1.Outcome Indicators in Randomized Controlled Trials of Traditional Chinese Medicine Intervention in Ulcerative Colitis

		                        			To systematically review randomized controlled trials (RCTs) of traditional Chinese medicine (TCM) intervention in ulcerative colitis (UC), and analyze the characteristics of these studies and their outcome indicators, thereby providing references for the design of future RCTs of TCM intervention in UC and offering evidence supporting the clinical application of TCM in UC. A computerized search was conducted in the China National Knowledge Infrastructure (CNKI), Wanfang Data, VIP, SinoMed, PubMed, Cochrane Library, EMbase, and Web of Science databases for RCTs of TCM intervention in UC published from January 2021 to August 2024. The risk of bias was assessed, and outcome indicators were qualitatively analyzed. A total of 555 RCTs were included, with a sample size of 44 853 participants. The largest sample size was 218 cases, and the smallest was 28 cases, with most studies focusing on 60-100 participants. Of the 386 RCTs that explicitly reported TCM syndrome types, the top three were large intestine dampness-heat syndrome (31.05%), spleen and kidney yang deficiency syndrome (12.47%), and spleen deficiency with dampness syndrome (9.17%). The interventions, ranked by frequency of use, included internal Chinese medicine compounds/preparations (64.5%), Chinese medicine compounds/preparations with retained enema (18.2%), internal Chinese medicine compounds/preparations + external TCM treatment (5.95%), and external TCM treatment alone (4.86%). The treatment duration was mainly 4-8 weeks (64.86%), with 61 studies (10.99%) reporting follow-up time. A total of 157 outcome indicators were used, with a frequency of 3 460 occurrences, classified into six domains: TCM syndromes and symptoms (346 occurrences, 10%), symptoms/signs (541 occurrences, 15.64%), physical and chemical examinations (2 119 occurrences, 61.24%), quality of life (107 occurrences, 3.09%), long-term prognosis (61 occurrences, 1.76%), and safety events (284 occurrences, 8.21%). The analysis reveals several limitations in the outcome indicators of TCM intervention in UC, including the lack of a basis for sample size calculation, non-standardized TCM syndrome classification, absence of trial design and registration, inadequate blinding and allocation concealment, adherence issues with interventions, imbalanced selection of surrogate and endpoint indicators, inconsistency in the timing of outcome measurements, design issues that require standardization, and ethical and safety concerns. It is recommended that future studies actively construct a set of core indicators for UC that include standardized TCM syndrome classification, clear efficacy evaluation indicators, key endpoint indicators, and reasonable measurement time points. Long-term prognostic impacts, comprehensive assessments of patients' quality of life, and consideration of economic benefits should be emphasized, providing a basis for the clinical practice of TCM in the treatment of UC.

3.Protective mechanism of rhubarb decoction against inflammatory damage of brain tissue in rats with mild hepatic encephalopathy: A study based on the PI3K/AKT/mTOR signaling pathway

		                        			ObjectiveTo investigate the role and possible mechanism of action of rhubarb decoction (RD) retention enema in improving inflammatory damage of brain tissue in a rat model of mild hepatic encephalopathy (MHE). MethodsA total of 60 male Sprague-Dawley rats were divided into blank group (CON group with 6 rats) and chronic liver cirrhosis modeling group with 54 rats using the complete randomization method. After 12 weeks, 40 rats with successful modeling which were confirmed to meet the requirements for MHE model by the Morris water maze test were randomly divided into model group (MOD group), lactulose group (LT group), low-dose RD group (RD1 group), middle-dose RD group (RD2 group), and high-dose RD group (RD3 group), with 8 rats in each group. The rats in the CON group and the MOD group were given retention enema with 2 mL of normal saline once a day; the rats in the LT group were given retention enema with 2 mL of lactulose at a dose of 22.5% once a day; the rats in the RD1, RD2, and RD3 groups were given retention enema with 2 mL RD at a dose of 2.5, 5.0, and 7.5 g/kg, respectively, once a day. After 10 days of treatment, the Morris water maze test was performed to analyze the spatial learning and memory abilities of rats. The rats were analyzed from the following aspects: behavioral status; the serum levels of alanine aminotransferase (ALT), aspartate aminotransferase (AST), interleukin-1β (IL-1β), interleukin-6 (IL-6), and tumor necrosis factor-α (TNF-α) and the level of blood ammonia; pathological changes of liver tissue and brain tissue; the mRNA and protein expression levels of phosphatidylinositol 3-kinase (PI3K), protein kinase B (AKT), and mammalian target of rapamycin (mTOR) in brain tissue. A one-way analysis of variance was used for comparison of continuous data between multiple groups, and the least significant difference t-test was used for further comparison between two groups. ResultsCompared with the MOD group, the RD1, RD2, and RD3 groups had a significantly shorter escape latency (all P<0.01), significant reductions in the levels of ALT, AST, IL-1β, IL-6, TNF-α, and blood ammonia (all P<0.05), significant alleviation of the degeneration, necrosis, and inflammation of hepatocytes and brain cells, and significant reductions in the mRNA and protein expression levels of PI3K, AKT, and mTOR in brain tissue (all P<0.05), and the RD3 group had a better treatment outcome than the RD1 and RD2 groups. ConclusionRetention enema with RD can improve cognitive function and inflammatory damage of brain tissue in MHE rats, possibly by regulating the PI3K/AKT/mTOR signaling pathway. 
		                        		
		                        		
		                        		
		                        	
4.Correlation of platelet parameter changes and prognosis in children with severe community-acquired pneumonia

		                        			Objective:To investigate the dynamic trend of platelet(PLT)count and mean platelet volume(MPV)in children with severe community-acquired pneumonia(SCAP)in PICU and their correlation with prognosis.Methods:A retrospective study was conducted in 215 SCAP children who were admitted to the PICU of Beijing Children's Hospital Affiliated to Capital Medical University from January 2016 to December 2019.According to the disease outcome,the patients were divided into improvement group ( n=184) and unrecovered group ( n=31).The changes of PLT count and MPV at admission,on the 2nd,3rd,and 7th days of hospitalization and before discharge were observed,and the relationship between changes in PLT parameters and poor prognosis was analyzed. Meanwhile,the correlation between thrombocytopenia on admission and on the 7th day of hospitalization and prognosis was further explored. Results:The PLT count of improvement group at admission,on the 2nd,3rd,and 7th days of hospitalization and at discharge[(328±159, 329±137, 362±159, 439±168, 510±171)×10 9/L] were significantly higher than those of unrecovered group [(210±142, 207±152, 267±143, 260±162, 343±159)×10 9/L]( P<0.05).Although the MPV of improvement group [(10.9±1.9)fL] on admission was significantly lower than that of the unrecovered group[(12.7±2.5) fL]( P<0.05),there was no significant difference in MPV between two groups on the 2nd,3rd,7th days of hospitalization and discharge( P>0.05).In addition,compared with the admission,children in improvement group had significantly higher PLT count on the 7th day of hospitalization and before discharge( P<0.05),but there was no significant change in unrecovered group( P>0.05).Compared with SCAP patients with thrombocytopenia at admission (PLT<100×10 9/L)( n=22),those with thrombocytopenia on 7th day of hospitalization had a significant higher rate of non recovery( P<0.05). Conclusion:The occurrence of thrombocytopenia on admission and after 7 days of hospitalization in children with SCAP is associated with poor prognosis.No significant increase or decrease in PLT count after 7 days of hospitalization is often indicative of poor prognosis.Dynamic monitoring of PLT parameter changes may help to better judge the prognosis of severe pneumonia.
		                        		
		                        		
		                        		
		                        	
5.Mediating effect of professional self-efficacy between professional values and willingness to work on gerontological care of nursing students in higher vocational colleges

		                        			Objective:To explore the mediating effect of professional self-efficacy between professional values and willingness to work on gerontological care of nursing students in higher vocational colleges.Methods:A total of 391 nursing students from Chengdu area full-time university were investigated by general information questionnaire, gerontological nursing career motivation questionnaire, professional values scale, and professional self-efficacy scale. Pearson correlation analysis and descriptive statistical analysis were performed by SPSS 21.0. Amos 26.0 was used to establish mediating effect model and verify the mediating effect.Results:The total score of 391 nursing students' willingness to work on gerontological care was (47.05±6.93), the willingness to work on gerontological care was positively correlated with professional self-efficacy and professional values ( r = 0.826, 0.775, respectively, all P<0.01), and professional self-efficacy played a partial mediating role between professional values and willingness to work on gerontological care, accounting for 64.82% of the total effect. Conclusions:The willingness to work on gerontological care of nursing students in higher vocational colleges is at a middle level. Professional values not only directly affect the willingness to work on gerontological care, but also indirectly take function through the professional self-efficacy. 6.PDHA1 promotes proliferation,invasion and metastasis of triple-nega-tive breast cancer cells

		                        			AIM:One of the important characteristics of the occurrence and development of triple-negative breast cancer(TNBC)is dysregulated cell metabolism.The aim of this study is to investigate the mechanism of pyruvate dehydrogenase E1 subunit alpha 1(PDHA1),a key enzyme component in aerobic glycolysis,affecting the proliferation,metastasis and invasion of TNBC.METHODS:(1)The expression levels of PDHA1 in breast cancer tissues and adja-cent tissues were analyzed by UALCAN database,KM-plotter database,Gene MANIA database and TCGA database.The expression of PDHA1 was compared according to tumor pathological stage,subtype classification and breast cancer bio-markers.The function of PDHA1 in TNBC was explored by gene enrichment analysis.(2)Immunohistochemistry assays were used to detect the expression of PDHA1 in human TNBC tissue and adjacent tissue samples.(3)Stable PDHA1 knockout and PDHA1 rescue TNBC MDA-MB-231 cells were constructed.The proliferation of MDA-MB-231 cells was de-tected by colony formation assay and cell counting assay.The regulatory effect of PDHA1 on the invasion and migration of MDA-MB-231 cells was detected by in vitro scratch assay and Transwell migration assay.RESULTS:Database analysis showed that the group with high PDHA1 expression in breast cancer had shorter survival and worse prognosis.In clinical specimens,the expression of PDHA1 in cancer tissues was higher than that in adjacent normal tissues.Knockout of PDHA1 inhibited the proliferation,metastasis,invasion and epithelial-mesenchymal transition of MDA-MB-231 cells.CONCLUSION:PDHA1 is overexpressed in TNBC,and it promotes cell proliferation and facilitates TNBC metastasis through the epithelial-mesenchymal transition pathway.
		                        		
		                        		
		                        		
		                        	
7.Application of virtual simulation experiment in the teaching of Laboratory Sciences for Virus Infection Testing

		                        			Objective:To investigate the application value of virtual simulation experiment in the teaching of Laboratory Sciences for Virus Infection Testing.Methods:Undergraduate students(n=58)majoring in Health Inspection and Quarantine of grade 2020 were selected as the research objects.The"Virtual simulation experiment of influenza virus isolation and identification in BSL-2 laboratory"was included in the teaching of Laboratory Sciences for Virus Infection Testing.The application effect of the virtual simulation experiment was evaluated by a self-designed questionnaire.Results:A total of 50 students completed the questionnaire effectively.After completing the virtual simulation experiment,96.0%of the students thought that the simulation degree of the experiment was high.Students could understand the instrument composition,internal structure,operation process,and matters needing attention.And 52.0%of the students thought that the experiment had an impact on autonomous learning.Students believed that the experiment could improve their interest in the course(88.0%),increase their mastery of theoretical knowledge(60.0%),and improve their ability to analyze and solve problems(46.0%).And 72.0%of the students thought that the running speed of the virtual simulation experiment was slow.The top three advantages of the experiment evaluated by students were easy operation,no damage to the instrument and no worry about the dangers in the experiment,and helpful to understand the knowledge.The top three disadvantages of this experiment were no improvement in practical ability,no familiarity with the use of instruments,different feelings from the real experiment and too idealized experimental process.And 66.0%of the students thought that virtual simulation experiments should complement traditional experimental teaching.Conclusions:Virtual simulation experiment can carry out dangerous experiments that are difficult to perform in traditional laboratories.It can improve students'mastery of theoretical knowledge,enhance their interest in learning,and improve their ability to analyze and solve problems.However,there are still some problems in its application,which should be combined with traditional experimental teaching to improve the teaching effect.
		                        		
		                        		
		                        		
		                        	
8.Study on the relationship between serum Autotaxin,Copeptin,LBP and prognosis in patients with hepatitis B virus-related decompensated cirrhosis complicated with liver failure

		                        			Objective To investigate the relationship between serum autotaxin,copeptin,lipopolysaccharide binding protein(LBP)and prognosis in patients with hepatitis B virus-related decompensated cirrhosis(HBV-DC)complicated with liver failure(LF).Methods 143 patients with HBV-DC complicated with LF who admitted to our hospital from February 2018 to August 2023 were selected as the research objects.Patients were followed up for 90 d,patients were divided into death group(55 cases)and survival group(88 cases)according to the prognosis,the levels of serum Autotaxin,Copeptin and LBP were compared between two groups.The clinical data of patients with HBV-DC complicated with liver failure were collected,the prognostic factors of HBV-DC complicated with LF patients were analyzed by univariate and multivariate Logistic regression models.The clinical value of serum Autotaxin,Copeptin and LBP alone or in combination in predicting the prognosis of patients with HBV-DC complicated with LF were analyzed by receiver operating characteristic(ROC)curve.Results 143 patients with HBV-DC complicated with LF 90 d follow-up,55 died,and 88 survived,with a mortality rate of 38.46%.Compared with survival group,the serum levels of Autotaxin,Copeptin and LBP in death group were significantly increased(P<0.05).Compared with survival group,the proportion of hospitalization time ≥14 d,the proportion of ascites,the proportion of hepatic encephalopathy,alanine aminotransferase,total bilirubin and model for end-stage liver disease(MELD)score in death group were significantly increased(P<0.05),and albumin was significantly decreased(P<0.05),there was no significant difference in age,gender,diabetes mellitus,hypertension,serum creatinine,platelet count and fibrinogen(P>0.05).Elevated total bilirubin,concurrent hepatic encephalopathy,elevated MELD score,and elevated serum Autotaxin,Copeptin,and LBP levels were risk factors for poor prognosis in patients with HBV-DC complicated with LF(P<0.05).ROC curve results showed that,the area under the curve(AUC),sensitivity and specificity of combined detection of serum Autotaxin,LBP and Copeptin in predicting poor prognosis of patients with HBV-DC complicated with LF were 0.930,85.45%and 88.64%,respectively,which were significantly better than those of single index detection.Conclusion The high expression of serum Autotaxin,Copeptin and LBP are related to the risk of short-term death in patients with HBV-DC complicated with LF,and the combined detection has a high clinical predictive value for the occurrence of short-term death in patients with HBV-DC complicated with LF.
		                        		
		                        		
		                        		
		                        	
9.Correlations of serum eukaryotic translation promoter 2α and activated transcription factor 4 levels with renal tissue injury and renal function in diabetic nephropathy patients

		                        			Objective To investigate the correlations of serum eukaryotic translation promoter 2α (eIF2α) and activated transcription factor 4 (ATF4) levels with the degree of renal tissue injury and renal function in patients with diabetic nephropathy (DN). Methods A total of 102 patients with DN (DN group) and 102 patients with simple diabetes (control group) were selected. According to the severe degree of renal tissue damage, the patients with DN were divided into microalbuminuria group (MG group, 35 cases) and dominant albuminuria group (PG group, 41 cases) and renal dysfunction group (RIG group, 26 cases). Serum levels of eIF2α, ATF4, urea nitrogen (BUN), creatinine (Scr), cystatin C (CysC) and estimated glomerular filtration rate (eGFR) were measured. Pearson correlation analysis was used to explore the correlations of eIF2α and ATF4 with BUN, Scr, CysC and eGFR; multivariate Logistic regression analysis was used to explore the risk factors of DN; receiver operating characteristic (ROC) curve was used to analyze the value of eIF2α and ATF4 in diagnostic of DN. 10.Preliminary study on the efficacy of subretinal injection of Aflibercept in the treatment of refractory polypoidal choroidal vasculopathy

		                        			Objective:To observe the efficacy and safety of subretinal injection of Aflibercept for the treatment of refractory or recurrent polypoidal choroidal vasculopathy (PCV).Methods:A prospective clinical research. From January to June 2022, 18 patients of 18 eyes with PCV diagnosed in The Affiliated Eye Hospital of Nanchang University were included in the study. All patients underwent best corrected visual acuity (BCVA), indocyanine green angiography and optical coherence tomography (OCT). The BCVA examination was performed using the international standard visual acuity chart, which was converted to logarithm of the minimum angle of resolution (logMAR) visual acuity during statistics. The large choroidal vessel thickness (LVCT), central retinal thickness (CRT), sub-foveal choroidal thickness (SFCT) and retinal pigment epithelium detachment (PED) height were measured by enhanced depth imaging technique of OCT. The choroidal vascular index (CVI) was calculated. There were 18 patients of 18 eyes, 11 males of 11 eyes and 7 females of 7 eyes. The age was (64.22±3.86) years old. The disease duration was (5.22±1.80) years. The patient had received intravitreal injection of anti-vascular endothelial growth factor (VEGF) drugs for (7.72±1.36) times. The logMAR BCVA of the affected eyes was 1.28±0.25. The SFCT, CRT, LVCT, PED height were (436.56±9.80), (432.44±44.29), (283.78±27.10), (342.44±50.18) μm, respectively, and CVI was 0.65±0.01. All eyes were treated with a single subretinal injection of 40 mg/ml Aflibercept 0.05 ml (including Aflibercept 2.0 mg). According to the results of OCT and BCVA after treatment, the lesions were divided into active type and static type. The active lesions were treated with intravitreal injection of Aflibercept at the same dose as before. Quiescent lesions were followed up. Examinations were performed 1-3, 6, 9 and 12 months after treatment using the same equipment and methods before treatment. The BCVA, LVCT, CRT, SFCT, PED height, CVI, interretinal or subretinal fluid, lesion regression rate, injection times, and complications during and after treatment were observed. The BCVA, SFCT, CRT, LVCT, PED height and CVI before and after treatment were compared by repeated measures analysis of variance.Results:Eighteen eyes received subretinal and/or intravitreal injection of Aflibercept (1.61±0.85) times (1-4 times). At the last follow-up, the polypoid lesions regressed in 4 eyes and PED disappeared in 1 eye. Compared with before treatment, BCVA ( F=50.298) gradually increased, CRT ( F=25.220), PED height ( F=144.16), SFCT ( F=69.77), LVCT ( F=136.69), CVI ( F=72.70) gradually decreased after treatment. The differences were statistically significant ( P<0.001). Macular hole occurred in 1 eye after treatment, and the hole closed spontaneously 3 months after treatment. No serious complications such as retinal tear, retinal detachment, endophthalmitis and vitreous hemorrhage occurred during and after treatment.
