1.Effect of Suanzaoren Tang Combined with Ziwu Liuzhu Acupuncture on Vertebral Artery Hemodynamics, Inflammatory Cytokines, and Neurotrophic Factors in Patients with Cervical Insomnia

		                        			ObjectiveTo explore the effect of Suanzaoren Tang combined with Ziwu Liuzhu acupuncture on the vertebral artery hemodynamics, inflammatory cytokines, and neurotrophic factors in the patients with cervical insomnia with syndrome of deficiency of both heart and spleen. MethodThe random number table method was employed to assign 164 patients with cervical insomnia with syndrome of deficiency of both heart and spleen treated in the First Clinical Medical School of Guangzhou University of Chinese Medicine from January 2018 to June 2021 into a control group and an observation group. The control group was orally administrated with 1-2 mg estazolam tablets before bed for 4 weeks, and the observation group with Suanzaoren Tang combined with Ziwu Liuzhu acupuncture for 4 weeks. The therapeutic efficacy and safety were observed. The Pittsburgh Sleep Quality Index (PSQI) score, polysomnography monitoring results, hemodynamics parameters of vertebral artery, and serum levels of inflammatory cytokines and neurotrophic factors were compared before and after treatment. ResultExcept 4 dropouts, the remaining 160 patients were included in this study, with 80 patients in each group. The observation group had higher total effective rate than the control group [92.50% (74/80) vs. 80.00% (64/80), χ2=5.270, P<0.05]. Compared with that before treatment, the therapies in both groups decreased the PSQI score, sleep latency time, awakening time, awakening times, serum levels of interleukin-1β (IL-1β), C-reactive protein (CRP), and tumor necrosis factor-α (TNF-α) (P<0.01). Meanwhile, they increased the proportion of rapid-eye-movement (REM) sleep, the diastolic blood flow velocity (Vd), systolic blood flow velocity (Vs), and mean blood flow velocity (MFV) of vertebral artery, as well as the serum levels of brain-derived neurotrophic factor (BDNF) and glial cell-derived neurotrophic factor (GDNF) (P<0.05, P<0.01). Moreover, the observation group had lower PSQI score, sleep latency time, awakening time, awakening times, and serum IL-1β, CRP, and TNF-α levels (P<0.01) and higher proportion of REM sleep, Vd, Vs, MFV of vertebral artery, and serum BDNF and GDNF levels (P<0.05, P<0.01) than the control group. ConclusionZiwu Liuzhu acupuncture combined with Suanzaoren Tang can improve blood circulation of vertebral artery, reduce the serum levels of inflammatory cytokine, and increase the serum levels of neurotrophic factors to improve the sleep quality of the patients with cervical insomnia with syndrome of deficiency of both heart and spleen. 
		                        		
		                        		
		                        		
		                        	
2. Discussion on several statistical problems in establishing quality standards of standard decoctions

		                        			 Since 2016, a number of studies have been published on standard decoctions used in Chinese medicine. However, there is little research on statistical issues related to establishing the quality standards for standard decoctions. In view of the currently established quality standard methods for standard decoctions, an improvement scheme is proposed from a statistical perspective. This review explores the requirements for dry matter yield rate data and index component transfer data for the application of two methods specified in “Technical Requirements for Quality Control and Standard Establishment of Chinese Medicine Formula Granules,” which include the average value plus or minus three times the standard deviation (X-±3SD) or 70% to 130% of the average value (X-±30%X-). The square-root arcsine transformation method is used as an approach to solve the problem of unreasonable standard ranges of standard decoctions. This review also proposes the use of merged data to establish a standard. A method to judge whether multiple sets of standard decoction data can be merged is also provided. When multiple sets of data have a similar central tendency and a similar discrete tendency, they can be merged to establish a more reliable quality standard. Assuming that the dry matter yield rate and transfer rate conform to a binomial distribution, the number of batches of prepared slices that are needed to establish the standard decoction quality standard is estimated. 3.Expressions of desmoglein 1 and desmoglein 3 in primary keratinocytes and two keratinocyte cell lines HaCaT and A431

		                        			Objective To detect the mRNA and protein expressions of desmoglein 1 (DSG1) and DSG3 in different types of keratinocytes (KCs).Methods Two keratinocyte cell lines HaCaT and A431,as well as primary keratinocytes from human abdomenal skin served as the object of this study.Direct immunofluorescence assay was performed to observe and quantify the expressions of DSG1 and DSG3,and quantitative PCR (qPCR) to determine the mRNA expressions of DSG1 and DSG3,in these cells.Results Both DSG1 and DSG3 were expressed in all the three types of keratinocytes,and the fluorescence intensity of DSG1 and DSG3 in HaCaT cells was higher than that in primary keratinocytes but lower than that in A431 cells.Similarly,all the keratinocytes expressed DSG1 and DSG3 mRNA,with the relative expression levels of DSG1 and DSG3 mRNA in primary keratinocytes being 291.7% and 237.4% of those in HaCaT cells respectively (both P < 0.01),and those in A431 cells being 0.1% and 18.8% of those in HaCaT cells respectively (both P < 0.05).Conclusions HaCaT cells,A431 cells and primary keratinocytes all can be used for the study of DSG1 and DSG3,of which,A431 cells show the strongest expressions of DSG1 and DSG3,and primary keratinocytes display the highest expressions of DSG1 and DSG3 mRNAs.
		                        		
		                        		
		                        		
		                        	
4.Clinical analysis of systemic sclerosis patients with pulmonary arterial hypertension

		                        			Objective To investigate the clinical features of systemic sclerosis( SSc)patients with pulmonary hypertension(PAH)ane its treatment approach ane prognosis. Methods The clinical information of 16 SSc patients with PAH(PAH group)were recoreee. Seventy-four SSc without PAH were servee as no-PAH group. Patients in PAH group were given the basic therapy inclueing oxygen therapy,anticoagulants,careiac, eiuretic,anti-rheumatic,pulmonary vasoeilator therapy. Results The rate of antinuclear antiboey in PAH ane non-PAH group were 87. 5%(14 / 16)ane 75. 7%(56 / 74),ane the eifference was statistically significant(P= 0. 508). Serum albumin,erythrocyte seeimentation rate at 1 h in PAH group were(32. 6 ± 4. 6)g/ L ane (48. 4 ± 29. 4)mm/ 1 h. The rate of acral lesion proteinuria,hematuria,ECG abnormal rate were 62. 5%(10 / 16),62. 5%(10 / 16),43. 8%(7 / 16),62. 5%(10 / 16)respectively in PAH group. Serum albumin, erythrocyte seeimentation rate at 1 h in PAH group were(35. 6 ± 5. 0)g/ L ane(31. 3 ± 26. 3)mm/ 1 h in non-PAH group. The rate of acral lesions,proteinuria,hematuria,ECG abnormal rate were 31. 1%(23 / 74),27. 0%(20 / 74),12. 2%(9 / 74),9. 5%( 7 / 74 ) respectively in non-PAH group. The eifferences were significant between in terms of all above ineices(P = 0. 033,0. 041,0. 018,0. 006,0. 003,0. 000). During follow-up,the eisease was in stable in 74 case of non-PAH. Among 16 case with PAH,1 mile case was lost,1 case with severe PAH eiee of severe pneumonia,pulmonary hypertension,right ventricular failure,respiratory failure,3 mile patient with PAH were with eevelopment of primary eisease ane the rest 11 cases of PAH patients were with lower interstitial lung eisease than that of the previous eetectee by chest HRCT. Oppler echocareiography measurement of pulmonary artery systolic pressure was(48. 9 ± 2. 4)mmHg before treatment ane then reeucee to(31. 5 ± 4. 5)mmHg in rest 11 cases(t = 22. 27;P = 0. 001)measuree by Doppler echocareiography. Careiac function was improvee euring followee up ane no other aeverse reactions were seen. Conclusion SSc patients merge multiple PAH show it associate with other organ eamage,ane has a poor prognosis. Early careiac Doppler ultrasoune shoule be performee in oreer to get early eiagnosis ane treatment. 5.Dermatosis papulosa nigra: a case report

		                        			A 36-year-old female presented with multiple dark brown papules on the chest and abdomen for more than 10 years,which had gradually increased in number.Physical examination revealed dozens of dark brown,flat papules measuring 1-3 mm in diameter in the chest and abdomen.Most of the lesions had a smooth surface,and some lesions gave a papilloma-like appearance,with no confluent trend.Biopsy of abdominal lesions showed mild hyperkeratosis of epidermis,acanthosis,extension of epidermal protrusions forming a reticulated appearance,horn pseudocysts in prickle cell layer,enhanced pigmentation of basal layer,and a sparse lymphocytic perivascular infiltrate in superficial dermis.A diagnosis of dermatosis papulosa nigra (DPN) was made.
		                        		
		                        		
		                        		
		                        	
6.Study on the exocellular polysaccharide of Ureaplasma urealyticum biofilm in vitro

		                        			Objective To investigate the extracellular polysaccharide distribution and components of Ureaplasma urealyticum (Uu) after biofilm having been developed in.Methods The standard serotype 3 and serotype 14 belong to biovar Parvo,and the standard serotype 4 and serotype 8 belong to biovar T960 were employed to form biofilrns in vitro.Scanning electron microscope and confocal laser scanning microscope were used to analysis the biofilms and extracellular polysaccharide.We used combination of two different labeled lectins,Canavalia ensiformis(FITC-ConA) and Erythrina cristagalli(ECA) which bind to specific polysaccharide residues to visualize extracellular polysaccharide in biofilms,and average uorescence intensity was evaluated Results All the strains can form the biofilmsin vitro.The biofilm was honeycomb-Like structures mainly,and extracellular polymeric substances accounts for majority of proportions.All the extracellular polysaccharide could be combined with FITC-ConA and ECA,and the total average fluorescence intensity of FITC-ConA was higher than ECA( P<0.001 ).Conclusion Ureaplasma urealyticum biofilm is honeycomb-like structures mainly.The extracellular polysaccharide contains,galactose,and N-acetyl glucan residual,and the glucose,mannose residual are the main components.
		                        		
		                        		
		                        		
		                        	
7.The clinical value of serum procalcitionin in the diagnosis of neonates septicemia

		                        			Objective To assess the application value of serum procalcitonin(PCT)for the differentiation between the neonates with or without septicemia.Methods PCT,C-reactive protein(CRP),blood and blood culture tests were done in 75 cases of neonatal septicemia as the therapy group,and 74 neonates without the signs of infection as the control group after the admission of our hospital One week later,the therapy group also re-examined the PTC and CRP tests.Furthermore,the results were analyzed.Results The levels of CRP(23.26±5.68)mg/L and PCT(7.61±4.53)ng/L in therapy group were dramatically reduced compared with(4.09±1.09)mg/L,(1.95±0.25)ng/L in therapy group,and(6.42±4.54)mg/L,(0.45±0.16)ng/L in the control group before therapy,the t values are 28.71,12.97 and 19.97,14.08 respectively,and p value are both less than 0.01.The positive rates of PTC,CRP and blood culture were 82.7%,48.0% 28.0% before the admition to hospital.The differences of PTC and CRP,PTC and blood culture had statistical significances(x2=5.64,P<0.05;x2=10.40,P<0.01).If 2.5 μg/L of PCT and 8mg/L of CRP would be regarded as cutoff point in our study,the sensitivity and specificity of PCT or CRP for diagnosis of neonatal septicemia was 82.7% and 80.3%,48.7% and 78.4%.Conclusion Our data suggested that the serum PTC is a valuable marker for the early diagnosis of neonatal septicemia.
		                        		
		                        		
		                        		
		                        	
8.The relationship between the serum carbohydrate antigen 125 level and the prognosis of the threatened abortion

		                        			Objective To explore the value of serum CA125 in prediction of the first-trimester threatened abortion.Methods The serum CA125 level of 78 patients with threatened abortionand( miscarriage success group and failure group) and 40 normal early pregnant women before treatment and after 1 ~ 4 weeks were tested by chemiluminescent immunoassay(CLIA) methods and the results were compared.Results The serum CA125 level of the failure group was significantly higher than that of the miscarriage success group[ (28.52 ± 19.12) x 103 U/L] and normal early pregnant women group [ (20.45 ± 9.55) × 103 U/L] ( t =- 1.28,- 1.24,all P < 0.05 ),and it increased by degree and by time,and the sensitivity and specificity of detection was 93.1% and 87.85% respectively.Conclusion Monitoring of serum CA125 had highly clinical value in predicting the prognosis of threatened abortion.
		                        		
		                        		
		                        		
		                        	
9.Efficacy and safety of two kinds of homemade sirolimus-eluting stents for treatment of acute ST segment elevation myocardial infarction

		                        			Objective To compare the efficacy and safety of two kinds of homemade sirolimus-eluting stents (Firebird and Excel) for treatment of acute ST segment elevation myocardial infarction (STEMI) in patients who underwent percutaneous coronary intervention (PCI). Methods The 249 consecutive patients with STEMI who underwent PCI were randomly divided into two groups: Excel group (n=136) and Firebird group (n=113). They were followed up for 6-24 months, and coronary angiography was reviewed average 12 months later. The primary endpoints were major adverse cardiac events, including death, reinfarction and target vessel revascularization. The second endpoints included late luminal loss and restenosis 12 months after treatment. Results There were no significant differences in baseline data, coronary arterial lesion before operation, and immediateness condition after PCI between the two groups (all P>0.05). Within follow-up, there were 2 (1.47%) death cases and 1 (0.88%) death case, 1 (0.74%) and 1 (0.88%) nonfatal myocardial infarction case, 2 (1.47%) and 2 (1.77%) target vessel revascularization cases in the two groups respectively (all P>0.05). There were no significant differences in late luminal loss of in-stent and in-segment, the rates of in-stent restenosis, in-segment restenosis and stent thrombosis, the in-stent minimal lumen diameter and in-segment minimal lumen diameter between the two groups (all P>0.05). Conclusions The two kinds of homemade sirolimus-eluting stents may have similar efficacy and safety in patients with STEMI treated with primary PCI.
		                        		
		                        		
		                        		
		                        	
10.mRNA expression of Fc gamma RⅡA on neutrophils from patients with Beh(c)et's disease

		                        			Objective To investigate the mRNA expression of Fc gamma RⅡA(FcγR ⅡA)on polymorphonuclear neutrophils(PMN)from patients with Beh(c)et's disease(BD).Methods Twenty-five patients with active BD and 20 healthy human controls were included in this study.Blood samples were obtained from all patients with active BD before treatment,from 15 patients with inactive BD after treatment and from healthy controls.PMN were isolated.The FcγR Ⅱ A mRNA expression on PMNs was detected by RT-PCR,and plasma myeloperoxidase (MPO)activity which represented neutrophil activation,was measured spectrophotometricaily.Results The relative expression level of FcγR Ⅱ A on PMN and plasma MPO aetivity were 1.80 1±0.829 and 32±5 U/L.respectively,in patients with active BD,0.820±0.625 and 27±4 U/L,respectively,in those with inactive BD,and 0.745 ±0.931 and 29±5 U/L,respectively,in normal controls;the differences were significant in the two parameters between the patients with active and inactive BD (both P<0.01),while no statistical difference was observed between inactive patients and normal human controls(P>0.05).There was a positive eorrelation between the expression level of FcγR Ⅱ A on PMN and plasma MPO activity in patients with BD(r=0.39,P<0.0 1).Conclusions The mRNA expression of FcγR ⅡA on neutrophils is up-regulated in patients with active BD.It is likely that FcγR Ⅱ A is involved in the activation of neutrophils in BD.
