1.Status quo and influencing factors of clinical practice level of 298 geriatric nursing specialist nurses

Objective:To investigate the clinical practice level and influencing factors of geriatric nursing specialist nurses.Methods:This study was a cross-sectional survey. Using the convenient sampling method, a total of 332 specialist nurses from 5 phases trained by Beijing Nursing Association from January to February 2021 were selected as the research objects. The general data questionnaire, Questionnaire on Clinical Practice Level of Geriatric Nursing Specialist Nurses, Questionnaire on Unit Support for Geriatric Nurses and Core Competence Self-Assessment Scale of Geriatric Nursing Specialist Nurses were used to investigate the nurses. Pearson correlation was used to analyze the relationship between unit support, self-assessment of core competence and practice level. Multiple linear regression was used to analyze the factors influencing the clinical practice level of geriatric nurses. A total of 332 questionnaires were distributed, 306 questionnaires were returned and 298 were valid questionnaires.Results:The total mean score of clinical practice of geriatric nursing specialist nurses was (3.42±0.83) . The results of multiple linear regression analysis showed that the work department, unit support and core competence were the influencing factors of the clinical practice level of geriatric nursing specialist nurses ( P<0.05) . Conclusions:Specialist nurses who work in the geriatric ward, have a high degree of unit support and have a high level of core competence have a higher clinical practice level.
2.Current status of asphyxia prevention and management for the elderly in 43 elderly care institutions

Objective:To explore the current situation and related factors of asphyxia prevention and management for the elderly in elderly care institutions, so as to provide a reference for improving the safety management of elderly care institutions.Methods:This research was a cross-sectional survey. From December 2020 to January 2021, convenience sampling was used to select 43 elderly care institutions from the China Aged Care Alliance. The self-designed questionnaire was used to collect the content related to asphyxia prevention and management for the elderly in institutions, and analyze its current situation and related factors. A total of 43 questionnaires were distributed in this survey, and 43 were recovered, with a recovery rate of 100%.Results:Among the 43 elderly care institutions, and 3 (6.98%) were state-run, and 22 (51.16%) were private, and 18 (41.86%) were public and private. The average occupancy rate was (67.63±20.59) %, and the average proportion of moderate to severe disability among the elderly residents was (80.10±20.25) %. The proportions of elderly care institutions that formulated systems and regulations for monitoring and early warning of asphyxia events, reporting of asphyxia events, analysis and improvement of asphyxia incidents in the elderly were 53.49%, 51.16%, and 51.16%, respectively. The proportions of elderly care institutions that carried out asphyxia risk screening, swallowing function assessment, and swallowing function training for the elderly residents were 65.12%, 51.16%, and 46.51%, respectively.Conclusions:Most elderly care institutions have problems in the prevention and management of asphyxia for the elderly, which needs to be improved. It is recommended that building a complete asphyxia prevention and management system, carrying out risk screening, assessment and training of swallowing function for high-risk groups, strengthening the professional training of agency leaders and caregivers, and seeking continuous external professional support so as to improve the safety management system of elderly care institutions, and ensure the quality of life and personal safety of the elderly.
3.Analysis of the demand and related factors for suffocation prevention training for nursing staff in elderly care institutions

Objective:To explore the demand for suffocation prevention training for nursing staff in elderly care institutions and analyze its influencing factors to provide reference for improving training strategies.Methods:In this cross-sectional study, from December 2020 to January 2021, the caregivers from 43 elderly nursing institutions under China Geriatric Care Alliance were selected by convenient sampling. Self-made questionnaires were used to collect personal information of caregivers as well as related experience and training needs of suffocation prevention for the elderly. Totally 526 questionnaires were recovered, of which 412 were valid, with an effective rate of 78.33%.Results:Among the 412 caregivers, 53 (12.86%) were registered nurses and 359 (87.14%) were senior caregivers. They had a high demand for suffocation prevention, identification, and emergency treatment training. The top three most demanded are the common causes of suffocation, the risk factors of suffocation for special elderly people, and the identification of abnormal physical signs to detect suffocation in time. There were statistically significant differences in the scores of content requirements for suffocation prevention training between institutions which had and had not experienced suffocation and caregivers who knew or did not know the medication status or suffocation history of the care recipients ( P<0.05) . Conclusions:Caregivers in elderly care institutions have a high demand for suffocation prevention training. It is recommended that elderly care institutions apply pre-prevention to the management of suffocation prevention for the elderly, and guide caregivers to fully understand the medication status, suffocation history, and swallowing function of the care recipients, and provide registered nurses and elderly caregivers with systematic training related to suffocation prevention.
4.Development and application of lens parameters calculating software based on B ultrasound images.

This project was aimed to develop a simple, convenient and reliable computer image processing software for the measurement of lens dimensions, including the radius of curvature of anterior lens surface (RCALS), the radius of curvature of posterior lens surface (RCPLS) and the lens volume (LV). On the basis of lens images captured by B ultrasound, our computer software was designed to calculate the three parameters of lens in accordance to geometry principle. This software comprises Program I and Program II, and they both possess different calculation methods. Then they were used in a group of normal volunteers who were recruited via randomization and the outcomes were compared. The results showed that the two programs were developed successfully, and the outcomes of RCALS, RCPLS and LV calculated by the two programs were similar. The standard deviation of Program II is smaller than that of Program I. The computer software for calculating RCALS, RCPLS and LV was characterized by scientific design and easy-to-use. In respect of calculating repetition, Program II was better than Program I, and it possesses prosperous prospect in terms of clinical research and application.

5.Effects of recombinant human erythropoietin on glutamate expression in the retina with acute high intraocular pressure in a rabbit model

Objective The neuroprotection provided by recombinant human erythropoietin(rhEPO)on the retina from ischemia-reperfusion injury has been confirmed but its mechanism is not fully understood.The present study aimed to investigate the effect of systemic administration of recombinant human erythropoietin(rhEPO)on the expression of glutamate in the retina after acute high intraocular pressure in vitro.MethodsThe acute high intraocular pressure models were established by the perfusion of physiological saline into anterior chamber of the lateral eye in forty-eight Japanese white rabbits.Other 6 Japanese white rabbits were as normal control group.The experimental rabbits were then equally divided into the model group and EPO group,and hypodermic injection of rhEPO was only performed in the EPO group.Glutamate expression in the retina in both groups was observed by immunohistochemistry on days 1,3,7,and 14 after retinal ischemia-reperfusion.Glutamate expression in another 6 rabbit retina without any treatment was determined as normal by the same method.The use of animal followed the Standard of Association for Research in Vision and Ophthalmology.ResultsNo positive expression of glutamate was observed in normal rabbit retina,but positive expression response of glutamate occurred in the rabbit retina of the model group.The number of positive expression cells in the EPO group was more than that in the model group at each time point(P<0.01).On day 14 after ischemia-reperfusion,the number of positive expression cells was 3.3±1.1 per high visual field in the retina of the model group but 0.3±0.2 in the retina of the EPO group,showing a significant decrease of positive expression cells in EPO group(P<0.01).ConclusionSystemic administration of rhEPO can down-regulate the expression of glutamate in the retina with acute high intraocular pressure.This process may be one of the mechanisms that rhEPO protects the retina from ischemia reperfusion injury.
6.Experimental study on cyclosporine A drug delivery system in prevention of posterior capsule opacification after intraocular lens implantation in rabbits

Objective To study the effect of cyclosporine A drug delivery system (CsA-DDS) on the prevention of posterior capsule opacification (PCO) after experimental intraocular lens implantation in rabbit eyes. Methods Twenty healthy New Zealand white rabbits, whose left eyes and right eyes were used respectively as experiment eyes and controls, were subjected to extracapsular lens extraction and artificial lens implantation. During the operation, CsA-DDS with poly (lactideco-glycolide) as carriers or empty DDS was implanted in the capsular bag for the experimental eyes and controls respectively. After the operation, anterior chamber reaction, intraocular pressure (IOP) and CsA concentration were monitored and twelve weeks after the operation, the eyes were extracted for histopathological and morphological examinations. Results There were no differences between the two groups in conjunctival congestion,IOP change and anterior chamber reaction. PCO was less severe in the experimental eyes than in the controls. Light microscopy revealed that posterior capsular membrane in the experimental eyes was slick, with no obvious proliferation,whereas in the controls, there were lens epithelial cell proliferation and cortex regeneration of different degrees.Morphological examination with electron microscope showed that in the experimental eyes, lens epithelial cells did not function actively and apoptosis occurred, whereas in the controls, epithelial cells presented active function. No marked ultrastructural changes were found in either group. Conclusion Cs-DDS can inhibit PCO after intraocular lens implantation in rabbit eyes and does not have toxic effects on the surrounding ocular tissues. Therefore, it has a good potential for clinical use in prevention of PCO.
7.Pharmcokinetic study of cyclosporin A in rabbit eyes by HPLC

Objective To study the pharmacokinetic characters of cyclosporin A(CsA) in aqueous humor in rabbit after implanting different dosages of CsA in eyes and to provide a theoretical basis for the treatment of after cataract. Methods ECCE was performed in all rabbit eyes. CsA-MS was injected into the anterior chamber and the capsular bag in left eyes as expression group and MS was given in the same way in right eyes as control group. The concentration of CsA in the aqueous humor was monitored with high-performance liquid chromatogram. The follow-up period was 4 weeks. The samples were separated on a C18 column at 60℃ and detected at 210nm. The mobile phase was acetonitrile-water (67∶33). Results The correlation analysis showed a positive correlation within the range of 0.13-1.25mg/L (r=0.9951) and the detection limit was 0.13mg/L. The accuracy was 95.91% and the inter-day and intro-day precision was less than 5%. CsA in aqueous humor sustained a high concentration within 2 weeks. There were no significant differences in t1/2Ka and CL between the two dosage groups. AUC and Cmax increased in a dose-dependent manner. Conclusion The sustain-released CsA ophthalmic gels provided significant ocular bioavailability in rabbit eyes and they can reach the therapeutic dose in order to inhibit after cataract.
8.Morphological study of uveoscleral pathway following topical Prazosin in free-sympathetic rabbit eyes

ObjectiveTo observe the morphology of the uveoscleral pathway following topical Prazosin(PZ) in rabbit free-sympathetic eyes.MethodsThe one lateral superior cervical ganglionectomy(SCG) were preformed on each rabbit.PZ drops were installed only on the SCG eyes on 14 days Postoperationly.Microamounts of fluorescein isothiocyanate-bovine serum albumin(FITC-BSA) was infused into anterior chamber of rabbit eyes as the teacer,two rabbits in each group were killed at 2,4,6,8,10,and 12h after PZ instillation.Degress of fluorescent intensity of ciliary body,suprachoroidal space,anterior and posterior sclera,and choroid were observed under fluorescence microscopy.ResultsThe IOP of the SCG eyes was lower after installation of PZ.But the reduced extent is below that of normal rabbit eyes;After installation of PZ,the fluorescent intensity of the ciliary body,suprachoroidal space,anterior and posterior sclera,and choroid was less stronger than the control eyes,but fader than at the same area of the normal eyes obviously.ConclusionThe outflow effects of PZ drops on increasing uveoscleral pathway were obviously decreased in the SCG eyes.It depends on the activity of sympathetic nerve system partially.Besides,there are other mechanisms.
9.MOLECULAR CLONING OF HUMAN NEUROTROPHIN-4 GENE

Objective Cloning and sequencing of the human neurotrophin-4(hNT-4) gene.Methods With the chromosomal DNA of human blood lymphocytes as template,hNT-4 coding genes were amplified by polymerase chain reaction(PCR) and recombinated into phage vector pGEM-T Easy,which were sequenced by using Sanger's single stranded DNA terminal termination method.Results The sequence of the cloned gene is completely the same as that reported in the literature(GenBank data base,M86528).Conclusion This study successfully cloning and sequenced the gene of mhNT-4,and it would be convenient for us to study the expression of mhNT-4 in eukaryote,and to continue the research on the gene therapy of Alzheimer's disease intensively.This study indicate that the hNT-4 is conservative in different races and individuals.
10.A HISTOPATHOLOGIC IMMUNOHISTOCHEMICAL AND ULTRASTRUCTURAL STUDY OF RABBIT CORNEA AFTER EXCIMER LASER PHOTOREFRACTIVE KERATECTOMY

Oblective To evaluate the corneal healing of non-epuality diopter response histopathologically,immunohistochemically and ultrastructurally after excimer laser photorefractive keratectomy(PRK) with SVS APEX PLUS(Summit Technology Inc. USA) excimer laser, and the effects of corticosteroid on the healing. Methods PRK on 6 white rabbits(12 eyes) was performed on right eye of the rabbit for an attampted correction of -4.00 diopter and on left eye for an attempted correction of -8.00 diopter. The rabbits were divided into 2 groups randomly and each group included 6 eyes: Group FLM (3 rabbits, 6 eyes) and group CM (3 rabbits, 6 eyes). Fluoromethalone was given to group FLM,and chloromycetin to group CM. On 10d, 30d and 100d ,the eyes of one rabbit in each group were enucleated randomly. Half of each cornea was prepared for electron microscope observation (SEM and TEM)and the rest embedded in OCT compound for immunohistochemical study to examine Ⅲ -C and FN. Results All eyes were reepithelialized within 3d after PRK. Subepithelial corneal haze was observed on 15d,which was dominant on 30 or 60d. On 100d postoperatively,corneal hazes of 11 eyes were grades 0 or 0. 5,only 1 eye(the left eye of group CM) was denser haze (grade 1). On 3d postoperatively, one or two layers of corneal epithelial cell covered the ablation zone. On 30d after PRK,the epithelial cells showed hyperplastic changes. The cells were larger and increased from normal 5 or 6 layers to 7 or 8 layers of cells on 100d after PRK,epithelium was clear with more bright epithelium. Microplicae and microvilli were less than before. The expression of Ⅲ -C and FN in group CM was significantly more evident than that in group FLM. Conclusion The study show that despite recovery of a continuous and smooth epithelial layer and nearly normal corneal tissues on 100d after PRK,abnormalities of both epithelium and superficial stroma can be detected in the area of ablation. The ablation depth of stroma can influence the appearance of corneal haze after PRK. The microplicae and microvilli of rabbit cornea epithelium become less after PRK,which can be one of cause leading to ocular dry sensation in some patients.
