1.FANG Dingya's Experience in Staged Treatment of Sjögren's Syndrome based on Dryness Toxin Theory

This paper summarized the experience of Professor FANG Dingya in staged treatment of Sjögren's syndrome from the perspective of dryness toxin. It is believed that the cause of Sjögren's syndrome is externally-contracted dryness, consumption of essence and fluid, congenital and acquired essence deficiency, depleted essence and insufficient blood, and the core mechanism is internal accumulation of dryness toxin. The treatment can be divided into three stages, that is dryness toxin transforming into fire-heat, damp-heat and phlegm-stasis, from the perspective of dryness metal qi transformation. It is emphasized to dispel pathogen mainly, to clear and moisten with yin-nourishing medicinals in supplementation, and to treat by stages based on syndrome differentiation. For dryness toxin with fire-heat, it is suggested to moisten dryness, resolve toxins and subdue fire, with self-made Runzao Jiedu Decoction (润燥解毒汤) in modification. For dryness toxin with damp-heat, the method of nourishing yin, clearing heat and draining dampness should be used, and Chunze Decoction (春泽汤) in modification is suggested. For dryness toxin with phlegm-stasis, it is recommended to unblock collaterals, disperse phlegm and dissipate stasis, with self-made Sanyu Xiaotan Decoction (散瘀消痰汤) in modification.
2.Study on Quality Evaluation of Didang Qigui Decoction by HPLC Fingerprint Combined with Multi-component Content Determination

Objective To establish an HPLC fingerprint of Dingdang Qigui Decoction and analyze and evaluate it using chemical pattern recognition technology;To determine the contents of 5 effective chemical components in Dingdang Qigui Decoction;To provide a basis for its quality control.Methods The analysis was performed on Agilent 5 TC-C18(2)column(250 mm×4.6 mm).The mobile phase comprised of acetonitrile-0.1%phosphoric acid aqueous solution with the gradient elution at a flow rate of 1.0 mL/min.The detection wavelength was set at 260 nm.The column temperature was maintained at 30℃and the injection volume was 10 μL.SPSS 26.0 and SIMCA 14.1 were used to perform clustering analysis and principal component analysis on the 10 batches of Didang Qigui Decoction.The landmark components for inter batch differences were selected through orthogonal partial least squares discriminant analysis(OPLS-DA).Results The HPLC fingerprint with eighteen common peaks of Didang Qigui Decoction in 10 batches of sample was established,and the similarities of samples were between 0.828 and 0.989.Five indicative components were identified and quantitatively analyzed by comparing with the reference substances,which were paeoniflorin,mauroisoflavone glucoside,hesperidin,cinnamaldehyde and aloe rhodopsin.The linear ranges was 10.000 0-320.000 0 μg/mL,2.500 0-80.000 0 μg/mL,10.000 0-320.000 0 μg/mL,10.000 0-320.000 0 μg/mL,0.078 1-5.000 0 μg/mL,respectively,and their mean recovery ranged from 100.30%to 104.09%.Clustering analysis and principal component analysis divided 10 batches of samples from Didang Qigui Decoction into 2 categories.Through OPLS-DA screening,hairy pistil isoflavone glycosides,paeoniflorin,and hesperidin were selected as landmark components for quality differences.Conclusion The quality evaluation method for Didang Qigui Decoction established in this study is simple,sensitive,accurate,and reproducible,which can provide a basis for the quality evaluation of Didang Qigui Decoction.
3.Research progress of anti-tumor effect and mechanism of CDDO and its derivatives

2-cyano-3,12-dioxooleana-1,9 (11)-dien-28-oic acid (CDDO) is a compound synthesized by taking oleanolic acid, a natural triterpene, as a precursor or precursor, and transforming three modifiable functional groups in the molecule through a series of chemical structure modification. In order to improve its anti-tumor activity, CDDO derivatives are further synthesized. In this paper, the research results of anti-tumor effects and mechanisms of CDDO and its derivatives in recent years are summarized. It is found that CDDO and its derivatives have a wide range of anti-tumor effects, and can show significant anti-tumor effects on breast cancer, pancreatic cancer, lung cancer and ovarian cancer at low concentrations such as micromole or even nanomole, among which CDDO methyl ester compound (CDDO-Me) and CDDO imidazolidinone compound (CDDO-Im) have the most obvious effects. CDDO and its derivatives exert anti-tumor activity mainly by inducing tumor cell apoptosis, and regulating metabolic reprogramming and immune microenvironment. The involved pathways mainly include Janus protein tyrosine kinase (JAK)/ signal transduction and transcription activation protein 3(STAT3) signal pathway, nuclear factor E2-related factor 2 (NRF2) signal pathway, phosphatidylinositol 3 kinase (PI3K)/protein kinase B (also known as Akt)/mammalian rapamycin target protein (mTOR) signal pathway, Wnt/β-catenin signal pathway, nuclear factor κB signal pathway.
4.Relationship between thyroid hormone level and obesity related indices in patients with type 2 diabetes mellitus

Objective:To investigate the relationship between serum thyroid hormone levels in the normal range and body weight, blood glucose, blood lipids, and other obesity-related indexes in patients with type 2 diabetes mellitus.Methods:Seventy obese patients with type 2 diabetes mellitus and ninety-two patients with type 2 diabetes mellitus with normal weight who were treated in the Nangang Branch of Heilongjiang Provincial Hospital from May 2020 to May 2021 were included in this study. Thyroid-stimulating hormone level was in the normal range (0.35-4.94 mU/L) in all participants. Serum levels of free triiodothyronine, free thyroxine, thyroid-stimulating hormone, thyroid peroxidase antibody, thyroglobulin antibody, triglyceride, total cholesterol, low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, fasting blood glucose, glycosylated hemoglobin, fasting C peptide, fasting insulin, systolic blood pressure, diastolic blood pressure, and serum uric acid were measured in all participants.Results:Free triiodothyronine level was positively correlated with fasting blood glucose and glycosylated hemoglobin levels ( r = 0.19, P = 0.021; r = 0.21, P = 0.017). Free thyroxine level was positively correlated with serum glycosylated hemoglobin level ( r = 0.25, P = 0.009) and negatively correlated with total cholesterol ( r = -0.17, P = 0.029). Thyroid-stimulating hormone level was positively correlated with body mass index as well as total cholesterol and low-density lipoprotein cholesterol levels ( r = 0.33, P < 0.001; r = 0.33, P < 0.001; r = 0.32, P < 0.001). Conclusion:Thyroid hormones in the normal range play an important role in the regulation of body weight, blood glucose, and blood lipids in patients with type 2 diabetes mellitus. Blood glucose level increases markedly in patients with relatively high free triiodothyronine and free thyroxine levels. The risks of obesity and dyslipidemia increase in patients with relatively high serum thyroid-stimulating hormone levels
5.Interpretation for Evidence-based Practice Guideline of Medication Therapy of High-dose Methotrexate in China

Evidence-based Practice Guideline of Medication Therapy of High-dose Methotrexate in China was published in the British Journal of Clinical Pharmacology in February 2022. The guideline followed the latest definition of clinical practice guideline and the methodology specification for the guideline development of WHO. The Grading of Recommendations Assessment , Development,and Evaluation (GRADE)approach was applied to rate the quality of evidence and determine the strength of recommendations. Finally ,this guideline presents 28 recommendations covering the whole process of clinical medication of high-dose methotrexate ,involving evaluation prior to administration (liver and renal function ,pleural effusion and ascites , comedication,genetic testing ),pre-treatment and routine dosing regimen (pretreatment of hydration and alkalization ,urine alkalization,routine dosing regimen ),therapeutic drug monitoring (necessity,method,timing,target concentration ),leucovorin rescue(rescue timing ,rescue regimen ,rescue dose optimization ),and management of toxicities (liver and kidney function monitoring,supportive treatment ,blood purification treatment ). This article aims to summarize and interpret the recommendations of this guideline ,so as to promote the better promotion and implementation of this guideline and provide comprehensive technical support and suggestions for whole-course individualized administration of high-dose methotrexate in China.
6.Application of virtual experiment in clinical microbiology inspection technology

Clinical microbiology examination technology experiment is an important part of clinical microbiology examination technology teaching. In the experimental teaching of clinical microbiology examination technology, the virtual simulation technology was combined with traditional teaching to give full play to the advantages of the virtual experimental platform. As to experimental projects that couldn't be carried out in traditional teaching and some important experimental projects, students could learn on the virtual experimental platform, and after learning, they would participate in the corresponding assessment. The perfect combination of the two can solve the problem of high experimental cost and limited experimental content in the current experimental class, make up for the shortcomings of traditional teaching, realize the sharing of teaching resources. Besides, it can strengthen the students' experimental operation skills and enhance the interest of learning for cultivation of application-oriented medical talents.
7. Artesunate attenuate chronic graft-versus-host disease by regulating Th17/Treg balance

Objective:
To investigate the effects of artesunate treatment on chronic graft-versus-host disease (cGVHD).
Methods:
Recipient BALB/c mice received 8 × 106 bone marrow cells with 8×106 spleen cells from B10D2 mice. Artesunate solubilized in acetone was injected intraperitoneally every day at the dose of 1 mg/kg at Day 28 after BMT. The clinical scores, survival and histopathological damage were analyzed. The frequency of Th17 and Tregs in PB and spleens from the mice were evaluated by flow cytometry. In addition, CD4+ T cells from the spleens of mice were cultured in vitro, then stimulated with artesunate, the frequency of Th17 and Tregs in these splenocytes were evaluated by flow cytometry.
Results:
Artesunate administration diminished clinical and histopathological damage, and improved the survival of cGVHD mice[(46.57±7.83)%

Medication safety is a top concern for medical institutions. Outpatient prescription standard is designed to standardize prescription, dispensing, and supervision for outpatient and emergency prescriptions at medical institutions. The standard covered prescription authorization management, prescription issuance, prescription dispensing, prescription saving and supervision. These four parts focus on risk exposure of patients′medication safety, and aim at safeguarding patient medication safety, which were formulated according to China′s laws and regulations, domestic and international industrial standards and technical specifications, as well as prescription conditions at medical institutions and experts opinions. The standard covers technical requirements and guidance, management measures and system development, serving as an important basis to guide medical institutions on standardize management of outpatient prescription and emergency prescription.
9.Effect of metformin combined with acupoint therapy with low frequency impulse stimulation on repairing neuropathy of patients with diabetic foot

Objective To investigate the effect of metformin combined with acupoint therapy with low frequency impulse stimulation on repairing diabetic neuropathy of patients with diabetic foot.Methods Totally 150 diabetic foot patients with neuropathy were randomly divided into observation group (n =75) and control group (n =75).The therapeutic effect,nerve conduction velocity,fasting blood glucose,postprandial 2 h blood glucose,glycosylated hemoglobin and adverse reactions were compared between the two groups.Results The total effective rate of the observation group was 92%,which was significantly higher than 76% of the control group (P <0.05).The SCV and MCV of the tibial nerve and peroneal nerve increased significantly after treatment in all the patients (P <0.05),and the SCV and MCV of the tibial nerve and peroneal nerve in the observation group were significantly higher than those in the control group (P < 0.05).After treatment,the levels of HbA1 C,FBG and 2 hPBG decreased significantly when compared with those before treatment (P < 0.05),and the levels of HbA1 C,FBG and 2 hPBG in the observation group were significantly lower than those in the control group (P < 0.05).The total incidence rate of complications in the observation group was 8%,which was significantly lower than 20% in the control group (P < 0.05).Conclusion Metformin combined with acupoint therapy with low frequency impulse stimulation can effectively improve the symptoms of neuropathy in patients with diabetic foot.
