1.Changes in cerebrospinal fluid leukocytes and 24-hour intrathecal IgG synthesis rate in the patients with positive intrathecal IgG synthesis

Objective:Laboratory evaluation of the relationship between cerebrospinal fluid and plasma indicators and intrathecal immunoglobulin G(IgG) synthesis in patients with neurological diseases, and establishment of a new diagnostic method for intrathecal IgG synthesis.Methods:This study retrospectively analyzed the content of IgG in cerebrospinal fluid samples and blood albumin in blood samples, and other test results of 410 patients with neurological diseases who visited Beijing Tiantan Hospital from 2019 to 2022. According to the results of oligoclonal bands in cerebrospinal fluid, patients were divided into intrathecal IgG synthesis group and non-intrathecal IgG synthesis group. The Mann Whitney U test was used for inter group comparison, and a bilateral test with P<0.05 indicates a statistically significant difference. Include indicators with differences between groups in logistic regression analysis, construct a predictive model, and compare it with the established quantitative formula IgG index. Results:There were significant differences in 10 indicators, including cerebrospinal fluid leukocyte count and 24-hour intrathecal IgG synthesis rate, between the intrathecal IgG synthesis group and the non-intrathecal IgG synthesis group, with P<0.05. The area under the curve (AUC) of intrathecal IgG synthesis was higher than the IgG index (AUC=0.920, 0.809, Z=31.178, P<0.001), the sensitivity was higher than the IgG index (0.825, 0.618), and the specificity was lower than the IgG index (0.876, 0.908). Conclusion:The combination of 10 indicators such as cerebrospinal fluid white blood cell count and 24-hour intrathecal IgG synthesis rate can improve the diagnostic efficacy and sensitivity of intrathecal IgG synthesis.
2.Discussion on Occurrence and Development of Non-alcoholic Fatty Liver Disease based on Traditional Chinese Medicine Chronomedicine

Circadian rhythm disorders are closely related to metabolic diseases, which can cause non-alcoholic fatty liver disease (NAFLD) by directly acting on the liver or indirectly affecting the liver through the liver-brain axis and intestinal flora. The rhythms of yin and yang, ying (营) and wei (卫), twelve hours and four seasons in traditional Chinese medicine (TCM) chronomedicine are similar to the connotations of modern biological rhythms. From the perspective of chronomedicine of TCM, the incidence of NAFLD is closely related to the abnormality of the daily rhythm of the waxing and waning of yin and yang, the daily rhythm of the circulation of ying and wei, the rhythm of twelve hours and four seasons. Through analyzing the rhythms related to the occurrence, development and prognosis of NAFLD, it is helpful to enhance the understanding of NAFLD in relation to time, so as to better guide the clinical diagnosis and treatment.
3.Construction of the evaluation index system for long-term care needs of community-dwelling elderly based on International Classification of Functioning, Disability and Health

Objective:To construct an evaluation index system for the long-term care needs of community-dwelling elderly.Methods:Based on the International Classification of Functioning, Disability and Health, a preliminary evaluation index system was formed using literature research and descriptive research. The evaluation index system was revised and improved through the expert consultation.Results:Two rounds of consultation were conducted on 20 experts. The effective response rates of the questionnaire for two rounds of expert consultation were 90.0% (18/20) and 94.4% (17/18), respectively. The expert opinion submission rates were 94.4% (17/18) and 29.4% (5/17), and the expert authority coefficients were 0.94 and 0.95, respectively. After two rounds of expert consultation, the coefficient of variation of each indicator was less than 0.15, and the Kendall harmony coefficients of the overall indicator and all levels of indicators were 0.277 to 0.441 (all P<0.01). The final evaluation index system consisted of three first-level indicators, 12 second-level indicators, and 49 third-level indicators. Conclusions:The construction process of the evaluation index system established is rigorous, the content is comprehensive, and the research methods and results are scientific and reliable. It can provide a basis for further developing long-term care needs assessment tools for community-dwelling elderly and establishing grading standards for long-term care needs assessment.
4.Study on the mental health status and its influencing factors among middle school students in Yi nationality areas: a case study of Xide County, Liangshan Prefecture, Sichuan Province

ObjectiveTo explore the mental health status and its influencing factors among middle school students in Xide County, Liangshan Yi Autonomous Prefecture, and to provide references for mental health interventions for local middle school students. MethodsUsing a cross-sectional study design, one junior middle school and one senior middle school in Xide County, Liangshan Prefecture, Sichuan Province, were randomly selected on September 16, 2021, and two classes per grade in each school involving 288 students were recruited. Subjects were assessed using Patients' Health Questionnaire Depression Scale-9 item (PHQ-9), Generalized Anxiety Disorder Scale-7 item (GAD-7), PTSD Checklist for DSM-5 (PCL-5), Multidimensional Scale of Perceived Social Support (MSPSS) and UCLA Loneliness Scale (ULS-3). Then the scores of above scales were compared among middle school students with different demographic characteristics, and binary Logistic regression analysis was conducted to screen the influencing factors of post-traumatic stress disorder (PTSD) symptoms. ResultsAmong the respondents, 17.71% (95% CI: 0.133~0.221), 8.68% (95% CI: 0.054~0.120), 2.78% (95% CI: 0.009~0.047) and 45.83% (95% CI: 0.400~0.516) were reported to have symptoms of depression, anxiety, PTSD and loneliness, respectively. Students in senior middle school scored lower on PCL-5 and ULS-3 than those in junior middle school [(6.46±8.91) vs. (8.87±9.42), t=2.202, P<0.05; (4.67±1.65) vs. (5.60±1.88), t=4.431, P<0.01]. Regression analysis denoted that depressive symptoms (OR=7.630, P<0.05) and anxiety symptoms (OR=3.789, P<0.01) were risk factors for PTSD symptoms among middle school students. ConclusionThe middle school students in Xide County, Liangshan Yi Autonomous Prefecture suffer a high prevalence rate of depressive symptoms and loneliness, and those in junior middle school are more likely to feel a sense of strong loneliness, furthermore, depressive symptoms and anxiety symptoms are risk factors for PTSD symptoms.
5.Procine recombinant NK-lysin inhibits hepatocellular carcinoma metastasis by downregulating FKBP3 and inhibiting oxidative phosphorylation and glycolysis: a proteomic analysis.

OBJECTIVE:
To investigate the potential mechanisms that mediate the inhibitory effect of porcine recombinant NKlysin (prNK-lysin) against liver cancer cell metastasis.
METHODS:
HPLC-tandem mass spectrometry was used to identify the differentially expressed proteins in prNK-lysin-treated hepatocellular carcinoma SMMOL/LC-7721 cells in comparison with the control and PBS-treated cells. GO functional annotation and KEGG pathway analysis of the differentially expressed proteins were performed using GO and KEGG databases. RT-qPCR was used to determine the mRNA expression levels of polypeptide-N-acetylgalactosaminotransferase 13 (GALNT13), transmembrane protein 51 (TMEM51) and FKBP prolyl isomerase 3 (FKBP3) in the cells, and the protein expression of FKBP3 was verified using Western blotting.
RESULTS:
Proteomic analysis identified 1989 differentially expressed proteins in prNK-lysin-treated cells compared with the control cells, and 2753 compared with PBS-treated cells. Fifteen proteins were differentially expressed between PBS-treated and the control cells, and 1909 were differentially expressed in prNK- lysin group compared with both PBS and control groups. These differentially expressed proteins were involved mainly in the viral process, translational initiation and RNA binding and were enriched mainly in ribosome, protein process in endoplasmic reticulum, and RNA transport pathways. RT-qPCR showed that compared with the control group, prNK-lysin treatment significantly increased the mRNA expressions of GALNT13 (P < 0.05) and TMEM51 (P < 0.01) and lowered FKBP3 mRNA expression (P < 0.05). Western blotting also showed a significantly decreased expression of FKBP3 protein in prNK-lysin-treated cells (P < 0.001).
CONCLUSION
Treatment with prNK-lysin causes significant changes in protein expression profile of SMMOL/LC-7721 cells and inhibits hepatocellular carcinoma metastasis by downregulating FKBP3 protein and affecting the cellular oxidative phosphorylation and glycolysis pathways.

6.Effect of immunochemotherapy administration sequence on efficacy and adverse reac-tions in patients with non-small cell lung cancer

Objective:To explore the effect of chemotherapy administration sequence combined with programmed cell death protein-1(PD-1)monoclonal antibody on the efficacy and immune-related adverse events(irAEs)in patients with advanced non-small cell lung cancer(NSCLC).Methods:The clinical data of 110 patients with advanced NSCLC treated at The First Affiliated Hospital of Zhengzhou University between November 2019 and January 2022 were retrospectively collected.The factors influencing irAEs were analyzed by univariate and multivariate Logistic regression analyses,while those influencing curative effect were analyzed by Kaplan-Meier curve analysis,Log-rank test,and univariate and multivariate Cox regression analyses.Results:Treatment with PD-1 monoclonal antibody after 2 days of chemotherapy(sequential treatment group,n=36)significantly prolonged progression-free survival(PFS)compared with PD-1 monoclonal antibody admin-istration on the same day of chemotherapy(simultaneous treatment group,n=74)(17.2 months vs.11.3 months,respectively;P<0.05).The disease control rate(DCR)was better in the sequential treatment group than in the simultaneous treatment group(94.4%vs.79.7%,re-spectively;P=0.045),while the objective response rate(ORR)did not differ significantly(69.4%vs.51.4%,respectively;P=0.072).The Cox re-gression analysis showed that cytokerat in 19 fragment(Cyfra21-1)and d-dimer(D-dimer)affected the efficacy of combination therapy(P<0.05).The Logistic regression analysis showed that age and lactic dehydrogenase(LDH)influenced the occurrence of irAEs(P<0.05),while the administration sequence did not significantly affect the occurrence of irAEs(P=0.130).Conclusions:Administration sequence influences the efficacy of combination therapy,and patients with advanced NSCLC who receive sequential therapy may experience better efficacy.Age and LDH are negatively correlated with the occurrence of irAEs.

Drinking culture has high significance in both China and the world, whether in the entertainment sector or in social occasions; according to the World Health Organization's 2018 Global Alcohol and Health Report, about 3 million people died from excessive drinking in 2016, accounting for 5.3% of the total global deaths that year. Oxidative stress and inflammation are the most common pathological phenomena caused by alcohol abuse (Snyder et al., 2017). Scutellarin, a kind of flavonoid, is one of the main active ingredients extracted from breviscapine. It exerts anti-inflammatory, antioxidant, and vasodilation effects, and has been used to treat cardiovascular diseases and alcoholic liver injury. Although scutellarin can effectively alleviate multi-target organ injury induced by different forms of stimulation, its protective effect on alcoholic brain injury has not been well-defined. Therefore, the present study established an acute alcohol mice brain injury model to explore the effect of scutellarin on acute alcoholic brain injury. The study was carried out based on the targets of oxidative stress and inflammation, which is of great significance for the targeted therapy of clinical alcohol diseases.

8.Advance in rehabilitation therapy with diastasis recti abdominis in postpartum women (review)

Diastasis recti abdominis (DRA) is common in postpartum women. DRA seriously affects the physiological and psychological health of patients. Rehabilitation is the main intervention for DRA. It is necessary to clarify the effectiveness of different rehabilitation intervention. Neuromuscular electrical stimulation and exercise therapy can effectively optimize DRA in postpartum period, but the best intervention is not clear. Multi-center researches with large sample should be conducted to compare the treatment parameters in the future.
9.The therapeutic effect of Chaihu Longgu Muli Decoction combined with western medicine in the treatment of post-stroke depression

Objective:To evaluate the clinical efficacy of Chaihu Longgu Muli Decoction in the treatment of post-stroke depression.Methods:A total of 130 patients with post-stroke depression who met the inclusion criteria from April 2019 to December 2020 were enrolled in the study. The patients were randomly divided into two groups with 65 cases in each group. The control group was treated with Flupentixol and melitracen tablets on the basis of routine treatment, and the observation group was treated with Chaihu Longgu Muli Decoction on the basis of routine treatment. Both groups were treated for 4 weeks. TCM syndrome scores were performed before and after treatment. The levels of serum Hcy and hs CRP, Plasma 5-HT and brain-derived neurotrophic factor (BDNF) were detected by ELISA, and the ratio of neutrophils to lymphoblasts (NLR) was recorded.Results:The response rate was 92.3% (60/65) in the observation group and 75.4% (49/65) in the control group. There was significant difference between the two groups ( χ2=6.87, P=0.009). After treatment, the serum Hcy [(11.87±1.56) μmol/L vs. (16.69±1.77) μmol/L, t=16.47] and hs-CRP [(7.81±1.66) mg/L vs. (16.45±4.87) mg/L, t=13.54] in the observation group were significantly lower than those in the control group ( P<0.01), and Plasma 5-HT [(150.41±17.54) μg/L vs. (123.24±16.02) μg/L, t=9.22], BDNF [(19.45±2.48) μg/L vs. (12.56 ± 2.23) μg/L, t=16.66] were significantly higher than those in the control group ( P<0.01). After treatment, the NLR and TCM syndrome scores in the observation group were significantly lower than those in the control group ( t values were 6.03 and 7.15, respectively, all Ps<0.01). Conclusion:The Chaihu Longgu Muli Decoction combined with western medicine treatment can correct the levels of Hcy, hs-CRP, 5-HT, BDNF and NLR in peripheral blood, alleviate the state of depression and anxiety of patients with post-stroke depression.
10.Optimization of detection conditions of pre-donation ABO blood type screening by sliding methods in fixed blood donation sites

【Objective】 To explore the quantitative value of key points for ABO blood group initial screening in fixed blood donation sites, so as to provide reference for standardized testing process of sliding method. 【Methods】 Several groups of experiments were carried out to illustrate the optimal conditions, including the serum dosage of monoclonal reagent, red blood cell dosage of blood sample, and reaction time in ABO blood group initial screening, by sliding method, and the quantitative value of key points of sliding method was preliminarily determined. Blood typing tests of 310 blood donor samples including type A, B, O, AB, subtype A and subtype AB were conducted to evaluate the effects of quantitative value of key points in the initial screening procedure. The test tube method would be conducted if the results are inconsistent with the fully automated blood grouping analyzer. The ABO subtypes suspected are identified by serological and molecular biological methods. 【Results】 The quantitative value of key points in initial screening procedure of sliding methods was as follows: 2 drops of reagent serum, 5-10 μL of whole blood and 3 minutes of reaction time. The concordance rate of ABO blood type screening comparison experiment in 310 blood donors was 100%. 【Conclusion】 ABO blood group initial screening by sliding method with quantitative value can effectively standardize the pre-donation blood type screening in fixed blood donation sites, and can meet the requirements of ABO blood group initial screening.
