1.Clinical observation of dupilumab in the treatment of severe asthma-COPD overlap

OBJECTIVE To investigate the clinical efficacy and safety of dupilumab in patients with severe asthma-chronic obstructive pulmonary disease (COPD) overlap (ACO). METHODS A retrospective analysis was conducted on 150 patients with severe ACO treated at Nantong Third People’s Hospital during Jan. 2022-Dec. 2023. Patients were divided into the control group [conventional therapy with inhaled corticosteroids (ICS)+long-acting beta-2 agonists (LABA)+long-acting muscarinic anticholinergic (LAMA), n=90] and the observation group (conventional therapy plus dupilumab, n=60) based on different treatment regimens. Propensity score matching (PSM) was performed at a ratio of 1.5∶1. Symptom control scores [Asthma Control Test (ACT) score, COPD Assessment Test (CAT) score] and pulmonary function parameters [forced vital capacity (FVC), forced expiratory volume in 1 second (FEV1), the FEV1 in percentage of predicted value (FEV1%pred), the ratio of FEV1 and FVC (FEV1/FVC), peak expiratory flow (PEF) and maximal mid-expiratory flow (MMEF)] before treatment and at 1, 3, and 12 months after treatment, as well as the levels of biomarkers [peripheral blood eosinophil (EOS) count, immunoglobulin E (IgE), C-reactive protein (CRP), fractional exhaled nitric oxide (FeNO)], annual acute exacerbation frequency, quality-of-life scores [Asthma Quality of Life Questionnaire (AQLQ) score, St George’s Respiratory Questionnaire (SGRQ) score, and Medical Outcomes Study 36-item Short Form Health Survey (SF-36) score] before treatment and 12 months after treatment, and the occurrence of adverse reactions were compared. RESULTS After PSM, 120 patients were included, involving 72 cases in the control group and 48 cases in the observation group. Compared with before treatment, both groups showed significant improvements in ACT scores at 1, 3, and 12 months after treatment, FVC, FEV1, FEV1%pred (except at 3 months after treatment in the control group), (except at 3 months after treatment in the control group), PEF (except at 3 and 12 months after treatment in the control group) and MMEF (including at 1 month after treatment in the observation group) at 3 and 12 months after treatment, as well as AQLQ and SF-36 scores at 12 months after treatment (P<0.05). CAT scores at 1, 3 and 12 months after treatment, annual acute exacerbation frequency and SGRQ score at 12 months after treatment, FeNO levels at 12 months after treatment in the control group, as well as EOS count, IgE, CRP and FeNO levels at 12 months after treatment in the observation group were significantly reduced (P<0.05). The improvements of above indicators in the observation group were mostly greater than in the control group (P<0.05). The incidence of adverse reactions did not differ significantly between the two groups (P>0.05). CONCLUSIONS Dupilumab provides superior efficacy over conventional therapy in improving clinical symptoms, pulmonary function, and quality of life, while reducing biomarker levels in patients with severe ACO, with a comparable safety profile to standard therapy.
2.Risk factors for cage retropulsion following transforaminal lumbar interbody fusion

BACKGROUND:Previous literature reported that the fusion cage moved more than 2 mm from its original position,which means that the fusion cage moved backward.At present,clinical observation has found that the factors leading to the displacement of the fusion cage are complex,and the relationship between these factors and the cage retropulsion is not clear. OBJECTIVE:To explore the risk factors related to cage retropulsion after lumbar interbody fusion. METHODS:Retrospective analysis was conducted in 200 patients who underwent transforaminal lumbar interbody fusion surgery with a polyetheretherketone interbody fusion from February 2020 to February 2022.According to the distance from the posterior edge of the vertebral fusion cage to the posterior edge of the vertebral body after the operation(the second day after the removal of the drainage tube)and 1,3,6 and 12 months after the operation,patients were divided into cage retropulsion group(≥2 mm)and cage non-retropulsion group(<2 mm).The factors that may affect cage retropulsion,such as age,gender,body mass index,bone mineral density,operation time,bleeding,endplate injury,preoperative and postoperative interbody height,cage implantation depth,cage size,and segmental anterior convexity angle,were analyzed by univariate and logistic regression analysis. RESULTS AND CONCLUSION:(1)Posterior displacement of the fusion cage occurred in 15 cases(15/200).The differences in basic information such as age and body mass index between the two groups were not statistically significant.(2)The results of the univariate analysis were that gap height difference,time to wear a brace,segmental anterior convexity angle difference,bone mineral density,and age were related to posterior migration of the cage.(3)The results of logistic regression analysis were that cage size,endplate injury condition,and depth of cage implantation were risk factors for cage retropulsion.(4)These findings suggest that cage retropulsion after lumbar interbody fusion is caused by multiple factors,including segmental anterior convexity angle difference,bone mineral density,cage size,endplate damage,time to wear a brace,and depth of cage implantation.

Objective To prepare a nano drug(PFOB@Lip-MMC)with liposome as the carrier,liquid perfluorooc-tyl bromide(PFOB)as core and mitomycin C(MMC)loading on the liposome shell and study its inhibitory effect on the proliferation of human pterygium fibroblasts(HPFs).Methods The thin film dispersion-hydration ultrasonic method was used to prepare PFOB@Lip-MMC and detect its physical and chemical properties.Cell Counting Kit-8,Cam-PI cell viability staining and flow cytometry were employed to detect the impact of different concentrations of PFOB@Lip-MMC on the via-bility of HPFs.DiI fluorescence labeled PFOB@Lip-MMC was used to observe the permeability of the nano drug to HPFs under a laser confocal microscope.After establishing HPF inflammatory cell models,they were divided into the control group(with sterile phosphate-buffered saline solution added),PFOB@Lip group(with PFOB@Lip added),MMC group(with MMC added),PFOB@Lip-MMC group(with PFOB@Lip-MMC added)and normal group(with fresh culture medi-um added)according to the experimental requirements.After co-incubation for 24 h,flow cytometer was used to detect the apoptosis rate of inflammatory cells,and the gene expression levels of interleukin(IL)-1β,prostaglandin E2(PGE2),tumor necrosis factor(TNF)-α and vascular endothelial growth factor(VEGF)in cells were analyzed by PCR.Results The average particle size and Zeta potential of PFOB@Lip-MMC were(103.45±2.17)nm and(27.34±1.03)mV,respec-tively,and its entrapped efficiency and drug loading rate were(72.85±3.28)%and(34.27±2.04)%,respectively.The sustained-release MMC of drug-loaded nanospheres reached(78.34±2.92)%in vitro in a 24-hour ocular surface environ-ment.The biological safety of PFOB@Lip-MMC significantly improved compared to MMC.In terms of the DiI fluorescence labeled PFOB@Lip-MMC,after co-incubation with inflammatory HPFs for 2 h,DiI fluorescence labeling was diffusely dis-tributed in the cytoplasm of inflammatory HPFs.The apoptosis rate of inflammatory HPFs in the PFOB@Lip-MMC group[(77.23±4.93)%]was significantly higher than that in the MMC group[(51.62±3.28)%].The PCR examination results showed that the gene transcription levels of IL-1 β,PGE2,TNF-α and VEGF in other groups were significantly reduced com-pared to the control group and PFOB@Lip group,with the most significant decrease in the PFOB@Lip-MMC group(all P<0.05).Conclusion In this study,a novel nano drug(PFOB@LIP-MMC)that inhibited the proliferation of HPFs was successfully synthesized,and its cytotoxicity was significantly reduced compared to the original drugs.It has good bio-compatibility and anti-inflammatory effects,providing a new treatment approach for reducing the recurrence rate after pte-rygium surgery.
4.Iodine nutritional status and influencing factors of pregnant women in Hangzhou City

Objective:To investigate the iodine nutritional status of pregnant women in Hangzhou City and analyze its influencing factors, in order to provide a basis for guiding pregnant women in Hangzhou City to supplement iodine scientifically.Methods:A stratified random sampling method was used to conduct a questionnaire survey on 1 400 pregnant women in Hangzhou City from March to October 2018. Random urine samples and household salt samples from pregnant women were collected, and the levels of urine iodine and salt iodine were measured using arsenic cerium catalytic spectrophotometry and direct titration, respectively. The iodine nutritional status of pregnant women with different basic characteristics and dietary levels were analyzed and compared (Kruskal-Wallis test), and the main influencing factors affecting the urinary iodine concentration level of pregnant women were identified (the ordinal multiple classification logistic regression analysis).Results:The median urinary iodine of pregnant women was 135.00 μg/L, and the qualified iodized salt consumption rate of pregnant women was 83.36% (1 167/1 400). In terms of basic characteristics, there were statistically significant differences in the distribution level of urine iodine among pregnant women with different pregnancy frequency, delivery frequency, and natural abortion frequency ( P < 0.05). In terms of diet, the frequency of consuming milk, yogurt, meat, and whether pregnant women consumed cabbage and cauliflower showed statistically significant differences in urinary iodine distribution levels ( P < 0.05). Ordinal multiple classification logistic regression analysis showed that the urine iodine levels of pregnant women who were pregnant twice were higher than those who were pregnant ≥3 times ( OR = 1.64, P = 0.003). Pregnant women who never or occasionally consumed yogurt had lower urine iodine levels than or equal to those who consumed 2 bottles of yogurt per day ( OR = 0.53, P = 0.044). Pregnant women who never or occasionally consumed meat and those who consumed meat once a week had higher urinary iodine levels than that who consumed meat ≥2 times per week ( OR = 1.40, 1.47, P < 0.05). Conclusions:The overall iodine nutrition of pregnant women in Hangzhou City is at an deficiency level. Pregnancy experience and dietary level are influencing factors on iodine nutrition of pregnant women. It is necessary to carry out in-depth health education for pregnant women, improve the dietary structure during pregnancy, and improve the abnormal iodine nutrition of pregnant women.
5.Frontal fibrosing alopecia

Frontal fibrosing alopecia is a primary lymphocytic cicatricial alopecia, and is generally considered to be a subtype of lichen planopilaris due to similar histopathological changes. Its etiology is still unclear. With the deepening of research on this disease, more and more cases of frontal fibrosing alopecia have been reported in China and other countries. This review summarizes research progress in pathogenesis, clinical and pathological characteristics, and treatment of frontal fibrosing alopecia.
6.Summary of the best evidence for home exercise rehabilitation of peritoneal dialysis patients

Objective:To retrieve, evaluate and integrate the best evidence of home exercise rehabilitation for peritoneal dialysis patients, so as to provide basis for formulating home exercise rehabilitation plan for peritoneal dialysis patients.Methods:Evidence-based problems were constructed according to the PICO (population, intervention, control and outcome) principle. The home exercise rehabilitation guideline, expert consensus, evidence summary, systematic review, and randomized controlled trial of peritoneal dialysis patients were systematically searched in the evidence-based resource databases such as UpToDate, British Medical Journal (BMJ) Best Practice, Guidelines International Network, and in the comprehensive databases such as PubMed, Embase, and Web of Science. The retrieval time limit was from the establishment of the database to November 20, 2021. Two researchers with evidence-based knowledge independently evaluated the quality of the article and the level of evidence.Results:A total of 10 articles were included, including one evidence summary, one practical suggestion, one expert consensus, two systematic reviews, and five randomized controlled trials. A total of 24 best evidences were summarized from 8 aspects, including applicable population, starting time, exercise evaluation, exercise prescription formulation, exercise type, exercise frequency and duration, exercise intensity and exercise precautions.Conclusions:The best evidence of home exercise rehabilitation for peritoneal dialysis patients provides a certain evidence-based basis for the selection of home exercise rehabilitation programs for peritoneal dialysis patients.
7.The value of intra-tumoral and peri-tumoral early dynamic contrast-enhanced MRI-based radiomics models in identifying benign from malignant in breast imaging-reporting and data system 4 breast tumors

Objective:To explore the value of radiomics model based on intratumoral and peritumoral early dynamic contrast-enhanced (DCE) MRI for identifying benign and malignant in breast imaging reporting and data system (BI-RADS) 4 tumors.Methods:A total of 191 patients diagnosed with BI-RADS 4 breast tumors by breast MRI examination with clear pathological diagnosis from January 2016 to December 2020 in the First Affiliated Hospital of Bengbu Medical College were analyzed retrospectively, including 77 benign and 114 malignant cases, aged 23-68 (46±10) years. The one-slice image with the largest area of the lesion of the second stage DCE-MRI images was selected to outline the region of interest, and automatically conformal extrapolated by 5 mm to extract the intra-tumoral and peritumoral radiomics features. The included cases were randomly divided into training and testing cohorts in the ratio of 8∶2. The statistical and machine learning methods were used for feature dimensionality reduction and selection of optimal radiomics features, and logistic regression was used as the classifier to establish the intratumoral, peritumoral, and intratumoral combined with peritumoral radiomics models. The independent risk factors that could predict the benignity and malignancy of breast tumors were retained as clinical-radiological characteristics by univariate and multivariate logistic regression to establish a clinical-radiological model. Finally, the intratumoral and peritumoral radiomics features were combined with clinical-radiological features to develop a combined model of the three. The receiver operating curve was used to analyze the predictive performance of each model and calculate the area under the curve (AUC),the AUC was compared by DeLong test. The stability of the three-component combined diagnostic model was tested by 10-fold cross-validation, and the model was visualized by plotting nomogram and calibration curves.Results:In the training cohort, the AUC of the three-component combined model for identifying benign and malignant BI-RADS 4 breast tumors was significantly higher than that of the intratumoral radiomics model ( Z=3.38, P<0.001), the peritumoral radiomics model ( Z=4.01, P<0.001), the intratumoral combined with peritumoral radiomics model ( Z=3.11, P=0.002), and the clinical-radiological model ( Z=3.24, P=0.001). And the AUC, sensitivity, specificity, accuracy, and F1-score of the three-component combined model were 0.932, 91.2%, 86.9%, 87.0% and 0.89, respectively. In the testing cohort, the three-component combined model also had the highest AUC value (0.875), and diagnostic sensitivity, specificity, accuracy and malignancy F1-score were 95.7%, 62.5%, 76.9%, and 0.89, respectively. The AUC calculated by 10-fold cross-validation was 0.90 (0.85-0.92), and the predicted curve of the three-component combined model in the calibration curve was in good agreement with the ideal curve. Conclusion:The three-component combined diagnostic model based on the intratumoral and peritumoral radiomics features and clinical-radiological features of early DCE-MRI has good performance and stability for identifying the benign and malignant in BI-RADS 4 breast tumors, and it can provide guidance for clinical decision non-invasively.
8.A case of cicatricial female pattern hair loss

A case of cicatricial female pattern hair loss was reported. A 36-year-old female patient presented with gradually aggravated hair loss for more than 10 years. Skin examination showed diffuse hair thinning on the scalp, thin and soft hairs, and some pencil eraser-sized areas of focal atrichia. TrichoScan examination revealed markedly decreased hair density on the forehead, variability in hair diameter greater than 20%, and increased proportions of vellus hairs. Dermoscopic examination showed increased numbers of vellus hairs, plenty of focal atrichia areas measuring 3 - 5 mm in diameter, loss of some follicular ostia, and confluent white dots. Histopathological examination of vertical and transverse scalp sections showed predominantly distributed miniaturized hair follicles with lichenoid folliculitis around the infundibulum and isthmus, concentrically layered perifollicular fibrosis, a marked decrease in the number of hair follicles compared with healthy people of the same age, increased proportions of vellus hairs, a large number of miniaturized hair follicles and follicular streamers, and formation of follicular micro-scars. The patient was diagnosed with cicatricial female pattern hair loss. She received topical treatment with 5% minoxidil liniment once a day, and alternate treatment with topical tacrolimus ointment and clobetasol propionate ointment, as well as oral spironolactone at a dose of 20 mg twice a day and compound glycyrrhizin capsules at a dose of 50 mg thrice a day. After half a year of treatment, there was no marked aggravation of hair loss, and the follow-up continued.
9.Prognostic Role of Immune-related Genes in Hepatocellular Carcinoma

Objective To identify the potential prognostic biomarkers of the immune-related genes signature for patients with hepatocellular carcinoma (HCC). Methods Original HCC data were downloaded from TCGA, and the immune activity of each sample was calculated by ssGSEA. HCC samples were divided into high and low immune cell infiltration groups by "GSVA" package and "hclust" package. The ESTIMATE algorithm scored the tumor microenvironment in each HCC sample. The "limma" package and Venn diagram identified effective immune-related genes. Univariate Cox, Lasso regression and multivariate Cox regression analyses were used to explore key genes. The "rms" package was used to create nomograms and draw calibration curves. Results Compared with the high immune cell infiltration group, the tumor purity of the samples in the low immune cell infiltration group was higher, the immune score, ESTIMATE score and stromal score were lower. In the high immune cell infiltration group, the immune components were more abundant, and the expression levels of TIGIT, PD-L1, PD-1, LAG3, TIM-3, CTLA4 and HLA family were higher. Multivariate Cox regression analysis showed that four immune-related genes (S100A9, HMOX1, IL18RAP and FCER1G) were used to construct the prognosis model. Compared with other clinical features, the risk score of this prognostic model was recognized as an independent prognostic factor. Conclusion This study identified the immune-related core genes which may be used in targeted therapy and immunotherapy of HCC.
10.Evaluation of the efficacy of complete nail plate avulsion in the treatment of five cases of retronychia

Objective:To evaluate the efficacy of complete nail plate avulsion in the treatment of retronychia.Methods:From June 2018 to November 2019, 5 patients with retronychia (5 toes) were collected from the Department of Dermatology, Shanghai Ninth People′s Hospital, Shanghai JiaoTong University School of Medicine, and treated with complete nail plate avulsion. In some cases, redundant skin tissues at the distal end of the great toe were removed. After the surgery, artificial nails were placed on the surface of the nail bed followed by the suturing.Results:In the 5 patients, the neonatal nail plate gradually grew over the nail bed, and pain was completely relieved. Within the follow-up of 6-12 months, the nail plates of the 5 patients all returned to normal.Conclusion:Complete nail plate avulsion is effective for the treatment of retronychia, and worthy of clinical promotion.
