1.Integrated Optical and Magnetic Navigation for Simplified Percutaneous Transforaminal Endoscopic Lumbar Discectomy: A Novel Approach

Objective:
This study aims to evaluate the clinical benefits of the integrated optical and magnetic surgical navigation system in assisting transforaminal endoscopic lumbar discectomy (TELD) for the treatment of lumbar disc herniation (LDH).
Methods:
A retrospective analysis was conducted on patients who underwent TELD for LDH at Beijing Chaoyang Hospital, Capital Medical University from November 2022 to December 2023. Patients treated with the integrated optical and magnetic surgical navigation system were defined as the navigation-guided TELD (Ng-TELD) group (30 cases), while those treated with the conventional x-ray fluoroscopy method were defined as the control group (31 cases). Record and compare baseline characteristics, surgical parameters, efficacy indicators, and adverse events between the 2 patient groups.
Results:
The average follow-up duration for the 61 patients was 11.8 months. Postoperatively, both groups exhibited significant relief from back and leg pain, which continued to improve over time. At the final follow-up, patients’ lumbar function and quality of life had significantly improved compared to preoperative levels (p < 0.05). The Ng-TELD group had significantly shorter total operation time (58.43 ± 12.37 minutes vs. 83.23 ± 25.90 minutes), catheter placement time (5.83 ± 1.09 minutes vs. 15.94 ± 3.00 minutes), decompression time (47.17 ± 11.98 minutes vs. 67.29 ± 24.23 minutes), and fewer intraoperative fluoroscopies (3.20 ± 1.45 vs. 16.58 ± 4.25) compared to the control group (p < 0.05). There were no significant differences between the groups in terms of efficacy evaluation indicators and hospital stay. At the final follow-up, the excellent and good rate of surgical outcomes assessed by the MacNab criteria was 98.4%, and the overall adverse event rate was 8.2%, with no statistically significant differences between the groups (p > 0.05).
Conclusion
This study demonstrates that the integrated optical and magnetic surgical navigation system can reduce the complexity of TELD, shorten operation time, and minimize radiation exposure for the surgeon, highlighting its promising clinical potential.

6.Genotyping of M protein gene of group A streptococcus infections among children in Xicheng District of Beijing in 2011-2024

Objective To understand the distribution of M protein gene (emm) of group A streptococcus (GAS) infections among children in Xicheng District of Beijing. Methods Throat swab samples from scarlet fever and pharyngeal infection cases were collected in sentinel hospitals of Xicheng District, Beijing from 2011 to 2024. GAS strains were isolated and identified, and emm gene was amplified and sequenced by PCR to determine the genotype. The differences in emm genotype between different groups were compared. Results A total of 3 130 throat swab samples were collected, and 400 GAS strains were isolated, with a positive rate of 12.78%. The highest positive rate was 19.93% in 2011. The positive rate of scarlet fever (45.71%) was higher than that of pharyngeal infection (6.14%) (P<0.001). There were 391 emm gene positive strains, and the differences in the positive rate of emm gene among different cases were statistically significant (P<0.001). A total of 7 genotypes and 27 gene subtypes were detected. Among different groups, the emm genotypes were mainly emm12 and emm1. The emm gene subtypes were mainly emm12.00 and emm1.00. Except for some years, the genotypes and their subtypes were dominated by emm12 and emm12.00, and the distribution differences of the two major genotypes and their subtypes were statistically significant from 2011 to 2019 (P<0.001). There were differences in genotypes and subtypes among different age groups (P=0.002). Conclusion The dominant types of emm genes in group A streptococcus infections among children were emm12 and emm1 in Xicheng District of Beijing from 2011 to 2024, and the dominant gene subtypes were emm12.00 and emm1.00. It is necessary to comprehensively strengthen the monitoring of the epidemic situation and genotype, timely grasp the distribution and variation of emm gene.

8.Establishment and validation of nomogram prediction model of cefoperazone/sulbactam-induced thrombocytopenia

OBJECTIVE To explore the predictive factors of cefoperazone/sulbactam-induced thrombocytopenia in adult inpatients, and to establish and validate the nomogram prediction model. METHODS Data of adult inpatients treated with cefoperazone/sulbactam in Xi’an Central Hospital from Jun. 30th, 2021 to Jun. 30th, 2023 were retrospectively collected. The training set and internal validation set were randomly constructed in a 7∶3 ratio. Singler factor and multifactor Logistic regression analysis were used to screen the independent predictors of cefoperazone/sulbactam-induced thrombocytopenia. The nomogram was drawn by using “RMS” of R 4.0.3 software, and the predictive performance of the model was evaluated by the receiver operating characteristic curve and C-index curve. Hosmer-Lemeshow goodness-of-fit test was used to evaluate the calibration degree of the model. Using the same standard, the clinical data of hospitalized patients receiving cefoperazone/sulbactam in Xi’an First Hospital in the same period were collected for external validation of the nomogram prediction model. RESULTS A total of 1 045 patients in Xi’an Central Hospital were included in this study, among which 67 patients suffered from cefoperazone/sulbactam-induced thrombocytopenia, with an incidence of 6.41%. After the false positive patients were excluded, 473 patients were included finally, including 331 in the training set and 142 in theinternal validation set. Multifactor Logistic regression analysis showed that age [OR=1.043, 95%CI (1.017, 1.070)], estimated glomerular filtration rate (eGFR) [OR=0.988,95%CI(0.977, 0.998)], baseline platelet (PLT) [OR=0.989, 95%CI(0.982, 0.996)], nutritional risk [OR=3.863, 95%CI(1.884, 7.921)] and cumulative defined daily doses (DDDs) [OR=1.082, 95%CI(1.020, 1.147)] were independent predictors for cefoperazone/sulbactam-induced thrombocytopenia (P<0.05). The C-index values of the training set and the internal validation set were 0.824 [95%CI (0.759, 0.890)] and 0.828 [95%CI (0.749, 0.933)], respectively. The results of the Hosmer-Lemeshow test showed that χ 2 values were 0.441 (P=0.802) and 1.804 (P=0.406). In the external validation set, the C-index value was 0.808 [95%CI (0.672, 0.945)], the χ 2 value of the Hosmer-Lemeshow test was 0.899 (P=0.638). CONCLUSIONS The independent predictors of cefoperazone/sulbactam-induced thrombocytopenia include age, baseline PLT, eGFR, nutritional risk and cumulative DDDs. The model has good predictive efficacy and extrapolation ability, which can help clinic identify the potential risk of cefoperazone/sulbactam-induced thrombocytopenia quickly and accurately.
9.Quality assessment of global obstructive sleep apnea guidelines

Objective·To evaluate the quality of clinical practice guidelines of obstructive sleep apnea(OSA)published worldwide.Methods·The guidelines of OSA were retrieved in PubMed,Embase,China National Knowledge Infrastructure(CNKI),Wanfang Data,SinoMed,MedSci,The Cochrane Library,and the websites such as Medlive,U.S.Preventive Services Task Force(USPSTF),National Institute for Health and Care Excellence(NICE),New Zealand Guidelines Group(NZGG),Scottish Intercollegiate Guidelines Network(SIGN),and Guidelines International Network(GIN)from establishment to December 2022.Two reviewers screened the literature and extracted the main information independently,using the Appraisal of Guidelines for Research and Evaluation Ⅱ(AGREE Ⅱ)and Reporting Items for Practice Guidelines in Healthcare(RIGHT)to evaluate the quality of the included OSA guidelines.Subgroup analysis was performed according to the publication regions of guidelines.The inter-evaluator consistency test was also performed and the results were expressed as the intra-class correlation coefficient(ICC).All the included guidelines were read entirely and the clinical questions they raised were summarized.Results·A total of 35 OSA guidelines were included.The ICC value of 0.975 showed a good inter-evaluator agreement.The results of AGREE Ⅱ showed that the average score of all guidelines was(63.60±16.45)%,with a minimum of 23.40%and a maximum of 91.67%.In the six domains,the scores of"Rigor of development"[(56.07±25.89)%]and"Applicability"[(53.57±15.52)%]were relative low.The average reporting rate of RIGHT of all the included guidelines was(67.84±20.03)%,with a minimum of 14.29%and a maximum of 94.29%,and the three domains with the lowest reporting rates were"Review and quality assurance"[(31.40±45.51)%],"Funding and conflict of interest declaration and management"[(56.43±33.95)%]and"Other aspects"[(56.19±36.85)%].Subgroup analysis showed that guidelines in Asian had a lower score in"Rigor of development"and a lower overall score of AGREE Ⅱ than the guidelines in America and Europe(both P<0.05),and the reporting rates in the domains of"Evidence"and"Other information"of RIGHT of the Asian guidelines were also lower than those in the guidelines in America and Europe(P<0.05).These guidelines focused on 42 clinical questions which were classified to 3 aspects,i.e.screening and diagnosis,treatment and long-term management of OSA.Conclusion·The quality of current global OSA guidelines varies a lot,and they need to be strengthened in terms of rigor of development,applicability,review and quality assurance,funding and conflict of interest declaration and management,especially those in Asia.
10.Different methods for treating pulmonary arteriovenous malformation under multi-disciplinary treatment model

Objective To observe the effectiveness and safety of different methods under multi-disciplinary treatment(MDT)model for treating pulmonary arteriovenous malformation(PAVM).Methods MDT were retrospectively performed in 31 patients with PAVM.The effectiveness and safety of interventional therapy,surgical treatment and conservative therapy for PAVM were compared.Results Among 31 cases of PAVM,22 cases underwent interventional therapies(interventional group),4 cases received surgical treatments(surgical group)and 5 cases underwent conservative therapies(conservative group).In interventional group,PAVM was successfully embolized in all 22 cases,with the technical success rate was 100%(22/22).Pleurisy was occurred in 3 cases(3/22,13.64%),while recurrence of PAVM was noticed in 4 cases(4/22,18.18%)during follow-up.No recurrence occurred in interventional group after the second interventional therapies.In surgical group,4 cases were successfully treated with thoracoscopic lobectomy,with the technical success rate of 100%(4/4).No postoperative complication occurred,while recurrence of PAVM was noticed in 2 cases(2/4,50.00%)during follow-up,including 1 case underwent interventional therapy and 1 case underwent conservative therapy.In conservative group,progressive PAVM was observed in 3 cases(3/5,60.00%),including 2 cases who were cured with interventional therapy and 1 case died of stroke after conservative therapy.Conclusion Individualized treatments of PAVM were feasible under MDT model.Compared with surgical treatments and conservative therapies,interventional therapies of PAVM were more effective and relatively safe.
