1.Marginal Zone Lymphoma with Recurrent Intestinal Obstruction After Multiple Chemotherapy: A Case Report

This article reports a diagnostically and therapeutically challenging case of small intestinal marginal zone lymphoma. The patient presented with recurrent abdominal pain as the chief complaint, and imaging revealed multifocal small bowel wall thickening with high uptake, multisegmental luminal stenosis, and proximal dilation. Initial diagnostic workup, including gastroscopy, colonoscopy, and enteroscopy with biopsy, failed to establish a definitive diagnosis. Empirical anti-tuberculosis therapy was ineffective. A repeat enteroscopic biopsy performed over eight months after symptom onset eventually confirmed the diagnosis of mucosa-associated lymphoid tissue (MALT) extranodal marginal zone lymphoma. Despite three different chemotherapy regimens, the patient's intestinal obstruction symptoms persisted, with imaging still showing multifocal bowel wall thickening and hypermetabolic activity. A critical diagnostic dilemma arose regarding whether the PET/CT-positive lesions represented residual lymphoma or fibrotic scarring, whether further chemotherapy adjustments were warranted, and whether surgical resection was necessary. Multidisciplinary discussion concluded that imaging had limited discriminatory value in this scenario and that surgical intervention should be pursued if feasible. The patient successfully underwent partial small bowel resection, with postoperative pathology confirming no residual lymphoma but significant fibrotic changes. The patient has since resumed a normal diet, with body weight nearly restored to pre-illness levels. This case highlights that fibrotic transformation is a common sequela of treated marginal zone lymphoma and that PET/CT may misleadingly suggest residual disease, potentially leading to unnecessary chemotherapy. Timely surgical intervention is crucial in such scenarios.
2.Influencing factors of phenobarbital treatment effect in rural epilepsy patients in Hubei Province

		                        			Objective  To analyze the main factors influencing the management effect of rural epilepsy prevention and control projects in Hubei Province, and to provide reference for further improving the management effect. Methods According to the phenobarbital treatment and management plan of the rural epilepsy project, the target population was screened and reviewed, and patients who met the inclusion criteria were enrolled into the project management. Regular follow-up visits, free drug treatment, health education and other measures were carried out, and all relevant information was collected and integrated into the survey data. After the data was reviewed level by level, SPSS20.0 software was used for statistical analysis.  Results  From January 1, 2015 to December 31, 2020, among patients treated and managed with phenobarbital in 6 project counties, 1430 patients were treated and managed for more than 1 year, of whom 1119 (78.25%) had no seizures or had more than 75% reduction in the number of seizures during the observation period. Compared with other age groups, the age group of 65 years and above had the highest markedly effective/effective ratio (95.77%). From the point of follow-up, the markedly effective/effective ratio of 5 years and above was the highest (91.51%). Compared with those who received no treatment prior to enrollment and those who received regular treatment, the patients receiving informal treatment had the lowest markedly effective/effective ratio (82.43%). 1213 cases (84.83%) had good compliance during the observation period, of whom 1062 cases (87.55%) had a reduction in the number of seizures by more than 50% compared with that before treatment. Univariate analysis showed that the age of patients, the length of follow-up, the treatment status before enrollment, the average daily dose of phenobarbital and the compliance of patients all had an impact on the management effect, and the difference was statistically significant (P<0.05). Multivariate analysis showed that the markedly effective/effective rate of patients in the age group of 65 years and above was 6.749 times that of the younger age group. Receiving informal treatment prior to enrollment was a risk factor for difficult-to-control epilepsy. The markedly effective/effective rate of patients receiving informal treatment was 0.29 times that of patients never receiving treatment. Good compliance was a protective factor for epilepsy control, and the markedly effective/effective rate of patients with good compliance was 2.058 times that of patients with poor compliance.  Conclusion  The epilepsy prevention and management project in rural areas has a significant effect on seizure control. Early treatment, standardized treatment, and improvement of treatment compliance are effective ways to improve the management effect of epilepsy patients.
		                        		
		                        		
		                        		
		                        	
3.Prognostic value of combined serum fibrinogen to albumin ratio and serum CA724 after radical resection for stage Ⅱ/Ⅲ gastric cancer

		                        			Objective:To explore the prognostic value of combined fibrinogen/albumin ratio (FAR) and CA724 in patients with stage Ⅱ/Ⅲ gastric cancer after radical resection.Methods:A total of 932 patients were enrolled in the study, and the best cut-off values of CEA, FAR, NLR and other variables were obtained through ROC curve analysis. According to the FAR-CA724 score, patients were divided into 3 groups: FAR-CA724=0 (CA724<3.43 ng/ml and FAR<0.083), FAR-CA724=1 (CA724≥3.43 ng/ml and FAR≥0.083) and FAR-CA724=2 (CEA≥3.43 ng/ml and FAR≥0.083).Results:After FAR-CA724 grouping, the patient's age (χ 2=12.02, P=0.002), gender (χ 2=15.91, P<0.001), tumor size (χ 2=18.22, P<0.001), hypertension (χ 2=6.35, P=0.042), tumor location (χ 2=26.09, P<0.001), degree of differentiation (χ 2=12.46, P=0.002) and pTNM staging (χ 2=6.68, P=0.035) are significantly different. Survival analysis showed that there were significant differences in OS between the three groups of patients (FAR-CA724=0, 1, and 2: 88.2%, 64.3% and 37.8%, respectively, P<0.001). By multivariate analysis FAR-CA724 is an independent risk factor affecting OS in patients with stage Ⅱ/Ⅲ gastric cancer after radical surgery. Conclusions:Preoperative FAR-CA724 may be a potential blood marker for predicting the prognosis of GC patients.
		                        		
		                        		
		                        		
		                        	
4.Empirical study on satisfaction of physical health education in primary schools of Ganjiang New District

		                        			To understand the status of physical health education in primary schools in Ganjiang New District, and to provide a reference for physical health education for educational managers.
		                        		
		                        			Methods:
		                        			Based on the client satisfaction index and national documents, satisfaction questionnaire survey was administered among parents of 789 primary school students from 5 school in Ganjiang New Area.Sixteen indicators including physical monitoring and hours of physical education.
		                        		
		                        			Results:
		                        			Modrate satisfaction level towards physical health education found among parents ranging from "4" to "3". The highest score for "sports teacher level" was 4.13 and the lowest score for "multimedia education" was 3.00.  The average value of "sports teacher training locations" was 4.08, which was higher than the "overall satisfaction" score of 3.67; the average value of "school sports activities" was 3.58, which was lower than "overall satisfaction". The five-dimensional sample has good independence and there was no multicollinearity; the coefficient of influence of  "Physical health of students" was 0.15(P<0.05).
		                        		
		                        			Conclusion
		                        			Parents satisfaction with the management of physical health education in the five primary schools is generally good, but they are dissatisfied with the lack of physical education teachers venues and teaching hours. It is recommended that parents correct the "five education concept" to improve health education, balance the strengths and weaknesses of school education to make up for shortcomings, and strengthen the home school society co-education mechanism to improve the physical health of primary school students in Ganjiang New District.
		                        		
		                        		
		                        		
		                        	
5.Clinical study on the safety and efficacy of dezocine combined with flurbiprofen axetil in postoperative analgesia in elderly patients undergoing upper abdominal surgery

		                        			Objective To explore the safety and efficacy of dezocine combined with flurbiprofen axetil in postoperative analgesia in elderly patients undergoing upper abdominal surgery.Methods 80 patients underwent abdominalsurgery were selected.They were divided into study group(T group) and control group(C group) according to the random number table,40 cases in each group.The visual analogue scale(VAS) and Ramsay sedation score were recorded at 2h,6h,12h,24h,48h after operation,respectively.The anesthesia-related complications such as sleepiness,chills,postoperative nausea and vomiting (PONV),and pulmonary infection in 48 hours were also recorded.Results There were no statistically significant differences in the VAS score between the two groups in the resting state and the active state(t =0.287,0.436,all P > 0.05).The VAS scores of T group were significantly lower than those of C group at 2h,6h,12h,24h and 48h after operation (t =4.096,4.241,5.510,6.121,5.752,3.542,3.866,4.948,5.542,4.030,all P < 0.05).There were no statistically significant differences in Ramsay sedation scores between the two groups at postoperative 2h,6h,12h,24h and 48h (t =0.323,0.226,0.541,0.297,0.771,0.795,all P > 0.05).There were no statistically significant differences in the incidence of drowsiness,chills,PONV,skin itching and pulmonary infection in the two groups (4 cases vs.6 cases,6 cases vs.5 cases,5 cases vs.3 cases,1 case vs.3 cases,5 cases vs.1 case,P =0.499,0.712,0.745,0.615,0.201).Conclusion The postoperative analgesic compatibility of dezocine combined with flurbiprofen axetil can effectively relieve the postoperative pain in elderly patients with upper abdominal surgery and provide good postoperative analgesic effect and does not increase the incidence of drug related complications.It is worth to promote in clinical.
		                        		
		                        		
		                        		
		                        	
6.Effects of dexmedetomidine on intraoperative sedation and levels of TNF-α, IL-1 and IL-6 in forearm traumatic patients with brachial plexus block

		                        			Objective To investigate the effects of dexmedetomidine on sedation and inflammatory response in patients undergoing brachial plexus anesthesia, to provide a reference for the selection of sedative drugs in forearm traumatic patients undergoing brachial plexus block anesthesia .Methods 80 forearm traumatic patients underwent trauma surgery with brachial plexus nerve block from July 2015 to June 2016 were selected and randomly divided into two groups of dexmedetomidine group (dexmedetomidine group, D group) and normal saline group (control group, C group), each group of 40 cases.The mean arterial pressure (MAP), heart rate (HR) and blood oxygen saturation (SpO2) were recorded before operation (T0), incision (T1), 15 min (T2), 30 min (T3).Interleukin-1 (IL-1), interleukin-6 (IL-6) and tumor necrosis factor-α(TNF-α) were measured at different time points. At the same time, the Ramsay sedation score and the degree of amnesia after operation were measured.Results There were no significant differences in MAP, HR and SpO2 before anesthesia between the two groups.In group D, MAP and HR were significantly lower than those in group C at T1, T2, T3 and T4, the difference was statistically significant (P<0.05).There was no statistical difference in SpO2 between the two groups.The Ramsay sedation score of group D at T1 to T4 was significantly higher than that of group C, the difference was statistically significant (P<0.05).The levels of IL-1β, IL-6 and TNF-αin group D were significantly lower than those in group C at T1 to T4, the difference was statistically significant (P<0.05).Conclusion Dexmedetomidine has good curative effect on forearm traumatic patients with brachial plexus block and less hemodynamics changes , which is beneficial to patients'forgotten of operation and inhibition of inflammatory reaction in forearm traumatic patients.
		                        		
		                        		
		                        		
		                        	
7.Value of detection of anti-herpes simplex virus type 2 antibodies in the diagnosis of genital herpes before in vitro fertilization

		                        			Objective To determine the prevalent herpes simplex virus (HSV) strain in patients with genital herpes (GH),and to evaluate the sensitivity,specificity,positive predictive value (PPV) and negative predictive value (NPV) of anti-herpes simplex virus type 2 (HSV2) IgG and IgM antibodies in the diagnosis of genital herpes (GH) before in vitro fertilization (IVF).Methods Totally,193 HSV2 clinical strains isolated in cell culture from the lesions of patients with GH in the Department of Dermatology,First Affiliated Hospital,Sun Yat-sen University between 2009 and 2011 were typed by using type-specific fluorescein isothiocyanate (FITC)-labelled anti-HSV monoclonal antibodies.Serum samples were obtained from 57 anti-HSV2 IgM/IgG antibody-positive females with suspected GH as well as their husbands (clinical observation group),68 HSV culture-positive patients diagnosed with GH (positive control group),and 120 children aged 8-12 years (negative control group).Enzyme-linked immunosorbent assay (ELISA) was performed to detect anti-HSV1/HSV2 IgG/IgM antibodies in these serum samples.Statistical analysis was carried out using chi-square test.Results There was a significant difference between the positive control group and negative control group in the positivity rate of anti-HSV1 IgG (89.71% (61/68) vs.40.80% (49/120),P < 0.01) and anti-HSV2 IgG (91.18% (62/68) vs.0,P < 0.01),but not in that of anti-HSV1 IgM (20.59% (14/68) vs.21.70% (26/120),P > 0.05) or anti-HSV2 lgM (13.24% (9/68)vs.13.30% (16/120),P > 0.05).In the clinical observation group,the positivity rate of anti-HSV1 and anti-HSV2 IgM antibodies,anti-HSV1 and anti-HSV2 IgG antibodies was 80.70% (46/57),91.23% (52/57),84.21% (48/57) and 14.04% (8/57) respectively in the females,19.30% (11/57),8.77% (5/57),87.71% (50/57),12.28% (7/57)respectively in the males,with significant differences in the positivity rate of anti-HSV1 and-HSV2 IgM antibodies (both P < 0.01),but not in that of anti-HSV 1 or-HSV2 IgG antibodies (both P > 0.05).The sensitivity,specificity,PPV and NPV were 13.24% (9/68),86.67% (104/120),36.00% (9/25) and 63.80% (104/163) respectively for anti-HSV2 IgM antibody in the diagnosis of GH,91.18% (62/68),100.00% (120/120),100.00% (62/62),and 95.24% (120/126) respectively for anti-HSV2 IgG antibody.Conclusions HSV2 prevails in the patients with GH in this region,while HSV1 only amounts to 5.18%.The type-specific anti-HSV2 IgG antibody shows a higher specificity,sensitivity,PPV and NPV in the diagnosis of GH than anti-HSV2 IgM antibody,hence,the type-specific anti-HSV2 IgG antibody is superior to anti-HSV2 IgM antibody in diagnosing GH before assisted reproduction.
		                        		
		                        		
		                        		
		                        	
8.A preliminary study on serum protein SP70 as a novel biomarker for the detection of non-small cell lung cancer

		                        			Objective To evaluate whether the protein SP70 could be used as a serum biomarker for the diagnosis of non-small cell lung cancer (NSCLC).Methods Polyclonal antibody was prepared by immunizing New Zealand rabbit with SPC-A1 cells.Sandwich ELISA was carried out by using newly-prepared polyclonal antibody(PcMb) coating assay plates,monoclonal antibody (McAb) NJ001 and HRP goat antimouse antibody as primary antibody and labeling antibody respectively.After optimizing the experiment conditions,serum from 175 lung cancer patients [ 80 NSCLC adenocarcinoma,70 NSCLC squamous carcinoma and 25 small cell lung cancer ( SCLC) ],25 benign lung disease ( BLD) patients and 300 healthy controls (HC) were examined.CEA,NSE,CYFRA21-1 were measured by ECLIA for comparison.Results Positive rates of NSCLC adenocarcinoma,NSCLC squamous carcinoma,SCLC and BLD were 68.8%,51.4%,16.0% and 12.0% respectively,obviously higher than that of HC (7.3%).NSCLC (adenocarcinoma,Squamous carcinoma) had significantly higher positive rate than SCLC (60.7% υs 16.0%,x2 =17.23,P<0.05)and BLD(60.7% υs 12.0%,x2 =20.41,P <0.05).Among 68 NSCLC patients who had definite staging,positive rates at early stage ( Ⅰ/Ⅱ,n=30) reached up to 76.7%.Meanwhile,positive rates of CEA,NSE and CYFRA21-1 (32.7%,18.0% and 37.3%) were significantly lower than the targeting antigen to McAb NJ001 in NSCLC(60.7% υs 32.7%,x2 =23.63,P <0.05;60.7%υs18.0%,x2 =57.22,P<0.05;60.7% υs37.3%,x2=16.34,P<0.05).Conclusions It showed high positive rates of SP70 in the serum of NSCLC patients,which suggested thai SP70 might be a potential valuable biomarker for the diagnosis of NSCLC.

		                        			Objective To investigate the diagnostic value of detection of protein SP70 in differentiating benign and malignant pleural effusion.Methods A case-control study was conducted from July 2011 to February 2012.108 cases of pleural effusion from patients with clinically proven lung cancers and 122 cases of benign pleural effusion were collected.SP70 was detected by Sandwich ELISA,while CEA,CYFRA21-1,NSE were measured by electrochemiluminescence immunoassay for comparison.Meanwhile,protein SP70 on exfoliated cells in pleural effusion was detected by direct immunofluorescence,and was compared with the results of HE staining.The differences between the groups were evaluated by the chisquare test Fisher' s exact test.Results Positive rates of SP70,CEA,CYFRA21-1,NSE were 72.2%,58.3%,52.8% and 30.6% in malignant pleural effusion,obviously higher than benign pleural effusio (9.8%,13.1%,23.0% and 19.7%).The specificity of SP70,CEA,CYFRA21-1,NSE were 90.2%,86.9%,77.0% and 80.3%,NSCLC had significantly higher positive rate than SCLC(74.3% >0.0%,P =0.02 < 0.05),detection of protein SP70 in malignant pleural effusion had significantly higher coincidence rate than HE staining(72.2% vs 47.2%,x2 =14.03,P < 0.05).Conclusion Determination of the protein SP70 in pleural effusion and in exfoliated cells,can improve the sensitivity and specificity of the diagnosis of malignant pleural effusion.
		                        		
		                        		
		                        		
		                        	
10.Losartan intervenes chronic heart failure of rats

		                        			ObjectiveTo study the reasons and mechanism of cardiomyocyte apoptosis in chronic heart failure by using Losartan and to provide a theoretical basis for the treatment of chronic heart failure. Methods The models of chronic heart failure were produced by injecting Adriamycin and Losartan as intervention agents, the expression of apoptotic protein Bax, Bcl-2 and channel protein ERK1, JNK1 and P38MAPK were detected by immunohistochemistry and RT-PCR.Cardiomyocyte apoptosis and myocardial ultrastructure are detected by transmission electron microscopy and TUNEL staining.Results Compared with the model group of heart failure, after Losartan treatment, the ultra structure of myocardial cells were significantly improved, Apoptosis index was decreased significantly (P <0.01), The level of Bax and JNK1 decreased (Bax χ~2=6.6149, P=0.0078; JNK1 q=22.0156,P<0.01). However, the expressions of ERK1 and Bcl-2 were significantly increased (ERK1 q=15.3514,P<0.01;Bcl-2 χ~2=6.81,P=0.0074).Conclusion The effect of Losartan on chronic heart failure is related closely with the pathway of ERK1 and JNK1, and no p38 MAPK pathway.
