1.Application effect of child-friendly gamified intervention strategies in visual acuity testing for children aged 2-4 years

Objective:To develop an E-chart visual acuity recognition card suitable for children aged 2-4 years and to design a child-friendly gamified intervention method to improve the success rate, compliance, and parental satisfaction in visual acuity testing for this age group.Methods:A non-concurrent historical control study design was employed, involving 143 children aged 2-4 years and parents who attended the ophthalmology outpatient clinic of Renmin Hospital of Wuhan University from January to June 2023. The children were divided into the control group and the intervention group based on their appointment times. Both groups underwent binocular visual acuity testing using the E-chart visual acuity recognition cards. The control group received conventional visual acuity testing, while the intervention group was subjected to a child-friendly gamified intervention. The effectiveness of the intervention was assessed by comparing the success rate of visual acuity testing, examination compliance, and parental satisfaction between the two groups.Results:Finally, the sample size of the control group and the intervention group were 69 and 74 cases. The children in the control group was (3.06 ± 0.47) years old, with 36 males and 33 females; whereas the children in the intervention group was (2.99 ± 0.45) years old, with 38 males and 36 females. The parents in the control group was (29.37 ± 4.00) years old, with 24 males and 45 females; whereas the parents in the intervention group was (29.35 ± 3.50) years old, with 27 males and 47 females. The success rate of visual acuity testing in the intervention group was significantly higher at 63.5% (47/74) compared to the control group′s 34.8% (24/69), with a statistically significant difference ( χ2=7.45, P<0.01). The compliance of the children during the examination in the intervention group, categorized as no grade I cases, 19 gradeⅡcases, 21 gradeⅢcases, and 34 grade Ⅳ cases, was significantly higher than that of the control group, which had 17 grade I cases, 23 gradeⅡcases, 15 grade Ⅲcases, and 14 grade Ⅳ cases ( Z=4.61, P<0.01). In terms of parental satisfaction, the intervention group′s satisfaction score was (4.53 ± 0.56) points, which was significantly higher than the control group′s score of (3.74 ± 0.78) points, indicating a statistically significant difference ( t=-4.88, P<0.01). Conclusions:The child-friendly gamified intervention significantly improved the success rate and compliance of visual acuity testing in children aged 2-4 years and increased parental satisfaction. This intervention provides an effective solution for visual acuity testing in children aged 2-4 years.
2.Mediating effect of compassion fatigue between organizational support and therapeutic relationships in psychiatric nurses

Objective:To investigate the current status of therapeutic relationships, organizational support, and compassion fatigue among psychiatric nurses, and to analyze the mechanism of action between these variables.Methods:This was a cross-sectional study. From August to September 2023, totally 310 psychiatric nurses were selected from three ClassⅢ Grade A psychiatric hospitals in Qingdao, Jinan, and Linyi by convenience sampling. Data were collected using a general information questionnaire, the Therapeutic Relationship Assessment Scale-Nurse (TRAS-N), the Perceived Organizational Support Scale (POSS), and the Chinese version of Compassion Fatigue short Scale (CCFS). SPSS 22.0 and the Process macro plugin were used for data analysis.Results:A total of 310 questionnaires were distributed, with 282 valid responses, yielding a response rate of 90.97%. The total score for TRAS-N among the 282 psychiatric nurses was (95.59±18.75), the total score for CCFS was 45.50 (24.75, 68.25), and the total score for POSS was (43.66±13.34). Therapeutic relationships were positively correlated with organizational support and negatively correlated with compassion fatigue, while organizational support was negatively correlated with compassion fatigue (all P< 0.01). Organizational support had a direct positive effect on therapeutic relationships (β=0.291, P<0.01) and also had an indirect effect through the mediating effect of compassion fatigue (β=0.212, P<0.01), with the mediation effect accounting for 42.15% of the total effect. Conclusions:Compassion fatigue partially mediates the relationship between organizational support and therapeutic relationships in psychiatric nurses. Nursing managers should enhance organizational support for psychiatric nurses to reduce their levels of compassion fatigue, thereby improving therapeutic relationships.
3.Evaluation of myocardial microcirculation abnormalities in patients with hypertrophic cardiomyopathy by myocardial contrast echocardiography combined with exercise stress test

Objective:To detect the abnormal changes of myocardial blood perfusion in patients with hypertrophic cardiomyopathy (HCM) by myocardial contrast echocardiography(MCE) combined with exercise stress test.Methods:Twenty-seven patients with clinically diagnozed of asymmetric HCM in Fuwai Central China Cardiovascular Hospital from May 2020 to April 2021 were selected as the HCM group, and 29 healthy subjects during the same period were selected as the control group. All patients underwent routine echocardiography, resting and exercise stress MCE. The myocardial perfusion parameters of each segment of interventricular septum in the 2 groups were quantitatively analyzed: the peak plateau intensity (A value), ascending slope of the curve(β value) and value of A×β. According to the end-diastolic myocardial thickness, the interventricular septum of the HCM group was divided into hypertrophic and non-hypertrophic segments, and the myocardial contrast parameters of the interventricular septum of the study group were compared with those of the control group. The myocardial blood flow reserve value of the two groups were calculated, and the correlation of myocardial blood flow reserve value with left ventricular mass index (LVMI) and left ventricular remodeling index (LVRI) were analyzed.Results:No matter at rest or under stress, the A value, β value and A×β value of ventricular septal hypertrophic and non-hypertrophic segments in the hypertrophic cardiomyopathy group were lower than those in the control group, and the differences were statistically significant (all P<0.05). Under stress, the A value, β value and A×β value of interventricular septal hypertrophic segments were lower than those in non-hypertrophic segments in the HCM group, and the differences were statistically significant (all P<0.05). The myocardial blood flow reserve in the HCM group was negatively correlated with LVMI and LVRI( r=-0.899, -0.676; all P<0.001). Conclusions:In patients with HCM under resting and exercise stress, microcirculation disorders were found in both hypertrophic and non-hypertrophic segments of the ventricular wall, and the myocardial blood flow reserve was negatively correlated with LVMI and LVRI.
4.Advices on the prevention and control of nosocomial infection of COVID-19 within children′s hospitals

The epidemic of COVID-19 has become the topmost public health threat worldwide. The authors suggested that in addition to strengthening the organization and leadership of the abovementioned work, greater attention be paid to establishing and improving the prevention and control mechanism.Furthermore, special efforts should be given to the safety of the medical workers, by strengthening their infection monitoring and outbreak management. Medical workers in different work areas and positions should be placed under careful protection with due cleaning and disinfection measures. The protection during specimen collection, transportation and medical waste management should also be prioritized. Such special issues in the management of pediatric patients, as allocation of nurses, the screening and management of caregivers, the problem of breastfeeding, and the disinfection of children′s toys and diapers were discussed. This paper also put forward management suggestions for the outpatient department, isolation ward and other key departments, which aim to guide the prevention and control of COVID-19 nosocomial infection in the pediatric outpatient and ward.

The pneumonia caused by the novel coronavirus (2019-nCoV), which began in December 2019, has become the most serious public health problem, threatening people's health and life. This threat is posing a severe challenge on the diagnosis and treatment of 2019-nCoV infection, the prevention and control of hospital cross infection of medical staff. It is suggested that in addition to strengthening the organization and leadership of the abovementioned work, establishing and improving the prevention and control mechanism deserve greater attention. Furthermore, special attention should be given to the safety of the medical staff, strengthening their infection monitoring and outbreak management. Medical staff in different work areas and positions should be placed under careful protection, cleaning and disinfection measures. 6. Clinical Study on Changes of Gastric Myoelectrical Activity and Autonomic Nerve Function in Cirrhotic Patients With Gastroesophageal Varices After Endoscopic Treatment

Background: The occurrence of gastrointestinal symptoms in cirrhotic patients with gastroesophageal varices (GOV) after endoscopic treatment is obvious, and the role of gastric myoelectrical activity (GMA) and autonomic nerve function imbalance in the development of gastrointestinal symptoms has not been clarified. Aims: To investigate the changes of GMA and autonomic nerve function in cirrhotic patients with GOV after endoscopic treatment. Methods: Twenty-five cirrhotic patients with GOV from May 2019 to October 2019 at the First Affiliated Hospital of Nanjing Medical University were enrolled, and 10 patients with gastric polyp were served as controls. Electrogastrogram (EGG) and heart rate variability (HRV) were detected before the operation, 1 day after the operation and 5 days after the operation in GOV group. For the gastric polyp group, EGG and HRV were detected before the operation and 1 day after the operation. Changes of GMA and autonomic nerve function were compared between the two groups. Results: No significant differences in GMA and autonomic nerve function were found between GOV group and gastric polyp group before and 1 day after the operation (P>0.05). LF, LF/HF were significantly increased 1 day after the operation in GOV group (P<0.05), while HF was significantly decreased (P<0.01), and the percentage of bradygastria was significantly increased (P<0.01). There were no significant differences in GMA and autonomic nerve function between pre-operation and 5 days after the operation in GOV group (P>0.05). There were no significant differences in GMA and autonomic nerve function 1 day after the operation compared with pre-operation in gastric polyp group (P>0.05). Compared with Child-Pugh A group, Child-Pugh B group had more obvious GMA abnormalities, and the difference was statistically significant (P<0.05). Conclusions: In cirrhotic patients with GOV, the percentage of bradygastria and the sympathetic activity increased, and the vagal activity decreased 1 day after the operation. These results suggest that GMA and autonomic nerve dysfunction may be related to the gastrointestinal symptoms after endoscopic treatment.
7. Influence of pharmaceutical care pathway on cancer-related fatigue and negative emotion in patients with advanced lung cancer undergoing chemotherapy

Objective:
To investigate the influence of pharmaceutical care pathway on cancer-related fatigue (CRF) and negative emotion in patients with advanced lung cancer undergoing chemotherapy.
Methods:
Patients with advanced lung cancer admitted to the oncology department of Yongkang first people′s hospital from January 2017 to June 2018 for chemotherapy were randomly divided into two groups:61 cases received conventional clinical pathway management (control group), and 64 cases received conventional clinical pathway combined with pharmaceutical pathway management (intervention group). Fatigue, anxiety, depression scores were compared between the two groups before and after 2, 4 cycles chemotherapy.

8.Expression and clinical significance of plasma miR-124 in acute ischemic stroke

Objective To investigate the expression and clinical significance of plasma miR-124 in acute ischemic stroke (AIS). Methods Forty patients with AIS were collected and 40 volunteers without history of AIS were set as control. Infarction volume was detected by MRI; plasma miR-124 expression level was measured by RTPCR technique and neural function was evaluated by NIHSS scores. Results Compared with that in the control group, plasma miR-124 level in AIS group was significantly reduced (P < 0.05). Plasma miR-124 level in AIS patients with infarction volume greater than 3 cm3 was significantly lower than that of AIS patients with infarction volume less than 3 cm3 (P < 0.05). Correlation analysis showed a negative correlation between miR-124 and infarction volume (r =-0.473, P < 0.05). Plasma miR-124 level in AIS with NIHSS score higher than 5 was significantly lower than that of AIS patients with NIHSS lower than 5 (P< 0.05). NIHSS score negatively correlated with the miR-124 level of AIS patients (r =-0.567, P < 0.05). Conclusion The plasma miR-124 is significantly reduced in patients with AIS, and negatively correlated with the cerebral infarction volume and NIHSS score.
9.Level of hope and its influencing factors in elderly patients with total knee anthroplasty

Objective? To investigate the hope level of elderly patients undergoing total knee arthroplasty (THA) and analyze its influencing factors, so as to provide evidence for improving the hope level and quality of life of patients after THA. Methods? A cluster random sampling method was used to select 308 patients with bilateral THA for the first time who were admitted to two ClassⅢ Grade A general hospitals in Harbin from January 2017 to July 2018. Questionnaire surveys were conducted using Herth Hope Scale, the Second Edition of Beck Depression Scale and Social Support Scale. Results? The total score of hope level in elderly THA patients was (29.96±8.72). Among them, 76.62% (236/308) had moderate to high level of hope and the total score of depression was (19.20±11.75) which was at a moderate level. There were significant differences in the hope level of patients with different gender, place of residence, educational level, marital status and family per capita monthly income (P< 0.05). The results of multiple stepwise regression analysis showed that age, sex, family income per capita, depression, negative coping style, subjective support and objective support were the influencing factors of elderly THA patients' hope level (P < 0.05). Conclusions? The THA patients' hope are at a medium level. Medical staff should pay attention to the relevant factors affecting the level of hope, adopt targeted nursing intervention measures, give psychological guidance to patients, provide social support, and further improve the level of hope of the THA patients.
10.The application value of whole-lesion method measuring hepatic abscess lesions in ADC map:a pilot study

Objective To explore the value of whole-lesion method measuring hepatic abscess lesions in ADC map.Methods 13 cases with hepatic abscess were measured by using three methods,drawing the outline of the lesion by manual operation,and placing the round ROI in the central area and the peripheral area in ADC map,respectively.The differences of the ADC values measured by the three methods were compared.The Friedman test was used for the mean of ADC values.The value differences of the groups were compared by the Bonferroni test.Results Among the 17 lesions,6 lesions located in the anterior segment of right lobe of liver,7 lesions in the posterior segment of right lobe of liver,1 lesion in the caudate lobe,1 lesion in the junction zone between the anterior segment of right lobe and the medial segment of left lobe,1 lesion in the medial segment of left lobe and 1 lesion in the lateral segment of left lobe of liver.The mean values from the three methods(×10-3 mm2/s) and the 95% confidence intervalwere:1.27±0.33,1.23± 0.84,1.29±0.44 and (1.21,1.34),(1.06,1.40),(1.20,1.38),respectively.The results of the Friedman test were x2=2.176,P=0.337,and there was no statistical significance between the differences.There were also no statistical significance by the Bonferroni test (P values >0.05/3=0.017).The 95 % confidence interval of the whole-lesion method was the narrowest,and the accuracy was the highest.The 95% confidence interval of the central method was the widest,and the accuracy was the lowest.Conclusion The whole-lesion measurement may be used as an analytical method for hepatic abscess in ADC map.
