1.Characterization of molecular transmission network and drug resistance in newly diagnosed HIV-1 population aged 50 years and above in Huzhou City, Zhejiang Province

ObjectiveTo analyze the characteristics of HIV-1 molecular network and pretreatment drug resistance genes in the middle-aged and elderly people aged ≥50 years in Huzhou City, Zhejiang Province, and to provide an evidence for the prevention and control of AIDS epidemic. MethodsA total of 332 samples from the newly reported and untreated AIDS patients aged ≥50 years in Huzhou City from January 2020 to December 2023 were collected, pol genes were amplified by reverse transcription polymerase chain reaction (RT-PCR) and nested polymerase chain reaction (nest⁃PCR). Phylogenetic trees analyzing the subtypes were constructed, and a molecular network with a gene distance threshold of 1.0% were constructed at the same time. Mutation sites of drug resistance-related genes were identified through the Data Analysis and Detection System of HIV-1 Resistance Gene Detection of Stanford University, USA. ResultsSequence samples of 308 patients were obtained, and9 genotypes were identified, including CRF07_BC in 172 cases (55.8%), CRF01_AE in 61 cases (19.8%), CRF08_BC in 43 cases (14.0%), CRF85_BC in 9 cases (2.9%), and CRF55_01B in 8 cases (2.6%), subtype B in 5 cases (1.6%), subtype C in 4 cases (1.3%), CRF67_01B in3 cases (1.0%), and unique recombination URF01_AE/07_BC in 3 cases (1.0%). When the gene distance threshold was 1.0%, 28 molecular clusters were formed, and 139 cases were connected to the network, with an access rate of 45.0%. The largest transmission cluster C1 contained 44 cases infected with CRF07_BC subtype, all of whom were heterosexually transmitted, and predominantly by males. A total of 30 patients were found to have low-grade or higher drug resistance mutations, and the pretreatment drug resistance rate was 9.7% (30/308). Among them, there were 5 cases (16.7%) of protease inhibitor (PI) related drug resistance mutations, and 26 cases (86.7%) of non-nucleoside reverse transcriptase inhibitors (NNRTI) related drug resistance mutations. ConclusionCRF07_BC is the subtype with the most clusters among the middle-aged and elderly infected patients aged ≥50 years in Huzhou City. Middle-aged and elderly transmission clusters are formed within the three counties of WX, NX and CX through related activities. Molecular network monitoring on newly reported cases aged ≥50 years in Huzhou City should be strengthened so that the new characteristics of epidemic changes can be detected in time, providing a scientific basis for adjusting AIDS prevention and control measures for the elderly.
2.Effects of narrative nursing in patients with retinal detachment undergoing surgery

Objective:To explore the effect of narrative nursing in patients with retinal detachment undergoing surgery.Methods:From February 2020 to August 2021, 40 patients with retinal detachment who underwent surgical treatment in the Department of Geriatrics and Ophthalmology of Beijing Tongren Hospital, Capital Medical University were selected as the research object by the convenient sampling method. According to the random number table method, the patients were divided into the observation group and the control group, 20 cases in each. The patients in the control group were given routine nursing, and the patients in the observation group received narrative nursing on this basis. Before and after the intervention, Self-Rating Anxiety Scale (SAS) , Self-Rating Depression Scale (SDS) , Memorial University of Newfoundland Scale of Happiness (MUNSH) , and Generic Quality of Life Inventory-74 (GQOLI-74) were used to investigate the two groups of patients. The hospitalization time and complications of the two groups of patients were recorded.Results:At discharge, the SAS and SDS scores of the observation group were lower than those of the control group, the GQOLI-74 and MUNSH scores of the observation group were higher than those of the control group, and the differences were statistically significant ( P<0.01) . The hospitalization time of the observation group was shorter than that of the control group, and the difference was statistically significant ( P<0.05) . There was no significant difference in the number of cases of postoperative intraocular hypertension between the two groups ( P>0.05) . Conclusions:Narrative nursing can effectively relieve anxiety and depression in patients with retinal detachment surgery, improve patients' well-being and quality of life, and shorten hospitalization time.
3.Changes of T cell immune mechanism in patients with acute exacerbation of chronic obstructive pulmonary disease

Objective To investigate the changes of T lymphocyte immune balance in the patients with COPD.Methods Forty-eightpatients with AECOPD from January 2015 to December 2016 in this hospital were selected as study group and 30 healthy subjects as control.T cells and cytokines in peripheral blood were detected by flow cytometry and ELISA,respectively.The levels of them were compared between the two groups.The correlations of them and principal components were analyzed among the two groups.Results (1) CD8+T cells in the study group decreased [23.8%(17.5%,30.1%) vs.28.2%(23.8%,31.5%),P<0.05],TregandTh17increased[8.5%(8.3%,9.0%) vs.5.6%(4.9%,6.1%),P<0.01;2.9%(2.8%,3.0%) vs.1.2%(1.2%,1.4%),P<0.01],respectively.(2)In the study group,IL-12 [54.97 pg/mL(31.20,161.59) pg/mL],IL-22 [17.70 pg/mL(15.60,35.99) pg/mL],IL-23[120.28 pg/mL(82.97,169.27) pg/mL],IL-23R [0.15 pg/mL(0.15,0.71)pg/mL]and TNF-α[1 000 pg/mL(1 000,1 000)pg/mL] were higher than that of the control group[31.20 pg/mL(31.20,56.23)pg/mL,15.60 pg/mL(15.60,15.60)pg/mL,78.10 pg/mL(78.10,99.08)pg/mL,0.15 pg/mL(0.15,0.15)pg/mL,722.16 pg/mL(494.25,941.44)pg/mL,P<0.05].(3)Correlation analysis showed that there was a negative correlation between Th17 and Treg (r=-0.883,P<0.01) in the study group (r=-0.883,P<0.01),and IL-22 was positively correlated with IL-23 (r=0.340,P<0.05).In the control group,there was a negative correlation between CD4+T cells and CD8+ T cells,Th17 and CD3+ T cells,Th17 and CD8+T cells (r was-0.578,-0.393 and-0.569,respectively,P<0.05),and a positive correlation between Th17 and Treg,IL-23 and CD4+T cells,TNF-α and DNT(r=0.403,0.440 and 0.392,respectively.(4) There were 5 main components among each group,but the variable factors differed in the study group from the controls,as that:Th17/Treg,Th17,Treg vs.CD4+ T/CD8+ T cells,CD4+ T cells,CD8+ T cells and CD4+T/CD8+T cells,CD8+ T cells vs.Th17/Treg,Treg and CD4+ T cells,CD3+ T cells vs.IL-12,IL-23R andIL-12,IL-23R vs.CD3+ T cells and IL-23,IL-22 vs.TNF-α,DNT.Conclusion The increasing immune function of Th17 cells with IL-22 and IL-23 may be involved in the pathological mechanism of COPD,which accompanied with the weakened toxicity of CD8+T cells and regulation of DNT.
4.Synthesis and inhibition of NO production activities of 1,6-O, O-diacylbritannilactone derivatives

Eight 1,6-O,O-diacetylbritannilactone (OABL) derivatives (compounds 1-8) were synthesized by esterification or reduction of 1-O-diacetylbritannilactone (ABL) isolated from Inula japonica.All derivatives were evaluated for their anti-inflammation activities through the determination of inhibition of nitric oxide (NO) production in lipopolysaccharide (LPS)-induced macrophages.As results,compounds 5-8 (IC50 < 2 μmol/L) exhibited more potent inhibition of NO production activities than the lead compound OABL.
5.Magnetic resonance imaging evaluation of hysteromyoma before and after uterine artery embolization

Objective To explore the significance of signal and volume change from magnetic resonance imaging (MRI) of hysteromyoma before and after uterine artery embolization (UAE) in the therapy evaluation. Methods MRI was performed in 30 patients (50 hysteromyoma) before and 3,6 and 12 months after UAE. They were grouped by location, signal and size. The MRI signal changes and the hysteromyoma's volume reduction ratio were measured. Results After 3,6,12 months, MRI of hysteromyoma was changed significantly, and all hysteromyomas had lower T2WI signals than before, some of which had higher T1WI signals. Hysteromyoma's volumes were progressively reduced, the majority of which shrinked significantly within 3 months. Evaluated by 12 month's volume changes, significant volume reduction was found in submucous fibroids, and significant difference was showed compared with intramural fibroids and subserosal fibroids (88.9 % vs. 73.7 % and 68.3 %, P=0.036, P=0.019), meanwhile,the latter two had no significant difference (P=0.384). The volume reduction rate in rich cell fibroids was higher than those in ordinary no degeneration fibroids and degeneration type, and there were significant differences (85.7 % vs. 72.1 % and 63.4%, P=0.038, P=0.014). Besides, the latter two had no significant difference (P=0.364). Large fibroids shrinked more obviously than small ones with significant difference (75.2 % vs. 59.6 %, χ2=4.563, P=0.044). Conclusion MRI is useful for the evaluation of efficacy in hysteromyoma before and after UAE, which can provide the better interventional treatment for the patients in regard to different sensitivity of hysteromyoma to UAE.
6.Identification of Risk Pathways and Functional Modules for Coronary Artery Disease Based on Genome-wide SNP Data

Coronary artery disease (CAD) is a complex human disease, involving multiple genes and their nonlinear interactions, which often act in a modular fashion. Genome-wide single nucleotide polymorphism (SNP) profiling provides an effective technique to unravel these underlying genetic interplays or their functional involvements for CAD. This study aimed to identify the susceptible pathways and modules for CAD based on SNP omics. First, the Wellcome Trust Case Control Consortium (WTCCC) SNP datasets of CAD and control samples were used to assess the joint effect of multiple genetic variants at the pathway level, using logistic kernel machine regression model. Then, an expanded genetic network was constructed by integrating statistical gene–gene interactions involved in these susceptible pathways with their protein–protein interaction (PPI) knowledge. Finally, risk functional modules were identified by decomposition of the network. Of 276 KEGG pathways analyzed, 6 pathways were found to have a significant effect on CAD. Other than glycerolipid metabolism, glycosaminoglycan biosynthesis, and cardiac muscle contraction pathways, three pathways related to other diseases were also revealed, including Alzheimer’s disease, non-alcoholic fatty liver disease, and Huntington’s disease. A genetic epistatic network of 95 genes was further constructed using the abovementioned integrative approach. Of 10 functional modules derived from the network, 6 have been annotated to phospholipase C activity and cell adhesion molecule binding, which also have known functional involvement in Alzheimer’s disease. These findings indicate an overlap of the underlying molecular mechanisms between CAD and Alzheimer’s disease, thus providing new insights into the molecular basis for CAD and its molecular relationships with other diseases.
7.Effects of different surface treatments on shear bond strength of diatomite-based dental ceramic

Objective To investigate the effects of different surface treatments on shear bond strength between diatomite-based dental ceramic and IPS e.max veneering ceramic.Methods Diatomite-based dental ceramic was randomly divided into acid-etched group,sandblasting group and untreated group,and then sintered with IPS e.max veneering ceramic,respectively.Shear bond strength were detected,and the interface microstructures and elemental distribution were observed by scanning electron microscope-energy dispersive spectrometer (SEM-EDS).Results Shear bond strength of acid-etched group was significantly higher than that of untreated group (P<0.01).SEM observation results showed that the interface of acid-etched group bonded closely.There was no statistical difference of the shear bond strength between the sandblasting group and untreated group (P>0.05).Conclusions Acid-etched treatment can significantly enhance the bond strength of diatomite-based dental ceramic and IPS e.max veneering ceramic.
8.Nursing of critically ill patients receiving continuous renal replacement therapy

Objective To investigate the application of continuous renal replacement therapy in critically ill patients and clinical nursing measures.Methods 100 critically ill patients receiving continuous renal replacement therapy were chosen from October 2009 to June 2011 in our hospital and were given nursing intervention.The biochemical indicators before and after treatment were compared and analyzed.Results The biochemical indicators in patients improved significantly after continuous renal replacement therapy.There was significant difference.Conclusions Effective and comprehensive nursing for critically ill patients in continuous renal replacement therapy can improve the patients vital signs,improve the survival rate of them,and it should be carried out in clinic.
9.Simultaneous determination of aflatoxin B1, B2, G1, G2 and ochratoxin A in Glycyrrhiza uralensis by HPLC-fLD after immunoaffinity column with online post-column photochemical derivatization.

OBJECTIVETo develop a method for the simultaneous determination of aflatoxin B1, B2, G1, G2 and ochratoxin A in Glycyrrhiza uralensis by HPLC-FLD after immunoaffinity column with online post-column photochemical derivatization.
METHODSample was extracted with MeOH: H2O (80:20) and cleaned up by immunoaffinity column. The toxins were separated by reversed-phase HPLC and the mobile phase was consisted of methanol and 0.5% acetic acid solution with gradient elution. The determination was carried out by fluorescence detector after photochemical derivatization.
RESULTThe detection limits of aflatoxin G2, G1, B2, B1 and ochratoxin A were 0.02, 0.06, 0.015, 0.03 and 0.25 microg x kg(-1), respectively. The recoveries of analytes were from 76.0% to 103% and the relative standard deviations (RSDs) were below 13%.
CONCLUSIONThe method is a simple, accurate and can be used to determine the contents of aflatoxin B1, B2, G1, G2 and ochratoxin A in G. uralensis simultaneously.

10.Quantitative determination of pinoresinol diglucoside in Eucommiae unloads by NIRS.

OBJECTIVETo establish a rapid determination method of pinoresinol diglucoside in Eucommiae unloads by near-infrared reflectance spectroscopy (NIRS).
METHODForty-one samples of E. unloads were collected from three different producing areas and their main component, namely pinoresinol diglucoside, was determined by HPLC. Corresponding data of samples were collected from 12 000 to 4 000 cm(-1) by near-infrared reflectance spectroscopy. The spectral pretreatment was optimized by OPUS software and the calibration equations between the content of pinoresinol diglucoside and spectrum data were constructed by partial least squares regression.
RESULTAvailable information could be extracted from spectra in the range from 7 502 to 4 597.6 cm(-1) after corrected by applying second derivative transformation and subtract a linear correction. Cross validation was used to prevent over-fitting. Good correlation existed between pinoresinol diglucoside content and NIR spectra ( R2 = 0.926 4, SEC = 0.029 and SEP = 0.066 2).
CONCLUSIONNIRS calibration equations developed in this study could be applied to the rapid analysis of the pinoresinol diglucoside content.
