1.Exploring the pathogenesis and treatment of cancer based on"body coldness and tumor heat"

Cancer is a prevalent and challenging disease that is difficult to treat.Western medicine recognizes the disruption of the immune and inflammatory microenvironment as a crucial factor in the development and progression of cancer.According to traditional Chinese medicine,cancer falls into the categories of"accumulation"and"abnormal masses".Based on the attributes of cold and heat in the overall and local conditions as well as the characteristic of the struggle between the vital qi and pathogenic factors at different stages of this disease,this article proposes the concept of"body coldness and tumor heat"to describe the pathogenesis of cancer formation and progression.This pathogenesis aligns with the disruption of the immune and inflammatory microenvironment in modern medical oncology.The article suggests a treatment approach that focuses on balancing cold and heat,nourishing and protecting yang qi,warming the kidneys,and strengthening the spleen to address the"coldness of the entire body".This approach also involves promoting qi circulation,eliminating dampness,phlegm,and stasis,and detoxifying and dispersing nodules to address the"heat in the tumor locally".By addressing deficiencies,eliminating pathogenic factors,and promoting circulation to alleviate stagnation,the aim is to restore the balance of yin and yang and improve the complex state of"coldness of the entire body"and"heat in the tumor locally".These interventions can ameliorate the disorder in the microenvironment and enhance clinical efficacy.
2.A study on communication mechanism of lung cancer cells in tumor microenvironment mediated by pleckstrin-2/miR-196a signal axis

Background and purpose:It is still a great challenge to clarify the signal molecules that mediate the communication between cancer-associated fibroblasts(CAFs)and tumor cells.These signal molecules are very important for cancer metastasis.The purpose of this study was to explore the communication mechanism of pleckstrin-2/miR-196a signal axis mediated by lung cancer cells in tumor microenvironment.Methods:Human lung adenocarcinoma cell line H1299 and human embryonic lung cell MRC-5 were selected as the research objects.H1299 cells were transfected with lentivirus(PLEK2)expressing PLEK2 and Vector control,and exosomes(Vector_exo,PLEK2_exo)were isolated after 24 h of transfection.MRC-5 cells were transfected with miR-196a mimetic or inhibitor.The expressions of PLEK2 and epithelial-mesenchymal transition(EMT)-related proteins were analyzed by Western blot.The expression of miR-196a was analyzed by polymerase chain reaction(PCR),and the metastasis and invasion ability of cells were determined by transwell assay.Six female BALB/c-nu mice were randomly divided into Vector group and PLEK2 group,with 3 mice in each group.Mice in each group were injected with H1299 cells transfected with Vector or PLEK2 through the tail vein.After 4 weeks,lung tissue was taken out for H-E staining and immunohistochemical staining to analyze the expression of α-smooth muscle actin(α-SMA).All animal experiments were approved by the ethics committee of First Hospital of Changsha City(Changsha Hospital,Xiangya School of Medicine,Central South University)(ethics number:EI-2021-103).Results:Compared with the Vector group,the number of pulmonary metastatic nodules and the expression of α-SMA in metastatic cancer in PLEK2 group increased significantly(P<0.001).Compared with Vector group,the expression level of miR-196a in H1229 cells in PLEK2 group increased significantly(P<0.05),and the expression level of miR-196a was significantly higher in PLEK2_exo than in Vector_exo(P<0.05).Compared with Vector_exo group,the expression levels of miR-196a,α-SMA and fibroblast activation protein(FAP)in MRC-5 cells in PLEK2_exo group increased significantly(P<0.05).Compared with the negative control(NC),the expression levels of α-SMA and FAP in MRC-5 cells transfected with miR-196a increased significantly(P<0.05).On the contrary,by transfection with miR-196a inhibitors(si-miR-196a#1 and si-miR-196a#),the expression levels of α-SMA and FAP were significantly inhibited(P<0.05).Compared with NC-CM group,the number of metastatic cells,invasive cells and the expression of vimentin in miR-196a-CM group increased significantly(P<0.001),and the expression of E-cadherin decreased significantly(P<0.001).In addition,compared with Vector_exo-CM group,PLEK2_exo-CM group had significant increase in number of metastatic and invasive cells and the expression of vimentin(P<0.01),and significant decrease in the expression of E-cadherin(P<0.001).Conclusion:Upregulation of PLEK2 can enhance the level of exosomes miR-196a derived from lung cancer cells,thereby promoting the activation of CAFs.The activated CAFs can further enhance the invasive ability of lung cancer cells.
3.A pedigree of pseudohypoaldosteronism type Ⅱ and review of literature

The clinical data, laboratory test, and gene mutations were collected from a family with pseudohypoaldosteronism type II(PHA2). The proband, aged 1 year and 7 months, presented with hyperkalemia(6.69 mmol/L; reference range 3.5-5.3 mmol/L), blood pressure of 110/68 mmHg(normal<106/61 mmHg, 1 mmHg=0.133 kPa), blood chloride of 111.5 mmol/L(reference 99-110 mmol/L), blood HCO 3- of 17.1 mmol/L(reference 22-29 mmol/L), estimated glomerular filtration rate of 128.5 mL·min -1·(1.73 m 2) -1[>90 mL·min -1·(1.73 m 2) -1], and blood renin concentration of 0.30 μIU/mL(reference 4.2-45.6 μIU/mL). The mother and maternal grandfather also exhibited normal renal function with hyperkalemia, hypertension, hyperchloremia, metabolic acidosis, and low renin. Genetic testing revealed a heterozygous missense mutation(c.1685A>G, p. E562G) in exon 7 of the no-lysine kinase 4(WNK4) gene. Treatment with hydrochlorothiazide was effective. Literature review comparing this E562G pedigree with other WNK4 variants suggested clinical heterogeneity of WNK4 mutations. For unexplained hyperkalemia, especially with concurrent hypertension, PHA2 should be considered early for genetic screening to prevent misdiagnosis or delayed diagnosis.
4.Status of medication deviation in hospital-home transition period in elderly patients with chronic heart failure and its influencing factors

Objective To analyze the current situation and influencing factors of medication deviation in hospital-home transition period in elderly patients with chronic heart failure (CHF). Methods A total of 178 elderly patients with chronic heart failure were selected as the study objects. General information of patients was collected. The 8-item Morisky Medication Adherence Scale (MMAS-8), Self-Efficacy for Appropriate Medication Use Scale (SEAMS) and family APGAR index questionnaire were analyzed. The occurrence of medication deviation was recorded one week after discharge. The influencing factors of medication deviation in elderly patients with CHF were analyzed. Results Fifty patients (28.09 %) had at least one medication deviation within one week after discharge. A total of 108 cases of medication deviation occurred in 178 patients. The main type of medication deviation was patient-derived causes. 5.Epidemiological characteristics of foodborne disease outbreaks in Shaoxing City from 2012 to 2022

Objective:
To investigate the epidemiological characteristics of foodborne disease outbreaks in Shaoxing City, Zhejiang Province, from 2012 to 2022, so as to provide the evidence for improving the foodborne disease control strategy.
Methods:
Foodborne disease outbreaks in Shaoxing City from 2012 to 2022 were collected from National Foodborne Disease Outbreak Monitoring System in China, including populations, places of outbreak, pathogenic factors and suspected foods. The temporal distribution, regional distribution, distribution of outbreak places and pathogenic factors of foodborne disease outbreaks were descriptively analyzed.
Results:
A total of 89 foodborne disease outbreaks were reported in Shaoxing City from 2012 to 2022, covering totally 699 patients, with an average annual attack rate of 6.35%. The outbreak peaked during the period between June and October (73 outbreaks, 82.02%), and family was the predominant place of outbreak (41 outbreaks, 46.07%). There were 83 outbreaks with known pathogenic factors, including 51 outbreaks caused by microbial factors, with Vibrio parahaemolyticus, Salmonella and norovirus as predominant pathogens, and 29 outbreaks caused by fungi and their toxins, which were all poisonous mushrooms poisoning, resulting in 2 deaths. In addition, there were 3 outbreaks caused by chemical factors.
Conclusions
The outbreak of foodborne diseases predominantly occurred in summer and autumn in Shaoxing City from 2012 to 2022. Family was the predominant place of outbreak, and toxic mushroom poisoning was the most lethal pathogenic factor.
6.Application of self-management-oriented network platform in depression rehabilitation

Objective:To explore the application and effects of the self-management-oriented network platform in depression rehabilitation.Methods:Totally 118 patients with depression discharged from a municipal mental health center and the communities that it governs between January and December 2017 were selected by convenient sampling and divided into the study group and the control group by Excel sheets, with 59 cases in each group, of which 2 cases dropped out from the study group. Patients in the control group received routine disease management, while patients in the study group received self-management and education via the depression network management platform after a personalized management plan was formulated upon discharge on this basis. After 6 months of intervention, the improvements in satisfaction, self-efficacy and suicidal risks were compared between the two groups.Results:After intervention, the satisfaction scores of visit time, visit attitude, visit content, and humanistic care of the study group were higher than those of the control group, and the differences were statistically significant ( P<0.05) ; the General Self-Efficacy Scale score of the study group was higher than that of the control group, and the difference was statistically significant ( P<0.05) ; the scores of despair, optimism, and sleep factors in suicidal ideation in the study group were lower than those of the control group, and the differences were statistically significant ( P<0.05) . Conclusions:The self-management-oriented network platform management model for discharged patients with depression can improve patient satisfaction, enhance their sense of self-efficacy, reduce the risk of suicide, and strengthen their self-management capabilities.
7. The value of serum adiponectin in patients with preeclampsia and its relationship with insulin resistance

Objective:
To investigate the clinical value of serum adiponectin in patients with preeclampsia and its relationship with insulin resistance.
Methods:
From May 2017 to May 2018, 80 cases of preeclampsia diagnosed and treated in obstetrics department of the Second People′s Hospital of Liaocheng were selected in the research.According to the severity of the disease, they were divided into severe preeclampsia group and mild preeclampsia group, with 40 cases in each group, and 40 healthy pregnant women in the same period were selected as the control group.The serum adiponectin level and insulin resistance index (HOMA-IR) were detected and compared among the groups, and the correlation between them was analyzed.
Results:

8. Epidemiologic characteristics and pathogen surveillance of hand, foot and mouth disease in Zhejiang province, 2009-2017

Objective:
To evaluate the epidemiologic characteristics of hand, foot and mouth disease in Zhejiang province between 2009 and 2017, so that scientific evidence could be provided for prevention and control of hand, foot and mouth disease.
Methods:
Spatial, temporal and population distribution of HFMD was analyzed. Real-time reverse transcription polymerase cain reaction was used to test Enterovirus A71 and Coxsackievirus A16 in samples.
Results:
Between 2009 and 2017, 1 108 093 HFMD cases were reported in Zhejiang with the prevalence of 226.24/100000; 2010, 2012, 2014 and 2016 had a higher prevalence than other years. Prevalence of HFMD peaked in April-July and September-October. Wenzhou, Taizhou and Ningbo had a higher prevalence than other cities. In total, 69.27% cases were children who were not enrolled in nursery school, and 65.67% were 1-3 years old. Pathogen surveillance showed that EV-A71 decreased in mild cases, whereas other enterovirus increased. However EV-A71 was still predominant in severe and fatal cases (56.0%).
Conclusions
Temporal and spatial distribution of HFMD is characteristic in Zhejiang province. EV-A71 predominated in severe cases and fatal cases, while other enterovirus (non-EV-A71, non CV-A16) were the main pathogen for mild cases.
9.Application of WeChat-based self-management education in patients with depression

Objective To explore the effects of WeChat-based self-management education on patients with depression. Methods Totally 108 patients with depression who were discharged from a municipal mental health center between January and December 2016 received self-management education with the WeChat platform for six months. The frequency of reading push contents on WeChat, Insight and Treatment Attitude Questionnaire (ITAQ), Medication Adherence Questionnaire and Beliefs About Medical Questionnaire (BMQ) were used to assess the patients one month and six months after the management. The patients' insight and treatment attitude as well as medication adherence before and after the introduction of WeChat-based self-management education was compared. Results After six months of WeChat-based self-management education, the patients with depression showed better frequency of reading disease publicity materials, personized guidance, WeChat games and watching video and pictures in push contents of WeChat than that after one month of self-management (P<0.05); the patients showed higher insight and treatment attitude scores, lower medication adherence scores and better scores in the dimensions of beliefs about medical than that after one month of self-management (P<0.01). Conclusions WeChat-based self-management education can effectively improve the adherence, perception to disease and rehabilitation of those depression patients discharged from hospital.
10.Self management status and influencing factors of schizophrenia patients in community

Objective To investigate the present situation and analyze the influence factors of self-management ability of schizophrenic patients in community. Methods A total of 122 cases of schizophrenic patients in community that have been discharged from Zhenjiang Mental Health Center were investigated and analyzed by general information questionnaire, Schizophrenia Self-Management Ability Scale, quality of life and Social Support Rating Scale. Results The scores of self-management ability of schizophrenic patients in community were (68.74 ± 10.24) points. Multiple stepwise regression analysis showed that long-term medication (t =2.351, P<0.05), degree of education(t =2.385, P<0.05), community rehabilitation guidance (t =3.813, P<0.01) and social support (t=2.768, P<0.01) were the influence factors of self-management ability of schizophrenic patients in community. Conclusions Schizophrenia patients in community medical staff should be aimed at strengthening the long-term medication in community rehabilitation guidance and social support, improvement, and strengthen the low degree of culture of patient care, to improve the ability of self-management.
