1.Clinical Observation on Patients With ST-elevation During Radiofrequency Catheter Ablation for Atrial Fibrillation

Objectives:This study aimed to elucidate the incidence,clinical characteristics and prognosis of ST-elevation during radiofrequency catheter ablation for atrial fibrillation(AF). Methods:Consecutive patients who underwent radiofrequency catheter ablation for AF in Tongren Municipal People's Hospital,Qixingguan District People's Hospital of Bijie City and Guizhou Provincial People's Hospital from January 2021 to August 2023 were enrolled in this study.All patients underwent CARTO three-dimensional electroanatomical mapping and radiofrequency ablation via atrial septal approach under local anesthesia.The ST-elevation of electrocardiogram was analyzed.Follow-up was performed at 1,3,6,and 12 months after radiofrequency ablation.AF recurrence and duration,use of antiarrhythmic drugs,and incidence of death,thromboembolism,bleeding,and perioperative complications were evaluated. Results:ST-elevation was observed in 5 out of 798 patients(0.62%).ST-elevation occurred after transseptal puncture in three patients and during PentaRay multielectrode mapping in two patients.Blood pressure was significantly increased in three patients with transient ST-elevation(<20 min)and hemodynamic collapse occurred in two patients with persistent ST-elevation(>20 min).Catheter ablation of AF was completed in 4 patients,1 patient suffered severe hemodynamic disorders during radiofrequency catheter ablation,and the procedure was stopped immediately,this patient died from multiple organ system failure on the fifth day after failed radiofrequency catheter ablation,and the other 4 patients had no perioperative complications.The mean follow-up was(6±3)months,only 1 patient developed short atrial tachycardia,and the other patients had no recurrent atrial fibrillation and palpitation. Conclusions:Transient or persistent ST elevation can occur in patients during AF ablation.Early detection and rapid management are needed to prevent severe hemodynamic instability and cardiogenic death.
2.Effect of evidence-based nursing practice in function rehabilitation of post-stroke neurogenic bladder

Objective:To evaluate the effect of evidence-based nursing practice for functional rehabilitation of post-stroke neurogenic bladder (PSNB) .Methods:From October 2020 to July 2021, 136 PSNB patients admitted to the Department of Neurology, Jiangyin People's Hospital were selected by convenience sampling as the research object. According to the random number table method, the patients were divided into the study group and the control group, with 68 cases in each group. The control group received routine rehabilitation nursing, while the study group was given evidence-based rehabilitation nursing based on the control group. The urodynamics, rates of bladder balance, urinary incontinence and urinary retention after intervention were compared between the two groups.Results:After three months of intervention, bladder compliance and bladder balance rate in the study group were higher than those in the control group, and the residual urine volume was less than that in the control group, with statistical differences ( P<0.05) . After three months of intervention, urinary retention rate in the study group was lower than that in the control group, and the difference was statistically significant ( P<0.05) . Conclusions:Evidence-based rehabilitation nursing can help to promote the rapid recovery of bladder balance in PSNB patients and improve the prognosis of patients.
3.Analysis of deafness gene variant screening of 7875 neonatal cases in Dongying area of Shandong.

OBJECTIVE:
To determine the types and frequency of deafness-related variants among 7875 newborns from Dongying area of Shandong Province.
METHODS:
One hundred loci of 18 common deafness genes were subjected to semiconductor sequencing. Variant site, frequency and distribution of the variants were analyzed.
RESULTS:
In total 552 deafness gene variants were detected among the 7875 newborns, which yielded a detection rate of 7.01%. Among these, common variant sites for GJB2, SLC26A4 and GJB3 genes were c.235delC, IVS7-2A>G and c.538C>T, respectively. The variant frequencies of matrilinear inheritance deafness genes MT-CO1, MT-RNR1, MT-TL1 and MT-TS1 were 0.38%, 0.25%, 0.1% and 0.01%, respectively. Four newborns were diagnosed with deafness, among which one had unilateral hearing loss. Analysis of the proportions of neonatal deafness-related variants in five counties of Dongying showed that the highest variant rate for the SLC26A4 gene compared with GJB2 was in Lijin county (51.76% vs. 40%), while the lowest was in Hekou county (30.77% vs. 56.41%).
CONCLUSION
The carrier rate of deafness-related variants in Dongying area is higher than other regions of China, which may be attributed to the increased types and variant sites covered by the semiconductor sequencing method compared with the chip method and time-of-flight mass spectrometry. Due to geographical and population aggregation factors, the proportion of deafness variants in the five counties of Dongying differed significantly. Above results may provide a guide for the prevention of congenital deafness in Dongying area.
4.Study on the correlation between IL-1β ,Hcy ,Cys-C and HbA1c levels and senile type 2 diabetic peripheral neuropathy

Objective To investigate the correlation of interleukin-1β (IL-1β) ,homocysteine (HCY) ,cysta-tin (Cys-C) and glycosylated hemoglobin (HbA1c) levels and senile type 2 diabetic peripheral neuropathy. Methods 104 patients with type 2 diabetes mellitus received in the hospital from June 2014 to December 2016 were selected as study group ,52 patients with peripheral neuropathy were served as study group A (n=52) and 52 patients with type 2 diabetes were served as study group B (n=52) ,and 52 healthy persons who under-went health assessement were taken as the control group.The Cys-C level was measured by immunoturbidime-try ,the levels of HCY and IL-1β were measured by enzyme-linked immunosorbent assay.The HbA1c level was measured by high performance liquid chromatography ,the IL-1β ,HCY ,Cys-C and HbA1c levels were com-pared in the three groups ,and the relationship between IL-1β ,HCY ,Cys-C and HbA1c levels and senile type 2 diabetic peripheral neuropathy were analyzed.Results Single factor variance analysis showed that the levels of IL-1β ,HCY ,Cys-C and HbA1c in the three groups were statistically significant (P<0.05) ;LSD-t test showed that the IL-1β ,HCY ,Cys-C and HbA1c levels in the study group B were higher than those in control group , and IL-1β ,HCY ,Cys-C and HbA1c levels in the study group A were higher than those in the study group B , and the difference was statistically significant (P< 0.05) ;logisticis regression analysis showed that IL-1β , HCY ,Cys-C ,HbA1c levels were associated with type 2 diabetic peripheral neuropathy (P<0.05) ;Spearman test showed that IL-1β ,HCY ,Cys-C ,HbA1c levels were positively correlated with type 2 diabetic peripheral neuropathy (P<0.05).Conclusion IL-1β ,HCY ,Cys-C and HbA1c levels are positively correlated with senile type 2 diabetic peripheral neuropathy in the elderly ,and the IL-1β ,HCY ,Cys-C and HbA1c levels can be used to determine type 2 diabetic peripheral neuropathy in clinical work.
5.Expression of miR-206 in serum and placental tissue of patients with gestational hypertension and its clinical significance

Objective To investigate the expression levels of miR-206 in serum and placenta tissues of patients with gestational hypertension and its clinical significance.Methods The expression levels of miR-206 in serum and placental tissue samples from 34 pregnant women with gestational hypertension,26 with mild preeclampsia,27 with severe preeclampsia and 32 normal pregnant women were detected by quantitative real-time PCR (qRT-PCR),and their clinical pathological data were collected for correlation analysis.Results Compared with normal pregnant women,the expression levels of miR-206 in sera and placental tissues of pregnant women with gesta tional hypertension,mild preeclampsia or severe preeclampsia decreased significantly in turn (all P < 0.05),and there was significant difference (F =79.10 and 129.18,respectively,both of P < 0.05).The correlation analysis showed that the expression levels of miR-206 in sera of normal pregnant women and pregnant women with gestational hypertension,mild preeclampsia or severe preeclampsia were positively related to that in placental tissues (r =0.402,0.512,0.416 and 0.397,respectively,all P < 0.05).In addition,the expression levels of miR-206 in serum and placental tissue of pregnant women with gestational hypertension were negatively related to systolic blood pressure,diastolic blood pressure and urinary protein level (P < 0.05),but positively to gestational age,albumin level and neonatal body weight (P < 0.05).Conclusion The expression of miR-206 in serum and placental tissue of pregnant women with gestational hypertension decreases significantly,which may be involved in the development and progression of gestational hypertension.
6.Risk factors of pathological upgrading in gastric mucosa lesions with low-grade intraepithelial neoplasia after endoscopic submucosal dissection

Objective To investigate the risk factors of pathological upgrading in gastric mucosal lesions with low-grade intraepithelial neoplasia (LGIN) after endoscopic submucosal dissection (ESD).Methods From January 2010 to December 2016,the complete clinical data of 326 patients pathologically diagnosed with gastric LGIN lesions before ESD were retrospectively analyzed.Single factor analysis of variance and multiple factor Logistic regression analysis were performed to analyze the risk factors of pathological upgrading after ESD.Results A total of 326 patients with gastric LGIN lesions diagnosed by preoperative biopsy before ESD were enrolled.Among them the postoperative pathological diagnosis of 244 cases (74.85%) were still LGIN,while the postoperative pathological diagnosis of 82 cases (25.15 %) were upgraded,of which 61 cases (18.71%) were upgraded to high-grade intraepithelial neoplasia and 21 (6.44%) were upgraded to gastric early cancer.The results of single and multiple factor analysis indicated that lesion size≥2.0 cm,deep depressed-type,surface erythema,lesion mucosa with ulceration and lesions with spontaneous bleeding were the risk factors of pathological diagnosis upgrading after ESD (F=5.37,6.44,4.56,7.56 and 7.78,respectively;all P<0.01),odds ratio (OR) value and 95% confidence interval (CI) were 4.086 (2.035 to 10.786),7.435 (2.845 to 19.862),3.205 (1.535 to 8.541),8.668 (3.365 to 21.457) and 7.056 (2.732 to 18.355).The age,gender and location of the lesion were not the risk factors.Conclusions Pathological upgrading is common in gastric lesions with LGIN after ESD.The lesions with high risk factors should be alerted and treated more actively.

Objective To summarize the characteristics of medical service patterns at Grenada station of Mission Harmony-2015 so as to provide reference for future overseas services in similar island countries .Methods After detailed analysis of actual conditions of the countries visited and efficient communication with host country health organizations , we made an appropriate adjustment of the medical service pattern , which involved multi-site screening of general patients , followed by intensive checkups and treatment aboard the hospital ship .Results The procedure of diagnosis and treatment was streamlined and the efficacy of medical services was significantly enhanced to meet the maximum medical demand of local residents.Grenada became a station with the largest number of patients who received top-quality fee-free healthcare during Mission Harmony-2015 .Conclusion Adjustment of medical service patterns according to the actual conditions could deliver a desired effect during overseas missions .
9.Characteristics of Surface Electromyogram of Forearm Muscles of Parkinson's Disease Patients with or without Load

Objective To explore the characteristics of surface electromyogram (sEMG) of the forearm muscles of Parkinson's disease patients with or without load. Methods 26 Parkinson's disease patients and 28 healthy individuals participated in this study. All the subjects performed isometric contraction for elbow flexion without load or with a load of 1.5 kg. The sEMG signals were collected by surface elec-trodes and processed by linear time and frequency domain method. Results The values of median frequency (MF) and mean power frequen-cy (MPF) were higher in the patients than in the healthy controls without load (P<0.05). The value of average EMG (AEMG) was lower in the patients than in the healthy controls with a load of 1.5 kg (P<0.001). For the patients, the values of MF and MPF were higher without load than with a load of 1.5 kg (P<0.001), and the value of AEMG was lower without load than with a load of 1.5 kg (P<0.001). For the healthy controls, only the value of AEMG was lower without load than with a load of 1.5 kg (P<0.001). Conclusion The values of MF and MPF are higher in Parkinson's disease patients than in the healthy controls without load while the value of AEMG is lower in Parkinson's dis-ease patients than in the healthy controls with load.
10.The rheology properties of common hydrophilic gel excipients.

To investigate theological properties of common hydrophilic gel excipients such as Carbopol based on viscosity, the viscosity was determined by rotation method and falling-ball method. Linear regression was made between ln(eta) and concentration, the slope of which was used to explore the relation between viscosity and concentration of different excipients. The viscosity flow active energy (E(eta)) was calculated according to Arrhenius equation and was used to investigate the relation between viscosity and temperature of different excipients. The results showed that viscosities measured by two methods were consistent. Concentration of guargum (GG) and hydroxypropylmethyl cellulose (HPMC) solution had a great influence on the viscosity, k > 5; while concentration of polyvinylpyrrolidone-K30 (PVP-K30) and polyethylene glycol 6000 (PEG6000) exerted a less effect on viscosity, k < 0.2; viscosity flow active energy of different excipients were close, which ranged from 30 to 40 kJ x mol(-1). Therefore, theological properties study could provide the basis for application of excipients and establish a foundation for the research of relation between excipients structure, property and function.
