1.Gut microbial assessment among Hylobatidae at the National Wildlife Rescue Centre, Peninsular Malaysia

		                        			 Objective:
		                        			High-throughput sequencing based on 16S ribosomal RNA genes was used to determine the baseline gut bacterial composition and identify potential pathogenic bacteria among three endangered gibbons housed in the NWRC. 
		                        		
		                        			Methods:
		                        			Feces were collected from 14 individuals (Hylobates lar, n = 9; Hylobates agilis, n = 4; and Symphalangus syndactylus, n = 1) from March to November 2022. Amplicon sequencing were conducted by targeting V3–V4 region. 
		                        		
		                        			Results:
		                        			The fecal microbial community of the study gibbons was dominated by Bacteroidetes and Firmicutes (phylum level), Prevotellaceae and Lachnospiraceae/ Muribaculaceae (family level), and Prevotella (and its subgroups) (genera level). This trend suggests that the microbial community composition of the study gibbons differed insignificantly from previously reported conspecific or closely related gibbon species. 
		                        		
		                        			Conclusions
		                        			and Relevance: This study showed no serious health problems that require immediate attention. However, relatively low alpha diversity and few potential bacteria related to gastrointestinal diseases and streptococcal infections were detected. Information on microbial composition is essential as a guideline to sustain a healthy gut condition of captive gibbons in NWRC, especially before releasing this primate back into the wild or semiwild environment. Further enhanced husbandry environments in the NWRC are expected through continuous health monitoring and increase diversity of the gut microbiota through diet diversification. 
		                        		
		                        		
		                        		
		                        	
2.Risk factors associated with low anterior resection syndrome: a cross-sectional study

		                        			 Purpose:
		                        			Oncological outcomes following rectal cancer surgery have improved significantly over recent decades with lower recurrences and longer overall survival. However, many of the patients experienced low anterior resection syndrome (LARS). This study identified the prevalence and risk factors associated with the development of LARS. 
		                        		
		                        			Methods:
		                        			This cross-sectional study involved patients who were diagnosed with rectal cancer and had undergone sphincter-preserving low anterior resection from January 2011 to December 2020. Upon clinic follow-up, patients were asked to complete an interviewed based questionnaire (LARS score) designed to assess bowel dysfunction after rectal cancer surgery. 
		                        		
		                        			Results:
		                        			Out of 76 patients, 25 patients (32.9%) had major LARS, 10 patients (13.2%) had minor LARS, and 41 patients (53.9%) had no LARS. The height of tumor from anal verge showed an association with the development of major LARS (P=0.039). Those patients with less than 8 cm tumor from anal verge had an increased risk of LARS by 3 times compared to those with 8 cm and above (adjusted odds ratio, 3.11; 95% confidence interval, 1.06–9.13). 
		                        		
		                        			Conclusion
		                        			Results from our study show that low tumor height was a significant risk factor that has a negative impact on bowel function after surgery. The high prevalence of LARS emphasizes the need for study regarding risk factors and the importance of understanding the pathophysiology of LARS, in order for us to improve patient bowel function and quality of life after rectal cancer surgery. 
		                        		
		                        		
		                        		
		                        	
3.Conversion rate and risk factors of conversion to open in laparoscopic appendicectomy

		                        			 Purpose:
		                        			Laparoscopic appendicectomy (LA) has several advantages over conventional open appendicectomy (OA). However, about 5% to 10% of LA patients still need to be converted to open surgery. Identifying risk factors that contribute to conversion to OA allows for early identification of patients who may benefit from primary OA. This study aimed to determine the conversion rate of LA to OA and to identify its associated risk factors among patients with acute or perforated appendicitis. 
		                        		
		                        			Methods:
		                        			A retrospective review of medical records was performed among patients with acute or perforated appendicitis who underwent LA between December 2015 and January 2017. With the use of multivariable logistic regression analyses, the predictors of conversion from laparoscopic to OA were investigated. 
		                        		
		                        			Results:
		                        			Out of 120 patients, 33 cases were converted to OA which gives a conversion rate of 27.5%. Among 33 patients who were converted to OA, 27 patients (81.8%) had perforated appendix, while in the LA group, perforated appendix cases consisted of 34.5% (P<0.001). Histopathology of the appendix was the predictor of conversion from LA to OA (adjusted odds ratio, 8.82; 95% confidence interval, 3.13–24.91; P<0.001). 
		                        		
		                        			Conclusion
		                        			The result from our study shows that the overall conversion rate for the study period was high. Patients with perforated appendicitis had a higher risk of conversion to OA. Therefore, preoperative diagnosis of perforated appendicitis may be paramount in predicting conversion to OA. 
		                        		
		                        		
		                        		
		                        	
4.Validation of Malay Language Translated Questionnaire on Adult Intensive Care Unit Nurses’ Perception and Involvement in End-of-life Care

		                        			Introduction: The study’s objective was to validate a Malay language translated questionnaire on end-of-life care 
to be used among nurses practicing in critical care areas. Methods: The English language questionnaire underwent 
forward and backward translations by four experts. The translated Malay language questionnaire was pilot tested on 
30 subjects and revised accordingly. The validation of the revised questionnaire was carried out on 250 nurses. The 
reliability of the translated questionnaire was checked. Cronbach alpha value of at least 0.70 suggests adequate internal consistency. The validity of the questionnaire was explored using Confirmatory Factor Analysis (CFA) and model 
fit tests were run to achieve fit test specific cut off values. The CFAs were run repeatedly with iterative item reductions 
until acceptable goodness of fit for the model was achieved. Results: All domains of the translated questionnaire 
showed reasonable to excellent reliability (Cronbach Alpha 0.687 to 0.922). Multiple CFAs were run and 13 out of 
46 items were excluded, and the final model fit improved substantially with the indices were within the acceptable 
threshold of good or reasonably fit, cut off values are in brackets [Chi-Square statistics 1.635 (≤ 2.0), Root Mean 
Square Error of Approximation 0.050 (< 0.05), Standardised Root Mean Square Residual 0.059 (≤ 0.08), Comparative 
Fit Index 0.911 (0.90-0.94), Tucker Lewis Index 0.900 (0.90-0.94), Akaike Information Criteria 13024, Bayesian Information Criteria 13334]. Conclusion: The psychometric properties of the final model indicated the Malay language

5.Morphological and molecular identification of Fusarium spp. and Colletotrichum spp. isolated from infected vanilla orchid

		                        			Aims:
		                        			This study aimed to isolate and identify fungi involved in causing diseases to Vanilla planifolia as well as to study
their pathogenicity level in causing disease.
		                        		
		                        			Methodology and results:
		                        			The diseased parts of vanilla plants were collected from vanilla farms located in Pahang and 
Sabah, Malaysia from May 2015 to May 2016. Diseases tissue transplantation was adopted to isolate the fungi for 
morphology identification prior to the polymerase chain reaction (PCR) amplification of internal transcribed spacer (ITS) 
regions using universal primers for fungi, ITS1 and ITS4. After being isolated, the fungi pathogenicity was tested on 
detached fresh and mature vanilla leaves. A total of 22 fungal isolates were identified, Fusarium fujikuroi and F. 
oxysporum were the two most recovered species, followed by Colletotrichum gloeosporioides, Fusarium sp., F. 
proliferatum and F. solani. Pathogenicity test revealed a significantly high pathogenicity of F. oxysporum and C. 
gloeosporiodes (p<0.01) on detached vanilla leaf, with high level of damage.
		                        		
		                        			Conclusion, significance and impact of study
		                        			This study provides valuable information on fungi-associated diseases

6.Fruit and vegetable intake among overweight and obese school children: A cluster randomised control trial

		                        			Introduction: Adequate daily intake of fruits and vegetables is crucial for the 
prevention of chronic diseases. This study aimed to determine the effects of My 
Body is Fit and Fabulous at School (MyBFF@school) with nutrition education 
intervention (NEI) on the stages of change for fruit and vegetable intake among 
overweight and obese secondary school children based on the trans-theoretical 
model (TTM). Methods: This was a cluster randomised controlled trial involving 15 
out of 415 eligible government secondary schools in central Peninsular Malaysia, 
which were randomly assigned into intervention (six schools; 579 school children) 
and control (nine schools; 462 school children). The intervention group was given 
NEI for 24 weeks, while the control group followed the existing school programme 
by the Ministry of Education. Results: There was no significant difference between 
the intervention and control groups for the stages of change, with majority at the 
maintenance stage after six months (intervention: 34.9%; control: 39.0%). The 
within group analysis showed a significant reduction after six months for those 
at the action stage (action and maintenance stage) from 68.0% to 60.4% in the 
intervention group and from 71.4% to 65.6% in the control group. However, there 
was a significant increase among those with adequate fruit and vegetable intake in 
the intervention group and no significant increase in the control group. Conclusion:
MyBFF@school with NEI based on TTM provided acceptable changes in fruit and

7.The Use of M2-Pyruvate Kinase as a Stool Biomarker for Detection of Colorectal Cancer in Tertiary Teaching Hospital: A Comparative Study

		                        			 Purpose:
		                        			Guaiac fecal occult blood test (gFOBT) has been the standard for colorectal screening but it has low sensitivity and specificity. This study evaluated the use of fecal tumor M2-pyruvate kinase (M2-PK) for detection of colorectal cancer and to compare with the current surveillance tool; gFOBT in symptomatic adult subjects underwent colonoscopy. 
		                        		
		                        			Methods:
		                        			Stool samples were collected prospectively from symptomatic adults who had elective colonoscopy from September 2014 to January 2016 and were analyzed with the ScheBo M2-PK Quick test and laboratory detection of fecal hemoglobin. 
		                        		
		                        			Results:
		                        			The results were correlated to the colonoscopy findings and/or histopathology report. Eighty-five subjects (age of 56.8 ± 15.3 years [mean ± standard deviation]) were recruited with a total of 17 colorectal cancer (20.0%) and 10 colorectal adenoma patients (11.8%). The sensitivity of M2-PK test in colorectal cancer detection was higher than gFOBT (100% vs. 64.7%). M2-PK test had a lower specificity when compared to gFOBT (72.5% vs. 88.2%) in colorectal cancer detection. The positive and negative predictive values were 47.2% and 100% for M2-PK test and 57.9% and 90.9% for gFOBT. 
		                        		
		                        			Conclusion
		                        			Fecal M2-PK Quick test has a high sensitivity for detection of colorectal cancer when compared to gFOBT, making it the potential choice for colorectal tumor screening biomarker in the future. 
		                        		
		                        		
		                        		
		                        	
8.Predictors of early postoperative hypocalcemia after total parathyroidectomy in renal hyperparathyroidism

		                        			 PURPOSE:
		                        			Patients with secondary hyperparathyroidism are at high risk for developing postoperative hypocalcemia. However, there are limited data regarding predictors of postoperative hypocalcemia in renal failure patient with secondary hyperparathyroidism. This study aimed to determine the clinical presentations of renal hyperparathyroidism and the predictors of early postoperative hypocalcemia after total parathyroidectomy.
		                        		
		                        			METHODS:
		                        			Data of patients with renal hyperparathyroidism who underwent total parathyroidectomy between January 2007 to December 2014 were reviewed retrospectively. Patients were divided into 2 cohort groups according to their serum calcium levels within 24 hours of parathyroidectomy: the hypocalcemia group (calcium levels of 2 mmol/L or less), and the normocalcemia group (calcium levels more than 2 mmol/L). With the use of multivariable logistic regression analyses, the predictors of early postoperative hypocalcemia after total parathyroidectomy in patients with renal hyperparathyroidism were investigated.
		                        		
		                        			RESULTS:
		                        			Among 68 patients, 56 patients (82.4%) were symptomatic preoperatively. Fifty patients (73.5%) presented with bone pain and 14 patients (20.6%) had muscle weakness. Early postoperative hypocalcemia occurred in 25 patients (36.8%). Preoperative alkaline phosphatase level was the predictor of early postoperative hypocalcemia (adjusted odds ratio, 1.004; 95% confidence interval, 1.001–1.006; P = 0.002).
		                        		
		                        			CONCLUSION
		                        			Results from our study show that most of the patients with renal hyperparathyroidism were symptomatic preoperatively and the most common clinical presentations were bone pain and muscle weakness. The significant predictor of early postoperative hypocalcemia after total parathyroidectomy was the preoperative alkaline phosphatase levels. 
		                        		
		                        		
		                        		
		                        	
9.Serological survey of Leptospirosis in high-risk rangers and wild animals from ex-situ captive centers

		                        			Leptospirosis is a bacterial disease caused by pathogenic Leptospira that infect
both human and animals. This study was conducted to determine the seroprevalence of
leptospirosis among rangers and wild animals in two ex-situ captive centers, Bukit Merah
Orangutan Island (BMOUI) and Taiping Zoo, Perak and to identify the risk factors responsible
for the leptospiral seropositivity. Blood samples from rangers and animals of BMOUI and
Taiping Zoo were taken to determine the presence of antibodies against Leptospira through
microscopic agglutination test (MAT) using 21 serovars of Leptospira commonly found in
Malaysia as antigens. Structured surveys in identifying risk factors were given to each
ranger from both study sites. It was observed that Rattus exulans (1/10) (10.0%), Rattus
rattus (1/5) (20.0%), Niniventer fulvescens (1/1) (100.0%), Callosciurus notatus (0/6)
(0.0%), Tupaia tana (1/1) (100.0%), Pongo pygmaeus (5/10) (50.0%) and BMOUI rangers
(8/18) (44.4%) were positive for leptospiral antibodies. Samples obtained from Taiping Zoo
also revealed the presence of leptospiral antibodies in R. rattus (0/19) (0.0%), R. exulans
(1/2) (50.0%) and rangers (2/5) (40.0%). Among the positive cases, most human and animal
samples from both study sites reacted with serovar Lepto 175. Our surveys indicated no
significant associations between seroprevalence of leptospiral antibodies with rangers’
age (p = 0.82), sex (p = 0.85), ethnicity (p = 0.65), educational level (p = 0.88) and working
experience (p = 0.82). In terms of risk factors, no significant associations between
seroprevalence of leptospiral antibodies with knowledge on leptospirosis (p = 0.82), working
hours (p = 0.53), smoking (p = 0.85), crossing rivers/pools/stagnant water while working
(p = 0.90) and wearing full personal protective equipment (PPE) (p = 0.73). This study
provides epidemiological data on leptospirosis in rangers and animals from BMOUI and
Taiping Zoo which is of paramount importance for improving strategies in prevention of the

10.Bacterial isolation of oral, rectum and anus swabs from Macaca fascicularis and Macaca namestrina in Kemasul, Pahang, Malaysia

		                        			Abstract
		                        			This study was conducted with the aim of isolating and identifying pathogenic bacterial communities from actively shedding anatomical sites of Macaca fascicularis and M. namestrina in Jambu Rias (JR) and Chemomoi (CM) in Kemasul Forest Reserve, Pahang and to determine the antibiotic susceptibility of these isolates. The findings show that M. fascicularis had higher bacterial density and ten different isolates were identified from these samples.
