1.Role of lipophagy in the prevention and treatment of nonalcoholic fatty liver disease

Nowadays,the prevalence of nonalcoholic fatty liver disease(NAFLD)is constantly rising in China and globally,and its incidence rate is increasing year by year,which has seriously affected human life and health.Lipophagy is molecular chaperone-mediated autophagy and has the functions of promoting lipolysis,maintaining the lipid homeostasis of hepatocytes,and alleviating hepatocyte fatty degeneration.Lipophagy has three main processes of lipid droplet catabolism,lipid droplet autophagy,and fatty acid β-oxidation,which are regulated by key genes,receptors,and enzymes.Currently,important advances have been achieved for the intervention methods of traditional Chinese medicine,Western medicine,diet,and exercise in the research on lipophagy,which provides new perspectives for the prevention and treatment strategies for NAFLD.
2.Regulatory effect of Diwu Yanggan Decoction on lysoglycerophospholipids in circulating exosomes in a mouse model of nonalcoholic fatty liver disease

Objective To evaluate the regulatory effect of Diwu Yanggan(DWYG)Decoction on lysoglycerophospholipids(Lyso-GPLs)in circulating exosomes in a mouse model of nonalcoholic fatty liver disease(NAFLD).Methods Circulating exosomes isolated from mouse serum by size exclusion chromatography were morphologically characterized using transmission electron microscope and examined for surface markers CD9,CD63 and TSG101 using Western blotting.Twenty-four male Kunming mice were randomized into 3 groups for normal feeding(control,n=8)or high-fat diet feeding for 1 week to induce NAFLD,after which the latter mice were given DWYG decoction(treatment group,n=8)or normal saline(model group,n=8)by gavage for 4 weeks.After the last treatment,blood samples were collected from the mice for testing serum TC,HDL-C,LDL-C,ALT and AST levels and isolating circulating exosomes.Using multivariate statistical analysis based on targeted metabolomics strategy,the potential biomarkers for Lyso-GPLs in the exosomes were screened.Results The isolated exosomes about 100 nm in size had a typical saucer-like structure with distinct double-layer membranes and a mean particle size of 137.5 nm and expressed the specific surface marker proteins CD9,CD63 and TSG101.The mouse models of NAFLD had significantly increased serum levels of TC,HDL-C,LDL-C and AST and lowered serum ALT level.A total of 43 Lyso-GPLs with significant reduction after DWYG Decoction treatment were identified in NAFLD mice.Conclusion DWYG Decoction can regulate Lyso-GPLs in circulating exosomes in NAFLD mice,which provides a new clue for studying the therapeutic mechanism of DWYG Decoction for liver disease.

Emerging SARS-CoV-2 variants have made COVID-19 convalescents susceptible to re-infection and have raised concern about the efficacy of inactivated vaccination in neutralization against emerging variants and antigen-specific B cell response. To this end, a study on a long-term cohort of 208 participants who have recovered from COVID-19 was conducted, and the participants were followed up at 3.3 (Visit 1), 9.2 (Visit 2), and 18.5 (Visit 3) months after SARS-CoV-2 infection. They were classified into three groups (no-vaccination (n = 54), one-dose (n = 62), and two-dose (n = 92) groups) on the basis of the administration of inactivated vaccination. The neutralizing antibody (NAb) titers against the wild-type virus continued to decrease in the no-vaccination group, but they rose significantly in the one-dose and two-dose groups, with the highest NAb titers being observed in the two-dose group at Visit 3. The NAb titers against the Delta variant for the no-vaccination, one-dose, and two-dose groups decreased by 3.3, 1.9, and 2.3 folds relative to the wild-type virus, respectively, and those against the Omicron variant decreased by 7.0, 4.0, and 3.8 folds, respectively. Similarly, the responses of SARS-CoV-2 RBD-specific B cells and memory B cells were boosted by the second vaccine dose. Results showed that the convalescents benefited from the administration of the inactivated vaccine (one or two doses), which enhanced neutralization against highly mutated SARS-CoV-2 variants and memory B cell responses. Two doses of inactivated vaccine among COVID-19 convalescents are therefore recommended for the prevention of the COVID-19 pandemic, and vaccination guidelines and policies need to be updated.
5.Integrated Traditional Chinese and Western Medicine Treatment for Acute Lymphoblastic Leukemia Complicated with Severe COVID-19 in Recovery Stage: A Case Report

This paper reported a case of severe COVID-19 in the recovery stage with acute lymphoblastic leukemia treated by integrated traditional Chinese and western medicine, with the intention of shedding light on the clinical diagnosis and treatment of similar conditions. The patient, who had acute lymphoblastic leukemia, developed COVID-19 infection during the bone marrow suppression period after chemotherapy. Treatment with western medicine was mainly anti-infection, symptomatic management, and supportive care. During the recovery stage, considering the patient's chemotherapy history and disease progression, the overall syndrome was identified as deficiency of both qi and yin and binding of phlegm and blood. Based on the “state-target” combined treatment strategy, herbal prescriptions were selected and modified to address the “deficiency state”, “disease target”, and “symptom target”. In addition to western medicine, the patient was administered with Shengmai Powder (生脉散) and Compound Zhebei Granules (复方浙贝颗粒) in its modifications to boost qi, nourish yin, and reinforce healthy qi, nourish and cool the blood, ultimately achieving satisfactory therapeutic effects.
6.Analysis of deafness gene variant screening of 7875 neonatal cases in Dongying area of Shandong.

OBJECTIVE:
To determine the types and frequency of deafness-related variants among 7875 newborns from Dongying area of Shandong Province.
METHODS:
One hundred loci of 18 common deafness genes were subjected to semiconductor sequencing. Variant site, frequency and distribution of the variants were analyzed.
RESULTS:
In total 552 deafness gene variants were detected among the 7875 newborns, which yielded a detection rate of 7.01%. Among these, common variant sites for GJB2, SLC26A4 and GJB3 genes were c.235delC, IVS7-2A>G and c.538C>T, respectively. The variant frequencies of matrilinear inheritance deafness genes MT-CO1, MT-RNR1, MT-TL1 and MT-TS1 were 0.38%, 0.25%, 0.1% and 0.01%, respectively. Four newborns were diagnosed with deafness, among which one had unilateral hearing loss. Analysis of the proportions of neonatal deafness-related variants in five counties of Dongying showed that the highest variant rate for the SLC26A4 gene compared with GJB2 was in Lijin county (51.76% vs. 40%), while the lowest was in Hekou county (30.77% vs. 56.41%).
CONCLUSION
The carrier rate of deafness-related variants in Dongying area is higher than other regions of China, which may be attributed to the increased types and variant sites covered by the semiconductor sequencing method compared with the chip method and time-of-flight mass spectrometry. Due to geographical and population aggregation factors, the proportion of deafness variants in the five counties of Dongying differed significantly. Above results may provide a guide for the prevention of congenital deafness in Dongying area.
7.Correlation between marrow edema and related MRI and clinical manifestations in patients with knee osteoarthritis

Objective To investigate the relationship between the marrow edema and general clinical index,quadriceps muscle area,and the meniscus grade of knee osteoarthritis(OA).Methods 72 patients were collected with knee OA in our hospital, underwent X-ray and routine MRI examination of knee, and the same X-ray and MRI were reviewed at 12 months later in different time point.The K-L grading, bone marrow edema score, meniscal grading and VAS score of each knee joint were evaluated.The t-test and Rank-sum test were used to compare the two groups of general data, Spearman was used to perform bivariate correlation analysis.Results The age and VAS score of bone marrow edema group at the initial follow-up were significantly lower than those without edema group(P<0.05),the degree of marrow edema was moderately correlated with age and VAS score, and was not significantly correlated with other indexes.12 months later, BMI and the area of quadriceps femoris in the group with marrow edema were different from those in the group without bone marrow edema(P<0.05), the degree of marrow edema was moderately correlated with age, quadriceps area and VAS.Conclusion The range of marrow edema was related to age, quadriceps area, BMI index and VAS score in MRI.The MRI measurement could reflect the progression of knee OA more than that of X-ray.It also revealed some factors related to the progression of knee OA.
8.Fuzhengkangai Particle Combination HAART Therapy in Patients with HIV/AIDS Clinical Curative Effect and Adverse Reactions of Clinical Research

Objective:to apply Fuzhengkangai AIDS particles combined therapy with effective antiretroviral therapy (HAART) at the beginning of the HIV/AIDS (human immunodeficiency virus/acquired immunodeficiency syndrome) patients,observe its clinical curative effect and improve the related adverse reactions.Methods:a multicenter,randomized,double-blind,placebo-controlled clinical research method,divided into experimental group and the control group (n =57),experimental group to "Fuzhengkangai particles" joint efficient antiretroviral therapy (HAART) antiviral treatment,control group to "Fuzhengkangai particles" placebo (5% low dose) joint efficient antiretroviral therapy (HAART) antiviral treatment,treatment for 3 months.Two groups of patients with simultaneous liver and kidney function,blood routine,CD4 + T lymphocyte count,the observation of TCM symptoms and signs,survival quality of integral.Results:after 12 weeks of treatment,the control group and experimental group,the experimental group significantly elevated levels of peripheral blood RBC,lower levels of CR,reduce the signs and symptoms (total score,fatigue,stay,and spontaneous perspiration)integral,higher levels of CD4 + T lymphocyte count and survival quality score,the difference had statistical significance (p < 0.05).Conclusion:Fuzhengkangai particles joint efficient antiretroviral therapy (HAART) antiviral treatment,can improve HAART after initial treatment of the patient's clinical symptoms,blood toxicity,the quality of survival,reduce the antiviral treatment of adverse reactions,combination drug alone can significantly increase the CD4 + T lymphocytes,so as to improve clinical curative effect.
9.Expression of LP-PLA2 mRNA in peripheral blood monocytes subsets of patients with type 2 diabetes mellitus

Objective To investigate the role of monocyte subsets in the pathogenesis of type 2 diabetes mellitus(T2DM), the expression was detected on lipoprotein-associated phospholipase A2(LP-PLA2) mRNA in peripheral blood monocytes subsets of type 2 diabetic patients. Methods After the peripheral blood of 32 type 2 diabetic patients and 22 healthy controls were collected,the percentages of the monocytes subsets were detected by flow cytometry. The expression of LP-PLA2 mRNA in monocytes subsets were detected by PCR. After monocyte subsets of healthy subjects were induced into macrophages and stimulated by the serum of type 2 diabetic patients for 24h,LP-PLA2 mRNA expression was detected. Results Compared with the control group, the percentage of classical monocytes in T2DM group was decreased, while the LP-PLA2 mRNA expression and percentage of nonclassical,intermediate monocytes were significantly increased(all P<0.05). In addition,the homeostasis model assessment for insulin resistance index was positively correlated with LP-PLA2 mRNA expression in non-classical(P<0.05)and intermediate monocytes(P<0.01)of type 2 diabetic patients. Compared with type 2 diabetic patients without carotid atherosclerosis, the levels of LP-PLA2 mRNA in intermediate and nonclassical monocytes were significantly higher than those in type 2 diabetic patients with carotid atherosclerosis(P<0.05). The level of LP-PLA2 mRNA in non-classical monocytes-derived macrophages stimulated with the serum of type 2 diabetic patients was significantly higher than that in the control group(P<0.05). Conclusion Monocytes subsets from type 2 diabetic patients are abnormally skewed toward to intermediate and non-classical monocytes with high expression of LP-PLA2 mRNA,which may play an important role in the pathogenesis of type 2 diabetes mellitus and atherosclerosis.
10.Stability of sodium nitroprusside for injection in two different dissolvants

Objective To study the stability of sodium nitroprusside for injection in 5% glucose injection and 0 .9%sodium chloride injection ,and provide reference for clinical rational drug use .Methods The content of sodium nitroprusside for injection in different dissolvants(5% glueose ,sodium chloride injection) within 26 h was determined by HPLC .The change of micro particle was measured by light blockage method in accordance with China pharmacopeia(2010 edition) ,and the appearance and UV visible absorption spectroscopy and pH ,the osmotic pressure of the mixtures were observed as well .Results The changes in appearnace ,UV visible absorption spectroscopy ,pH value ,content and microparticle were not significant .Conclusion Sodium nitroprusside for injection were stable within 26 h after mixing with different dissolvants (5% glueose ,sodium chloride injection) .
