1.Open Abdominal Management Among Non-Trauma Patients: The Appropriate Duration and a New Clinical Index

		                        			 Purpose:
		                        			Despite widespread adoption of open abdominal management (OAM), there is currently no threshold criterion for OAM duration for non-trauma patients. Moreover, there is a positive relationship between morbidity and the duration of OAM, but an uncertain relationship with patients’ age. Therefore, a novel clinical index for the duration of open abdominal management (IDOM) was developed based on the patient’s age and risk of severe complications following OAM to indicate the maximum tolerable number of days of OAM based on the individual’s age. The utility of this new index was evaluated. 
		                        		
		                        			Methods:
		                        			This retrospective study included 65 non-trauma patients managed with an open abdomen (OA) from August 2015 to August 2018. The IDOM was developed based on the patient’s age. The result indicated the maximum number of OA days. Patients’ demographic and operative variables were examined and patient data was assigned to one of two groups according to whether the actual number of OA days was above or below the calculated IDOM. Prevalence of complications between these groups was compared. Measures of validity were employed to assess the utility of the IDOM for patient complications. 
		                        		
		                        			Results:
		                        			Sixty-five patients were included. The above-the calculated IDOM group exhibited a significantly longer OA and higher rates of wound complications and postoperative respiratory complications compared with the below the calculated IDOM group. The IDOM predicted the incidence of OA-related complications with a sensitivity of 72.4%, and a specificity of 80.6%. 
		                        		
		                        			Conclusion
		                        			The IDOM is a potentially useful tool for appropriate duration at the outset of OA. 
		                        		
		                        		
		                        		
		                        	
2.Effect of Coix-seed Reactive Derivatives(CRD) on Cold Sensitivity of Female Hands and Feet: A Randomized Controlled Trial

		                        			We evaluated the effect of Coix-seed Reactive Derivatives (CRD) on cold sensitivity of female hands and feet. Ninety females were assigned randomly to 3 groups and CRD(2g/day, 4g/day or placebo) was administrated for 8weeks. Significant improvement of cold sensitivity was observed in both the 2g/day and 4g/day groups. It was speculated that a naringenin and a quercetin as active ingredients of adlay hull may play a critical role in ameliorating effect on blood flow.

		                        			In this report, we presented the case of a 45-year-old man who developed intractable eczema on the thumb-index interdigital fold for over 3 years. Coix-seed Reactive Derivatives(CRD) was administrated at a dose of 2.2g/day per day. The lesion cured markedly within 7 months. 		                        			Well water could be a stable source of drinking water. Recently, the use of well water as drinking water has been encouraged in developing countries. However, many kinds of disorders caused by toxic elements in well drinking water have been reported. It is our urgent task to resolve the global issue of element-originating diseases. In this review article, our multidisciplinary approaches focusing on oncogenic toxicities and disturbances of sensory organs (skin and ear) induced by arsenic and barium are introduced. First, our environmental monitoring in developing countries in Asia showed elevated concentrations of arsenic and barium in well drinking water. Then our experimental studies in mice and our epidemiological studies in humans showed arsenic-mediated increased risks of hyperpigmented skin and hearing loss with partial elucidation of their mechanisms. Our experimental studies using cultured cells with focus on the expression and activity levels of intracellular signal transduction molecules such as c-SRC, c-RET, and oncogenic RET showed risks for malignant transformation and/or progression arose from arsenic and barium. Finally, our original hydrotalcite-like compound was proposed as a novel remediation system to effectively remove arsenic and barium from well drinking water. Hopefully, comprehensive studies consisting of (1) environmental monitoring, (2) health risk assessments, and (3) remediation will be expanded in the field of environmental health to prevent various disorders caused by environmental factors including toxic elements in drinking water.

8.Effective Use of Coix-seed Reactive Derivatives (CRD) in Managing Photoaging of the Skin: A Case Report

		                        			Ultraviolet (UV)-induced wrinkle and pigmentation are very common in clinical practice. In this report, we presented the case of a 58-year-old man who developed wrinkle and pigmentation around right hand, face and neck for ten years. Coix-seed Reactive Derivatives (CRD) was administrated orally 2.1g twice a day for 6 months. These lesions were improved in the third month, and a dramatic response was obtained at the end of 6 months of treatment. 9.Successful Treatment of Severe Friction Melanosis with Coix-seed Reactive Derivatives (CRD): A Case Report

		                        			Friction melanosis is a cutaneous entity characterized by a brownish pigmentation distributed on the skin over bony regions of various sites including a trunk or the limbs, after the use, for many years, of a nylon towel or scrub brush to clean the skin. Moreover, repeated trauma due to friction of clothes on the skin over bony prominences are another factors characterizing its pathogenesis. In this report, we presented the case of a 58-year-old man who developed severe friction melanosis around both knees and lower extremities for about 20 years. Coix-seed Reactive Derivatives (CRD) was administrated orally 2.1g twice a day for 6 months. The lesions of pigmentation diminished markedly within 3 months.
