1.Pathogenesis of T2DM with cognitive impairment in Baotou area and the improvement effect of GLP-1 receptor agonists

Objective To analyze the onset characteristics of type 2 diabetes mellitus (T2DM) with cognitive impairment in Baotou area, and study the improvement effect of GLP-1 receptor agonists. Methods A total of 320 patients with T2DM admitted between September 2021 and September 2022 were selected and divided into the observation group with T2DM and cognitive dysfunction and the negative control group without cognitive dysfunction according to their cognitive function status , Among the 160 cases in each group; Patients with type 2 diabetes and cognitive impairment were randomly divided into a treatment group and a control group, 80 cases in each group; the control group was treated with conventional treatment, and the treatment group was additionally treated with semaglutide; Logistics multiple regression model was used to analyze T2DM The related risk factors of cognitive impairment in patients were assessed by the Mini-Mental State Examination (MMSE) score to evaluate the cognitive function of the patients. Results Multivariate regression model showed that course of disease, age, vitamin D, HbA1c, LDL-C, BMI, Hcy, Lp-PLA2, TNF-α, IL-6 and folic acid levels were also independent risk factors for cognitive impairment in T2DM patients (P<0.05); There was a significant positive correlation between GLP-1 receptor agonists and cognitive function recovery in T2DM patients with cognitive impairment (P<0.05). Conclusion The onset of T2DM with cognitive impairment in Baotou area is often accompanied by a long course of disease, older age, abnormal levels of vitamin D, HbA1c, LDL-C, BMI, Hcy, Lp-PLA2, TNF-α, IL-6 and folic acid, and GLP -1 receptor agonists have a clear role in improving the cognitive function of patients.
2.Effect of Banxia Xiexintang-containing Intestinal Absorption Solution on Migration and Invasion of PMN-MDSCs in Gastric Cancer Microenvironment

ObjectiveTo observe the effect of Banxia Xiexintang containing intestinal absorption solution (BXCIAS) on migration and invasion of polymorphonuclear myeloid-derived suppressor cells (PMN-MDSCs) in gastric cancer microenvironment. MethodThe complex solution (containing 0.63 g·mL-1 crude drug) was prepared. Gastric cancer cells were subjected to non-contact co-culture with PMN-MDSCs in Transwell chamber to create gastric cancer microenvironment. Cell counting kit-8 (CCK-8) assay was used to screen the optimal intervention concentration and time of BXCIAS on PMN-MDSCs for subsequent experiment. The blank group, model group, FAK inhibitor group, and BXCIAS groups (26%, 18%, and 10%) were designed. Scratch assay and Transwell assay were employed to detect the migration and invasion ability of PMN-MDSCs, and enzyme-linked immunosorbent assay (ELISA) to measure the expression of vascular endothelial cell growth factor (VEGF) and matrix metalloproteinase-9 (MMP-9) in tumor microenvironment. The expression levels of PMN-MDSCs pathway-related proteins FAK, phosphorylated (p)-FAK, protein tyrosine kinase (Src), and p-Src were detected by Western blot. ResultThe inhibition rates of PMN-MDSCs by 5%, 50%, 75%, and 100% BXCIAS at 48 h were higher than those at 24 h (P<0.05, P<0.01). The inhibition rate of PMN-MDSCs by 50% BXCIAS at 72 h was lower than that at 48 h (P<0.01), and the inhibition rates by 5% and 100% BXCIAS at 72 h were higher than those at 48 h (P<0.05, P<0.01). There was no significant difference in the inhibition rate by other concentration levels at 48 h. The half-maximal inhibitory concentration (IC50) at 48 h was 18.09%, indicating that 18% BXCIAS and 48 h were the optimal concentration and time, respectively. The migration distance of PMN-MDSCs was large (P<0.01), and the number of migrating and invading cells increased (P<0.01) in the mode group compared with those in the blank group. Compared with model group, FAK inhibitor and BXCIAS at different concentration decreased the migration distance of PMN-MDSCs (P<0.01), and the number of migrating and invading cells (P<0.01), especially the 26% BXCIAS (P<0.01). The expression of PMN-MDSCs pathway-related proteins FAK, p-FAK, Src and p-Src (P<0.01) and the expression of VEGF and MMP-9 (P<0.01) were higher in the model group than in the blank group. Compared with model group, FAK inhibitor and BXCIAS (26%, 18%, 10%) decreased the expression of FAK, p-FAK, and Src (P<0.01), and FAK inhibitor and 18% BXCIAS reduced the expression of p-Src (P<0.01), and the expression of VEGF and MMP-9 (P<0.01). ConclusionBXCIAS can inhibit the migration and invasion of PMN-MDSCs by down-regulating the expression of FAK, p-FAK, Src, and p-Src proteins in the FAK signaling pathway of PMN-MDSCs in gastric cancer microenvironment.
3.Effect of Banxia Xiexintang-containing Intestinal Absorption Solution on PMN-MDSCs Apoptosis in Gastric Cancer Microenvironment

ObjectiveTo observe the effect of Banxia Xiexintang (BXT)-containing intestinal absorption solution on the apoptosis of polymorphonuclear myeloid-derived suppressor cells (PMN-MDSCs) in gastric cancer microenvironment. MethodBXT-containing intestinal absorption solution was prepared, and gastric cancer cells and PMN-MDSCs were non-contact co-cultured in Transwell chamber to establish gastric cancer microenvironment. Cell counting kit-8 (CCK-8) assay was used to screen the optimal intervention concentration and time of 0-100% BXT-containing intestinal absorption solution prepared by 0.63 g·mL-1 reconstitution solution. Cells were classified into blank group, model group, oxaliplatin group (10 mg·L-1), and BXT (26%, 18%, 10% BXT-containing intestinal absorption solution) group, and the apoptosis of PMN-MDSCs was detected by flow cytometry. The expression of apoptosis-related B-cell lymphoma 2 (Bcl-2), Bcl-2-associated X protein (Bax), and cysteine-aspartic acid protease-3 (Caspase-3) in PMN-MDSCs was detected by Western blot. ResultAfter treatment for 24 h and 48 h, the PMN-MDSCs-inhibiting rate was increased by 5%, 50%, 75%, and 100% BXT-containing intestinal absorption solution compared with that in the blank group (P<0.05, P<0.01). At 72 h, the PMN-MDSCs-inhibiting rate by 50% BXT-containing intestinal absorption solution was lower than that at 48 h (P<0.01), and the PMN-MDSCs-inhibiting rate by 5%, 75%, and 100% BXT-containing intestinal absorption solution showed no significant difference from that at 48 h. Moreover, the half-maximal inhibitory concentration (IC50) at 48 h was 18.40%. Thus, 18% BXT-containing intestinal absorption solution and 48 h were the optimal intervention concentration and time. The survival rate of PMN-MDSCs in model group was higher than that in the blank group (P<0.05), and the apoptosis rate was lower than that in the blank group (P<0.05). Compared with model group, BXT containing intestinal absorption solution lowered the survival rate and raised apoptosis rate of PMN-MDSCs (P<0.05), particularly the 26% BXT-containing intestinal absorption solution (P<0.05). The expression of Bax and Caspase-3 in PMN-MDSCs was lower in the model group than in the blank group (P<0.05), and the expression of Bcl-2 was higher in the model group than in the blank group (P<0.05). The expression of Caspase-3 in PMN-MDSCs increased (P<0.05) and the expression of Bcl-2 decreased (P<0.05) in oxaliplatin group and BXT group compared with those in the model group. The expression of Bax rose in oxaliplatin group and BXT group (10% BXT-containing intestinal absorption solution) (P<0.05). ConclusionBXT can induce the apoptosis of PMN-MDSCs by regulating the expression of apoptosis-related proteins Bax, Caspase-3, and Bcl-2 in gastric cancer microenvironment.
4.Mechanism of Banxia Xiexintang in Prevention and Treatment of Gastric Cancer: A Review

Gastric cancer (GC) is one of the common malignant tumors, and the incidence and mortality of GC in China rank first in the world. At present, the pathogenesis of GC has not been fully clarified. Although surgery, radiotherapy, chemotherapy, targeted therapy, and immunotherapy have achieved good results in the treatment of GC, there are still many complications, decreased sensitivity, and severe side effects. Banxia Xiexintang, derived from Treatise on Cold Damage and Miscellaneous Diseases(《伤寒杂病论》), has been clinically used for more than 2000 years with the effects of combining cold and warm drugs, dissipating mass, and relieving stuffiness, and is a classic prescription for treating digestive tract diseases in later generations. Through clinical observation and experimental research, it is found that Banxia Xiexintang and its single drugs have good effect in preventing and treating GC. Chinese medicine has multi-component and multi-target characteristics and can treat GC through various mechanisms. Therefore, it is necessary to carry out systematic and in-depth research from the aspects of molecular biology and network pharmacology, and comprehensively reveal the mechanism of Banxia Xiexintang in preventing and treating GC. At present, the mechanism of Banxia Xiexintang in treating GC mainly focuses on inducing apoptosis of GC cells, inhibiting proliferation, migration, and invasion of GC cells, protecting peritoneal mesothelial cells, inhibiting peritoneal metastasis of GC cells, regulating GC microenvironment, and inhibiting the malignant transformation of bone marrow mesenchymal stem cells (BMSCs). This research group is committed to the prevention and treatment of GC with Banxia Xiexintang, aiming to comprehensively reveal the mechanism of action and the pharmacodynamic material basis of Banxia Xiexintang in the prevention and treatment of GC, and provide an important scientific basis for further clinical application of Banxia Xiexintang. After searching CNKI, PubMed, Wanfang Data, VIP, and other databases, this paper summarized Banxia Xiexintang in the treatment of GC from the aspects of prescription basis, material basis, network pharmacology, clinical and experimental studies, etc., so as to provide references for further research on pharmacological effect of Banxia Xiexintang and its application in the clinical treatment of GC.
5.Analysis of the disease spectrum of the patients in the pediatric emergency department of the grade A of level Ⅲ women and children′s hospital in Guangzhou area

Objective:Based on 108, 591 cases of pediatric emergency visits in a Level Ⅲ Grade A women and children′s hospital in Guangzhou area, we analyze the disease spectrum and epidemiological characteristics, and summarize the characteristics of patient flow changes.These investigations will provide an basis for scientific decision-making for manpower and material resource management of pediatric emergency and hospital workflow design.Methods:The children admitted to the Pediatric Emergency Department of the Zhujiang New Town District of Guangzhou Women and Children′s Medical Center from October 2016 to September 2018, including night emergency and inpatient observations, were analyzed according to the admission date, admission time, gender, age, initial diagnosis and etc.Results:There were more boys than girls in the emergency department, whose ratio was 1.46∶1 (64 480∶44 111 cases). The age of children ranged from 0 to 17 years old, with a median of 11 (23, 48) months.The age distribution was mainly under 5 years old, accounting for 84.14% (91 336/108 591). During the whole year, the number of children in July was the most, accounting for 10.53% (11 433/108 591), and the children in February were the least, accounting for 6.04% (6 555/108 591). The highest visit time of the whole day was 22-23 pm, accounting for 18.83% (20 443/108 591). The most of the diagnosis was respiratory disease, accounting for 53.83% (66 522/123 576). A total of 1 057 critically ill children were received, accounting for 0.97% (1 057/108 591). A total of 911 accidental injuries were received, accounting for 0.84% (911/108 591). Acute upper respiratory tract infection was the most among all diagnoses, accounting for 34.47% (42 541/123 576).Conclusion:Children in the pediatric emergency department of Guangzhou Women and Children′s Medical Center are mainly under 5 years old, and the number of children is the most in July of the year.The main disease is respiratory diseases.Medical staff can be trained according to the actual situation, and the disease spectrum can be updated in time to provide convenience for emergency rescue and improve service level.
6.Clinical Feature Analysis of 525 Cases of Patients with Ulcerative Colitis

Objective: To conduct retrospective analysis of clinical features of patients with ulcerative colitis in JiangsuProvince Hospital of Traditional Chinese Medicine (TCM) . Methods: Ulcerative colitis medical records in jiangsuprovince hospital of TCM were selected and used to retrospectively analyze the feature such as gender, age, course of thedisease, clinical type, stage, severity, and pathological parts. Results: The study include 290 cases of male and 235 casesof female with the ratio of 1.2:1. Peak incidence was between 20 to 60 years old. Dietary factor was the most commoncause, followed by fatigue, environment, medicine and emotional factors. There were 28 (5.33%) patients at remissionstage, and 497 (94.67%) patients at active stage. In patients at active stage, there were 78 (15.69%) patients newlydiagnosed, and 419 (84.31%) patients with chronic recurrent type. There 92 (18.51%) rectum type patients, 156 (31.39%) left half colon type patients and 249 (50.10%) extensive colon type patients. There were 275 (55.33%) mild patients, 149 (29.98%) moderate patients and 73 (14.69%) severe patients. The main symptoms were bloody purulent stool, diarrheaand abdominal pain. There were 375 (71.43%) patients with bloody purulent stool, and symptom disappearance rate was89.07% after treatment. There were 335 (63.81%) patients with diarrhea, and symptom disappearance rate was 71.64%after treatment. There were 319 (60.76%) patients with abdominal pain, and symptom disappearance rate was 85.89%after treatment. There were 3 cases of complications. Drug therapy was mainly based on integrated Chinese and westernmedicine (86.29%) . For Chinese medicine treatment, there were 453 patients (87.45%) with oral Chinese medicine, 401 patients (77.41%) with Chinese medicine enema, and 10 patients (1.93%) with foot bath. For western medicine treatment, there were 392 patients (75.68%) treated with oral amino salicylate drugs, 100 patients (19.31%) with anal used aminosalicylate drugs, 88 patients (16.99%) with glucocorticoid, 18 patients (3.47%) with immunosuppressive agent and 441 patients (85.14%) with microecological preparation. Conclusion: The types of ulcerative colitis patients in our hospitalwere mainly chronic recurrent type at active stage with mild to moderate and extensive colon type. The treatment wasmainly based on integrated Chinese and western medicine, with significant improvement in symptoms of bloody purulentstool, diarrhea and abdominal pain. 99.61% patients were discharged with a better condition
7.Cold Saponification Method for Determination of 5 Lutein Stereoisomers in Dairy Products by High Performance Liquid Chromatography

A cold saponification method for determination of 5 lutein stereoisomers in dairy products by high performance liquid chromatography ( HPLC ) was developed. Samples were cold-saponified at room temperature and extracted by n-hexane/petroleum/dichloromethane ( 2: 2: 1 , V/V/V ) . Then 5 lutein stereoisomers were separated on a YMC C30 column with gradient elution using methanol/methyl tert-butyl ether as the mobile phase, and data were acquired by a photodiode array detector at wavelength of 445 nm. The calibration curve was linear in the range of 0 . 127-5 . 082 mg/L with correlation coefficient of 0 . 9999 , and the recoveries were from 96 . 7% to 102 . 2% with the RSDs in the range of 4 . 1%-5 . 4% ( n=6 ) . The limit of detection was 0 . 010 μg/g ( S/N=3 ) , and the limit of quantification was 0 . 030 μg/g ( S/N=10 ) . By presenting results of good accuracy, precision and sensitivity, this method validates its suitability for routine analysis of 5 lutein stereoisomers in dairy products.

This study was aimed to discover core agent for the treatment of ulcerative colitis and explore the medication rules . A total of 525 ulcerative colitis medical records in the Jiangsu Province Hospital of TCM were selected from 2009 to 2013 . The records were input into the structured information acquisition system of clinical diagnosis and treatment . The complex network analysis was used to analyze core drugs of prescription and drug compatibility after data mining and rule processing . The results showed that the core drugs are Diyu , Huanglian, Muxiang, Baishao, Xianhecao, Danggui, Chaobaizhu, Huangqin, Zicao, Yiyiren, Fuling, Shanyao. It was concluded that data mining can be an objective method in the analysis of core drugs and compatibility in the treatment of ulcerative colitis. It can also be used to guide the clinical prescription medication.
9.The relation between gastroesophageal reflux disease with or without esophageal injury and esophageal motility and acid reflux

Objective To study the relation of esophageal motility and acid reflux between gastroesophageal reflux disease (GERD) patients with and without esophageal injury.Methods A total of 25 patients who met the GERD diagnosis criteria were conducted esophageal-gastric-duodenal endoscopy examination and followed with high-resolution esophageal manometry.The changes of esophageal pH postprandial were also detected.The differences of esophageal motility and acid reflux time between GERD patients with and without esophageal injury were compared.Patients were divided into non-acid reflux group and acid reflux group according to DeMeester score.The changes of esophageal motility indexes of these two groups were compared.Wilcoxon test and t test were used for statistical analysis.Results There was no significant difference in baseline value of lower esophageal sphincter (LES) pressure,remained LES pressure,speed of the esophageal peristalsis,the time of esophageal pH<4 and DeMeester score between groups with and without esophageal injury (all P>0.05).But esophageal contraction amplitude of group with esophageal injury was lower than that of group without esophageal injury (31.9 mm Hg vs 64.2 mm Hg,1 mm Hg=0.133 kPa,Z=-2.37,P=0.02).There was no significant difference in baseline value of LES pressure,remained LES pressure,speed of the esophageal peristalsis between acid reflux group and non acid reflux group (all P>0.05).But the esophageal contraction amplitude of acid reflux group was lower than that of non acid reflux group (36.4 mm Hg vs 71.8 mm Hg,Z=-2.25,P=0.02).Conclusions There was no significant correlation between the LES pressure and esophageal acid reflux with esophageal injury in GERD patients.Esophageal peristalic contraction may be associated with esophageal injury and acid reflux.
10.Effects of Scutellaria baicalensis stem-leaf total flavonoid on proliferation of vassel smooth muscle cells stimulated by high triglyceride blood serum.

OBJECTIVETo determine the effects and related mechanism of Scutellaria baicalensis stem-leaf total flavonoid (SSTF) on vascular smooth muscle cells (VSMCs) proliferation induced by high triglyceride blood serum (HTG).
METHODVSMCs isolated from rat aorta were cultured in vitro and proliferation was stimulated by HTG, SSTF was added to influence the proliferation of VSMCs. The proliferation and cell cycle of VSMCs were determined by MTT assay and flow cytometry, respectively. CO released into the culture media was quantitated by measuring carbon monoxide hemoglobin (COHb). The protein expressions of HO-1 and extracellular signal-regulated kinase (ERK/p-ERK) were detected by Western blot analysis.
RESULT500 mg x L(-1) SSTF could obviously suppress the cell multiplication by HTG's induction, remarkably increase the production of COHb in VSMCs, obviously suppresse the mitotic cycle progress of VSMCs (P < 0.05, P < 0.01), in the time and dosage dependence. Furthermore, 500 mg x L(-1) SSTF remarkably declined the ERK/p-ERK protein expression (P < 0.01), but did not have the influence on the HO-1 protein's expression.
CONCLUSIONSSTF inhibited the proliferation of VSMCs directly by blocking cell cycle progression, and the ERK signal transduction way possibly participated in the cytoprotection of SSTF.
