1.Progress in the detection of latent tuberculosis infection

ObjectiveTo introduce the three main techniques for tuberculosis screening currently used in China, to systematically evaluate their accuracy in diagnosing latent tuberculosis infection (LTBI), so as to provide scientific basis and recommendations for the formulation of China’s tuberculosis screening strategy. MethodsLiterature on the diagnosis of tuberculosis by tuberculin skin test (TST), interferon-γ release assay (IGRA), and recombinant Mycobacterium tuberculosis fusion protein (EC) skin test from January 1, 2010 to August 22, 2024 was comprehensively retrieved from PubMed, China National Knowledge Infrastructure (CNKI), and Wanfang Database through computerized search. Besides, all the literature was screened in accordance to the inclusion criteria for diagnostic tests, and characteristic information of the literature selected was extracted simultaneously. Meta-analysis was performed using Stata 17.0 software, with a random-effects model used for weighted quantitative synthesis of included literature, calculating pooled sensitivity, specificity, positive likelihood ratio, negative likelihood ratio, and their 95% confidence intervals (CI). ResultsA total of 543 relevant articles were retrieved, with 105 ultimately included. Among them, 33 articles reported diagnostic data for TST, with a pooled sensitivity of 0.68 (95%CI: 0.62‒0.73), specificity of 0.67 (95%CI: 0.60‒0.73), positive likelihood ratio of 2.0 (95%CI: 1.7‒2.5), and negative likelihood ratio of 0.48 (95%CI: 0.40‒0.58). Ninety-four articles reported the diagnostic value of IGRAs test, with a pooled sensitivity of 0.88 (95%CI: 0.87‒0.89), specificity of 0.82 (95%CI: 0.79‒0.84), positive likelihood ratio of 4.8 (95%CI: 4.2‒5.6), and negative likelihood ratio of 0.15 (95%CI: 0.13‒0.17). Data on EC skin test was limited, but preliminary analysis showed that it had high sensitivity and specificity. ConclusionIGRA has a significant advantage in diagnosing LTBI, and EC skin test also shows good diagnostic performance, although relevant data is limited. TST remains suitable for large-scale screening due to its cost-effectiveness.
2.Bioinformatics Analysis and Validation of Cuproptosis-related Genes in Wilson Disease

ObjectiveTo explore the role of cuproptosis and identify cuproptosis-related genes in Wilson disease (WD) through bioinformatics analysis and clinical validation,providing implications and directions for the diagnosis and treatment of WD. Methods(1) Screening of target genes: The differentially expressed genes (DEGs) between WD and healthy control were obtained from GeneCards,and the cuproptosis-related genes were obtained from Gene Expression Omnibus (GEO) and published literature.The cuproptosis-related genes in WD were obtained by intersection.Through gene ontology (GO) and Kyoto Encyclopedia of Genes and Genomes (KEGG) enrichment analyses,the specific biological process,functions or metabolic pathways of cuproptosis-related genes in WD were predicted.Molecular docking and PyMOL visualization were then performed to analyze and verify the potential regulatory mechanism of Gandou Fumu Decoction for cuproptosis.(2)Validation of target genes: The blood samples of 15 WD patients treated in the department of encephalopathy and 15 healthy volunteers undergoing physical examinations in the health management center were randomly collected from the First Affiliated Hospital of Anhui University of Chinese Medicine.The expression levels of target genes were determined by Western blot and real-time PCR. Results(1) A total of 3 607 DEGs in WD were obtained from GSE107323 in GEO,and 68 cuproptosis-related genes were obtained from GeneCards and published literature.Twelve common target genes were obtained by intersection,including three up-related genes(SQSTM1,MIF1,and TAX1BP1) and nine down-regulated genes(CP,SERPINE1,AOC3,GPX4,SLC27A5,VEGF-A,PDHB,PDK1,and ATP7B).The common target genes were mainly enriched in monocarboxylic acid metabolism,oxidoreductase activity,negative regulation of molecular functions,which mainly involved HIF-1,ferroptosis and other signaling pathways.Molecular docking and PyMOL visualization results showed Gandou Fumu Decoction had good binding ability with the cuproptosis-related genes PDK1,SERPINE1,VEGFA,and AOC3 in WD.(2)A total of 30 blood samples were collected,including 15 WD patients and 15 health volunteers.Western blot results showed that expression levels of target genes were consistent with the results obtained by bioinformatics analysis.RT-qPCR results showed that compared with healthy volunteers,WD patients had down-regulated mRNA levels of SERPINE1,GPX4,SLC27A5,and VEGF-A and up-regulated mRNA levels of SQSTM1 and MIF1(P<0.05). ConclusionThe expression levels of cuproptosis-related genes in WD patients are consistent with the results predicted by bioinformatics analysis.The characteristic preparation Gandou Fumu Decoction of Xin'an Medicine showed good binding abilities with the cuproptosis-related genes in WD.Cuproptosis may play a key role in the pathophysiological mechanism of WD,which can provide a new target for the diagnosis and treatment of WD.
3.The epidemiology and risk factors of delayed diagnosis in newly reported HIV/AIDS cases in Xuhui District, Shanghai, 2018‒2022

ObjectiveTo investigate the trends and significant determinants of delayed HIV diagnosis (DHD) among newly reported HIV/AIDS cases in Xuhui District, Shanghai between 2018 and 2022. MethodsIn the newly reported HIV/AIDS cases, patients died within one year without accident, HIV/AIDS cases with CD4 cell count <200 cells·μL-1, and AIDS cases with a CD4 cell count between 200 to 499 cells·μL-1 were defined as delayed diagnosis. Univariate and multivariate logistic analysis were employed to explore the influencing factors of DHD. ResultsAmong the 862 newly reported HIV/AIDS cases, The DHD rate was 39.79% without statistically significant difference by year(χ2=4.508, P=0.342). Patients with CD4 cell count <200 cells·μL-1 contributed the largest proportion of DHD. During 2018‒2022, the DHD rate declined among HIV/AIDS patients who were younger than 35 years old or 45‒65 years old, never married, original diagnosis from tertiary specialized hospitals. Patients who were 65 years or older, married or divorced, with heterosexual transmitted HIV/AIDS, and original diagnosis from other types of testing and tertiary metropolitan hospital, had sustainably higher DHD rates. The number of HIV screening and diagnosed from voluntary counseling and testing (VCT) decreased during the COVID-19 epidemic, while the DHD rate increased sharply. Multivariate logistic regression analysis suggested the DHD rates were higher among older age, other types of testing(OR=3.805, 95%CI: 2.260‒6.406)and pre-operative testing(OR=2.411, 95%CI: 1.424‒4.081). Patients who received CD4 test in 15 days had a higher DHD rate compared to the cases received CD4 test exceeding 90 days (OR=0.336, 95%CI: 0.216‒0.522). ConclusionThere is no significant decrease of delayed HIV diagnosis rate in Xuhui District in recent years, and the number of HIV tests has decreased in 2022. Monitoring of newly reported HIV/AIDS should be conducted continuously. Expansion of HIV antibody screening should be conducted in non-infectious departments and inpatient departments in healthcare institutions, particularly metropolitan hospitals. Assistance should be provided to clinicians and elderly patients for improving their ability to recognize and perceive the risk of HIV/AIDS, in order to enhance early diagnosis and subsequent treatment.
4.Clinical and imaging features of invasive Klebsiella pneumoniae liver abscess syndrome

Objective To observe the clinical and imaging features of invasive Klebsiella pneumoniae liver abscess syndrome(IKPLAS).Methods Data of 68 patients with Klebsiella pneumoniae liver abscess(KPLA)were retrospectively analyzed.The patients were divided into IKPLAS group(n=25)or non-IKPLAS group(n=43)according to extrahepatic invasive infection or not.Clinical data as well as CT and/or MRI findings were compared between groups.Results The patients'age was lower,while glycated hemoglobin and D-dimer levels were higher in IKPLAS group than those in non-IKPLAS group(all P<0.05).Hepatic venous thrombophlebitis was detected in 18 cases in IKPLAS group and 6 cases in non-IKPLAS group,while arterial phase abnormal high perfusion around abscess was noticed in 10 cases in IKPLAS group and 28 cases in non-IKPLAS group,both being significantly different between groups(both P<0.05).The extrahepatic infection in IKPLAS group mainly observed in lungs(19/25,76.00%).Conclusion The ages were lower,while glycated hemoglobin and D-dimer levels were both higher in IKPLAS than in the other KPLA patients.Often appeared hepatic venous thrombophlebitis and extrahepatic infection mainly affecting lungs were imaging characteristics of IKPLAS.
5.Role of TLR4/ERK1/2 signaling pathway in decidual tissue of unexplained spontaneous abortion

Objective:To investigate the expression of Toll-like receptor 4(TLR4)and extracellular signal-regulated protein kinase 1/2(ERK1/2)in decidua tissue of patients with unexplained spontaneous abortion and their correlation.Methods:The expres-sions of TLR4,ERK1/2 and p-ERK1/2 in decidua tissues of 32 patients with unexplained spontaneous abortion(abortion group)and 32 normal pregnancy(control group)were detected by immunohistochemistry and Western blot,respectively.The correlation between TLR4 and p-ERK1/2 in abortion group were analyzed by Pearson hierarchical correlation analysis.Results:In immunohistochemical experiments,the cytoplasm of decidua cells in the two groups is the expression locus of TLR4,ERK1/2 and p-ERK1/2,the expression of the three proteins were different,and the expressions of TLR4 and p-ERK1/2 in abortion group were significantly higher than that in control group(P<0.01).There was no significant difference in ERK1/2 expression between abortion group and control group(P>0.05);In decidua tissue samples of abortion group,the protein level of TLR4 was higher than that of control group(P<0.05);the pro-tein level of p-ERK1/2 was significantly higher than that of control group(P<0.01),and the protein level of ERK1/2 in decidua tissue of abortion group was not statistically different from that of control group(P>0.05).TLR4 was positively correlated with p-ERK1/2 expression in abortion group(r=0.890,P<0.01).Conclusion:Abnormal activation of TLR4/ERK1/2 signaling pathway may be one of the mechanisms of unexplained spontaneous abortion.
6.Meta-analysis of risk factors for multi-drug resistant organisms infections after liver transplantation

Objective:To systematically evaluate influencing factors of multi-drug resistant organisms (MDROs) infection in patients undergoing liver transplantation and to provide reference for infection prevention and control.Methods:The relevant literatures on the influencing factors of MDROs infection after liver transplantation included in PubMed, Medline, Embase, Wed of Science, China Biology Medicine disc, Wanfang Database, China National Knowledge Infrastructure and VIP were searched by computer, and the search time was from the establishment of databases to September 1, 2023. The languages were limited to Chinese and English. Two evaluators screened literature and extracted data, and used The Newcastle Ottawa Scale to evaluate the quality of the final included literature. Meta-analysis was performed using RevMan5.4 software.Results:Finally, a total of 17 literatures were included, including five case-control studies and 12 cohort studies. The results of Meta-analysis showed that ClassⅡ toⅣ hepatic encephalopathy, renal insufficiency, colonization of drug-resistant organisms before transplantation, performing tracheal intubation, tracheal intubation time greater than or equal to 48 h, tracheal intubation time greater than or equal to 72 h, length of ICU stay, use of antibiotics before transplantation, postoperative bile leakage, postoperative fungal culture of positivity, reoperation after transplantation and hemodialysis were risk factors for MDROs infection after liver transplantation ( P<0.05) . Conclusions:There are many influencing factors for multi-drug resistant organisms infection in liver transplant patients after surgery. Medical staff should be vigilant and identify relevant risk factors, conduct infection risk assessment and take preventive and control measures based on risk factors to improve the prognosis of liver transplant patients.
7.Sexually transmitted infections (STIs) and related factors among pregnant women with non-local household registration in Xuhui District, Shanghai

ObjectiveTo study the sexually transmitted infections (STIs) in pregnant women with non-local household registration in Xuhui District and analyze the risk factors for STIs. MethodsFrom April 2020 to March 2022, pregnant women with non-local household registration who received their first prenatal examination in a general hospital in Shanghai Xuhui District were selected to conduct a status survey of STIs. Logistic regression model was used to analyze the influencing factors of bacterial vaginitis and ureaplasma uaplasma infection. ResultsThe top three infection rates in the pregnant women were Ureaplasma urealyticum (13.2%), bacterial vaginosis (9.8%) and mycotic vaginitis (4.7%). Age between 25 and 35 years (aOR=0.53,95%CI: 0.28‒0.98) and monthly income ≥8 000 yuan (aOR=0.30,95%CI: 0.11‒0.82) were significantly correlated with ureaplasma uaplasma infection. Pregnancy number of 2 (aOR=4.95, 95%CI: 1.59‒15.46), first sexual relationship occurred before marriage (aOR=2.83, 95%CI: 1.04‒7.74), husband's alcoholism (aOR=3.83, 95%CI: 1.08‒13.56), high school education (aOR=0.27, 95%CI: 0.08‒0.93), and husband's travel history (aOR=0.30, 95%CI: 0.12‒0.79) were significantly correlated with bacterial vaginitis. ConclusionPregnant women with more gestation times, first sexual intercourse before marriage and husband with heavy drinking are more likely to be infected with bacterial vaginosis. Pregnant women with younger age and lower income are more likely to be infected with Ureaplasma urealyticum. Follow-up and monitoring should be strengthened in these groups.
8.Latent tuberculosis infection and screening method among close contacts in schools in Xuhui District,Shanghai from 2020 to 2022

ObjectiveTo investigate the latent tuberculosis infection (LTBI) of close contacts in schools of Xuhui District, and to explore the tuberculin skin test (TST)- interferon-γ release assay (IGRA) two-step method in order to discover the screening strategy of tuberculosis in Xuhui District. MethodsClose contacts of tuberculosis in schools of Xuhui District from 2020 to 2022 were selected as research subjects. Screening was conducted using symptom questionnaire, TST, chest X-rays, IGRA, and the information including the etiological results and grade of the index cases, as well as gender, age, and relationship with the index cases of the research subjects were collected. ResultsTotally 615 close contacts of 32 tuberculosis cases occurred in the schools were finally included. Of the 609 close contacts who completed tuberculosis infection screening and underwent TST testing, 153 TST(+) individuals underwent IGRA testing. The final LTBI rate was 4.6%, and the pulmonary tuberculosis detection rate was 163 per 100 000. The relationship with the index cases was an influencing factor for LTBI. The IGRA positivity rate was higher among close contacts with TST ≥15 mm than among those with 10 mm≤ TST <15 mm (χ2=14.41, P<0.05). ConclusionThe latent tuberculosis infection among close contacts of school tuberculosis cases in Xuhui District remains serious. TST-IGRA two-step method can assist in the accurate diagnosis of LTBI and pulmonary tuberculosis cases.
9.The effect of kinesio taping on the head and neck mobility of children with congenital muscular torticollis

Objective:To explore any effect of kinesio taping on the head and neck mobility of children with congenital muscular torticollis.Methods:A total of 52 children with congenital muscular torticollis were randomly divided into a treatment group and a control group, each of 26. All were given 20 minutes of routine rehabilitation training daily, 5 days a week for 4 weeks. The treatment group was additionally taped up with kinesio tape for no more than 48 hours once every 3 days for 4 weeks. Before and after the treatment the angles of head tilt and neck lateral flexion and the gaps in left and right neck rotation were measured for both groups.Results:Significant improvement was observed in both groups but there was significantly more improvement among the treatment group. The treatment had a cure rate (46.15%) and an improvement rate (50.00%) significantly superior to those of the control group and a significantly lower rate of ineffectiveness.Conclusion:Kinesio taping along with routine rehabilitation training can better improve the head and neck mobility of children with congenital muscular torticollis.
10.The effect of virtual reality balance training on the balance and the ability in the activities of daily living of children with cerebral palsy

Objective:To observe the effect of virtual reality (VR) balance training on the balance and ability in the activities of daily living (ADL) of children with cerebral palsy.Methods:Forty children with spastic cerebral palsy were divided at random into an intervention group (21 cases) and a control group (19 cases). All received 30 minutes of daily routine rehabilitation training, including therapeutic exercise, occupational therapy and home rehabilitation training. Everyone also received 20 minutes of balance training 5 days a week for 3 months. That included sitting and standing balance training and training in transferring the center of gravity. Beyond that the intervention group additionally trained on the Biomaster virtual reality training system doing image matching, playing football and skiing. Before the experiment and after 1 and 3 months, both groups′ balance trajectory length, peripheral area, and the unit area the trajectory length were documented and also using the Berg balance scale (BBS). ADL ability was quantified using the modified Barthel index (MBI).Results:①After the 3 months of training there was a significant improvement in the average trajectory length, peripheral area and unit area trajectory length of both groups sitting and standing. The average values of the intervention group were, however, significantly superior to the control group′s averages. ②After the 3 months there were also significant improvements in the groups′ average BBS and MBI scores, again with significantly greater improvement in the intervention group.Conclusion:Virtual reality balance training can significantly improve the balance function and ADL ability of children with cerebral palsy.
