1.Artificial intelligence as a proficient tool in detecting pulmonary tuberculosis in massive population screening programs: a case study in Chennai, India

Objective: To evaluate the performance of Genki, a computer-aided detection (CADe) software, in detecting tuberculosis (TB) using chest radiography in a mobile TB screening program in Chennai, India.Materials and Methods: Genki, an AI-based CADe software, was employed in four mobile diagnostic units in remote areas of Chennai, India for screening TB. Patients from remote areas of Chennai who visited the vans and registered in the screening program underwent chest radiography, and the acquired X-ray scans were analyzed using Genki, which provided an assessment of each scan as either “TB suggestive” or “TB not suggestive”. Subsequently, sputum or swab from the patients with “TB suggestive” results was collected to confirm the diagnosis.Results: In total, 25,598 patients were screened between January and December 2022. When the annotations from the expert radiologists were considered to be true, Genki demonstrated an aggregated sensitivity of 98%, specificity of 96.9%, and accuracy of 96.9% in detecting TB from chest X-ray scans of the screened population. Furthermore, it exhibited a sensitivity, specificity, and accuracy of >95%, >94%, and >94%, respectively, for both sexes (male and female) and all age groups (14–35, 36–60, and ≥61 years).Conclusion: Genki demonstrated excellent value as a TB screening tool in remote locations in Chennai, India. Employing a CADe-based approach for systematic TB screening is cost-effective and reduces workload in high-burden and low-resource settings.
2.Is Quadriceps-Strengthening Exercises (QSE) in MedialCompartment Knee Osteoarthritis with Neutral and Varus Malalignment a Paradox? – A Risk-Appraisal of StrengthTraining on Disease Progression

Introduction: The present inquiry seeks to investigate
whether the current regimens of QSEs (QuadricepsStrengthening Exercises) aggravate the disease while
mitigating symptoms.
Materials and methods: A comparative study was
conducted on 32 patients with medial compartment
osteoarthritis of knees. While the neutral group of 16 patients
was constituted of those with an anatomical-lateral-femorotibial-angle (aFTA) 176-180º, varus group comprised an
equal number of patients with an aFTA >180º. A home-based
12-week strength-training program involving weekly visits
to hospital for supervised sessions was administered. The
outcome measures were visual-analog-scale (VAS), medial
patello-femoral joint tenderness (MPFJT), time-up-and-gotest (TUGT), stair-climb test, step test, WOMAC, IKDC
scores, aFTA, hip-knee-ankle (HKA) angle, lateral-tibiofemoral-joint-separation (LTFJS), and horizontal-distancefrom-centre-of-knee-to-Mikulicz-line.
Results: There was a significant increase in quadriceps
strength (p<0.01) in both groups. Values for neutral group
with VAS score (p=0.01), MPFJT (p=0.01), TUGT (p=0.01),
timing of the stair climb test (p=0.01), WOMAC (p<0.01),
and IKDC (p=0.03) were better compared to varus group
with VAS score (p=0.13), MPFJT (p=0.03), TUGT (p=0.90),
timing of stair climb test (p=0.68), WOMAC (p<0.02), and
IKDC (p=0.05). Varus group also showed an increase in
aFTA and LTFJS in 12 patients, increase in HKA in 11, and
increase in horizontal distance from the centre of knee to the
Mikulicz line in 7 patients.
Conclusion: The present study brings to the fore the
paradoxical role played by QSEs in management of medial
knee OA. While there is a radiological progression of the
disease in both neutral and varus mal-aligned knees more so
in the latter than the former.

Percutaneous techniques for femoral arterial access are increasingly being performed due to advances in endovascular cerebral procedures, as they provide a less morbid and minimally invasive approach than open procedures. Common complications associated with this peripheral puncture include hematoma, bleeding, pseudoaneurysm, arteriovenous fistula, retroperitoneal bleeding, inadvertent venous puncture, dissection, etc. The retrograde femoral access is currently the most frequently used arterial access as it is technically straightforward, allows for the use of larger size sheaths and catheters, allows repeated attempts, etc. Although being technically less challenging, grave complications can occur due to hardware failure. Here, we present a case of unruptured posterior inferior cerebellar artery (PICA) aneurysm, who underwent uneventful diagnostic cerebral digital substraction angiography (DSA) via right femoral artery route on first attempt, but on second attempt for therapeutic intervention, landed up with stuck guide wire and faced decannulation difficulty due to unravelling of guide wire and multiple knot formation, which was finally removed after multiple attempts at pulling and improvised manoeuvres. Such cannulation and decannulation difficulties have been reported multiple times for central venous access, but extremely rarely for femoral routes, making this case a rarity and worth reporting.
4.The memory ameliorating effects of novel N‑benzyl pyridine‑2‑one derivatives on scopolamine‑induced cognitive deficits in mice

Background:
Alzheimer’s disease (AD), the most common form of progressive dementia in the elderly, is a chronic neurological disorder that decreases cognitive ability. Although the underlying cause of AD is yet unknown, oxidative stress and brain acetylcholine shortage are the key pathogenic causes.
Results:
The current study shows that these derivatives have the potential to improve memory in mice by inhibiting scopolamine-induced acetylcholinesterase activity, oxidative and nitrosative stress, and improving locomotor activity and muscle grip strength in the rota rod test. When compared to the illness control, the memory-enhancing potential of novel N-benzyl pyridine-2-one derivatives was highly significant (P < 0.0001).
Conclusions
The observed memory ameliorating effect of novel N-benzyl pyridine-2-one makes them as a a good choice for treatment of individuals with cognitive impairment

Developmental anomalies of internal carotid artery (ICA), being rare entities, are mostly asymptomatic by themselves because of good collateral supply. However, when present with other associated intracranial anomalies requiring treatment, there can be catastrophic consequences, if special attention is not paid to this condition. We present a case of 36 years old male, who reported to our emergency department with complaints of headache and loss of consciousness. He was diagnosed as a case of ruptured anterior communicating aneurysm with subarachnoid hemorrhage and agenesis of left ICA with trans-cavernous anastomosis. He underwent clipping of aneurysm and was discharged uneventfully. This report highlights the importance of skillful microsurgical clipping in extremely high-risk conditions, in contemporary era of hybrid neurosurgeons.
6.Genetic Clarification of Auricularia heimuer Strains Bred and Cultivated in Korea Using the ITS and IGS1 rDNA Region Sequences

Auricularia is one of the broadly cultivated edible mushrooms in Korea. Most of the Korean Auricularia strains used for cultivation and breeding are known as A. auricula-judae. Recently, this species has been reported to belong to a species complex. Therefore, this study was carried out to genetically clarify the bred and cultivated Korean A. auricula-judae strains. The internal transcribed spacer (ITS) and IGS1 rDNA region sequences were determined from 10 A. auricula-judae strains by PCR and sequencing. Variation in the nucleotide sequence and sequence length of the two rDNA regions were found among the seven A. auricula-judae strains. A maximum-likelihood (ML) phylogenetic tree based on the ITS sequences clearly placed all the 10 Korean A. auricula-judae strains in the A. heimuer clade of the A. auriculajudae complex. A. heimuer is diverged from A. auricula-judae. An ML phylogenetic tree based on the IGS1 sequences revealed the close relationship between Korean A. heimuer strains to Chinese A. heimuer strains. But each strain could be distinguishable by the IGS1 sequence. Furthermore, progeny strains in the seven Korean strains could be differentiated from their parental strains by the IGS1 sequence based phylogenetic tree. Our results are expected to be used to complement the distinction of domestic Auricularia cultivars.
7.Efficacy of hepatitis B vaccination in patients with ulcerative colitis: a prospective cohort study

Background/Aims:
Response to vaccine in patients with inflammatory bowel disease is lower than in the general population. We aimed to evaluate the efficacy of hepatitis B virus (HBV) vaccination in patients with ulcerative colitis (UC) versus controls.
Methods:
We prospectively compared antibody response to HBV vaccination in 100 patients with UC versus controls. HBV vaccination was given to all the cases and controls at 0, 1 and 6 months. Anti-hepatitis B surface (anti-HBs) titers were then measured 4 weeks after the first and the third dose. Adequate immune response (AIR) was considered if the anti-HBs titer was >10 IU/L and effective immune response (EIR) if the anti-HBs titer was >100 IU/L.
Results:
Median anti-HBs titer was lower in patients with UC than controls (67 IU/L vs. 105 IU/L, P<0.01). AIR and EIR were significantly lower in patients than in controls (82% vs. 96%, P=0.001; 41% vs. 66%, P<0.001, respectively). Univariate analysis showed that age <30 years, mild to moderate severity of disease, disease duration <5 years, male sex, post first dose anti-HBs titer >2 IU/L and non-exposure to corticosteroids, azathioprine and biologicals were predictors of AIR in patients with UC (P<0.05). Multivariate analysis revealed that only non-exposure to corticosteroids, azathioprine and biologicals, male sex, and disease duration <5 years were independent predictors of AIR.
Conclusions
Response rate to the HBV vaccination in patients with UC was significantly lower as compared to the controls. Male sex, shorter disease duration, and non-exposure to immunomodulators were independent predictors of AIR.
8.Epstein-Barr Virus-associated Mixed Lymphoepithelioma-like Carcinoma and Adenocarcinoma of the Gall Bladder: An Unusual Entity

Schmincke described lymphoepithelioma as an undifferentiated carcinoma with abundant lymphoid stroma in the nasopharynx. Tumors with a similar histomorphology in extrapharyngeal areas have been referred to as lymphoepithelioma-like carcinoma (LELC). The association of an Ebstein-Barr virus (EBV) infection with lymphoepithelioma is well established in the nasopharynx but not so well at the extrapharyngeal sites. Only four cases of LELC have been reported in the gall bladder, of which all were negative for the EBV. This paper reports the first case of an EBV-associated mixed gall bladder carcinoma exhibiting a distinct phenotype of LELC and adenocarcinoma with mucinous differentiation. The EBV was confirmed by the strong granular membranous and cytoplasmic expression of LMP-1 (latent membrane protein-1) on immunohistochemistry and nuclear EBER RNA on chromogen in-situ hybridization in the tumor. This is the first case of LELC positive for EBV in the gall bladder. LELC has a more favorable prognosis than conventional adenocarcinoma or squamous cell carcinoma, irrespective of the site. Although a higher T stage and nodal metastasis were exceptional in the present case in contrast to the previous cases, the EBV-associated lymphocytic response might limit the disease spread and confer better overall survival and prognosis in these patients. Nevertheless, more prospective studies with a larger cohort will be needed to understand the pathogenesis, biological behavior, and prognosis of this rare entity.
9.COVID-19 Rapid Antigen Test: Role in Screening Prior to Gastrointestinal Endoscopy

Background/Aims:
The severe acute respiratory syndrome coronavirus 2 pandemic has affected the gastrointestinal (GI) endoscopy units globally owing to the risk of transmission. We present our data on the use of rapid antigen test (RAT) as a screening tool prior to endoscopy to prevent the transmission of coronavirus disease (COVID-19).
Methods:
This study was a retrospective analysis of patients who underwent any GI endoscopic procedure from July 2020 to October 2020 at a tertiary referral center in New Delhi, India. All patients underwent screening for COVID-19 using RAT, and endoscopy was performed only when the RAT was negative. The data are presented as numbers and percentages.
Results:
A total of 3,002 endoscopic procedures were performed during the study period. Only one endoscopic procedure was performed in a COVID-19 positive patient. A total of 53 healthcare workers were involved in conducting these procedures. Only 2 healthcare workers (3.8%) were diagnosed COVID-19 positive, presumably due to community-acquired infection, during this period.
Conclusions
The COVID-19 RAT is easily usable as a simple screening tool prior to GI endoscopy during the COVID-19 pandemic.
10.Association between Psychological Status and Functional Outcome in Surgically Managed Fractures around Hip in Geriatric Patients - A Prospective Study

Introduction: Fractures around the hip in the geriatric
population not only lead to functional but also psychological
impairment. Psychiatric disturbances can be associated with
poor participation in rehabilitation, increased risk of falling
again, and higher rates of mortality. The present study was
undertaken to assess the association between the
psychological status and functional outcome of surgically
managed elderly Indian patients who had sustained fractures
around the hip.
Materials and methods: The present study was a hospital
based prospective, single centre study. One hundred and two
geriatric patients who had sustained hip fracture and had
been managed surgically, having no cognitive dysfunction,
living independently, having unhindered walking capability
before the fracture, were included in the study. They were
called for follow-up at 3rd, 6th, and 12th month after the hip
surgery. Psychological assessment was done by the Hospital
Anxiety and Depression Scale (HADS) and functional
outcome by using the Harris hip score (HHS).
Results: Our study did not show association between
psychological status and functional outcome except in one
sub-group. Significant correlation was observed between the
psychological status and functional outcome in most of the
patients in the extra-capsular group. We have identified
improvement in the depression, anxiety and functional
scores during the follow-up.
Conclusion: We conclude that psychiatric disturbances in a
geriatric patient after undergoing a surgery for hip fracture
may lead to poor recovery. We recommend that all such
geriatric patients should undergo a psychological assessment
and proper therapy should be instituted to achieve good
functional recovery
