1.Clinical observation of doxycycline in the treatment of macrolide-unresponsive Mycoplasma pneumoniae pneumonia in children under 8 years of age

OBJECTIVE To investigate the efficacy and safety of doxycycline in the treatment of macrolide-unresponsive Mycoplasma pneumoniae pneumonia (MUMPP) in children under 8 years of age. METHODS The medical records of children with MUMPP admitted to the Women and Children’s Hospital Affiliated to Ningbo University were collected from January 1st, 2023 to December 31st, 2023. They were divided into doxycycline group (44 cases), doxycycline combined with methylprednisolone group (35 cases), and azithromycin combined with methylprednisolone group (35 cases) according to the treatment methods. Doxycycline group was given Doxycycline hyclate enteric-coated capsules; doxycycline combined with methylprednisolone group was given Doxycycline hyclate enteric-coated capsules and Methylprednisolone sodium succinate for injection; azithromycin combined with methylprednisolone group was given Azithromycin for injection and Methylprednisolone sodium succinate for injection. Treatment courses of 3 groups lasted for 10 d. The fever reduction rate, the time of fever reduction and improvement rate of lung infection absorption were compared among the three groups. The occurrence of adverse drug reactions was recorded during their hospitalization and followed up within 5 months after discharge. RESULTS The fever reduction rats 48, 72 h after treatment and improvement rate of lung infection absorption in doxycycline group and doxycycline combined with methylprednisolone group were significantly higher than azithromycin combined with methylprednisolone group; the time of fever reduction was significantly shorter than azithromycin combined with methylprednisolone group (P<0.05); there was no statistical significance in the difference between the doxycycline group and the doxycycline combined with methylprednisolone group (P>0.05). There was no statistical significance in the incidence of rash, vomiting, abdominal pain, diarrhea, and elevated transaminases among the three groups during hospitalization and within 5 months after discharge (P>0.05). None of the children treated with doxycycline suffered from tooth discoloration or enamel hypoplasia. CONCLUSIONS Doxycycline has good efficacy and safety in therapy of MUMPP in children under 8 years of age; adjunctive coadministration of low-dose glucocorticoids does not necessarily result in significant additional efficacy.

3.Regulating the structure of bacterial cellulose by altering the expression of bcsD using CRISPR/dCas9.

Gluconacetobacter xylinus is a primary strain producing bacterial cellulose (BC). In G. xylinus, BcsD is a subunit of cellulose synthase and is participated in the assembly process of BC. A series of G. xylinus with different expression levels of the bcsD gene were obtained by using the CRISPR/dCas9 technique. Analysis of the structural characteristics of BC showed that the crystallinity and porosity of BC changed with the expression of bcsD. The porosity varied from 59.95%-84.05%, and the crystallinity varied from 74.26%-93.75%, while the yield of BC did not decrease significantly upon changing the expression levels of bcsD. The results showed that the porosity of bacterial cellulose significantly increased, while the crystallinity was positively correlated with the expression of bcsD, when the expression level of bcsD was below 55.34%. By altering the expression level of the bcsD gene, obtaining BC with different structures but stable yield through a one-step fermentation of G. xylinus was achieved.

5.The analysis and thoughts on traditional Chinese medicine international cooperation projects between China Academy of Chinese Medical Sciences and Europe

The paper analysed the scale,character,cooperation countries,funds and achievements of the traditional Chinese medicine (TCM) international projects between China Academy of Chinese Medical Sciences (CACMS) and Europe from 2007-2017.The results showed that there were 44 projects between CACMS and Europe;the number of Chinese medicine,clinical and basic research projects were 21,9 and 6 respectively;the number of projects between CACMS and Austria,and between CACMS and Gemany were 20 and 8 respectively;the funds from Ministry of Science and Technology and CACMS accounted for relatively high proportions.Trough these projects between CACMS and Europe,the TCM centers were established,the key technologies were introduced,and the professionals were trained.But there were still some questions such as the regional maldistribution of cooperation countries,few acupuncture research,and little project funds.
6.Analysis and study on current status of European students in traditional Chinese medicine colleges and universities

The paper analyzed the scale, educational background, degree, and financial source of the European students in Chinese TCM colleges and universities by using excel 2013. The results showed that there were 19 TCM colleges and universities recruiting European students, and the number of campus students, enrollment students, graduation students were 409, 257 and 280 in 2017 respectively. The campus students in 2017 was highest since five years ago, but was less than the Afro-Asia states. Training was main form of studying abroad. Most campus students were at their own expenses, or received the help from Chinese government. The education for European students in Chinese TCM colleges and universities has made some achievement, but there were still some questions: the European students was not too much, the education's attraction was not strong, and the financial help from international organization and foreign government was scarce.
7.Effects of Rapamycin and mammalian target of rapamycin - small interfering RNA (mTOR siRNA)on proli-feration,apoptosis and extracellular matrix in premature rats lung fibroblasts exposed to hyperoxia

Objective To investigate the effects of Rapamycin and mammalian target of rapamycin - small in-terfering RNA (mTOR siRNA)on the proliferation,apoptosis and collagen Ⅰ(COLⅠ),collagen Ⅲ(COLⅢ)and fi-bronectin(FN)in premature rats lung fibroblasts exposed to hyperoxia. Methods 900 mL/ L volume fraction of oxygen was used to establish hyperoxia - damaged cell models,and the premature rats lung fibroblasts were divided into air control group,hyperoxia group,hyperoxia + rapamycin group and mammalian target of rapamycin - small interfering RNA transfection group. Cell proliferation was assessed by using 3 -(4,5 - Dimethylthiazol - 2 - yl)- 2,5 - dipheny-ltetrazolium bromide assay. Apoptosis were detected by Annexin V - FITC and propidium lodide (PI)double staining. The expressions of COLⅠ,COLⅢ and fibronectin was assessed by using enzyme linked immunosorbent assay and Bcl - 2,P53 and pro - fibrotic factors of connective tissue growth factor(CTGF)and transforming growth factor β(TGF - β)by using Western blot. Results Compared with the air control group,the proliferation of lung fibroblasts decreased and the apoptosis increased in the hyperoxia group,while the contents of COLⅠ(28. 30 ± 0. 53 vs. 17. 43 ±0. 37),COLⅢ(27. 86 ± 1. 02 vs. 17. 43 ± 0. 37)and fibronectin(32. 87 ± 0. 42 vs. 21. 57 ± 0. 47),P53(0. 810 ± 0. 119 vs. 0. 160 ± 0. 018),TGF - β(0. 580 ± 0. 108 vs. 0. 210 ± 0. 008)and CTGF(0. 590 ± 0. 017 vs. 0. 220 ± 0. 007)were also increased but the expression of Bcl - 2(0. 150 ± 0. 004 vs. 0. 600 ± 0. 130)protein was decreased, and the differences were all statistically significant (all P < 0. 01). Conclusion Rapamycin and mTOR siRNA can protect lung injury caused by hyperoxia and have a certain inhibitory effect on pulmonary fibrosis,and mTOR siRNA effect is more obvious,so the mechanism may be through the inhibition of mTOR signaling pathway.
8.Expression of ANLN in HCC and predictive value for long-term survival of patients after liver transplantation

Objective To explore the expression of ANLN in HCC and study the clinical value of ANLN expression for HCC patients after liver transplantation.Methods The protein and mRNA expression of ANLN was detected by immunohistochemistry and RNA-seq from TCGA respectively.Chi-square test and t test were used for correlation analysis between ANLN expression and clinicopathological characteristics.The predictive value of ANLN expression for HCC patients after liver transplantation was estimated by log-rank test and cox proportional hazards regression model.Results The positive protein expression rate of ANLN in HCC detected by immunohistochemistry was 37.0% (34/92),significantly higher than 6.5 % (6/92) in para-carcinoma non-tumor tissue (P<0.001,x2 =25.044).Upregulation of ANLN mRNA expression in HCC was also detected by the analysis of RNA-seq from the TCGA (P < 0.000 1).The positive ANLN protein expression was positively correlated with AFP>400 ng/L (P<0.001,x2 =11.952) and tumor size >8 cm (P =0.034,x2 =4.506).The independent risk factors for poorer 5-year survival of patients after liver transplantation were confirmed,including positive ANLN protein expression (P =0.031,OR =1.965,95 %CI =1.064-3.630),tumor size >8 em (P =0.003,OR =2.841,95 %CI =1.437-5.617),worse differentiation degree (P =0.001,OR =3.613,95% CI =1.646-7.928),peritumor intravascular cancer emboli (P =0.041,OR =1.896,95%CI =1.028-3.498) and tumor necrosis or hemorrhage (P=0.010,OR=2.195,95 %CI=1.211-3.979).Conclusion The expression of ANLN in HCC is upregulated and the positive protein expression indicates the poor prognosis for long-term survival of patients after liver transplantation.
9.The primary application of S-Detect technique in the diagnosis of thyroid tumors

Objective To evaluate the value of S-Detect technology of ultrasonography in the diagnosis of thyroid tumors. Methods Ninety-three thyroid tumors in 93 patients were enrolled in the group.A varied image features of the thyroid masses in gray-scale ultrasonography were analyzed by S-Detect technology and experienced doctor separately.The results were compared and the diagnostic ability were also compared between the two methods. Results There were 44 malignant tumors and 49 benign tumors in these thyroid nodules.The sensitivity of S-Detect technology in the diagnosis of thyroid malignant tumors was higher,up to 88.7%. In the five image features of the thyroid tumors in gray-scale ultrasonography,the result of composition of the mass obtained by S-Detect was the most consistent with that of the doctors and Kappa value was 0.89.Conclusions S-Detect is a kind of computer-aided diagnosis system,which is suitable for the ultrasound beginners in the diagnosis of thyroid tumors.
10.Effects of infusion per unit of weight on emerging pulmonary complications in patients with cirrhosis after liver transplantation

Objective To explore the effects of infusion per unit of weight on pulmonary edema and acute respiratory distress syndrome (ARDS).Methods The clinical data of 70 patients with cirrhosis who had accepted liver transplantation were retrospectively collected,including the age,height,weight,BMI,preoperative liver function,indexes during liver transplantation and the postoperative intake and output records in the first 5 days,and the emerging postoperative pulmonary complications (EPPCs) in the first 2 weeks were screened.The relationship between clinical data and new pulmonary edema and ARDS was analyzed.Results The incidence rate of NPPCs was 82.9 %,in which the incidence rate of pleural effusion,atelectasis,pulmonary edema,ARDS and pulmonary infection was 60.0%,14.7%,8.6%,31.4% and 10% respectively.In the pulmonary edema group (n =6),the preoperative Child-Pugh score (9.3 ± 1.6),the total volume (2 667 ± 1 164) ml and the volume of unit weight (39.4 ± 19.0) ml/kg of plasma transfusion,the total volume (1 417 ± 376) ml of artificial colloid during operation,and the input of unit weight (53.2 ± 9.3 ml/kg) on the 3rd day after operation were significantly different from those (7.6 ± 1.9,1 753 ± 1 040 ml,24.2 ± 15.7 ml/ kg,2 347 ± 1 088 ml,and 44.6 ± 10.1 ml/kg) in the group (n =64) without pulmonary edema.Their P values in the order were 0.028,0.046,0.029,0.046,and 0.046.In the ARDS group (n =22),the plasma transfusion volume per unit of weight during operation (31.3 ± 20.4 ml/kg),the total balance volume equaled with the difference of input and output (1 504 ± 894 ml) and the balance volume per unit of weight (22.1 ± 13.1 ml/kg) on the first postoperative day and the total input volume per unit of weight on the third postoperative day (49.0 ± 10.1 ml/kg) were significantly distinguished with those (22.7 ± 13.4 ml/kg,910 ± 684 ml,12.7 ± 9.9 ml/kg,and 43.6 ± 9.9 ml/kg) in the group (n =48) without ARDS.The P values in the order were 0.045,0.003,0.001 and 0.042 respectively.Conclusion The incidence rate of NPPCs in the patients with cirrhosis receiving the liver transplantation is relatively higher.In order to reduce the risk of NPPCs,based on the hemodynamic stability during operation,the artificial colloids should be appropriately increased and excessive plasma transfusion reduced.In addition,the redundant input should be limited according to the weigh,in the first and third postoperative days.
