1.Development and validation of a diagnostic model based on machine learning algorithms for the development of interstitial lung diseases in patients with rheumatoid arthritis

		                        			Objective:Screening factors that might influence rheumatoid arthritis (RA) complicating interstitial lung diseases (ILD) by constructing and validating a model for early diagnostic.Methods:The study subjects were composed of 712 RA patients in the Department of Rheumatology and Immunology of the Second Hospital of Shanxi Medical University during December 2019 to October 2022. Fifty-two variables such as their demographic data, clinical symptoms, and laboratory indexes were collected. Patients were categorized into RA-only group and RA-ILD group with or without the occurrence of ILD disease. After data preprocessing, subjects were randomly assigned to the modeling and validation groups in a 7:3 ratio.Univariate analysis comparing baseline characteristics of the two groups of patients. Feature selection was performed using LASSO and SVM-RFE regression algorithms.Screening indicators were analyzed by logistic regression and the results were used to develop a nomograms model for the early diagnosis of RA complicating interstitial lung disease; and the modeling group was evaluated for its performance for internal assessment of the model and internal validation using data from the validation group.Results:A total of 712 subjects participated in the study, of which 498 in the modeling group and 214 in the validation group. Univariate analysis showed that the differences between the two groups were statistically significant ( P<0.05) in 18 characteristic indexes, including male, gender, age, smoking history, drinking history, number of swollen joints, number of painful joints, use of prednisone, WBC, ESR, CRP, IL-2, IL-10, IL-17, TNF-α, INF-γ, AFA family, APF, and serum albumin. The LASSO algorithm identified 13 risk variables for RA-ILD, the SVM-RFE algorithm identified 12 variables for RA-ILD, and the intersecting risk variables were male, age, history of alcohol consumption, number of painful joints, prednisone acetate, IL-2, AFA family, TNF-α, serum albumin, and IL-10. The results of multifactorial logistic regression analysis confirmed that the differences between males [ OR(95% CI)=3.61(2.11, 6.18)], gender, age [ OR(95% CI)=1.05(1.03, 1.08)], number of painful joints [ OR(95% CI)=1.03(1.01, 1.06)], IL-2 [ OR(95% CI)=0.91 (0.84, 0.99)], and TNF-α[ OR (95% CI)=1.06 (1.02, 1.10)] were statistically significant ( P<0.05) and were independently influences on ILD complicated by RA. The modeling and validation groups that were used to construct early diagnostic Nomograms had high calibration curve accuracies, and the model had a high diagnostic power, which was mainly demonstrated by the receiver operating characteristic (ROC) area under the curve (AUC) and decision curve analysis(DCA), the model modeling group had an AUC of 0.76 (95% CI=0.71, 0.81), with net benefit rates of 3%~82% and 93%~99%, whereas the model validation group had an AUC of 0.71 (95% CI=0.64, 0.79), with net benefit rates of 5%~11%, 14%~60% and 85%~89%. Conclusion:Male, gender, age, number of painful joints, IL-2, and TNF-α are independent factors for RA complicated with ILD, and the Nomogram model constructed has good performance in early diagnosis of the disease.
		                        		
		                        		
		                        		
		                        	
2.Development of the robotic digestive endoscope system and an experimental study on mechanistic model and living animals (with video)

		                        			Objective:To develop a robotic digestive endoscope system (RDES) and to evaluate its feasibility, safety and control performance by experiments.Methods:The RDES was designed based on the master-slave control system, which consisted of 3 parts: the integrated endoscope, including a knob and button robotic control system integrated with a gastroscope; the robotic mechanical arm system, including the base and arm, as well as the endoscopic advance-retreat control device (force-feedback function was designed) and the endoscopic axial rotation control device; the control console, including a master manipulator and an image monitor. The operator sit far away from the endoscope and controlled the master manipulator to bend the end of the endoscope and to control advance, retract and rotation of the endoscope. The air supply, water supply, suction, figure fixing and motion scaling switching was realized by pressing buttons on the master manipulator. In the endoscopy experiments performed on live pigs, 5 physicians each were in the beginner and advanced groups. Each operator operated RDES and traditional endoscope (2 weeks interval) to perform porcine gastroscopy 6 times, comparing the examination time. In the experiment of endoscopic circle drawing on the inner wall of the simulated stomach model, each operator in the two groups operated RDES 1∶1 motion scaling, 5∶1 motion scaling and ordinary endoscope to complete endoscopic circle drawing 6 times, comparing the completion time, accuracy (i.e. trajectory deviation) and workload.Results:RDES was operated normally with good force feedback function. All porcine in vivo gastroscopies were successful, without mucosal injury, bleeding or perforation. In beginner and advanced groups, the examination time of both RDES and ordinary endoscopy tended to decrease as the number of operations increased, but the decrease in time was greater for operating RDES than for operating ordinary endoscope (beginner group P=0.033; advanced group P=0.023). In the beginner group, the operators operating RDES with 1∶1 motion scaling or 5∶1 motion scaling to complete endoscopic circle drawing had shorter completion time [1.68 (1.40, 2.17) min, 1.73 (1.47, 2.37) min VS 4.13 (2.27, 5.16) min, H=32.506, P<0.001], better trajectory deviation (0.50±0.11 mm, 0.46±0.11 mm VS 0.82±0.26 mm, F=38.999, P<0.001], and less workload [42.00 (30.00, 50.33) points, 43.33 (35.33, 54.00) points VS 52.67 (48.67, 63.33) points, H=20.056, P<0.001] than operating ordinary endoscope. In the advanced group, the operators operating RDES with 1∶1 or 5∶1 motion scaling to complete endoscopic circle drawing had longer completion time than operating ordinary endoscope [1.72 (1.37, 2.53) min, 1.57 (1.25, 2.58) min VS 1.15 (0.86, 1.58) min, H=13.233, P=0.001], but trajectory deviation [0.47 (0.13, 0.57) mm, 0.44 (0.39, 0.58) mm VS 0.52 (0.42, 0.59) mm, H=3.202, P=0.202] and workload (44.62±21.77 points, 41.24±12.57 points VS 44.71±17.92 points, F=0.369, P=0.693) were not different from those of the ordinary endoscope. Conclusion:The RDES enables remote control, greatly reducing the endoscopists' workload. Additionally, it gives full play to the cooperative motion function of the large and small endoscopic knobs, making the control more flexible. Finally, it increases motion scaling switching function to make the control of endoscope more flexible and more accurate. It is also easy for beginners to learn and master, and can shorten the training period. 3.The practice and analysis of TCM characteristic nursing out-patient cooperation in spleen and stomach disease specialization

		                        			Objective Exploring the feasibility and effectiveness of collaborative visits between doctors and nurses in tra-ditional Chinese medicine specialty nursing clinics for spleen and stomach diseases.Methods Using the principle of hospital se-lection to determine the visiting caregiver,and collaborating with specialist physicians to establish a specialized Traditional Chi-nese Medicine specialty nursing clinic for spleen and stomach diseases.By conducting a questionnaire survey on patients who visi-ted the Traditional Chinese Medicine specialty nursing outpatient clinic for spleen and stomach disease in 2023 through medical and nursing collaboration and in 2019 through individual visits by caregivers,we analyzed their satisfaction with medical nursing services.Results Since the implementation of collaborative visits between traditional Chinese medicine specialty nursing clinics for spleen and stomach diseases,the number of nursing clinics has increased from 35 people/month in 2019 to 148 people/month in 2023,and the visit time has been increased from 1 time/week in 2019 to 3 times/week in 2023.The satisfaction level of medi-cal nursing services among patients who received collaborative visits from nursing clinics in 2023 was higher than that of patients who received individual visits from nurses in 2019,and the difference was statistically significant(P<0.05).Conclusion The implementation of collaborative outpatient visits for traditional Chinese medicine specialty nursing in the spleen and stomach dis-ease specialty has expanded the connotation of traditional Chinese medicine nursing services,met the growing individualized and diversified needs of patients,and improved their satisfaction with medical nursing services.
		                        		
		                        		
		                        		
		                        	
4.Risk factors and predictive models for liver cancer after sustained virologic response in hepatitis C

		                        			Hepatitis C is one of the main causes of liver cancer.With the application of direct-acting antiviral agents,more than 95%of patients can achieve the eradication of hepatitis C virus and obtain sustained virologic response(SVR).Effective antiviral therapy can change the natural course of hepatitis C and reduce the risk of liver cancer;however,some patients are still affected by age,sex,liver fibrosis,diabetes,hepatic steatosis,alcohol consumption,and genetic factors and become the high-risk population of liver cancer.Therefore,it is needed to further clarify and improve the identification and prediction of high-risk populations of liver cancer after SVR of hepatitis C.This article reviews the risk factors and predictive models for liver cancer after SVR in patients with hepatitis C,in order to provide a basis for identifying the high-risk population of liver cancer after SVR of hepatitis C in clinical practice.
		                        		
		                        		
		                        		
		                        	
5.Human papillomavirus vaccination behaviors among gynecological outpatients based on extended unified theory of acceptance and use of technology

		                        			Objective:
		                        			To investigate the factors affecting human papillomavirus (HPV) vaccination behaviors among gynecological outpatients based on extended unified theory of acceptance and use of technology (UTAUT2), so as to provide insights into the development of HPV vaccination behavioral interventions.
		                        		
		                        			Methods:
		                        			 Patients at ages of 45 years and younger that were admitted to the outpatient department of gynecological of Shanxi Provincial People's Hospital from October 2021 to August 2022 were recruited, and the factors affecting HPV vaccination behaviors were identified using UTAUT2.
		                        		
		                        			Results:
		                        			A total of 431 female outpatients were enrolled, including 163 patients at ages of 36 to 45 years (37.82%), 272 cases with an educational level of college degree and above (63.11%) and 253 patients with per capita monthly household income of more than 3 000 Yuan (58.70%). The coverage of HPV vaccination was 24.36%, and the main cause of non-vaccination was difficulty in high-valent HPV vaccine appointment. Price value, social impact and efficacy expectation posed a positive impact on HPV vaccination behaviors via intention of vaccination (β=0.11, 0.08, 0.07, all P<0.05) and intention of vaccination and effort expectancy (β=0.10, 0.07, 0.06, all P<0.05), and effort expectancy played a mediating effect between intention of vaccination and vaccination behaviors (β=0.28, P<0.05).
		                        		
		                        			Conclusion
		                        			Efficacy expectation, social impact, price value, intention of vaccination and effort expectancy may positively affect HPV vaccination behaviors.
		                        		
		                        		
		                        		
		                        	
6.Construction of intervention program for physical and mental adjustment for primary caregivers of cancer patients based on network cognitive behavior therapy

		                        			Objective:Based on cognitive behavioral therapy, to construct a physical and mental adjustment intervention plan for the main caregiver of cancer patients through the network platform.Methods:Through evidence-based literature published from July 2012 to July 2022 screening and evaluation, combined with qualitative interviews for 10 primary caregivers of cancer patients, the intervention plan for physical and mental adjustment of the main caregivers of cancer patients was preliminarily formulated. After consultation with Delphi experts (15 cases) through two rounds, the intervention plan was finally determined.Results:In the two rounds of expert letter inquiries, 15 questionnaires were distributed and 15 valid questionnaires were recovered. The effective recovery rate was 100.00% and the expert authority coefficient were 0.89 and 0.90 in the two rounds of expert letter inquiries respectively, the Kendall harmony coefficients were 0.279 and 0.323 respectively, and the differences were all statistically significant ( P<0.01). The intervention plan for physical and mental adjustment ofthe main caregivers of cancer patients included 5 first-level indicators (basic knowledge, symptom education, home care knowledge, relaxation training, social support), 27 second-level indicatorsand 54 third-level indicators. Conclusions:The method of the psychosomatic regulation intervention program is scientific and practical, which can be initially applied to the psychological adjustment of the main caregivers of cancer patients, so as to provide a reference for improving their negative emotions.

8.Urinary microalbumin to creatinine ratio and α1-microglobulin to creatinine ratio and their influencing factors in people aged ≥40 years old in 10 counties of Shanxi province

		                        			Objective:To investigate urinary microalbumin to creatinine ratio (ACR) and α1-microglobulin to creatinine ratio (MCR) of people aged 40 years old and above in Shanxi province, and analyze the influencing factors of abnormal ACR and MCR, and to provide evidence for the prevention and control of chronic kidney diseases.Methods:It was a cross-sectional study. The data came from a screening study of chronic kidney diseases conducted by Shanxi Provincial People's Hospital from April to November 2019, involving aged 40 years old and above from 10 counties (Ningwu county, Yu county, Yangqu county, Lin county, Shouyang county, Zezhou county, Huozhou city, Hejin city, Linyi county and Ruicheng county) in Shanxi province. The related data were collected through questionnaire surveys, physical examinations, and blood and urine sample collection. Urinary α1-microglobulin, creatinine, and microalbuminuria were measured. Urinary ACR and MCR were calculated using urinary creatinine correction. ACR abnormality was defined as ≥30 mg/g, and MCR abnormality was defined as >23 mg/g. Covariate analysis was used to control confounding factors, and adjusted urinary ACR and MCR of 10 counties were calculated. Spearman correlation analysis and chi-square test were performed to analyze the factors associated with abnormal urinary ACR and MCR. Logistic regression analysis model was used to identify the influencing factors of abnormal urinary ACR and MCR.Results:A total of 12 285 residents were enrolled in the study, including 5 206 males (42.4%) and 7 079 females (57.6%). The median age was 58.0 (51.0, 66.0) years old. The median urinary ACR was 7.5 (4.5, 15.7) mg/g, and the median urinary MCR was 10.2 (6.4, 16.2) mg/g. A total of 1 572 individuals (12.80%) had urinary ACR abnormality and 1 450 individuals (11.80%) had urinary MCR abnormality. Yangqu county, Yuxian county, and Ningwu county had higher urinary ACR with (35.58±3.04) mg/g, (34.08±4.50) mg/g and (32.09±3.19) mg/g, respectively. The urinary MCR was generally similar among the 10 counties and Yangqu county had higher urinary MCR with (13.86±0.41) mg/g. In addition to Yu county, female individuals had higher urinary ACR compared to males in other counties, whereas female individuals had lower urinary MCR compared to males in 10 counties. Multivariate logistic regression analysis results showed that elevated triglyceride, fasting blood glucose, glycated hemoglobin, systolic blood pressure, diastolic blood pressure, age, body mass index and gender were independent influencing factors of abnormal urinary ACR and MCR (all P<0.05). Elevated blood homocysteine and low educational level were independent influencing factors of urinary MCR abnormality (both P<0.05). Conclusions:There are differences of gender and region in urinary ACR and MCR among individuals aged 40 years old and above in the 10 counties of Shanxi province. Triglyceride, fasting blood glucose, glycated hemoglobin, systolic blood pressure, diastolic blood pressure, age, gender, and body mass index are independent related factors of abnormal urinary ACR and MCR. 9.Disease burden of prostate cancer among residents at ages of 60 years and older in Zhangjiagang City from 2006 to 2022

		                        			To investigate the trends in incidence, mortality and disease burden of prostate cancer among residents at ages of 60 years and older in Zhangjiagang City, Jiangsu Province from 2006 to 2022, so as to provide insights into improvements in the prostate cancer control strategy.
		                        		
		                        			Methods:
		                        			The incidence and mortality of prostate cancer among residents at ages of 60 years and older in Zhangjiagang City from 2006 to 2022 were collected from the Chronic Diseases Monitoring and Management System in the National Health Information Platform of Zhangjiagang City, and the crude incidence and mortality of prostate cancer were calculated and standardized to data from the sixth national population census in China in 2010. Based on the disease burden of prostate cancer captured from the 2019 Global Burden of Disease Study datasets, the years of life lost due to premature death (YLL) and years of life lived with disability (YLD) and disability-adjusted life years (DALY) due to prostate cancer were measured, and trends in incidence, mortality and disease burden of prostate cancer were analyzed using average annual percent change (AAPC).
		                        		
		                        			Results:
		                        			The crude incidence, standardized incidence, crude mortality, standardized mortality, crude DALY rate and standardized DALY rate of prostate cancer were 89.85/105, 83.87/105, 32.31/105, 25.45/105, 546.39/105 and 483.50/105 among residents at ages of 60 years and older in Zhangjiagang City from 2006 to 2022, which all appeared a tendency towards a rise (AAPC=5.346%, 4.219%, 6.648%, 3.697%, 4.198% and 2.200%, all P<0.05). The crude incidence, mortality and DALY rate of prostate cancer all appeared a tendency towards a rise with age (all P<0.05), with a tendency towards a rise seen for the crude incidence of prostate cancer among residents at ages of 60 to 64 years, 65 to 69 years and 70 to 74 years (AAPC=4.888%, 8.086% and 3.005%, all P<0.05), and a tendency towards a rise for the crude mortality and DALY rate among residents at ages of 80 years and older (AAPC=10.243% and 9.693%, both P<0.05).
		                        		
		                        			Conclusion
		                        			The incidence and mortality of prostate cancer showed a tendency towards a rise among residents at ages of 60 years and older in Zhangjiagang City from 2006 to 2022, and the disease burden due to prostate cancer continued to increase, with a more remarkable increasing tendency seen for the incidence of prostate cancer among residents at ages of 60 to 74 years and for the mortality among residents at ages of 80 years and older.
		                        		
		                        		
		                        		
		                        	
10.Epidemiological characteristics and Spatial-temporal clustering of hand, foot and mouth disease in Shanxi province, 2009-2020.

		                        			Objective: To analyze the epidemiology and spatial-temporal distribution characteristics of hand, foot and mouth disease (HFMD) in Shanxi province. Methods: The data of HFMD in Shanxi province from 2009 to 2020 were collected from notifiable disease management information system of Chinese information system for disease control and prevention and analyzed by descriptive epidemiology, Joinpoint regression, spatial autocorrelation analysis and spatio- temporal scanning analysis. Results: A total of 293 477 HFMD cases were reported in Shanxi province from 2009 to 2020, with an average annual incidence of 67.64/100 000 (293 477/433 867 454), severe disease rate of 5.36/100 000 (2 326/433 867 454), severe disease ratio of 0.79%(2 326/293 477), mortality of 0.015/100 000 (66/433 867 454), and fatality rate of 22.49/100 000 (66/293 477). The reported incidence rate, severe disease rate, mortality rate and fatality rate of HFMD showed decreasing trends. The main high-risk groups were scattered children and kindergarten children aged 0-5. The incidence of HFMD had obvious seasonal variation, with two peaks every year: the main peak was during June-July, the secondary peak was during September-October and the peak period is from April to November. A total of 13 942 laboratory cases were confirmed, with a diagnosis rate of 4.75% (13 942/293 477), including 4 438 (35.11%, 4 438/293 477) Enterovirus A71 (EV-A71) positive cases, 4 609 (33.06%, 4 609/293 477) Coxsackievirus A16 (CV-A16) positive cases, and 4 895 (31.83%, 4 895/293 477) other enterovirus positive cases. There was a spatial positive correlation (Moran's I ranged from 0.12 to 0.58, all P<0.05) and the spatial clustering was obvious. High-risk regions were mainly distributed in Taiyuan in central Shanxi province, Linfen and Yuncheng in southern Shanxi province, and Changzhi in southeastern Shanxi province. Spatial-temporal scanning analysis revealed 1 the most likely cluster and 8 secondary likely clusters, of which the most likely cluster (RR=2.65, LLR=22 387.42, P<0.001) located in Taiyuan and Jinzhong city, Shanxi province, including 12 counties (districts), and accumulated from April 1, 2009 to November 30, 2018. Conclusions: There was obvious spatial-temporal clustering of HFMD in Shanxi province, and the epidemic situation was in decline. The key areas were the districts in urban areas and the counties adjacent to it.
