1.Construction and value of a vestibular function calibration test recognition model based on dual-stream ViT and ConvNeXt architecture

Objective To improve the efficiency and accuracy of videonystagmography calibration test results while enabling effective recognition of saccadic undershoot waveform by developing a dual-stream architecture-based deep learning model. Methods A vestibular function calibration test recognition model with cross-modal feature fusion was constructed by integrating vision transformer (ViT) and a modified ConvNeXt convolutional network. The model utilized trajectory pictures and spatial distribution maps as inputs, employed a multi-task learning framework to classify calibration data, and to directly evaluate undershoot waveform. Results The model showed outstanding performance in assessing calibration compliance. The accuracy, sensitivity, specificity of the model in left side, middle, and right side were all greater than 90%, and AUC values were all greater than 0.99, with 97.66% of optimal accuracy (middle), 98.98% of optimal sensitivity (middle), 96.87% of optimal specificity (right side), and
2.Effect of transcutaneous auricular vagus nerve stimulation on tourniquet-induced hypertension in patients undergoing anterior cruciate ligament reconstruction

Objective:To evaluate the effect of transcutaneous auricular vagus nerve stimulation (taVNS) on tourniquet-induced hypertension (TIH) in the patients undergoing anterior cruciate ligament reconstruction.Methods:Seventy-four patients of either sex, aged 18-60 yr, of American Society of Anesthesiologists Physical Status classification I or II, with body mass index of 18-30 kg/m 2, undergoing elective anterior cruciate ligament reconstruction under general anesthesia combined with preoperative femoral nerve block, were divided into 2 groups ( n=37 each) using a random number table method: sham stimulation group (group SS) and group taVNS. Group SS received stimulation on the ear lobe and the tail of the helix of the left ear. Group taVNS received stimulation on the cymba concha and the earlobe of the left ear. Both groups received stimulation from 1 h before induction of anesthesia until the end of the procedure (frequency of 30 Hz, pulse width of 300 μs, and amplitude of the strongest current that could be tolerated by the patient in the absence of pain). The tourniquet inflation pressure was 280 mmHg, with an inflation time of 60-90 min. Systolic blood pressure, diastolic blood pressure and heart rate were recorded before tourniquet inflation to assess the development of intraoperative TIH. The consumption of intraoperative propofol, remifentanil, nitroglycerin, esmolol, norepinephrine and atropine was recorded, and the occurrence of postoperative nausea and vomiting, skin itching and headache and dizziness was also recorded. Results:Compared with group SS, the incidence of TIH and the number of patients used nitroglycerin were significantly reduced ( P<0.05), and no significant changes were found in the other parameters in group taVNS ( P>0.05). Conclusions:taVNS can decrease the occurrence of TIH in the patients undergoing anterior cruciate ligament reconstruction.
3.Analysis of the status quo and influencing factors of social alienation in young adults undergoing radioactive iodine therapy for papillary thyroid carcinoma

Objective:To understand the status quo of social alienation in young adults undergoing radioactive iodine (RAI) therapy for papillary thyroid carcinoma (PTC) , analyze its influencing factors, and provide references for the formulation of clinical nursing interventions.Methods:Totally 206 young adults with PTC who underwent RAI therapy at the First Affiliated Hospital of Zhengzhou University from January 2021 to May 2022 were selected by convenience sampling. These patients were surveyed using a general information questionnaire, General Alienation Scale (GAS) , Social Support Rating Scale (SSRS) , and Hospital Anxiety and Depression Scale (HADS) . Multiple linear regression analysis was utilized to analyze the influencing factors of social alienation in young adults undergoing RAI therapy for PTC.Results:The score of social alienation in the young adults undergoing RAI therapy for PTC was (39.39±4.93) . Univariate analysis results indicated that there was a statistically significant difference in the scores of social alienation among the young PTC patients with different marital statuses, levels of disease awareness, employment statuses, family per capita monthly incomes, and tumor stages ( P<0.05) . Multiple linear regression analysis revealed that the influencing factors of social alienation included the level of disease awareness, family per capita monthly income, tumor staging, social support score, and anxiety and depression score ( P<0.05) . Conclusions:The overall level of social alienation in young adults undergoing RAI therapy for PTC is moderate. Healthcare providers should timely assess and adopt targeted intervention measures to enhance patients' sense of social participation.
4.Effect of Internet-based health education in patients with rheumatoid arthritis

Objective:To explore the effect of health education based on the Internet in patients with rheumatoid arthritis (RA) .Methods:From January 2019 to January 2020, convenience sampling was used to select 296 patients with RA in Xinxiang Municipal Central Hospital. According to the random number table method, the patients were divided into the control group and the observation group, each with 148 cases. The control group conducted routine health education, and the observation group implemented health education based on the Internet. The treatment compliance, Chronic Disease Self-Management Behavior Scale, the Medical Outcomes Study 36-Item Short Form Health Survey (SF-36) , and Disease Activity Score 28 (DAS28) scores were compared between the two groups after intervention.Results:The treatment compliance of the observation group was 46.62% (69/148) , which was higher than 12.84% (19/148) of the control group, and the difference was statistically significant ( P<0.05) . After the intervention, the self-management score and scores of all dimensions of the observation group were higher than those of the control group, and the SF-36 score was higher than that of the control group, and the DAS 28 score was lower than that of the control group, and the differences were statistically significant ( P<0.05) . Conclusions:Health education based on the Internet can improve the treatment compliance, self-management ability and quality of life of RA patients, and reduce the disease activity.
5.Effect of disease-based intelligent fusion carrier APP in patients with rheumatoid arthritis

Objective:To explore the effect of disease intelligent fusion carrier based on the Smart Assistant for Rheumatism (SSDM) APP and the Fengyun Assistant APP of the National Rheumatology Data Center (CRDC) in patients with rheumatoid arthritis.Methods:From September 2020 to September 2021, a total of 186 rheumatoid arthritis patients admitted to Xinxiang Central Hospital were enrolled using convenient sampling method, and divided into the control group and the observation group with 93 cases each by the random number table method. The control group received CRDC APP management, and the observation group carried out disease intelligent fusion carrier management based on SSDM APP and CRDC APP. The self-management ability score, compliance score, and disease activity degree were compared between the two groups.Results:One month after intervention and three months after intervention, the scores of compliance and self-management ability of the the observation group were higher than those of the control group, and the differences were statistically significant ( P<0.05) . Conclusions:The disease intelligent fusion carrier management based on SSDM APP and CRDC APP can help rheumatoid arthritis patients improve self-management awareness and medical compliance behavior.
6.Summary of best evidences for perioperative hydration in prevention of contrast-induced acute kidney injury in patients undergoing coronary intervention

Objective:To search and summarize the relevant evidences for perioperative hydration in prevention of contrast-induced acute kidney injury (CIAKI) in patients undergoing coronary intervention, and to provide evidence-based basis for clinical nursing staff to develop personalized perioperative hydration plan and prevent CIAKI.Methods:Evidence-based nursing was used to search evidences for perioperative hydration in prevention of CIAKI in patients undergoing coronary intervention in UpToDate, National Guideline Clearing-house (NGC) , European Society of Cardiology (ESC) , JBI Evidence Summary Database, Cochrane Database, PubMed, Wanfang Database, China National Knowledge Infrastructure (CNKI) and China Biomedical Literature Database (CBM) , including evidence summary, guidelines, expert consensus and systematic reviews. The search time was from the establishment of the database to February 2019. Two researchers evaluated the quality of included literatures and extracted evidence from literatures that met the quality standards.Results:A total of 9 articles of evidence were included, including 1 evidence summary, 3 guidelines, 3 expert consensus and 2 systematic evaluations. Finally, a total of 10 pieces of evidence from 5 aspects, including CIAKI risk assessment, hydration fluid type, hydration path and timing, hydration rate and urine volume monitoring, were summarized.Conclusions:This study summarizes the best evidence for perioperative hydration in prevention of CIAKI in patients undergoing coronary intervention and provides an evidence-based basis for clinical medical staff.
7.Decannulation of dysphagic patients after a tracheotomy

Objective:To explore the value of flexible endoscopic evaluation of swallowing (FEES) in guiding the decannulation of neurological disease patients with dysphagia after a tracheotomy.Methods:The FEES results of 188 neurological disease patients with dysphagia who had undergone a tracheotomy were analyzed retrospectively. The utility of FEES evaluation indexes (including glottis activity, the classification of pharyngeal secretions and residues as well as penetration-aspiration grade) for predicting the success of decannulation was explored.Results:One hundred and nine of the patients (the success group) were decannulated successfully and 79 (the failure group) were not, a success rate of 57%. The abnormal glottis activity rate among the failure group was 55%, significantly higher than among the success group (21%). The pharyngeal secretion classifications and penetration-aspiration grades among the success group were also significantly lower than among the failure group, on average. The average course of recovery from the tracheotomy was 184 days in the success group, significantly shorter than that of the failure group (292 days). No significant differences in residues were observed.Conclusion:The glottis activity, secretions and intake aspiration evaluated using FEES are of great value for guiding the decannulation of neurological disease patients with dysphagia after a tracheotomy.
8.DNA assembly technologies: a review.

DNA assembly is the core technology of synthetic biology. With the development of synthetic biology, researchers have developed different DNA assembly technologies that rely on DNA polymerase or DNA ligase, and also have developed some non-enzyme-dependent DNA assembly techniques to facilitate the automation of DNA assembly. The assembly of large fragments of DNA from a few hundred kb to Mb is mostly dependent on microbial recombination. In this paper, the three types of DNA assembly technologies, including enzyme-dependent, non-enzymatic and in vivo homologous recombination, are reviewed.

9.Research progress of swallowing function assessment among patients after partial laryngectomy

10.Genotype distribution of HPV infection among women in Changzhou

Objective To observe the human papillomavirus(HPV) prevalence and its distribution character among females in Changzhou and to provide epidemiological data for the prevention of cervical cancer and development of HPV vaccine .Methods The clinical material consisted of 32215 women ,who were from Changzhou Hospital T .C .M ,Changzhou Maternal and Child Health Care ,The Second People's Hospital of Changzhou and Changzhou Wujin Hospital from January 2014 to June 2016 .17 kinds of high-risk subtypes (HPV16 ,18 ,31 ,33 ,35 ,39 ,,45 ,51 ,52 ,53 ,56 ,58 ,59 ,66 ,68 ,73 ,82) and 6 kinds of low-risk subtypes (HPV6 ,11 ,42 ,43 ,81 ,83) were detected .All cases were divided into six groups according to the age (<20 ;>21 to 30;31 to 40 ;41 to 50 ;51 to 60 ;and>61) and the age distribution of women infected with HPV and genotypes of HPV were analyzed .Results Out of 32215 cervical cell specimens ,8396 cases were detected HPV infections with the position rate of 26 .06% .In seventeen types of high-risk HPV ,HPV 16 (4 .55% ) were the most common subtype ,followed by 52 (4 .43% ) ,58 (2 .75% ) ,53 (2 .42% ) ,51 (2 .16% ) ,68 (1 .85% ) ,56 (1 .73% ) ,33 (1 .40% ) ,18 (1 .38% ) ,66 (1 .33% ) ,59 (1 .26% ) ,39 (1 .08% ) ,31 (1 .02% ) ,35 (0 .89% ) ,45 (0 .42% ) ,73 (0 .26% ) ,and 82 (0 .15% ) .Six types of low-risk HPV were detected ,of which HPV 81 (2 .96% ) was the most common subtypes ,followed by 43 (1 .86% ) ,42 (1 .62% ) ,6 (1 .27% ) ,11 (0 .80% ) and 83 (0 .26% ) .Conclusion The predominant genotypes of HPV 16 and ,52 infection in female reproductive tract are the highest .
