1.Mediating effects of loneliness and depressive symptoms on family function and life satisfaction among rural elderly patients with chronic diseases

Objective:
To analyze the mediating effects of loneliness and depressive symptoms on family functioning and life satisfaction among rural elderly patients with chronic diseases, so as to provide the basis for improving the life satisfaction of this population.
Methods:
Rural elderly patients with chronic diseases aged ≥60 years in Qiannan Buyi and Miao Autonomous Prefecture, Guizhou Province were selected using a multi-stage stratified random cluster sampling method from June to September 2022. Basic information such as gender, age, and chronic diseases were collected. Family function, life satisfaction, loneliness and depressive symptoms were evaluated using Family Care Index Scale, the Satisfaction with Life Scale, the b-item Revised VCLA Loneliness Sale and the 15-item Geriatric Depression Scale, respectively. The structural equation model was constructed using Amos software to analyze the mediating effects of loneliness and depressive symptoms on the relationship between family function and life satisfaction. The Bootstrap method was employed to test the mediating effects.
Results:
A total of 1 145 rural elderly patients with chronic diseases were recruited, including 517 males (45.15%) and 628 females (54.85%). Among the participants, 657 individuals (57.38%) were aged 60-<71 years, and 540 individuals (47.16%) had three or more chronic diseases. The scores for family function, life satisfaction, loneliness, and depressive symptoms were (3.90±1.18), (18.88±5.25), (12.88±2.99), and (6.65±2.26), respectively. Mediating effect analysis showed that family function had a direct positive effect on life satisfaction (β=0.179, 95%CI: 0.126-0.231). It also indirectly positively influenced the life satisfaction of rural elderly patients with chronic diseases through the independent mediating effect of depressive symptoms (β=0.035, 95%CI: 0.021-0.054) and the chained mediating effect of loneliness and depressive symptoms (β=0.021, 95%CI: 0.013-0.030). The mediating effect of depressive symptoms accounted for 14.89% of the total effect, while the chained mediating effect of loneliness and depressive symptoms accounted for 8.94% of the total effect.
Conclusion
Good family function can directly enhance the life satisfaction of rural elderly patients with chronic diseases and can also indirectly improve their life satisfaction by reducing loneliness and depressive symptoms.
2.Effect of opioid-sparing analgesia on incidence of sepsis in severely burned patients: a retrospective cohort study

Objective:To evaluate the effects of opioid-sparing analgesia on the incidence of sepsis in severely burned patients in the retrospective cohort study.Methods:The clinical data from patients with severe burns admitted to three teaching hospitals in Guangdong from 2011 to 2020 were retrospectively extracted and analyzed. The patients were divided into 2 groups based on the analgesic regimen within 30 days after injury: continuous opioids analgesia group (continuous opioid infusion at a relative constant rate for more than 72 h) and opioid-sparing analgesia group (patient-controlled intravenous analgesia/intermittent administration/opioid-free analgesia). Patient′s age, severity of burn, inhalation injury and basal pain score at rest were matched by the propensity score at a 1∶1 ratio. The primary outcome measure was the occurrence of sepsis within 90 days of admission. Secondary outcome measures included 30-day and 90-day all-cause mortality, clinical diagnosis of multiple organ dysfunction syndrome, and prevalence of burn wound infection. The amount of opioid used was also recorded.Results:A total of 328 severely burned patients were finally enrolled, with 145 patients in continuous opioid analgesia group and 183 patients in opioid-sparing analgesia group, and 110 pairs of patients (220 cases) were finally matched by the propensity score.Compared with continuous opioid analgesia group, the total consumption of opioid, daily consumption per analgesia, and consumption per burn area were significantly decreased, and the incidence of sepsis and wound infection was decreased( P<0.05), and no significant change was found in the incidence of multiple organ dysfunction syndrome, 30-day and 90-day all-cause mortality in opioid-sparing analgesia group( P>0.05). Conclusions:Compared with the continuous opioid analgesia regimen, opioid-sparing analgesia can reduce the risk of sepsis in severely burned patients.
3.Analysis of the effect of modified electroconvulsive therapy on cognitive function and therapeutic effect in patients with depression

Objective To observe the effect of modified electroconvulsive therapy combined with venlafaxine on cognitive function and its efficacy in patients with depression.Methods 60 patients with depressive disorder were selected,and they were randomly divided into study group and control group according to the random number table method,30 cases in each group.The patients in the study group accepted modified electroconvulsive therapy for 10 times combined with venlafaxine sustained-release tablets treatment,while the patients in the control group were only treated with venlafaxine.Both two groups were treated for 6 weeks.The cognitive function was assessed by the Repeat able Battery for the Assessment of Neuropsycholgical Status (RBANS) and the therapeutic effect was evaluated by Hamilton Depression(HAMD) Scale before and after treatment.Results The scores of immediate memory,delayed memory,speech function,visual breadth and attention of both two groups after treatment were significantly higher than those before treatment(all P < 0.05),and the attention score of the study group was (109.96 ± 19.01) points,which was higher than (100.32 ± 17.13)points of the control group (t =2.062,P < 0.05).The HAMD scores after treatment in both two groups were significantly lower compared with those before treatment (all P < 0.05),and the HAMD score of the study group was (7.23 ± 4.19)points,which was lower than (10.27 ± 5.99)points of the control group (t =2.273,P < 0.05).The total effective rate of the study group(96.7%) was significantly higher than that of the control group (86.7 %) (x2 =6.546,P < 0.05),and the cure rate of the study group (60.0%) was also higher than that of the control group (36.7%) (x2 =10.901,P < 0.05).Conclusion Modified electroconvulsive therapy combined with venlafaxine treatment can effectively improve the cognitive function of depressive patients,and its effect is significantly better than monotherapy.
4.Ability of independent creative learning of students majoring in preventative medicine should be promoted rather than be taught

Preventive medicine is a highly practical vocation; the students majoring in this major should be trained with abilities of active learning and flexible applying.Nowadays,the main teaching methods in many colleges and universities were assigning homework,communicating on the internet and posing questions in the class,which were only taught by the teacher.The promotion model may promote the knowledge application for the students,with the heuristic teaching in the class based on the virtual or scenic status,practice in scientific research,social service and experiments in the real sites.Through promotion model,students can combine the theoretical knowledge with the reality,learn creatively and apply the knowledge flexibly.
5.Effects of sinomenine on the psychic dependence on morphine and the brain cyclic AMP level in mice

AIM To explore the effects of sinomenine (Sin) on psychic dependence induced by morphine (Mor) in mice and its relation to the cAMP content of the cerebral cortex. METHODS A biased procedure induced the strong place preference. The effect of Sin (10, 30, 60 mg?kg -1 , ip) on the rewarding property of morphine was observed in a conditioned place preference(CPP) paradigm in mice,at the same time, the change of cAMP content in cerebral cortex was measured by the radioimmunoassay(RIA). RESULTS Sin significantly shortened the time of staying in morphine-pair compartment and lowered the cAMP level of mice. CONCLUSION Sin could attenuate effects of psychical dependence induced by Mor, its mechanism may be related to decrease of cAMP level in the brain, and Sin did not display psychical dependence in the experiment.
6.INVESTIGATION OF THE DIETARY PATTERN AND NUTRITIONAL LEVELS OF THE INHABITANTS IN JIANGSU PROVINCE

A study on the dietary pattern and nutritional levels of the inhabitants in Jiangsu province was carried out by means of both analysis of food consumption and dietary survey. The results showed that the problem of basic need for food has been resolved in this province since the 1980's mid-period, but animal food consumption was still low relatively.Compared with the data obtained from dietary survey, the amount of grain consumed per capita per day obtained from calculation of food consumption was about 70 g more and the amount of animal food 18 g less. The intakes of energy and nutrients obtained from the two methods were different, but from animal sources showing about 4% higher in the latter. This suggested that the results from dietary survey was nearer to the actual status.The results indicated a tendency lying in the period of adjustment of food pattern. It also demonstrated the imbalance status for some nutrients intake.The difference of the food pattern and nutritional level between rural and urban was distinct and the difference also existed in intercountries. The health index of the inhabitants was better in the recent years but the correlation between the physical index and nutritional level hasn't been established yet.We suggested to increase animal food and beans intake properly, However the animal fat intake must be controlled. A goal was presented about the dietary pattern for the inhabitants in Jiangsu province in 2000. Various demands should de given according to the various area conditions.In addition, some discussion about the DDP score method has been made.
