1.Clinical Evidence of Oral Chinese Patent Medicines in Treatment of Chronic Kidney Disease: A Scoping Review

ObjectiveTo review the information and clinical studies of oral Chinese patent medicines (CPMs) for chronic kidney disease (CKD). MethodThe CPMs for treating CKD were retrieved from the Pharmacopoeia of the People's Republic of China, National Essential Drugs List, and Medicine List for National Basic Medical Insurance, Employment Injury Insurance and Maternity Insurance. China National Knowledge Infrastructure(CNKI), VIP, Wanfang Data, SinoMed, PubMed, Embase, Cochrane, and Web of Science were searched for the clinical trials of the treatment of CKD by CPMs from their inception dates to September 25, 2022. A database was established with the collected CPMs, and then the general conditions of the clinical trials were analyzed and presented visually. ResultA total of 16 CPMs for CKD were included in this study, including 5 classical traditional Chinese medicine (TCM) prescriptions involving Rehmanniae Radix and 11 new CPMs. The indications of the TCM prescriptions did not mention the corresponding western disease names, and those of the new CPMs mainly included chronic renal insufficiency, chronic renal failure, and chronic nephritis. Four CPMs were prepared with single Chinese medicine or active components. Specifically, Bailing Preparation and Jinshuibao Preparation were mainly prepared with the powder of Cordyceps, and the main components of Haikun Shenxi capsules and Huangkui capsules were fucoidan sulfate and the flower extract of Abelmoschi Corolla, respectively. The CPMs mainly exerted tonifying and eliminating effects on the lung, spleen, and kidney. A total of 892 clinical trials were screened out, covering all the areas in China and presented an increasing trend. Bailing Preparation was the most studied, followed by Niaoduqing Preparation. Among the 892 studies, 475 focused on single CPMs without combination with other CPMs or therapies. These studies mainly compared between conventional intervention and conventional intervention + CPM, which accounted for 75.58%. The 475 studies covered different kidney diseases, such as chronic kidney disease, chronic renal failure, nephrotic syndrome, diabetic kidney disease, IgA nephropathy, and membranous nephropathy, and involved a variety of populations including the elderly and children. Thirty-six studies evaluated TCM syndromes, reflecting the characteristics and advantages of TCM treatment. ConclusionThere are abundant oral CPMs for CKD, with varied efficacy and characteristics for different kidney diseases. However, the instruction manuals of the oral CPMs are not detailed or standard. According to the clinical research evidence in this field, the research on oral CPMs for CKD is characterized by a wide scope, rich study types, and wide disease coverage, while the sample size and quality remain to be improved.
2.A retrospective cohort study on postoperative radiotherapy and postoperative chemoradiotherapy for soft tissue sarcomas

Objective:To analyze the differences in clinical outcomes and toxicities between postoperative radiotherapy alone and postoperative radiochemotherapy for soft tissue sarcoma (STS), as well as the related factors affecting clinical prognosis of STS patients.Methods:Retrospective analysis of patients diagnosed with primary STS admitted to Zhejiang Cancer Hospital from May 2012 to May 2019 was performed, who received adjuvant radiotherapy after surgery, combined with or without postoperative chemotherapy. A total of 100 patients were enrolled and divided into postoperative radiotherapy group ( n=52) and postoperative radiochemotherapy group ( n=48). The median follow-up time was 65 months (24-124 months). The local recurrence-free survival (LRFS), distant metastasis-free survival (DMFS), overall survival (OS), and treatment-related toxicities were recorded in two groups. The survival rate was calculated by Kaplan-Meier analysis. Log-rank test was used for univariate analysis, and Cox model was used for multivariate analysis. Results:In multivariate analysis, the maximum tumor diameter was an independent predictor of local tumor recurrence ( HR=4.80, 95% CI=1.16-19.85, P=0.031), distant metastasis ( HR=4.67, 95% CI=1.53-14.26, P=0.007) and OS ( HR=4.10, 95% CI=1.35-12.48, P=0.013). In addition, the degree of myelosuppression in patients in postoperative radiochemotherapy was significantly higher than that in their counterparts in postoperative radiotherapy group ( P<0.001). Conclusions:In the limited number of patients, radiochemotherapy has no advantages over radiotherapy alone in distant metastasis or survival rate. Besides, it increases toxicities, but the overall tolerability is favorable. It is necessary to conduct prospective randomized studies in a large population and subgroup analysis of histological subtypes, aiming to obtain results with better reference value.
3.Effect of amino acid site modification on stability of foot-and-mouth disease virus-like particles.

The stability of virus-like particles (VLPs) is currently the main factor affecting the quality of foot-and-mouth disease VLPs vaccines. In order to further improve the quality of the VLPs vaccine of foot-and-mouth disease (FMD), three amino acid modification sites were designed and screened through kinetic analysis software, based on the three-dimensional structure of FMDV. The three mutant recombinant plasmids were successfully prepared by the point mutation kit, transformed into Escherichia coli strain BL21 and expressed in vitro. After purification by Ni ion chromatography column, SDS-PAGE proved that the three amino acid mutations did not affect the expression of the target protein. The results of the stability study of three FMD mutant VLPs obtained by in vitro assembly show that the introduction of internal hydrophobic side chain amino acids made the morphology of VLPs more uniform (N4017W), and their stability was significantly improved compared to the other two VLPs. The internal hydrophobic force of the capsid contributes to the formation of VLPs and helps to maintain the stability of the capsid, providing new experimental ideas for improving the quality of VLPs vaccines, and helping to promote the development of VLPs vaccines.

4.Analysis on the effects of individualized surgical method for treating pyriform sinus fistula in children

Objective:To explore the curative effects of individualized surgical method for treating pyriform sinus fistula (PSF) in children.Methods:A total of 43 PSF children treated in the Department of Pediatric Surgery of Henan Provincial People′s Hospital from June 2015 to November 2019 were selected, with 41 cases on the left side and 2 cases on the right side.There were 27 cases in the quiescent inflammatory infection stage and 16 cases in the acute inflammatory infection stage.According to patients′ condition, low-temperature plasma radiofrequency ablation, fistulectomy, abscess incision and drainage or combined operation were conducted.The postoperative complications and recurrence were observed.Results:Among 43 cases, there were 19 cases treated with low-temperature plasma radiofrequency ablation, 11 cases treated with low-temperature plasma radiofrequency ablation combined with abscess incision and drainage, 5 cases treated with low-temperature plasma radiofrequency ablation combined with fistulectomy, and 8 cases treated with fistulectomy.Four cases had postoperative hoarseness and recovered within 1-5 months.One case occurred pseudo healing after abscess incision and drainage, and healed after incision and debridement.Totally, 4 cases recurred, and the follow-up time was from 3 months to 57 months.Conclusions:Low-temperature plasma radiofrequency ablation has the advantages of minimally invasive, safe and effective, which provides a new treatment for PSF in children.Based on the condition of children, the choice of individualized treatment can significantly improve the curative effects.
5.Application of online PBL teaching in clinical teaching of anesthesia nursing

Objective:To explore the effect of online PBL teaching in clinical practice of anesthesia nursing during the coronavirus disease 2019 (COVID-19) epidemic.Methods:From January 2020 may 2020, 84 clinical nursing students who received anesthesia nursing teaching in our hospital were selected and randomized into the control group ( n=42) and the experimental group ( n=42). According to the different teaching methods, the control group adopted routine online teaching mode and the experimental group adopted online PBL teaching mode. SPSS 21.0 was used to compare scores of the theoretical knowledge, nursing evaluation, nursing plan, clinical operation and total scores of the two groups. Results:There was no significant difference in theoretical knowledge between the two groups ( P>0.05). The nursing evaluation, nursing plan, clinical operation and total scores of the anesthesia nursing students in the experimental group were significantly higher than those in the control group( P<0.05), and the experimental group had a significantly higher degree of teaching satisfaction than the control group ( P<0.05). Conclusion:During the special period of the COVID-19 epidemic, the online PBL teaching has achieved good teaching effect of online practice of anesthesia nursing specialty, which has ensured "classes suspended but learning continues", and improved the comprehensive examination results of the students. This teaching mode has gained good teaching satisfaction, worthy of clinical promotion and further study.
6. Relationship between morphological characteristics and prognosis of non-nasopharyneal EBV-associated carcinoma

Objective:
To analyze the pathological features and their influence on the clinical outcome of non-nasopharyngeal EBV-associated carcinomas.
Methods:
One hundred and twenty cases of non-nasopharyngeal EBV-associated carcinoma confirmed by in situ hybridization were identified at Zhejiang Cancer Hospital from January 1, 2006 to May 1, 2018, and the clinicopathological data were collected and analyzed using Kaplan-Meier survival analysis, Cox univariate and multivariate analysis.
Results:
One hundred and twenty cases were involved in the study; the male to female ratio was 1∶1; patients′ age range was 24 to 89 years (median 50 years). The primary sites were large parotid glands (62 cases), lung(26 cases), stomach(15 cases), and others (oral, oropharynx, larynx, cervix, liver; totally 17cases). Non-nasopharyngeal EBV-associated cancer could be divided into two histological types according to the amount of interstitial lymphocytes: type Ⅰ was "lymphoepithelial-like carcinoma" and rich in stromal lymphocytes; type Ⅱ lacked lymphocytic infiltration. Ninety-eight primary tumor samples could be classified morphologically: 43 cases were as type Ⅰ and 55 cases as typeⅡ; the distribution of type Ⅰ was 57.4% (27/47) in large parotid glands, 20.8% (5/24) in lung, 4/13 in stomach, and 7/14 in other sites. Complete treatment and survival data were obtained for 114 patients. According to the TNM staging criteria of WHO, 52 patients were at early stages (Ⅰ-Ⅱ) and 62 were at advanced stages (Ⅲ-Ⅳ); 102 patients underwent surgery. Seventy-four patients received adjuvant chemotherapy before or after surgery, and 52 patients received local radiotherapy. 7.Preparation of Graphene/Gold Nanoparticles/Glucose Oxidase Flexible Electrode Based on Poly(ethylene terephthalate) Film and Its Application in Detection of Glucose

The graphene prepared by chemical vapor deposition onto copper foils was transferred onto a poly(ethylene terephthalate) (PET) soft substrate by the aid of poly-methyl methacrylate (PMMA).The soft G/AuNPs/GOD composite electrode based on PET substrate was fabricated using a protocol in which the uniform distribution of Au nanoparticles (AuNPs) was firstly obtained by controlling the evaporation of gold sol on a graphene surface, then thioglycolic acid (TGA) was modified on the AuNPs through Au-S bond, and finally glucose oxidase (GOD) was immobilized on the surface of AuNPs through acylation reaction between TGA and the GOD.Glucose was detected in the linear range from 0.05 to 10.55 mmol/L with a linear correlation coefficient (r) of 0.9955.The detection was performed in phosphate buffer solution (pH 7) at 25℃ with a working potential of 0.6 V (vs.SCE electrode), and the detection limit was 1 μmol/L (3σ).The G/AuNPs/GOD flexible electrode based on PET substrate provided a new pathway to detect glucose in special environments using wearable equipment, which enlarged the applied field of glucose detection.
8.Turnover intention of nursing staff in a tertiary TCM hospital

Objective To investigate the turnover intention of nursing staff in a Beijing tertiary traditional Chinese medicine ( TCM ) hospital. Methods Self-designed questionnaire was used to investigate 295 nursing staff from a tertiary TCM hospital. The questionnaire included the general information of nurses, problems that nurses faced with and turnover intention. Results Nurses satisfied with current job or somewhat satisfied with current job accounted for 80%; 86% of nurses once had job burnout; and 35% of nurses had turnover intention. Influencing factors of nurses′ turnover intention contained wages, working pressure, large workload, etc. Conclusions Labor unions of hospitals and the administrative departments should be more concerned about the nurses, provide relief measures, and improve the social benefits of nurses.

10.Stratified study of high-risk colorectal neoplasm population in patients undergoing coronary artery angiography examination

Objective To investigate the difference between Asia-Pacific Colorectal Screening (APCS) scoring system and colorectal cancer sequential screening criteria issued by the Health Ministry of China (China sequential criteria) in the evaluation of high-risk colorectal neoplasm in patients undergoing coronary artery angiography (CAG) examination.Methods The data of 870 patients aged from 40 to 74 who underwent CAG examination were retrospectively analyzed.The measurement data were analyzed by t test and the count data were aralyzed by x2 test.Results There were 72 patients aged from 40 to 49 years old.Among them,eight patients were stratified as high-risk population according to the Chinese sequential criteria; however there was no high-risk population by APCS.There were 798 patients aged from 50 to 74 years old.There were 460 patients stratified as high-risk population by APCS.The percentage of CAG negative group (34.7%) was significantly lower than that of the coronary artery disease (CAD) group (68.0%,x2 =77.74,P<0.01).According to the Chinese sequential criteria,and there were 134 patients stratified as high-risk population,and there was no significant difference between the CAG negative group (17.7 %) and the CAD group (16.4%,P>0.05).Among the patients aged from 50 to 74 years old without family history of colorectal cancer in first-degree relatives,72 cases (29.0%) of the CAG negative group and 316 cases (57.5%) of the CAD group were stratified as high-risk according to APCS,however not stratified as high-risk by the Chinese sequential criteria.About 90.5 % (351/388) of them were male smokers.According to APCS,30 cases (12.1%) of the CAG negative group and 32 cases (5.8%) of the CAD group were stratified as middle-risk population,however stratified as high-risk population by the Chinese sequential criteria.About 75.8% (47/62) of them were female non-smokers.Conclusions The percentage of patients stratified as high-risk population by APCS was higher than that by the Chinese sequential criteria.In patients aged from over 50 to 74 years old and without family history of colorectal cancer in first-degree relative,APCS maybe overestimated the risk degree of colorectal neoplasm in male smokers and underestimated the risk degree in female non-smokers.
