1.Drug use patterns in the treatment of high-risk cervical HPV infection based on data mining

Objective To explore the rule of drug use in treating high-risk cervical human papilloma virus(HPV)infection by using data mining method.Methods Medical records and prescriptions of patients with high-risk HPV infection treated at Wang Jin Outpatient Department of Lishui Hospital of Traditional Chinese Medicine Affiliated to Zhejiang Chinese Medical University from January 1,2021 to December 1,2022 were collected.SPSS Modeler 18.0 and SPSS 25.0 were used for drug use frequency,association rules,clustering and other data analysis.Results A total of 100 prescriptions were included,involving a total of 162 Chinese materia medicas,with a total of 1445 times of Chinese materia medicas.The drugs with the highest frequency of use were mainly tonic drugs,antipyretic drugs,regulating vital energy drugs and tranquillizing drugs.Complex network analysis showed that there were close correlation among hedyotis,dried tangerine peel,immature tangerine peel,silktree albizia bark and buffalo horn.Three potential new prescriptions were explored.Conclusion The disease was mostly caused by deficiency in origin and excess in pathological products,and the treatment was to treat both symptoms and root causes,and to attack and supplement,with the treatment of clearing heat and removing toxicity,eliminating dampness,benefiting qi for activating blood circulation and nourishing yin,supplemented by nourishing heart for tranquillization.
2.Analysis of clinical data of necrotizing fasciitis secondary to intestinal fistulas and screening the mortality risk factors

Objective:To analyze the clinical data and to screen the mortality risk factors of necrotizing fasciitis (NF) secondary to intestinal fistulas (NFsIF).Methods:This study was a retrospective observational study. The data of all NFsIF cases who met the inclusion criteria and were admitted into Shandong Provincial Hospital Affiliated to Shandong First Medical University (hereinafter referred to as our unit) from January 2000 to October 2023, and in PubMed, Web of Science, Scopus, China National Knowledge Infrastructure, and Chinese Medical Journal Network databases from its establishment to October 2023 were retrieved and screened. Based on clinical outcomes, the cases were divided into survival group (47 males and 24 females) and death group (16 males and 7 females), and the mortality rate was calculated. Clinical data of patients in the two groups including age, underlying diseases (most related to NF), symptom duration before presentation, white blood cell count, causes of NF, signs of peritonitis, scope of NF involvement, and intestinal management and wound management measures were compared and analyzed to screen the risk factors of death in 94 patients with NFsIF.Results:A total of 94 valid cases were collected, including 90 patients reported in the literature and 4 patients admitted to our unit, with the mortality rate of patients being 24.5% (23/94). Univariate analysis showed that there were no statistically significant differences in age, underlying diseases, symptom duration before presentation, white blood cell count, causes of NF, signs of peritonitis, scope of NF involvement between patients in the two groups ( P>0.05); there were statistically significant differences in intestinal treatment and wound treatment between the two groups (with χ2 values of 17.97 and 8.33, respectively, P<0.05). Multivariate logistic regression analysis showed that both intestinal treatment measures and wound treatments measures were independent risk factors for death in 94 NFsIF patients, among which first-stage colostomy+late-stage reconstruction and negative presssure therapy had higher protective effects (with odds ratios of 0.05 and 0.27, respectively, 95% confidence intervals of 0.01-0.33 and 0.08-0.88, respectively, P<0.05). Conclusions:The mortality risk of patients with NFsIF is high. Based on comprehensive treatments, active intestinal and wound treatment may be the key to avoid death, with first-stage colostomy+late-stage reconstruction and negative pressure therapy having higher protective effects.
3.Exploring the effect of Xinjia Erjia Dihuang Tang on follicular development in PCOS model rats based on the Kisspeptin/GPR54 system

Objective To investigate the development of follicles in polycystic ovary syndrome(PCOS)model rats treated with Xinjia Erjia Dihuang Tang and its possible mechanism.Methods Twenty-eight female SD rats with regular estrous cycle were selected and randomly divided into normal control group,model group,Chinese medicine group and Western medicine group,with 7 rats in each group.In addition to the normal control group,the other three groups were continuously given 0.1mg/(kg·d)of letrozole-carboxymethyl cellulose suspension to construct the PCOS rat model.Since the 22nd day,the traditional Chinese medicine group was given 5.268g/(kg·d)of Xinjia Erjia Dihuang Tang,the western medicine group was given 0.286mg/(kg·d)of ethinylestradiol cyproterone tablet,and the normal control group and model group were given 10ml/(kg·d)of distilled water.After 3 weeks,the ovarian coefficients of the rats in all groups were compared,and the morphological changes of the ovarian tissues of the rats in all groups were observed by hematoxylin-eosin staining.The serum hormones of the rats in all groups were detected by enzyme-linked immunosorbent assay,and the ovarian kisspeptin(Kp)and G-protein-coupled receptors(GPR54)protein expression levels were detected by Western blot.Results Compared with the normal control group,the follicles in the ovary of the model group showed more cystic dilatation and atresia,and the number of granular cell layers decreased,the ovarian coefficient of the model group was increased;Serum levels of testosterone(T),luteinizing hormone(LH)and Kp were increased in model group;Serum levels of follicle stimulating hormone(FSH)and estradiol(E2)and protein expressions of Kp and GPR54 in ovarian tissue of rats in the model group were significantly decreased(P<0.05).After treatment with Chinese medicine,compared with the model group,the number of follicles in the Chinese medicine group decreased,the layer number of granulosa cells increased,there were nearly mature follicles,and a small amount of corpus luteum existed,serum LH,T and Kp levels were decreased,serum FSH and E2 levels and ovarian Kp and GPR54 protein expressions were significantly increased(P<0.05).Conclusion The development of follicles in PCOS model rats was improved by the intervention of Xinjia Erjia Dihuang Tang.The therapeutic mechanism may be to affect the release of FSH and LH by regulating Kisspeptin/GPR54 system,so as to affect hormone content,adjust ovarian function,and improve follicle development and ovulation ability.
4.Optimization of the extraction technology for the raw drugs of Sanse powder gel paste

OBJECTIVE To optimize the extraction technology for the raw drugs of Sanse powder gel paste. METHODS SD rats were divided into blank group, model group, traditional technology group, water extraction group and ethanol extraction group, with 5 rats in each group. Anterior cruciate ligament transection was used to construct knee osteoarthritis model, and the pharmacodynamic effects of different extraction methods on arthritic rats were investigated. Analgesic experiments were conducted using cold and hot pain thresholds and pain mediators calcitonin gene-related peptide (CGRP), cyclooxygenase-2 (COX-2), substance P (SP), and prostaglandin E2 (PGE2) contents as indicators. HE staining was performed on the synovial membrane of rats to observe the degree of synovial cell proliferation, inflammatory infiltration and vascular invasion, and anti-inflammatory experiments were conducted using protein and mRNA expressions of interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α) and IL-6 as indicators. The analgesic and anti-inflammatory effects were compared among those groups. In the orthogonal test, ethanol dosage, extraction time and extraction times were used as evaluation factors, and the contents of casticin, strychnine and toxiferine were taken as evaluation indicators; comprehensive score was calculated. The validation experiments were carried out after optimizing the extraction technology of the raw drugs of Sanse powder gel paste. RESULTS Compared with the model group, the cold and heat pain thresholds of drug administration groups (except for the traditional technology group) were all increased significantly (P<0.05), while the contents of pain (No.Y2021rc02) mediators CGRP, COX-2, SP and PGE2 were all decreased significantly (P<0.05). HE staining showed that inflammatory cell infiltration, fibrosis and collagen deposition were 炎。E-mail:liuzixiu3221@126.com decreased in the administration groups; a small amount of capillary proliferation could be found; the protein and mRNA expressions of inflammatory factors such as IL-1β, IL-6 and TNF-α were decreased significantly in synovial tissue of rats in administration groups (P<0.05). Compared with the traditional technology group, most indicators of the ethanol extraction group were significantly reduced (P<0.05), and only heat pain threshold and mRNA expression of IL-6 in rats were decreased significantly in the water extraction group (P<0.05). The optimal extraction technology of the raw drugs of Sanse powder gel paste included suitable dose of Sanse powder, 8-fold 55% ethanol, heating reflux extraction for 90 minutes, extracting twice. The results of 3 times of verification experiments showed that the average contents of casticin, strychnine and toxiferine were 0.007%, 0.092%, and 0.214%, respectively; RSD were all less than 5%. CONCLUSIONS The optimized extraction technology for the raw drugs of Sanse powder gel paste is stable and feasible, which can improve the efficacy of the preparation.
5.Comparison of Effect of Euodiae Fructus Processed with Different Proportions of Glycyrrhizae Radix et Rhizoma on Oxidative Stress and Autophagy in Mouse Liver Under Toxic Dose

ObjectiveTo investigate the principle and scientific connotation of Euodiae Fructus(EF) processed with Glycyrrhizae Radix et Rhizoma(Gly) by comparing the effects of unprocessed products of EF(UEF) and processed products of EF with the different proportions of Gly(GEFs) at toxic doses on oxidative stress and autophagy in the liver of mice. MethodSeventy mice were randomly divided into 7 groups, namely the control group, the UEF group, the group of the processed products of EF without Gly(PEF) and 4 groups of GEFs(the mass ratios of EF to Gly were 100∶3, 100∶6, 100∶12 and 100∶24, respectively, hereinafter referred to as the processed products of EF with the mass ratios of 100∶3, 100∶6, 100∶12 and 100∶24 of Gly). The mice were given purified water, the decoction of UEF, PEF and GEFs by gavage at a dose of 30 g·kg-1. PEF and GEFs were prepared according to the method under EF in the 2020 edition of Chinese Pharmacopoeia. Levels of alanine aminotransferase(ALT) and aspartate aminotransferase(AST) were determined by ultraviolet-visible spectrophotometry, hematoxylin-eosin(HE) staining was used to evaluate the pathological changes of liver tissue, the level of reactive oxygen species(ROS) was detected by fluorescence method, the mRNA expression of heme oxygenase-1(HO-1), quinone oxidoreductase-1(NQO1), glutathione-S-transferase 1(GSTA1), Kelch-like epichlorohydrin-associated protein 1(Keap1) and p62 were measured by Real-time fluorescence quantitative polymerase chain reaction(Real-time PCR), western blot was used to detect the protein expression of phosphorylated mammal target of rapamycin(p-mTOR), phosphorylated ribosomal p70 S6 protein kinase(p-p70S6K), p62, microtubule-associated protein 1 light chain 3Ⅰ(LC3Ⅰ) and LC3Ⅱ. ResultCompared with the control group, after 7 d of administration, the increase in body mass of mice in the UEF group began to slow down and the difference gradually increased, and the liver body index significantly increased(P<0.01), pathomorphological observation showed that the structure of hepatic lobules was disordered, and local hepatic sinuses were narrowed or disappeared, and there were inflammatory infiltration and local bleeding, the levels of ALT and AST in serum and ROS in liver tissue were significantly increased(P<0.01), and the expressions of Keap1, HO-1, NQO1, GSTA1, p62 mRNA and p-mTOR, p-p70S6K, p62 protein in liver tissue were significantly decreased(P<0.01), and LC3Ⅱ/LC3Ⅰ was significantly increased(P<0.01). Compared with the UEF group, the body mass of mice increased, and the liver body index, the levels of ALT and AST in serum, and the level of ROS in liver tissue all decreased in the groups of PEF and GEFs. Among these groups, only the liver lobules in GEF(100∶6) group were intact, and the size of liver sinuses was close to that in the control group. The mRNA expressions of Keap1, HO-1, NQO1, GSTA1 and p62 in liver tissue showed an overall upward trend in the groups of PEF and GEFs. Among these groups, only the ones of the above mRNA in the GEF(100∶6) group had a significant increase(P<0.05, P<0.01). The protein expressions of p-mTOR, p-p70S6K, p62 and LC3Ⅱ/LC3Ⅰ had a callback in the groups of PEF and GEFs, of which the protein expressions of p-mTOR, p-p70S6K and LC3Ⅱ/LC3Ⅰ in the GEF(100∶6) group and the expression of p62 protein in the GEF(100∶24) group had the largest callback. Except for p-mTOR protein, other protein expressions were statistically significant(P<0.05, P<0.01). ConclusionThe hepatotoxicity of EF is closely related to its ability to induce oxidative stress, which leads to pathological autophagy and hepatocyte damage. This ability can be reduced by the processing with different proportions of Gly, especially the ratio of 100∶6.
6.Method establishment and clinical practice for concentration determination of caffeine and its metabolites in urine

OBJECTIVE To establish a method for concentration determination of caffeine and its three metabolites, theophylline, paraxanthine and theobromine in urine, and apply it in clinical practice. METHODS Using caffeine-13C3-d3 as internal standard (IS), and the urine samples were protein precipitated with acetonitrile; HPLC-MS/MS method was adopted to determine the concentrations of caffeine and its three metabolites. The determination was performed on Waters ACQUITY UPLC® BEH HILIC column with mobile phase consisting of 60 mmol/L ammonium acetate (A)-acetonitrile (B) (gradient elution) at the flow rate of 0.5 mL/min. The column temperature was set at 38 ℃ , and the sample size was 2 μL. The electrospray ionization detection was operated in a positive mode by multiple reaction monitoring. The detection ions for quantitative analysis were m/z 195.1→110.0 for caffeine, m/z 181.1→124.0 for theophylline, m/z 181.1→124.0 for paraxanthine, m/z 181.1→138.0 for theobromine, and m/z 198.1→ 140.1 for IS. The above method was used to determine the concentrations of caffeine and its three metabolites in the urine of 19 infants with apnea of prematurity (AOP). RESULTS The linear ranges of mass concentration of caffeine, theophylline, paraxanthin and theobromine were 0.200-200, 0.050-50.0,0.050 0-50.0, and 0.100-100 μg/mL, respectively. The lower limits of quantification were 0.200, 0.050, 0.050 and 0.100 μg/mL (r>0.990), respectively. RSDs of intra-day and intra- day precision were not above 10.37%, and matrix factors were 85.68%-109.90%; extraction recoveries were 93.53%-109.40% (RSD≤15%), and RSDs of stability tests were all lower than 15%. The concentrations of caffeine and its three metabolites in the urine of 19 cases were (27.346±7.951), (0.351±0.223), (0.428±0.395) and (0.472±0.374) μg/mL, respectively. CONCLUSIONS The established HPLC-MS/MS method is simple, sensitive and can be used for the determination of caffeine and its three metabolites in urine samples of AOP.
7.LC-MS Combined with Network Pharmacology to Explore the Mechanism of “Wenyujin Rhizoma Concisum-Angelicae Sinensis Radix” Essential Oil in the Treatment of Knee Osteoarthritis

OBJECTIVE To extract the essential oil of “Wenyujin Rhizoma Concisum-Angelica Sinensis Radix”, and to qualitatively analyze the chemical composition of the extracted essential oil by LC-MS. Network pharmacology was used to explore the mechanism of the essential oil acting on knee osteoarthritis(KOA). METHODS The essential oil of “Wenyujin Rhizoma Concisum-Angelicae Sinensis Radix” was extracted by steam distillation, the components of the essential oil were analyzed by LC-MS. PubChem, Swiss Target Prediction, GeneCards, DAVID and other databases were used to predict disease targets and pathways of active component in prevention and treatment of KOA. The key genes were analyzed by Gene ontology(GO) analysis and Kyoto Encyclopedia of Genes and Genomes(KEGG) pathway enrichment analysis. Cytoscape was used to construct a visual network map of “active component-disease targets”. AutoDock and Pymol were used to verify the molecular docking between the target with the highest degree target and the active components. RESULTS A total of 59 compounds were identified in the “Wenyujin Rhizoma Concisum-Angelicae Sinensis Radix” essential oil, and the main components were curzerenone, isocurcumenol, ligustilide, etc. Through network pharmacology, 28 potential targets of essential oil acting on KOA were screened, including CTSK, PTGS1, PTGS2 and ESR1, etc. KEGG enrichment analysis predicted that “Wenyujin Rhizoma Concisum-Angelicae Sinensis Radix” essential oil mainly targeted TNF signaling pathway, osteoclast differentiation, IL-17 signaling pathway, relaxin signaling pathway, MAPK signaling pathway to treat KOA. The results of molecular docking showed that the screened active components and their corresponding target proteins had positive binding activity. CONCLUSION LC-MS combined with network pharmacology is used to preliminaries explore the main active components and the potential targets and mechanism of action of “Wenyujin Rhizoma Concisum-Angelicae Sinensis Radix” essential oil in the intervention of KOA, providing ideas for the further study of “Wenyujin Rhizoma Concisum-Angelicae Sinensis Radix” essential oil.
8.Summary of the best evidence on non-pharmacological interventions for orthostatic hypotension in patients with Parkinson disease

Objective:To search for and summarize the best evidence on non-pharmacological interventions for orthostatic hypotension in patients with Parkinson disease.Methods:Based on the 6S model, the relevant guidelines, clinical decisions, evidence summaries, systematic reviews and expert consensus on non-pharmacological interventions for orthostatic hypotension in Parkinson disease patients were systematically retrieved from domestic and foreign databases. The search time was from the establishment of the database to December 2022. Two researchers independently screened the literature, evaluated the quality of the included literature, and extracted and summarized evidence that met the quality standards.Results:A total of 15 articles were included, including 4 guidelines, 2 clinical decision-making articles, 1 evidence summary article, 3 systematic evaluations and 5 expert consensus articles. A total of 24 pieces of best evidence were summarized from 7 aspects, including purpose, evaluation, capacity intervention, exercise intervention, posture intervention, physical intervention, health education and support.Conclusions:The best evidence on non-pharmacological intervention for orthostatic hypotension in patients with Parkinson disease can provide a reference for the practice of clinical medical staffs. It is suggested to apply the best evidence in combination with the patient's condition, preference and clinical environment, so as to reduce the incidence of orthostatic hypotension in patients with Parkinson disease and to ensure the safety of patients.
