1.The 5-year relative survival rate among cancer patients in Henan province of China, 2015-2019

Objective:To analyze the 5-year relative survival rate of cancer in Henan province based on cancer registration data.Methods:Cancer survival data were extracted from the cancer registration database of Henan province with the diagnosis date between January 1, 2010 and December 31, 2019 were included. The closing date of follow-up was set as December 31, 2019. The 5-year relative survival rate of cancer was calculated using the period survival analysis method and the Ederer II method in the R package "periodR", and the interest period was between 2015 and 2019.Results:During the period of 2015-2019, the overall 5-year relative survival rate of cancer patients in Henan province was 43.6%, and after age-standardization, it was 40.2%. The overall 5-year relative survival rate showed the characteristics of higher survival rate in females than males (45.9% vs 34.7%, Z=39.60, P<0.001) and higher survival rate in urban areas than rural areas (44.9% vs 39.1%, Z=12.97, P<0.001). The 5-year relative survival rate for cancer patients among children aged 0-14 was 60.2%, and for adults aged 15 and above, it was 43.5%, which was standardized to 40.2% after age adjustment. There are two types of cancers with a standardized 5-year relative survival rate exceeding 70% (thyroid cancer at 82.2% and breast cancer at 71.6%), and four cancers with a rate below 30% (pancreatic cancer at 18.2%, liver cancer at 19.6%, lung cancer at 24.0%, and gallbladder cancer at 26.6%). Conclusion:The cancer 5-year survival rate in Henan Province is lower than that of the national average, indicating the need for continued enhancement of cancer prevention and control measures.
2.Role and mechanism of ABI3BP in angiotensin Ⅱ-induced endothelial progenitor cell dysfunction

Objective To investigate the role and mechanism of ABI family member 3-binding pro-tein(ABI3BP)in dysfunction of endothelial progenitor cells(EPC)induced by angiotensin Ⅱ(Ang Ⅱ).Methods EPC were divided into sh-RNA negative control(sh-NC)group(transfected with LV-scramble-shRNA+PBS),sh-ABI3BP group(transfected with LV-ABI3BP-shRNA+PBS),sh-NC+Ang Ⅱ group(transfected with LV-scramble-shRNA+Ang Ⅱ)and sh-ABI3BP+Ang Ⅱ group(transfected with LV-ABI3BP-shRNA+Ang Ⅱ)to investigate the effect of silencing ABI3BP on the dysfunction of EPC induced by Ang Ⅱ.Transwell assay,adhesion assay,Matrigel tube formation assay,and TUNEL staining were performed respectively to detect the migration,adhesion and tube formation abilities and cell apoptosis in above cell groups.The expression chan-ges in integrin-β1/FAK/P53 signaling pathway were detected by Western blotting.Results Com-pared to the sh-NC group,the sh-NC+AngⅡ group showed significant decreases in the numbers of migratory cells,adhesion cells,and tubule formation,along with increases in the apoptotic rate and the expression levels of Integrin β1,p-FAK,and P53(P<0.05).While,the sh-ABI3BP+AngⅡ group had obviously more migratory cells(88.67±8.33 vs 62.33±7.37 units,P<0.05),adhe-sion cells(104.33±6.03 vs 68.33±10.05 units,P<0.05),and tubule formation(36.33±3.21 vs 19.33±3.06 units,P<0.05),while decreased apoptotic rate and expression levels of integrin-β1,p-FAK and P53 protein when compared with the sh-NC+AngⅡ group(P<0.05).Conclusion Ang Ⅱ can up-regulate the expression of ABI3BP,and knockdown of ABI3BP can improve Ang Ⅱ-induced EPC dysfunction,which may be related to its inhibition on integrin-β1/FAK/P53 signa-ling pathway.
3.Pharmacoeconomic evaluation of trastuzumab deruxtecan versus chemotherapy in the second-line treatment of advanced breast cancer with HER-2 low expression

OBJECTIVE To evaluate the economics of trastuzumab deruxtecan versus the physician-selected chemotherapy (TPC) regimen in the second-line treatment of advanced breast cancer with epidermal growth factor receptor 2 (HER-2) low expression from the perspective of the Chinese healthcare system. METHODS Based on the data of DESTINY-Breast04 clinical trial, the dynamic Markov model was constructed. The time frame of the model simulation was 10 years, and the cycle was 3 weeks. Taking cost, quality-adjusted life year (QALY) and incremental cost-effectiveness ratio (ICER) as the model output indicators, the discount rate of 5% was applied, and 3 times China’s per capita gross domestic product (GDP) in 2023 was taken as the willingness-to-pay (WTP) threshold value. Cost-utility analysis was used to evaluate the economics of the two treatment regiments in the hormone receptor-positive cohort and all patient cohorts, and uncertainty analysis was used to verify the robustness of the basic analysis result. RESULTS The results of the basic analysis showed that compared with the TPC regimen, the ICER value of trastuzumab deruxtecan regimen edu.cn were 1 045 655.76 and 906 404.99 yuan/QALY in the hormone receptor-positive cohort and all patients, respectively, both exceeding the WTP threshold (268 074 yuan/QALY). The results of single factor sensitivity analysis showed that progression-free survival utility value, the price of trastuzumab deruxtecan and progression disease utility had a significant influence on the model results. The results of probability sensitivity analysis showed that when the WTP threshold was 3 times China’s per capita GDP in 2023, the probability of economic viability of trastuzumab deruxtecan was 0. The results of scenario analysis showed that when the patient assistance program for trastuzumab deruxtecan was considered, the probability of trastuzumab deruxtecan regimen being economical was 0. However, when the price of trastuzumab deruxtecan was reduced by 70%, the probability of its being cost-effective was significantly increased to 82.80%. CONCLUSIONS At a WTP threshold of 3 times China’s per capita GDP in 2023, the trastuzumab deruxtecan regimen is not cost-effective compared to TPC regimen for the second-line treatment of advanced breast cancer with HER-2 low expression. Reducing the price of trastuzumab deruxtecan by region can improve its cost-effectiveness.

5.Trends and influencing factors of endurance performance of Han college students in Hunan Province during 1985-2019

Objective:
To analyze the trends and influencing factors of endurance performance of 19-22 years old college students in Hunan Province from 1985 to 2019, so as to provide objective and scientific basis for sports and health work in colleges and universities.
Methods:
A retrospective analysis was conducted on the data of 14 490 college students aged 19-22 in Hunan Province from 8 consecutive National Student Physical Fitness and Health Surveys conducted from 1985 to 2019. The analysis indexes were 1 000 m running for boys and 800 m running for girls.
Results:
From 1985 to 2019, the endurance running time of 19-22 years old Han college students in Hunan Province showed an obvious trend of decline. The 1 000 m running time of urban and rural male students increased by 41.9 and 45.4 s on average, and the 800 m running time of urban and rural female students increased by 29.5 and 30.6 s on average, respectively. Multiple linear regression analysis showed that age ( β =0.17), urban students (rural students as reference; β =0.44), GDP ( β =0.94) and urbanization level ( β = 0.44 ) were positively correlated with the average endurance running time of males. Urban students ( β =0.92), GDP ( β = 1.38 ) and Engel coefficient ( β =0.93) were positively correlated with the average endurance running time of females. BMI ( β =-0.47) was negatively correlated with the females mean time of endurance running ( P <0.05).
Conclusion
The endurance performance of Han college students in Hunan Province showed a declining trend from 1985 to 2019,which is associated with age, urban and rural distribution, regional GDP, Engel s coefficient, urbanization level and BMI. Effective measures should be taken to improve the physical quality of college students.

8. Effect and molecular mechanism research of Astragalus membranaceus on inhibiting intestinal absorption of six alkaloids of Aconitum carmichaelii in spleen deficiency rats

Objective: To investigate the effect and the mechanism of Astragalus membranaceus (Huangqi in Chinese, HQ) extract on the intestinal absorption of six alkaloids of Aconitum carmichaelii (Fuzi in Chinese, FZ) in rats with spleen deficiency and provide novel insights into the application of HQ on modulating intestinal barrier. Methods: Four-week-old male Sprague-Dawley rats were fed with Xiaochengqi Decoction to induce the spleen deficiency model for 40 d. Single-pass intestinal perfusion model were used to study the effects of HQ extract on the absorption of alkaloids. Protein expression and mRNA levels of MRP2 and BCRP and tight junction proteins (TJ, including Claudin-1, Occludin and ZO-1) were measured using Western blot and real-time PCR, respectively. The location and expression of TJ protein was also investigated by the immunofluorescence method. Results: Compared with the normal group, the protein expression of MRP2, BCRP and TJ proteins in the model group were significantly down-regulated. After oral administration of HQ, the alkaloid absorption in intestinal villi was inhibited, MRP2, BCRP and TJ proteins were up-regulated, the green fluorescence staining of Claudin-1, Occludin, and ZO-1 was enhanced, and a thick layer of mucus was deposited on the surface of the epithelium of the intestinal cavity. Conclusion: HQ as an intestinal barrier modulator improves the physiological changes of the intestinal environment of spleen deficiency to reduce the absorption of toxic components, leading to a decrease in the absorption of drug-like molecules.

Malignancies are diseases resulting from an imbalance of cell growth and proliferation, endangering human health and life. Currently, there is no clinically effective treatment for tumors. Tumor cells may alter cell adhesion and tumor cell migration and movement by degrading the extracellular matrix, generating vascular factors, affecting epithelial-mesenchymal transformation, or altering the tumor microenvironment. The mechanisms which lead to multidrug resistance (MDR) are the regulation of membrane proteins, apoptosis-regulated gene expression, enzyme-mediated multidrug resistance, DNA damage repair, and epithelial-stromal transformation, resulting in ineffective treatment of tumors. Therefore, the search for natural, safe, and effective chemosensitizers has become a critical part in tumor research. Due to the increasing use of Chinese medicine in cancer treatment, researchers have conducted more extensive studies on its monomers and compounds. In addition, the mechanisms of Chinese medicine in inhibiting tumor invasion and metastasis and reversing drug resistance are gradually unraveled. The monomers and compounds of Chinese medicine may inhibit tumor invasion, metastasis, and drug resistance by enhancing the sensitivity of chemotherapy drugs and adjuvant properties. Furthermore, they can also improve the tolerance of patients to chemotherapy drugs, relieve side effects of chemotherapy, reduce the chance of recurrence, and prolong the life of patients. The development of traditional Chinese medicine plays an important role in reducing tumor recurrence and metastasis, reversing drug resistance, prolonging the prognosis of cancer patients, improving their quality of life, and prolonging their survival time. Currently, various types of Chinese medicines have been proven to be capable of reducing tumor invasion and metastasis, and reversing drug resistance. The present article reviewed development and findings of Chinese medicine as an anti-tumor invasion, anti-metastasis, and anti-tumor resistance therapy in recent years, in order to provide ideas for future research on anti-tumor effect of active components in Chinese medicine.
10.SWOT analysis of influenza vaccination promotion of primary care staff based on the perspective of the supplier, customer, and management

Objective:To analyze the situation of influenza vaccination among primary healthcare workers, find out the problems, and explore the strategies and measures to promote influenza vaccination among grass-roots medical staff.Methods:From April to May 2021, key insider interviews and literature research were carried out based on the perspectives of influenza vaccine suppliers (influenza vaccine manufacturers), consumers (primary medical institutions and primary healthcare workers), and managers (governments at all levels, health administrative departments and disease prevention and control departments). The SWOT (strengths, weaknesses, opportunities, and threats) analysis technique was used to comprehensively evaluate the current situation of influenza vaccination among grass-roots healthcare workers, and a SWOT analysis matrix was established.Results:Influenza vaccination of grass-roots healthcare workers have advantages and opportunities, including primary medical and health institutions' vital influenza vaccination accessibility, influenza vaccine safety is higher, COVID-19 outbreak improves the public awareness of respiratory infectious diseases and vaccine production enthusiasm, coronavirus vaccination has strengthened the capacity of the vaccine distribution system. There are also disadvantages and threats such as the high price of influenza vaccine, insufficient supply, low awareness of influenza vaccine vaccination among grass-roots healthcare workers, lack of demand assessment mechanism on influenza vaccine, poor vaccine deployment, structural imbalance in vaccine supply in different areas, and severe vaccine waste. SWOT analysis matrix of the influenza vaccination status of grass-roots healthcare workers was established, forming dominant opportunity (SO) strategy, dominant threat (ST) strategy, inferior opportunity (WO) strategy, and inferior threat (WT) strategy.Conclusion:Measures should be taken by the supplier, the demand—side, and the management side to improve the influenza vaccine coverage rate of primary healthcare workers, but the emphasis should be on the coordination and management of the management side.
