Background and Objectives: Vitamin D, as a steroid hormone, has multiple effects on human body and its deficiency has been associated with an increased risk of heart failure (HF) and unfavorable outcomes. The present study investigated the prevalence of vitamin D deficiency (VDD) and its relationship with cardiometabolic parameters in patients hospitalized for HF living in the city of Recife (latitude 8° South). Methods: Analytical cross-sectional study, with men and women aged 40–64 years. The HF group was recruited during hospitalization due to decompensation. A matched control group was recruited from the general endocrine clinics. Vitamin D status was assessed by measuring serum 25-hydroxyvitamin D (25OHD), considering deficiency when 25OHD <20 ng/mL (<50 nmol/L). Results: A total of 243 patients were evaluated (HF group: 161, control group: 82). Lower serum 25OHD levels were observed in the HF group (25.2±9.4 vs. 30.0±7.7ng/mL; p<0.001), as well as a higher prevalence of VDD (27.3% vs. 9.8%; prevalence ratio, 2.80; 95% confidence interval, 1.38–5.67; p=0.002). In patients with HF, VDD was associated with diabetes mellitus (65.9% vs. 41.0%; p=0.005) and female sex (65.9% vs. 44.4%; p=0.015). In the subgroup with VDD, higher values of hemoglobin A1c (7.9% [6.0–8.9] vs. 6.2% [5.7–7.9]; p=0.006) and dyslipidemia were also observed. Conclusions We found higher rates of VDD in patients hospitalized for HF and this was associated with deleterious laboratory metabolic parameters.

Purpose: This study was conducted to evaluate the correlation between persistent serum lactate elevation and brain magnetic resonance imaging (MRI) in children with status epilepticus (SE). Methods: In this prospective analytical study, serum lactate levels were measured 24 hours after episodes of SE, and brain MRI was performed within 7 to 14 days after SE termination. MRI abnormalities were classified as acute encephalopathy (AE) grade I to III. The Kruskal-Wallis test was utilized for statistical analysis. Results: The study included 42 participants with SE, of whom 85.70% were boys, with a mean age of 4.94 years. Viral encephalitis was the most common diagnosis, accounting for 47.60% of cases. Elevated serum lactate levels were detected in 71.40% of patients, and approximately 47.60% exhibited abnormal MRI findings consistent with AE grade I. The median serum lactate levels for AE grades I, II, and III were 1.50, 3.10, and 0.78 mmol/L, respectively. Two patients died, and 66.70% experienced neurologic sequelae. A significant correlation (P=0.021) was observed between persistent serum lactate elevation and abnormal brain MRI findings. Conclusion In children with SE, AE grade I was the most common neuroimaging pattern observed, and persistently elevated serum lactate level was correlated with abnormal brain MRI findings.

Objectives: Disorders of amino acid metabolism fall under the category of inborn errors of metabolism that can be managed with a protein-restricted diet. However, adherence to such a diet often poses challenges, leading to low treatment engagement. Consequently, there is a pressing need for new resources to aid in dietary self-monitoring. The goal is to develop and implement “AminoApp,” an application tailored for dietary self-monitoring in patients with inborn errors of metabolism who are on a low-protein diet. Methods: The design and development of the application adhered to the user-centered design method. This approach emphasizes active participation and collaboration between users and designers/researchers throughout all stages of product development, including requirement gathering, prototype development, and evaluation. Usability was evaluated using the System Usability Scale, which has been validated in Portuguese. Results: The application’s features include a food diary, a food consultation area, exam records, a recipe calculator, and reports on diet composition and metabolic control. The usability test included four patients on a low-protein diet, three caregivers, and three healthcare professionals. The average usability score was 84.9, with averages of 77.5 for patients, 85.8 for caregivers, and 91.6 for professionals, indicating that the application is user-friendly. Conclusions AminoApp is the first application developed in Brazil designed to assist in managing inborn errors of metabolism that require a protein-restricted diet. It was found to be easy to use, and the initial results are promising. Further research is necessary to evaluate the impact of the application on metabolic control and treatment adherence.
