1.Concept Development of Political Competence for Nurses

PURPOSE: The purpose of this study was to define and clarify the concept of political competence for nurses.METHODS: A hybrid model method was used to investigate the dimensions, attributes, and definitions of the concept. In the theoretical stage of the study, literature on nursing, politics, and other discipline were reviewed. In the fieldwork stage, individual in-depth interviews and focus groups interviews were conducted with politically seasoned experts or activists who had an understanding of the concept of political competence for extensive descriptions in nursing and field of health care.RESULTS: The concept of political competence was represented in four dimensions as political knowledge, political efficacy, political interaction, and political activity. In the political knowledge dimension, there were three attributes, namely, political knowledge, political information and systematic analysis ability. The political efficacy dimension had three attributes of internal political efficacy, external political efficacy, and self-pride of nursing profession. The political interaction dimension had three attributes of organizations and community service, networking, and persuasive power. The political activity dimension had six attributes of political leadership, political expression, assertive behavior, political advocacy, political participation, and policy intervention.CONCLUSION: This concept development might provide a basic understanding of developing a measurement tool and for constructing a theory promoting nurses' political competence.

2.Rapid Antigen Detection Test for Diagnosis of Bacterial Pharyngitis

Pharyngitis is a prevalent disease of the upper respiratory tract that requires treatment with an antibiotic. Group A streptococci (GAS) are the most frequent etiologic agents of bacterial pharyngitis. Because GAS are susceptible to penicillin, routine antibiotic susceptibility testing is not needed. Generally, patients with bacterial pharyngitis have high fever, cervical lymphadenopathy and tenderness, and tonsillar exudative discharge without symptoms of the common cold (e.g., cough, rhinorrhea, and sneezing). However, differentiating bacterial pharyngitis from viral pharyngitis based only on their clinical manifestations is problematic. Therefore, a bacterial culture or a rapid antigen detection test (RADT) is required for the diagnosis of bacterial pharyngitis. Although bacterial culture is the gold standard for diagnosis of bacterial pharyngitis, its accuracy is affected by the technical expertise of the technician, and there is a delay of 1–2 days before the results become available. In contrast, the sensitivity of RADT has increased to over 90%, making them suitable for screening purposes. The result of a RADT is available within 5–10 minutes, obviating the need for a second visit to obtain the results of culture. Use of a RADT would enable the optimal antibiotic to be administered earlier, reducing the overuse of antibiotics.

3.An Analysis of Pediatric Emergency Nursing Practice and Nursing Competence among Emergency Department Nurses

PURPOSE: The purpose of this study was to examine the frequency, perceived importance and competence of pediatric emergency nursing practice (PENP) in nurses who cared for pediatric patients in the emergency department. METHODS: This cross-sectional descriptive study analyzed 175 emergency department nurses caring for pediatric patients at 7 university hospitals with more than 500 beds, located in Seoul and Gyeonggi Province. The measurement tool was modified from the Classification of Standard Nursing Activities to measure the frequency, importance of PENP, and nursing competence. It comprised 143 items in 16 domains of PENP. RESULTS: The most frequent nursing practice was the domain of ‘nursing records and environmental management’ and the least frequent practice was the ‘research and consulting’ domain. The nursing care domain perceived to be most important by nurses was ‘specialized intensive nursing care’. The nursing care domain with the highest level of nursing competence was ‘hygiene care’, and the domain with the least level of nursing competence was the ‘research and consulting’. CONCLUSION: These results will be utilized as basic data for future pediatric emergency nursing education initiatives and for establishing priorities of nursing policy to improve health care for children admitted to the emergency department.

4.Concept Analysis of Hospital Nurses' Job-esteem

PURPOSE: This study was conducted to identify and define the concept of job-esteem of the hospital nurses. METHODS: Data were analyzed using the hybrid model, introduced by Schwartz-Barcott and Kim. Data were obtained from a relevant literature review and also from the in-depth interviews of twelve nurses with clinical experience of more than 3 years. RESULTS: The concept of job-esteem of the hospital nurses was finally analyzed with 5 attributes and 13 indicators of 3 dimensions. The attributes of the personal dimension were professional self-awareness, professional competence, the attribute of organizational dimension was respect and recognition of the organization, the attributes of social dimension were social trust and respect, driving forces of job retention. CONCLUSION: The job-esteem of hospital nurses is defined as beliefs and values used to evaluate or as expectations of self in the job through which hospital nurses van find professional self-awareness and competence, identify respect and recognition of the organization, recognize the driving forces of social trust, respect and job maintenance. This study is meaningful in that the concept of job respect of hospital nurses was analyzed and basic data for the development of a job-esteem scale for hospital nurses was provided.

5.Development of the Patient-Centered Nursing Culture Scale for Hospitals

PURPOSE: This study aimed to develop a scale measuring the Patient-Centered Nursing Culture (PCNC) and provide a basic tool to improve PCNC in Korea. METHODS: A conceptual framework and construct factors were extracted through extensive literature review and in-depth interviews with nursing professionals. In total, 59 items were derived based on the pilot survey. Data were collected from 357 nurses working at general hospitals and analyzed for verifying the reliability and validity of the scale. RESULTS: Nine factors containing 54 items were extracted from the exploratory factor analysis to verify the construct validity. The nine factors were top management leadership, policy and procedure, education and training, middle management leadership, supportive teamwork, nursing workplace environment, professional competence, patient-centered nursing activity, and nurses' values. These items were verified by convergent, discriminant, and concurrent validity testing. The internal consistency reliability was acceptable (Cronbach's α=.96). CONCLUSION: The developed PCNC scale is expected to be used as the tool for the development of theory and improvement of PCNC, the empirical testing for cause and effect of PCNC, the development of interventions, education and training programs for improving PCNC, and indicators for evaluation or accreditation of hospital service quality.

6.Development and Effects of the Self-determination Improvement Program for Nursing Students' Professional Competence Enhancement

PURPOSE: The purposes of this study were to develop a self-determination improvement program for enhancing nursing students' professional competence and to evaluate effects of the intervention on nursing professionalism, communication skills, and career decision-making autonomy among nursing students. METHODS: The study was in a pre-test, post-test 1, and post-test 2 for a nonequivalent control group design. The participants were 56 students enrolled a nursing school or college (experimental group: 28; control group: 28). The self-determination improvement program was provided in 12 sessions over 4 weeks. Data was collected from August to September 2018. Data were analyzed using SPSS/WIN 25.0 program with χ² test, Fisher's exact test, the independent t-test, and repeated measurement ANOVA. RESULTS: Participants receiving the self-determination improvement program reported increased nursing professionalism, communication skills, and career decision-making autonomy that was significantly different from those in the control group. CONCLUSION: Our study findings suggest that the self-determination improvement program could be effective for enhancing career decision-making autonomy by strengthening nursing students' professionalism and communication skills. Nursing educational program based on self-determination may be helpful for nursing students who need to prepare themselves as competent nurses in future profession.

7.Factors Influencing Managerial Competence of Frontline Nurse Managers

PURPOSE: The purpose of this study was to investigate the relationship between nursing practice environment, resilience, job stress, communication skills, and managerial competence of frontline nurse managers and identify factors influencing their managerial competence. METHODS: A descriptive research was carried out with 148 frontline nurse managers in six general hospitals. From August 16 to October 7, 2016 data were collected using a questionnaire. Data were analyzed with descriptive statistics, independent t-test, One-way ANOVA, Scheffé test, Pearson correlation coefficients, and multiple regression using IBM SPSS/WIN 22.0. RESULTS: Managerial competence of frontline nurse managers was positively correlated with nursing practice environment (r=.41 p < .001), resilience (r=.45, p < .001), communication skills (r=.38, p < .001) and was negatively correlated with job stress (r=−.27, p < .001). The factors influencing managerial competence were nursing practice environment (β=.29, p=.002) and resilience (β=.28, p=.007) in that order. The input variables explained 30.8% of managerial competence. CONCLUSION: The results suggest that a policy guideline is needed to enhance managerial competence of frontline nurse managers. The policy guideline should include achieving an adequate level of nurse staffing to improve the nursing practice environment and providing frontline nurse managers with educational support and administrative assistance to increase their resilience.

9.Patient Safety Teaching Competency of Nursing Faculty.

PURPOSE: The purpose of this study was to investigate patient safety teaching competency of nursing faculty and the extent of teaching patient safety topics in the nursing curriculum. METHODS: A national survey was conducted with full-time nursing faculty in 4-year nursing schools. Regional quota sampling method was used. An online survey was sent to 1,028 nursing faculty and 207 of them were completed. Among the 207, we analyzed data from 184 participants. The revised Health Professional Education in Patient Safety Survey was used. Data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, Pearson's correlation analysis, and multiple linear regression analyses. RESULTS: The faculty's self-confidence was lower than their perceived importance of patient safety education. The mean score of teaching patient safety was 3.52±0.67 out of 5, and the contents were mostly delivered through lectures. The extent of faculty's teaching varied depending on faculty's clinical career, teaching subjects, participation in practicum courses, and previous experience of patient safety education. The significant predictors of the extent of teaching patient safety were the faculty's self-confidence in teaching patient safety (β=.39) during clinical practicum, their perceived importance of patient safety education during lectures (β=.23), and the teaching subject (β=.15). CONCLUSION: To enhance the competency of nursing faculty for effective patient safety education, a patient safety education program tailored to faculty characteristics should be developed and continuously provided for faculty. In addition, it is necessary to improve patient safety curriculum, strengthen clinical and school linkages, and utilize various education methods in patient safety education.

10.The Effects of Violence Coping Program Based on Middle-Range Theory of Resilience on Emergency Room Nurses' Resilience, Violence Coping, Nursing Competency and Burnout.

PURPOSE: The purpose of this study was to examine the effects of a violence coping program (VCP) based on Polk's middle-range theory of resilience on nursing competency, resilience, burnout, and the ability to cope with violence in nurses working in emergency rooms. METHODS: A quasi-experimental study, with a nonequivalent control group and a pretest-posttest design, was conducted. Participants were 36 nurses who worked in emergency rooms and had experienced violence; 18 nurses from D hospital and 18 nurses from C hospital were assigned to the experimental and control groups, respectively. The experimental group received the VCP twice per week for 8 weeks. RESULTS: Levels of resilience, F=59.41, p<.001, active coping behavior, χ²=33.09, p<.001, and nursing competency, F=59.41 p<.001, increased significantly and levels of passive coping behavior, χ²=22.92, p<.001, and burnout, F=52.74, p<.001, decreased significantly in the experimental group. CONCLUSION: The results suggest that the VCP could be an effective strategy for reducing burnout and improving resilience, active coping behavior, and nursing competency. Therefore, it would be a useful intervention for improving the quality of nursing care provided in emergency rooms.
