1.Distribution and drug resistance characteristics of Acinetobacter baumannii in the environment of a general hospital in Xuhui District of Shanghai from 2018 to 2023

ObjectiveTo analyze the distribution, drug resistance characteristics, and changing trends of Acinetobacter baumannii (AB) isolated from environmental surfaces and healthcare workers’ hands in a grade Ⅱ level A general hospital in Xuhui District of Shanghai from 2018 to 2023, and to provide reference for infection control in the hospital. MethodsEnvironmental samples were collected quarterly from critical surfaces and healthcare workers’ hands in the intensive care unit (ICU), geriatrics, and respiratory departments from 2018 to 2023. Clinical isolates were obtained from all patients with AB infections in ICU, geriatrics, respiratory department, rehabilitation department, infectious diseases department, emergency department, cardiology department, and orthopedics of the hospital from 2018 to 2023. Retrospective analyses were performed on AB detection rates, strain origins, resistance rates to commonly used antimicrobial agents, and resistance gene features, comparing the antimicrobial resistance between clinically isolated strains and environmentally isolated strains. ResultsFrom 2018 to 2023, a total of 1 416 samples were collected from the hospital and a total of 272 strains of AB were detected, with a positive detection rate of 19.21%. The detection rate gradually decreased year-on-year (χ2trend=45.290, P<0.001). The majority of samples originated from patient-contacted items (34.56%, 94/272), followed by shared items (26.84%, 73/272) and healthcare worker-contacted items (15.07%, 41/272). From 2018 to 2023, the resistance rate of AB on environmental surfaces and healthcare workers’ hands to commonly tested antibiotics in the hospital ranged from 10% to 40%. The resistance rates to cefotaxime (42.52%) and piperacillin (38.58%) were relative high, while the resistance to polymyxin E (1.57%), polymyxin B (2.36%), and doxycycline (3.94%) maintained low. The annual fluctuations in resistance to cefotaxime, piperacillin, ceftriaxone, tobramycin, doxycycline, minocycline and cotrimoxazole were statistically significant (all P<0.05). There were statistically significant differences in the resistance of clinical and environmental isolates to ampicillin/sulbactam, cefepime, ceftazidime, subamphetamine, meropenem, piperacillin, aztreonam, gentamicin, tobramycin, minocycline, ciprofloxacin, levofloxacin, and cotrimoxazole in the hospital from 2018 to 2023 (all P<0.05). The resistance rate of clinical isolates was generally high, especially to β-lactam and quinolone drugs, which were mostly above 80% [such as cefepime (93.86%), cefotaxime (97.37%), imipenem (98.25%), and ciprofloxacin (99.12%)]. The resistance rate of environmental isolated strains to similar antibiotics was relatively lower, mostly concentrated at 10%‒30%. The whole-genome sequencing of 34 carbapenem-resistant Acinetobacter baumannii (CRAB) strains isolated from the hospital environment in 2023 revealed that the main resistance mechanism was overexpression of efflux pumps (51.97%), followed by changes in target sites (32.46%). Among the 34 CRAB strains, carbapenem resistance genes OXA-23 and OXA-51 were detected in 6 strains (17.65%), while genes such as KPC, IMP, VIM, and SIM were not detected. ConclusionFrom 2018 to 2023, AB in the hospital environment exhibited high resistance rates to certain antimicrobial agents and carried multiple resistance genes, indicating a potential transmission risk. It is necessary to further strengthen bacterial resistance monitoring and hospital infection control, and use antibiotics reasonably.
2.The value of multi slice spiral CT in the diagnosis of bilateral Wilms tumor in children

Objective To investigate the application value of multi slice spiral computed tomography(MSCT)in the diagnosis of bilateral Wilms tumor(BWT)in children.Methods The clinical and CT data of 7 children with BWT confirmed by clinical,imaging and pathology were analyzed retrospectively,and all cases underwent CT plain scanning and enhancement CT.Results Two cases were complicated with hypospadias,1 case with cryptorchidism,and 1 case with WAGR syndrome.There were 19 lesions in 14 kid-neys in 7 cases.The lesions were single in 10 kidneys and multiple in 4 kidneys on CT.CT flat scan showed that there were 3 solid lesions,16 cystic lesions,and there were different degrees of necrotic cystic changes inside,2 combined with bleeding,10 calcification,15 clear boundaries,and 4 exudation around.CT enhancement showed that the tumor body was unevenly strengthened,the necrotic cystic area was not strengthened,and the residual renal parenchyma was significantly strengthened.The typical signs were crescent signs and cuddle-ball signs,with 1 case of left renal venous thrombosis and 1 case of the invasion of renal pelvis.Conclusion MSCT can be used in the preoperative diagnosis of BWT in children,to evaluate the effect of chemotherapy and the postoperative evaluation,and to follow-up with and without recurrence and metastasis,so as to provide an important value for clinical diagnosis and treatment.
3.Effects of moxibustion combined with acupuncture on nasal resistance and quality of life in allergic rhinitis patients with lung qi deficiency and cold syndrome

Objective:To discuss the effects of moxibustion combined with acupuncture on nasal resistance and quality of life in allergic rhinitis (AR) patients with lung qi deficiency and cold syndrome.Methods:Randomized controlled trial was conducted. Totally 82 patients of AR were divided into two groups according to random number table method, with 41 cases in each group. The control group received routine intervention, and the observation group received moxibustion combined with acupuncture intervention on the basis of routine intervention. The treatment for both groups lasted for 4 weeks. The severity of nasal symptoms was evaluated using the Total Nasal Symptom Score (TNSS) before and after treatment. The Rhinoconjunctival Quality of Life Questionnaire (RQLQ) was used to evaluate the quality of life, patient nasal resistance was measured, and clinical efficacy was evaluated.Results:The total effective rate was 95.12% (39/41) in the observation group and 75.61% (31/41) in the control group, with statistical significance ( χ2=4.78, P=0.029). The scores and total scores of nasal itching, sneezing, runny nose and congestion in the observation group after treatment were lower than those in the control group ( t values were 4.45, 4.73, 4.64, 4.68, 9.09, respectively, P<0.01). The scores of daily activities, sleep, non-nasal/eye symptoms, behavioral problems, nasal symptoms, eye symptoms, and emotion were lower than those in the control group ( t values were 4.83, 4.63, 4.50, 5.32, 5.56, 5.29, 4.84, respectively, P<0.01). The nasal resistance of the observation group after treatment [(0.15±0.03) Pa/(cm?s) vs. (0.21±0.03) Pa/(cm?s), t=9.06] was lower than that of the control group ( P<0.001). Conclusion:Moxibustion combined with acupuncture in patients with AR can further reduce nasal resistance and improve their quality of life and clinical efficacy.
4.Construction and validation of a risk prediction model for early hungry bone syndrome in maintenance hemodialysis patients post-parathyroidectomy

Objective:Parathyroidectomy(PTX)is an effective treatment for refractory secondary hyperparathyroidism(SHPT),but it can lead to hungry bone syndrome(HBS),significantly threatening the health of maintenance haemodialysis(MHD)patients.While previous studies have analyzed the risk factors for HBS post-PTX,the predictive performance and clinical applicability of these risk models need further validation.This study aims to construct and validate a risk prediction model for HBS in MHD patients with SHPT post-PTX. Methods:A retrospective analysis was conducted on 368 MHD patients with SHPT who underwent PTX at Changsha Jieao Nephrology Hospital from January 2020 to December 2021.Patients were divided into a HBS group and a non-HBS group based on the occurrence of HBS.General data,surgical information,and biochemical indicators were compared between the 2 groups.Multivariate logistic regression was used to identify factors influencing HBS,and a risk prediction model was established.The model's performance was evaluated using receiver operator characteristic(ROC)curves,decision curves,and calibration curves.External validation was performed on 170 MHD patients with SHPT who underwent PTX at the Third Xiangya Hospital of Central South University from January to December 2022. Results:The incidence of HBS post-PTX in MHD patients with SHPT was 60.60%.Logistic regression analysis identified preoperative bone involvement(OR=3.908,95%CI 2.179 to 7.171),preoperative serum calcium(OR=7.174,95%CI 2.291 to 24.015),preoperative intact parathyroid hormone(iPTH)(OR=1.001,95%CI 1.001 to 1.001),preoperative alkaline phosphatase(ALP)(OR=1.001,95%CI 1.000 to 1.001),and serum calcium on the first postoperative day(OR=0.006,95%CI 0.001 to 0.038)as independent risk factors for HBS(all P<0.01).The constructed risk prediction model demonstrated good predictive performance in both internal and external validation cohorts.The internal validation cohort showed an accuracy of 0.821,sensitivity of 0.890,specificity of 0.776,Youden index of 0.666,and area under the curve(AUC)of 0.882(95%CI 0.845 to 0.919).The external validation cohort showed an accuracy of 0.800,sensitivity of 0.806,specificity of 0.799,Youden index of 0.605,and AUC of 0.863(95%CI 0.795 to 0.932). Conclusion:Preoperative bone involvement,serum calcium,iPTH,ALP,and serum calcium on the first postoperative day are influencing factors for HBS in MHD patients with SHPT post-PTX.The constructed risk prediction model based on these factors is reliable.
5.Distribution characteristics of antibiotic resistance genes in the domestic water of residents in Haimen, Jiangsu Province

ObjectiveAntibiotic resistance genes (ARGs) have received wide attention all over the world. The purpose of this study was to explore the bacterial community structure, the types and levels of antibiotic resistance genes in a water body in east China, and to compare and analyze the characteristics of microbial species distribution and antibiotic resistance gene distribution in various water environments. MethodsA total of 10 households in Haimen City, Jiangsu Province were selected and their surrounding water environment samples were collected. 21 water samples including river water (4), Mingou water (9) and well water (8) were collected for metagenomics sequencing, assembled with MetaWRAP, annotated with CARD database, and analyzed with R software. ResultsIn various water bodies, the dominant bacteria phyla was Proteobacteria, the dominant bacteria genera were Deuterostomia, Pseudomonas, Flavobacteriales and Streptomycetaceae. The ARGs annotated were mainly composed of quinolones, aminoglycosides, macrolides and beta-lactams antibiotic resistance genes. The top four relative abundance of resistance genes were macB, RanA, evgS and TxR, The average absolute abundance and expression of resistance genes in well water and Mingou water were higher than those in river water. ConclusionMultiple ARGs are detected to varying degrees in well water, river water, and Mingou water bodies, and the expression of resistance genes in well water and Mingou water bodies is higher than that in river water bodies, possibly due to human production and living activities.
6.Value of fetal ultrasonography index in twin fetal heart malformation screening at 11-13 + 6 weeks of gestation

Objective:To investigate the clinical value of fetal ultrasonography in the screening of congenital heart malformations in twins at 11-13 + 6 weeks of gestation. Methods:A retrospective cohort study method was used.Cases were collected from Beijing Obstetrics and Gynecology Hospital, Capital Medical University, Beijing Maternal and Child Health Care Hospital (January 2012 to December 2016) and the Fourth Hospital of Shijiazhuang City (January 2014 to December 2018). The twins with singleton or twins with cardiac malformation were screened out as the cardiac malformation group, and the twins with normal delivery during the same period were selected as the control group. Ultrasound data such as the nuchal translucency (NT), crown-rump-length (CRL), chorionicity, conception method, NT discordance, CRL discordance, NT discordance ≥20% incidence rate between twins, and the CRL discordance ≥10% incidence rate between twins in the two groups at 11-13 + 6 weeks were compared and analyzed. Results:①There were 30 cases in the cardiac malformation group (including 27 twins with singleton cardiac malformation and 3 twins with twin cardiac malformation) and 1 906 cases in the control group. ②The incidence rates of NT value ≥95th percentile and NT value ≥99th percentile in one or two pregnancies were significantly higher in the cardiac malformation group than in the control group (20.00% vs 2.20%, P<0.001; 10.00% vs 0.63%, P=0.002), the area under the ROC curve (AUC) for cardiac malformation screening was 0.589 and 0.549, respectively; CRL discordance in cardiac malformation group was higher than that in control group ( P=0.018), the incidence of CRL discordance ≥10% in the cardiac malformation group was higher than that in the control group (26.67% vs 12.70%, P=0.024), the AUC of cardiac malformation screening was 0.570; there were no significant differences in the incidence of NT discordance, the incidence of NT discordance ≥20%, pregnancy pattern and chorionicity between the two groups (all P>0.05). ③Multivariate Logistic regression analysis was performed using the incidence rates of NT value ≥95% and NT value ≥99%, CRL discordance ≥10%, and NT discordance ≥20%. The incidences of fetal NT value ≥95% and NT value ≥99% were risk factors for cardiac malformation in twins ( OR=11.095, 105.778; 95% CI=4.311-28.558, 16.984-658.796). Conclusions:Ultrasound at 11-13 + 6 weeks of gestation showing thickened value of one or two fetuses, and the CRL discordance≥10%, indicates an increased risk of cardiac malformation in the twins; NT value ≥95% and NT value ≥99% is a risk factor for heart defects in twins.
7.The effect of miRNA-34b/c-5p on neurokinin-1 receptor-truncated expression in breast cancer

Objective:To investigate the effect of miRNA-34b/c-5p on the expression of neurokinin 1 receptor-truncated(NK1R-Tr)in breast cancer and the effect of miRNA-34b/c-5p and NK1R-Tr on the migration and invasion abilities of breast cancer cells.Methods:Real-time fluorescence quantitative PCR(RT-PCR)was used to detect the expression of miRNA-34b/c-5p and NK1R-Tr in 50 breast can-cer specimens that were collected at Tianjin Medical University Cancer Institute&Hospital from February to May 2013.Western blot analysis was performed to detect the expression of miRNA-34b/c-5p and NK1R-Tr in breast cancer cell lines MDA-MB-231 and MCF-7. Scratch and Transwell assays were carried out to explore the effects of miRNA-34b/c-5p and NK1R-Tr on the migration and invasion abilities of MDA-MB-231 cells.Results:The expression of miRNA-34b/c-5p and NK1R-Tr in breast cancer tissues and cells were signifi-cantly negatively correlated.The relative expression of NK1R-Tr in breast cancer patients with lymph node metastasis was 5.75,and the relative expression of NK1R-Tr in patients with non-lymph node metastasis was 4.29.The relative expression was significantly dif-ferent between the two groups(P=0.026).Overexpression of miRNA-34b/c-5p and knockdown of NK1R-Tr could significantly inhibit the migration and invasion abilities of MDA-MB-231 cells(all P<0.001).Conclusions:The expression of miRNA-34b/c-5p and NK1R-Tr was significantly negatively correlated.MiRNA-34b/c-5p and NK1R-Tr might be potential therapeutic targets for inhibiting the invasion of breast cancer cells.

Objective Controversies exist as to the influences of the characteristics of lymph node metastasis ( LNM) on the prognosis of low-grade endometrial carcinoma ( LGEC) .The aim of this study was to investigate the risk factors of LNM and their im-pact on the prognosis of LGEC. Methods This study included 218 LGEC patients treated by retroperitoneal lymph node dissection. We analyzed the pathologic characteristics of LNM and its relationship with the survival rate of the patients. Results LNM was ob-served in 37 of the 218 patients (16.97%), including 20 cases of pelvic (54.05%), 6 cases of para-aortic (16.22%), and 11 ca-ses of both pelvic and para-aortic ( 29.73%) LNM.Independent risk factors of LNM included deep myometrial infiltration ( OR:5.21, 95%CI:2.77-9.81), cervical stromal involvement (OR:3.15, 95% CI:1.12-8.35), lymphovascular invasion (OR:1.15, 95%CI:1.02-1.30), and abnormally high serum CA125 (OR:3.46, 95%CI:1.56-7.67) (P<0.05).There were sig-nificant differences in the 3-year survival rate between the patients with LNM and those without LNM (83.8%vs 95.0%, P<0.05) as well as in the 3-year tumor-free survival rate (73.0% vs 90.1%, P<0.05). Conclusion Deep myometrial infiltration, cervical stromal involvement, lymphovascular invasion, and abnormally high serum CA125 are the risk factors of LNM in LGEC patients, and LNM affects the prognosis of the LGEC patients.
10.Analysis of the correlated factors of puncture-related pain in patients with the arteriovenous fistula undergoing maintenance hemodialysis

Objective To investigate the current situation of puncture-related pain in patients with the arteriovenous fistula undergoing maintenance hemodialysis and its correlated factors. Methods A total of 180 hemodialysis patients with the arteriovenous fistula undergoing maintenance hemodialysis were investigated with demographic data questionnaire, visual analogue scale and pain self-efficacy questionnaire. Results The effective questionnaires rate was 93.89%(169/180). The median score of puncture-related pain was 5 scores, the score of pain self-efficacy was (31.42±14.59) scores;multivariate analysis demonstrated that the puncture-related pain was positively correlated with the patient gender, using time of arteriovenous fistula and pain self-efficacy ( β=0.152,-0.017,-0.409, P<0.05), OR (95%CI)were 2.069 (1.206-3.148), 0.803(0.710-0.984), 1.004(0.886-1.431). There was negative correlation between the puncture-related pain score and pain self-efficacy in these patients, r=-0.647, P<0.01. Conclusions Puncture-related pain is common sense in patients with the arteriovenous fistula undergoing maintenance hemodialysis, some factors including the patient gender, using time of arteriovenous fistula and pain self-efficacy influence this feeling.
