1.Gender-specific Associations between Tobacco Use and Body Mass Index among School-going Adolescents in Malaysia: Results from the National Health Morbidity Survey 2017

Tobacco uses and obesity are major global public health concerns, with adolescence being a critical period for the onset of these risk factors. In Malaysia, rising trends in adolescent smoking and obesity necessitate further investigation. This study examined gender-specific associations between different types of tobacco use and body mass index (BMI) among school-going adolescents. Data were derived from the NHMS 2017, a nationwide cross-sectional survey using a two-stage stratified random sampling design (N=27,497). Complex sample logistic regression was employed to assess the odds of past 30-day tobacco use by BMI category (underweight/normal weight, overweight, obese) stratified by gender. Confounders, including age, ethnicity, physical activity, fast food consumption, and mental health, were controlled for in the analysis. Adolescents classified as obese had a significant association with tobacco use, but this was not observed in overweight individuals. Male smokers were significantly less likely to be obese, whereas female smokers had a higher likelihood of obesity (AOR = 1.59, 95% CI: 1.25–2.02). Specifically, female users of e-cigarettes (AOR = 1.86, 95% CI: 1.36–2.56), smokeless tobacco (AOR = 1.66, 95% CI: 1.14–2.43), and other tobacco products (AOR = 1.62, 95% CI: 1.05–2.52) exhibited a higher risk of obesity. No significant association was found among male smokers. Female adolescent smokers showed a greater likelihood of obesity, emphasizing the need for gender-specific interventions. To mitigate these risks, public health efforts should incorporate education, policy measures, and lifestyle promotion. Further research is warranted to explore underlying biological mechanisms

Pandemic fatigue encompasses exhaustion, emotional strain, and decreased motivation due to prolonged pandemic
effects. This scoping review examines pandemic fatigue issues related to COVID-19, aiming to suggest effective management strategies. We assessed 2,558 records from 1st of December 2019 until 27th of March 2022 in PubMed,
Science Direct, ProQuest, and ClinicalKey. Following Arksey and O’Malley (2005) and PRISMA-ScR principles, we
selected 31 studies including research papers employing either quantitative or qualitative methods and editorials
and other summary articles after excluding redundant and irrelevant works. The authors provide six critical answers
to understand better and manage pandemic fatigue, including the need for more research and support, the collaboration between key stakeholders and the community, and the use of information to address pandemic fatigue. The
review found that pandemic fatigue is prevalent among certain groups, such as students and healthcare workers.
Factors contributing to pandemic fatigue include age, gender, high perceived severity of COVID-19, low trust in
government or health organizations, and low social connectedness. The review also identified an instrument, the
COVID-19 Pandemic Fatigue Scale, that can be used to measure pandemic fatigue. The review concluded that
identifying pandemic fatigue as early as possible is absolutely important and people and their government should
work together to handle the fatigue. Many countries have acknowledged the presence hence manage it differently.
Nonetheless the strategy suggested by WHO has provided an excellent framework to tackle the pandemic fatigue.
3.Development and Feasibility Assessment of a Nutrition Screening Guideline in Older Patients for Use by Healthcare Staff in Health Clinic Setting: An Action Research Study

Introduction: Older persons are more susceptible to malnutrition. However, malnutrition identification through
nutrition screening is not routinely performed in Malaysia’s health clinics due to no specific nutrition screening
guideline and validated tool in this setting. This study aimed to develop a nutrition screening guideline for older patients and assess its feasibility for use by healthcare staff in health clinic setting. Methods: This study was conducted
in three phases. In Phase I, needs assessment was conducted amongst healthcare staff. Development of nutrition
screening guideline in Phase II involved scoping review and validation amongst six experts and twelve healthcare
staff. In Phase III, twenty-two healthcare staff participated in feasibility assessment using in-depth interviews after
completing nutrition screening in older patients. Interview data was analysed thematically using NVivo Software
12.0. Results: A Malay language version of nutrition screening guideline in A4-sized with 31 pages was developed
and validated. Items with a content validity index ≥0.83 from experts review were retained, while <0.83 were revised
accordingly. Validation amongst healthcare staff showed positive responses. The guideline was then assessed for
feasibility, where 110 older patients (mean age= 68.7±6.1 years) were screened by healthcare staff using the newly
developed nutrition screening guideline. Four themes emerged from in-depth interviews: 1) Ease of use; 2) Identification and management of malnutrition; 3) Acceptability; and 4) Implementation of nutrition screening. Conclusion:
The newly developed and validated nutrition screening guideline is well-accepted and feasible for healthcare staff to
screen older patients in health clinic setting for timely malnutrition identification and management.
4.A Qualitative Study Exploring the Impact of Covid-19 Pandemic on Teaching and Learning of Family Medicine Course among Undergraduates in a Local University in Malaysia

Introduction: The COVID-19 pandemic significantly impacted the global teaching and learning process (TnL). Unfortunately, to date, not many qualitative studies have been published specifically on the impact of COVID-19 on
the Family Medicine course, particularly in Malaysia. Hence, this study aimed to explore the impact of COVID-19
pandemic on the teaching and learning experience of undergraduate students undergoing their Family Medicine
course at a local university. Methods: A qualitative study using focused group discussion (FGD), was conducted
among undergraduate students during the lockdown period. A semi-structured interview guide was used to interview
20 students, in their fifth undergraduate year, undertaking their fourth to fifth week of a total six-week course. They
were selected using purposive snowball sampling method. There were four focus group discussions (FGD) with five
students in a group. All interviews were audio-taped, transcribed verbatim, and the contents were analysed using
the standard content analysis framework. Subsequently, thematic content analysis was conducted, and three major
themes were produced. Results: The three major themes were (1) facilitators to learning, (2) barriers to learning, and
(3) rooms for improvement. Conclusion: Although COVID-19 pandemic had significant impact on TnL of Family
Medicine course among undergraduates, the students remained optimistic and proposed some improvement from
their point of view. However, a creative, realistic, effective, and impactful way of TnL, particularly in clinical aspects
should be developed and experimented. Technological progress and advancement should permit this idea to be
achievable and implemented in near future.
5.The Osseous Pathology of Purpura Fulminans in a TwoYear-Old Child: A Case Report

Purpura fulminans (PF) is a severe clinical manifestation of
Neisseria meningitides infection that is associated with high
mortality rates in children. Survivors are frequently left with
debilitating musculoskeletal sequelae. There is a paucity of
reports on the musculoskeletal pathology of purpura
fulminans. We report on a 2-year-old boy with purpura
fulminans due to meningococcemia. The child developed
distal gangrene in both the upper and lower limbs.
Amputations were done for both lower limbs. Histological
examination of the amputated specimens showed an
inflammatory process and features of osteonecrosis. The
latest follow-up at the age of 6 years showed a right knee
valgus due to asymmetrical growth arrest of the proximal
tibia. PF and its complications are challenging to treat and
may require a multidisciplinary approach to improve
patient’s functional ability.
6.Exploring the norms of eating-out practice among adults in Malaysia

Introduction: Eating-out is common in almost all countries, including Malaysia,
but this frequent practice may affect human health. In Malaysia, data on eating-out
is limited. This study aimed to assess the proportion of eating-out, to assess the
association between socio-demographic factors and eating patterns, and to compare
energy and nutrient intakes between people eating-out and eating-in. Methods:
This cross-sectional study was conducted among 100 adults aged 30 to 70 years old.
Three-day food diaries were used to collect data on dietary intake. Eating-out was
defined as eating foods prepared outside the home. Respondents who ate outside for
at least one meal per day, for two or three days per week were considered as those
who frequently practised eating-out. Results: A total of 84% of respondents who
ate out had significantly higher sodium intake than those who ate at home (2934
mg/day vs. 2165 mg/day, p=0.025). Foods and drinks that were most commonly
consumed outside were nasi lemak, roti canai, rice, ayam masak kicap, vegetable
soup, tomyam, rice vermicelli soup (mee-hoon soup), hot teh-o, iced tea, and orange
juice. Occupation (p=0.004) and location type (p=0.001) were associated with eatingout. Government and semi-government workers (61%) and urban population (57%)
had higher percentage of eating-out compared to eating at home (19% and 12%,
respectively). Conclusion: More than two-thirds of our respondents ate out and this
habit was related to poor diet quality with excessive intake of sodium. Interventions
are needed to improve the diet quality of the overall eating-out behaviour among
targeted population.
7.Development of nutrition screening guideline content for use by healthcare staffs in older adults in health clinic setting: A scoping review

Introduction: Malnutrition, specifically undernutrition, in community-dwelling
older adults reduces their well-being and predisposes to diseases. Therefore, timely
malnutrition identification through nutrition screening is needed to identify at risk
and malnourished patients. This study aimed to develop the content of nutrition
screening guideline to facilitate healthcare staffs in health clinics to administer a
validated nutrition screening tool specifically for older adults. Methods: A scoping
review was conducted electronically using SCOPUS, PubMed, ProQuest Health &
Medical Complete, and Cochrane databases. The systematic search was performed
up to 31st December 2021. Search terms were created for identification of eligible
and related articles. Inclusion and exclusion criteria were determined for the
systematic search. The search was limited to English and Malay languages, and
full text articles with no limitation of years. All data were extracted and analysed,
guided by the PRISMA extension for scoping reviews (PRISMA-ScR). Results:
From 728 identified articles, 18 articles were included in the analysis. Identified
information for the guideline content were: introduction, aims and objectives,
definition of malnutrition, prevalence of malnutrition, implementation of nutrition
screening, guidance on administering items in validated tools, and guidance on
anthropometric measurements. Meanwhile, format and flow charts from established
guidelines served as references for the guideline development process. Conclusion:
Appropriate content to develop a nutrition screening guideline has been identified
based on this review. Development of a guideline based on this content can facilitate
healthcare staffs to perform timely nutrition screening in older adults.
8.Development, Validation and Acceptability of a Newly Developed Nutrition Resource Kit for At-Risk and Malnourished Elderly in Health Clinics Setting

Introduction: The elderly population is highly vulnerable to malnutrition, including those in the community. The use
of nutrition education as part of nutrition intervention is able to improve their nutritional status. Hence, provision of a
nutrition resource kit addressing the needs of at-risk and malnourished elderly would be advantageous. This research
aimed to develop, validate and evaluate the acceptance of a newly developed nutrition resource kit, which served
as an educational material among at-risk and malnourished elderly in Malaysian health clinics. Methods: This study
was conducted in three phases: Phase I comprised of qualitative needs assessment to identify the types of nutrition
resource kit needed; Phase II was the development and validation of the nutrition resource kit; while Phase III was
acceptance evaluation, which involved individual in-depth interview and triangulation. Results: An A5-sized booklet with incorporation of an educational video in a QR code was developed. Both printed material and educational
video had excellent content and face validity. Suggestions by experts and elderly from validation were considered
and revision was done accordingly. Acceptance evaluation revealed four themes: 1) positive acceptance, 2) elderly-friendly, 3) valuable, and 4) individual preference. Triangulation data revealed that all elderly positively evaluated
both resources. Additional suggestions given by elderly were considered for improvement. Conclusion: This newly
developed nutrition resource kit, both in the printed and electronic format, was well-accepted among the at-risk and
malnourished elderly. It could be used as an important reference for the elderly, especially those attending health
clinics, in treating and preventing malnutrition.
9.Application of HRM Analysis in Detection of PDGFRA Exon 10 Polymorphism in CML Patients with Imatinib Resistance

Introduction: Imatinib mesylate has been widely used as a standard treatment for chronic myeloid leukemia (CML).
It acts as a selective competitive inhibitor of the BCR-ABL tyrosine kinase. Despite the excellent efficacy on CML
treatment, some patients developed resistance to the treatment. Mutation in the PDGFRA may be one of the factors
involved in the mechanism of resistance that affects the response to imatinib. The mutational status of PDGFRA is
highly relevant for prognosis and treatment prediction in CML patients. Thus, this study is intended to establish and
validate a High Resolution Melting (HRM) analysis for PDGFRA exon 10 c.1432 T>C polymorphism in CML patients.
Methods: High resolution melting (HRM) analysis was used to identify the c.1432 T > C polymorphism in PDGFRA
exon 10 (n =86; response = 43; resistance = 43). The results from HRM analysis were compared and validated with
Sanger sequencing. The association between the polymorphism and treatment response was assessed by statistical
analysis using binomial logistic regression analysis. Results: HRM analyses showed two different melt curves. One
curve followed the shape of the reference, homozygous wild type (TT) and the other curve showed a different melting profile than the reference with the TC genotype (heterozygous variant). The results revealed that heterozygous
variant (TC) genotype showed a high risk of acquiring resistance with an OR of 3.795; 95% CI: 1.502-9.591, with
a statistically significant association, p = 0.005. HRM analysis also showed 100% sensitivity and specificity in the
detection of PDGFRA exon 10. Conclusion: The HRM analysis of PDGFRA exon 10 c.1432 T>C was successfully
established. The exon 10 c.1432 T>C polymorphism shows a higher risk for the development of resistance toward
imatinib treatment.
10.Types of Nutrition Resource Kit Needed by Community Living Elderly in Malaysian Health Clinic Setting: A Qualitative Study

Introduction: Elderly population is at high-risk to suffer from malnutrition and nutritional inadequacy. The use of
nutrition resource kit which provides nutritional guidance could be helpful to improve their dietary intake. There is
no study performed previously on the type of nutrition resource kit that can be used to overcome and prevent malnutrition among Malaysian elderly. Therefore, this study aimed to identify the types of nutrition resource kit needed
by community living elderly in Malaysian health clinic setting. Methods: Qualitative study was conducted to explore
the types of nutrition resource kit needed by community living elderly in health clinics. Recruitment for in-depth
individual interview targeted elderly aged ≥60 years old who met the inclusion and exclusion criteria and received
primary healthcare services in four health clinics in Kuantan, Pahang. Interview questions addressed the knowledge
on existing nutrition educational materials, preference for types of nutrition resource kit and opinion on technology-based materials. Data were audiotaped, transcribed verbatim and thematically analyzed using NVivo software
version 12. Results: 21 participants involved in this study (mean age=67.24 ±6.98 years). Five key themes identified
from the data: provision of nutrition resource kit; preference for printed version; eye-friendly; facilitate understanding
with diagram; and support for technology-based materials. This study also found that majority of elderly preferred for
A5-sized booklet for nutrition resource kit. Conclusion: Future development of nutrition resource kit tailored to the
need of elderly is needed to deliver nutritional guidance effectively and served as an important reference for them to
overcome malnutrition.
