retrospective Study of the Effect of Spinopelvic Parameters on Fatty Infiltration in Paraspinal Muscles in Patients With Lumbar Spondylolisthesis

Objective:
The effect on fat infiltration (FI) of paraspinal muscles in degenerative lumbar spinal diseases has been demonstrated except for spinopelvic parameters. The present study is to identify the effect of spinopelvic parameters on FI of paraspinal muscle (PSM) and psoas major muscle (PMM) in patients with degenerative lumbar spondylolisthesis.
Methods:
A single-center, retrospective cross-sectional study of 160 patients with degenerative lumbar spondylolisthesis (DLS) and lumbar stenosis (LSS) who had lateral full-spine x-ray and lumbar spine magnetic resonance imaging was conducted. PSM and PMM FIs were defined as the ratio of fat to its muscle cross-sectional area. The FIs were compared among patients with different pelvic tilt (PT) and pelvic incidence (PI), respectively.
Results:
The PSM FI correlated significantly with pelvic parameters in DLS patients, but not in LSS patients. The PSM FI in pelvic retroversion (PT > 25°) was 0.54 ± 0.13, which was significantly higher in DLS patients than in normal pelvis (0.41 ± 0.14) and pelvic anteversion (PT < 5°) (0.34 ± 0.12). The PSM FI of DLS patients with large PI ( > 60°) was 0.50 ± 0.13, which was higher than those with small ( < 45°) and normal PI (0.37 ± 0.11 and 0.36 ± 0.13). However, the PSM FI of LSS patients didn’t change significantly with PT or PI. Moreover, the PMM FI was about 0.10–0.15, which was significantly lower than the PSM FI, and changed with PT and PI in a similar way of PSM FI with much less in magnitude.
Conclusion
FI of the PSMs increased with greater pelvic retroversion or larger pelvic incidence in DLS patients, but not in LSS patients.

3.Observation and imaging analysis of signs of ankylosing spondylitis in spinal specimens

Objective To provide anatomical,radiological,and clinical diagnostic and therapeutic references for ankylosing spondylitis and spinal surgical operations.Methods Non-measurement spinal observations,X-ray examinations,and measurements were performed on two spinal specimens,along with digital image acquisition and processing.Results The first specimen included thoracic vertebra 7(T7)to lumbar vertebra 3(L3),with an average total length of 29.7 cm;the second specimen ranged from cervical vertebra 7(C7)to lumbar vertebra 2(L2),with an average total length of 38.3 cm.The specimens showed partial or complete calcification of ligaments,ossification of the small joints and intervertebral discs,and osteoporosis;The anterior-posterior diameter(width)of the vertebral foramen was narrower than that of a normal adult,while most of the superior-inferior diameter(height)was wider.Radiographically,the anterior longitudinal ligament calcification appeared as dot-like or striated,but it was actually flaky in the actual specimens.The specimens provided views of the facet joints,costovertebral joints,and intervertebral foramina that was difficult to demonstrate on two-dimensional X-ray images.Conclusion As ankylosing spondylitis progresses,the range of motion in spinal bending and rotation decreases,as does the extent of thoracic expansion,thereby affecting respiration and complicating procedures such as intraspinal anesthesia and sacral canal injections.In terms of diagnosis,bone specimens and X-ray films allow us to understand the development process and severity of ankylosing spondylitis more directly and accurately.
4.The importance,feasibility,and ideas of construction of a vertical ethical review committee for the Center for Disease Control in Shanghai

The construction of a vertical ethical review committee for the Center for Disease Control(CDC)in Shanghai will be an important exploration of public health ethics in the field of disease prevention and control,which will provide a practical field for strengthening the public health ethics norms and management of the disease control system and enhancing the awareness of public health ethics of practitioners,as well as plays a positive role in promoting modern public health governance in CDC.The paper demonstrated the importance of construction from three aspects,including the concept,disciplinary scope,and governance practice of public health,namely,the vertical ethics review committee is a tangible means and tool for public health governance to follow the constraints and norms of public health ethics,and is a constraining carrier and concentrated embodiment of the value of population health orientation of the practicing public health ethics.Subsequently,it clarified the feasibility of the social environment of the close relationship between the CDC providing the construction of vertical ethics review committees,as well as the feasibility of the homogeneity characteristics of practice and scientific research in the CDC promoting the work scenarios and fields.Furthermore,from the perspective of management,the construction principles such as system principles,humanistic principles,responsibility principles,and efficiency principles were substantiated.The construction framework was proposed in terms of construction objectives,subjects,objects,and norms,to provide an innovative practice for the conception of the construction of regional public health ethics committees.

Objective To observe the efficacy and safety of Morinda officinalis oligosaccharides in the continuation treatment of mild and moderate depression.Methods An open,single-arm,multi-center design was adopted in our study.Adult patients with mild and moderate depression who had received acute treatment of Morinda officinalis oligosaccharides were enrolled and continue to receive Morinda officinalis oligosaccharides capsules for 24 weeks,the dose remained unchanged during continuation treatment.The remission rate,recurrence rate,recurrence time,and the change from baseline to endpoint of Hamilton Depression Scale(HAMD),Hamilton Anxiety Scale(HAMA),Clinical Global Impression-Severity(CGI-S)and Arizona Sexual Experience Scale(ASEX)were evaluated.The incidence of treatment-related adverse events was reported.Results The scores of HAMD-17 at baseline and after treatment were 6.60±1.87 and 5.85±4.18,scores of HAMA were 6.36±3.02 and 4.93±3.09,scores of CGI-S were 1.49±0.56 and 1.29±0.81,scores of ASEX were 15.92±4.72 and 15.57±5.26,with significant difference(P<0.05).After continuation treatment,the remission rate was 54.59%(202 cases/370 cases),and the recurrence rate was 6.49%(24 cases/370 cases),the recurrence time was(64.67±42.47)days.The incidence of treatment-related adverse events was 15.35%(64 cases/417 cases).Conclusion Morinda officinalis oligosaccharides capsules can be effectively used for the continuation treatment of mild and moderate depression,and are well tolerated and safe.
6.Establishment of a predictive scoring model and preventive measures for bladder neck contracture after laparoscopic enucleation of the prostate with urethra preservation

Objective:To establish a predictive scoring model for bladder neck contracture(BNC)after laparoscopic enuclea-tion of the prostate with preservation of the urethra(Madigan surgery)and explore the preventive measures against this postoperative complication.Methods:We included 362 cases of BPH treated by laparoscopic Madigan surgery from January 2019 to March 2022(45 with and 317 without postoperative BNC)in the training group and another 120 cases treated the same way in the verification group,collected the clinical data on the patients and evaluated the results of surgery.Using the least absolute shrinkage and selection operator(LASSO)and multivariate logistic regression,we analyzed the risk factors for postoperative BNC and constructed a predictive scoring model for evaluation of the factors.Results:Compared with the baseline,the IPSS,quality of life(QOL)score and postvoid residual urine volume(PVR)were significantly decreased(P<0.05)while the maximum urinary flow rate(Qmax)remarkably in-creased(P<0.05)in the BPH patients at 3 months after surgery.Eight non-zero characteristic predictors were identified by LASSO regression analysis.Multivariate logistic regression analysis showed that short clinical experience of the surgeon,concurrent prostatitis,bladder rinse solution temperature<34℃,catheter blockage,urethral balloon injection volume>40 ml and postoperative constipation were independent risk factors for postoperative BNC(P<0.05).The best cut-off value was 2.36 points in both the training and the verification groups.The results of evaluation exhibited a high discriminability of the predictive scoring model.Conclusion:Laparo-scopic Madigan surgery is a safe and effective method for the treatment of BPH.Short clinical experience of the surgeon,concurrent prostatitis,bladder rinse solution temperature<34℃,catheter blockage,water injected into the urethral balloon>40 ml and postop-erative constipation were independent risk factors for postoperative BNC.The predictive scoring model constructed in this study has a good discriminability and is simple and feasible,contributive to the prediction of postoperative BNC in BPH patients undergoing laparo-scopic Madigan surgery.
7.Effects of hesperidin on gentamicin-induced acute kidney injury,inflammation and apoptosis in mice

AIM To observe the ameliorative effects of hesperidin on gentamicin-induced acute kidney injury(AKI)in mice,and to explore the correlation between the effects and inflammation and apoptosis.METHODS The mice were randomly divided into the normal group,the model group,the verapamil group(21.43 mg/kg)and the low and high dose hesperidin groups(100,200 mg/kg).With the AKI model established by intraperitoneal injection of gentamicin,the mice of each group underwent 14 days corresponding drug administration.After the intervention,the mice had their levels of 24-hour urine protein,renal index and renal function indexes detected;their levels of IL-10,IL-6 and TNF-α in serum and kidney tissue detected by ELISA;their renal pathological damage detected by HE staining;their renal apoptosis detected by TUNEL staining;their renal mRNA expressions of caspase-3,Bcl-2 and Bax detected by RT-qPCR;and their renal expressions of cleved caspase-3 and PI3K/Akt signaling pathway related proteins detected by Western blot.RESULTS Compared with the model group,the hesperidin groups displayed decreased levels of 24-hour urinary protein and renal index(P<0.05);improved renal pathological damage in varying degrees;decreased levels of serum BUN,Scr and CysC(P<0.05);increased IL-10 levels of serum and renal tissue(P<0.05);decreased levels of IL-6 and TNF-α(P<0.05);decreased renal apoptosis rate,caspase-3,Bax mRNA expressions and cleaved caspase-3 protein expression(P<0.05);and increased expressions of Bcl-2 mRNA and PI3K/Akt proteins(P<0.05).CONCLUSION Hesperidin can ameliorate gentamicin-induced AKI in mice,which is related to its anti-inflammatory efficacy and anti-apoptotic activity via PI3K/Akt signaling pathway.
