1.Clinical Immunological Characteristic and Influencing Factors Analysis of HIV/AIDS Patients Infected with EBV and HCMV

		                        			Objective To investigate the basic information of human immunodeficiency virus/acquired immune deficiency syndrome(HIV/AIDS)patients who infected with Epstein-Barr virus(EBV)or human Cytomegalovirus(HCMV),collect the relevant clinical immunological data and analyze the influencing factors.Method A total of 1 093 HIV/AIDS patients treated in the First Hospital of Changsha from January to December 2022 and underwent EBV and HCMV screening were collected.Flow cytometry was used to detect the CD4+T lymphocytes.Fluorescence quantitative PCR was applied for HIV-RNA,EBV-DNA,and HCMV-DNA testing.Statistical analysis was carried out by using SPSS 27.0,and logistic regression was used to analyze the risk factors of HIV/AIDS patients complicated with viral infection.Results Among 1 093 HIV/AIDS patients,the positive rates of EBV-DNA and HCMV-DNA were 48.22%(527/1 093)and 19.03%(208/1 093),respectively.As the number of CD4+T lymphocytes increased,the positive rates of EBV-DNA and HCMV-DNA decreased,and the differences was statistically significant(χ2=39.50,143.0,all P<0.001).As the level of HIV-RNA increased,the positive rates of EBV-DNA and HCMV-DNA increased,and the differences were statistically significant(χ2=46.18,124.3,all P<0.001).The patients receiving antiretroviral therapy(ART)significantly decreased the positive rates of EBV-DNA and HCMV-DNA(χ2=30.60,96.59,all P<0.001).There was a significant negative correlation between the number of CD4+T lymphocytes and the level of HIV-RNA(r=-0.49,P<0.001).Logistic regression analysis showed that the CD4+T lymphocyte count<200/μl(OR=1.46,95%CI:1.02~2.08,P=0.037),HIV-RNA load>200 copies/ml(OR=1.70,95%CI:1.18~2.44,P=0.004)and the age>30 years old(OR=2.15,95%CI:1.44~3.19,P<0.001)were risk factors for HIV/AIDS patients infected with EBV.Without regularly receiving ART(OR=1.83,95%CI:1.10~3.02,P=0.019),HIV-RNA load>200 copies/ml(OR=2.56,95%CI:1.50~4.35,P<0.001)and the CD4+T lymphocyte count<200/μl(OR=4.61,95%CI:2.57~8.28,P<0.001)were risk factors for HCMV infection in HIV/AIDS patients.Conclusion To reduce the possibility of opportunistic infection in HIV/AIDS patients,the surveillance of EBV and HCMV and regular ART should be strengthened,especially when the number of CD4+T lymphocytes decreases(<200/μl),the level of HIV RNA increases(>200 copies/ml)or the age>30 years old.
		                        		
		                        		
		                        		
		                        	
2.Preliminary study on high throughput screening small molecules targeting Gram-negative bacilli outer membrane protein BamA

		                        			Objective:High-throughput screening to obtain small molecular compounds against Gram-negative bacilli by targeting BamA outer membrane protein.Methods:The sybyl-X2.1 software was used to perform high-throughput virtual screening of small molecular compounds in Chemdiv compound library based on the molecular docking. The top 150 hits by high-throughput screening were re-screened through in vitro biological experiments. The top 4 small molecules with obvious antibacterial activity were selected for in-depth molecular docking analysis, and the small molecule 8308-0401 with the highest docking score was selected for further experiments. The antibacterial effect of 8308-0401 combined with rifampicin was tested by checkerboard assay. Finally, the affinity between 8308-0401 and BamA was tested by plasma surface resonance assay. Results:The docking score of the top 150 hits calculated by high-throughput virtual screening had a mean value of 5.63. In vitro biological experiments showed that small molecules 8308-0401, 8365-1335, C066-2507 and L582-0346 exhibited strong antibacterial activity. Among those molecules, 8308-0401 showed the highest molecular docking score, and synergistic antibacterial activity against both types of strains and clinical isolates when combined with rifampicin. 8308-0401 has a strong affinity to BamA with binding a constant of 182 μmol/L. Conclusion:The small molecule 8308-0401 exerts antibacterial activity against Gram negative bacilli by targeting the outer membrane protein BamA.
		                        		
		                        		
		                        		
		                        	
3.Determination of 9 halogenated alkanes in toothpaste by GC⁃MS

		                        			ObjectiveTo establish a GC-MS method for the detection of 9 halogenated alkanes in toothpastes. MethodsVarious brands of toothpastes on the market were collected as the samples. Methanol was used as the extraction solution. After shaking, vortex and centrifugation, the supernatant was filtered by 0.45 μm microporous filter membrane. The content of 9 halogenated alkanes including chloromethane, dichloromethane, chloroform, 1,1,1-trichloroethane, carbon tetrachloride, 1,2-dichloroethane, 1,1,2-trichloroethane, 1,2-dibromoethane, 1,2,3-trichloropropane in the filtrate were determined by GC-MS. ResultsThe 9 halogenated alkanes showed a good linear relationship in the concentration range of 0.03‒1.00 μg·mL-1. The correlation coefficient was ≥0.998 7 with the detection limits of 0.015‒0.056 mg·kg-1. The spiked recoveries were 83.2%‒108.9% with the RSD of 2.0%‒9.8%. In the actual sample analysis, 1,1,2-trichloroethane and 1,2,3-trichloropropane were respectively detected in 6 toothpaste samples from 5 brands. ConclusionThe method is simple, sensitive, accurate, and has good spiked recoveries. 		                        			 Irritable bowel syndrome (IBS) is a functional gastrointestinal disorder that is characterized by abdominal pain and disordered bowel habits. The etiology of IBS is multifactorial, including abnormal gut-brain interactions, visceral hypersensitivity, altered colon motility, and psychological factors. Recent studies have shown that the intestinal microbiota and its metabolites short chain fatty acids (SCFAs) may be involved in the pathogenesis of IBS. SCFAs play an important role in the pathophysiology of IBS. We discuss the underlying mechanisms of action of SCFAs in intestinal inflammation and immunity, intestinal barrier integrity, motility, and the microbiota-gutbrain axis. 6.Effect of exposure to typical phthalate esters on estrogen homeostasis during pregnancy

		                        			ObjectiveTo study the relationship between the exposure to two kinds of phthalate esters (PAEs) [Di-N-butyl phthalate,(DBP) and Di-(2-ethylhexyl)phthalate (DEHP)] and estrogen homeostasis in pregnant women. MethodsIn 2021, we classified the Jiading District of Shanghai into five geographical areas, east, west, south, north and central. A total of 151 pregnant women from each area were selected for questionnaire survey, with random urine samples during first, second, and third trimesters collected. A DBP metabolite [Mono-N-butyl phthalate (MBP)] and two DEHP metabolites [Mono(2-ethylhexyl) phthalate (MEHP), Mono(2-ethyl5-oxohexyl) phthalate, (MEOHP)] and three estrogens [estrone (E1), 17β -estradiol (E2), and estriol (E3)] in urine were determined by ultra-performance liquid chromatography tandem quadrupole time-of-flight mass spectrometry. After a natural logarithmic transformation of PAEs metabolite levels and estrogen concentration, multivariable linear regression was used to control potential confounders and determine the relationship between PAEs metabolite levels and estrogen concentration. ResultsThe detection rates of three PAEs metabolites in urine of pregnant women were more than 98%. The median corrected concentrations of MBP, MEHP and MEOHP were 5.18, 0.59 and 4.23 mg·kg-1, respectively. During the whole pregnancy, MEOHP was positively correlated with E1 (β=0.450, 95%CI: 0.057‒0.844), and MBP was positively correlated with E3 (β=0.250, 95%CI: 0.034‒0.465). Stratified by trimesters, MBP was positively correlated with E3 in the first trimester (β=0.428, 95%CI: 0.103‒0.752). MEOHP was positively correlated with E1 in the second trimester (β=0.734, 95%CI: 0.130‒0.752), and had a possitive trend with E1 in the third trimester (β=0.744, 95%CI: -0.140‒1.629). In addition, MEHP had a negative correlation with E1 in the second trimester (β=-0.498, 95%CI: -1.063‒0.066). MEOHP had a positive correlation trend with E2 (β=0.628, 95%CI: -0.101‒1.356) in the third trimester. ConclusionPAEs exposure may interfere with estrogen homeostasis during pregnancy and differs by trimesters. 7.Evaluation of characteristics of carotid plaques and immediate outcomes after carotid artery stenting in diabetic and non-diabetic patients by optical coherence tomography

		                        			Objective:To evaluate the characteristics of carotid plaque and the immediate outcomes after carotid artery stenting (CAS) in diabetic and non-diabetic patients by optical coherence tomography (OCT).Methods:Patients underwent CAS and OCT before and after operation in the Department of Neurology, Jinling Hospital from January 2014 to March 2019 were enrolled retrospectively. The clinical features, the characteristics of carotid plaque on OCT and the immediate outcomes after CAS were compared between diabetic group and non-diabetic group. The risk factors of stent malapposition were analyzed.Results:A total of 46 patients were enrolled. Their age was 64.02±8.32 years and 41 were males (89.1%). There were 20 patients (43.5%) in the diabetes group and 26 (56.5%) in the non-diabetes group. The proportions of atherosclerotic plaque with thin fibrous cap (40.0% vs. 7.7%; χ2=5.166, P=0.023), plaque rupture (55.0% vs. 23.1%; χ2=4.945, P=0.026) and macrophage infiltration (60.0% vs. 30.8%; χ2=3.930, P=0.047) in the diabetic group were significantly higher than those in the non-diabetic group. Multivariate logistic regression analysis showed that older age (odds ratio [ OR] 1.208, 95% confidence interval [ CI] 1.033-1.413; P=0.018), coronary heart disease ( OR 15.953, 95% CI 1.142-222.952; P=0.040), alcohol consumption ( OR 6.192, 95% CI 1.098-34.923; P=0.039) and lower systolic blood pressure ( OR 0.944, 95% CI 0.894-0.997; P=0.037) were independently associated with stent malaposition. Conclusion:Compared with the non-diabetic patients, carotid plaque in diabetic patients may be more unstable. Older age, coronary heart disease, alcohol consumption and lower systolic blood pressure were associated with stent malaposition after carotid stenting. OCT can reveal the characteristics of carotid plaque and the immediate outcomes after CAS, which can provide strong evidence for treatment decision.

		                        			Objective To investigate influence on the quality of life of psychological intervention combined with hydrochlorothiazide on the treatment of the patients undergoing heart valve replacement. Methods The control group after heart valve replacement surgery were accepted the routine drug combined with hydrochlorothiazide, at this basis, the study group were received targeted nursing interventions. The changes of quality of life (SF-36) before and after cardiac valve replacement in the two groups were recorded. The effect in the two groups were compared. Results There was no statistically significant in SF-36 in the two groups before heart valve replacement; After treatment, the SF-36 scale scores in the 2 groups were significantly higher than that before treatment, and the SF-36 scale score in the study group was significantly higher than that in the control group (P<0.05). 9.Effect and mechanism of ulinastatin on the efficacy of ventilator-associated pneumonia

		                        			Objective To investigate the effect of ulinastatin in adjuvant treatment of ventilator-associated pneumonia(VAP)and preliminary clinical efficacy of the therapy mechanism.Methods 76 patients with ventilator-associated pneumonia from January 2015 to February 2016 in Qingyuan People's Hospital of Zhejiang Province were selected and randomly divided into control group and observation group,38 cases in each group.Two groups were given mechanical ventilation,phlegm,anti infection,rehydration,nutritional support and other conventional treatment,the observation group on the basis of routine treatment for ulinastatin adjuvant therapy,comparison of two groups of treatment,the simultaneous determination of serum interleukin-6(IL-6),C-reactive protein(CRP)and procalcitonin(PCT)and tumor necrosis factor alpha(TNF-alpha)levels were measured before and after treatment.Results The total efficiency of the observation group was 94.74%,significantly higher than the control group 78.95%,the difference was statistically significant(P<0.05),the two groups after treatment of serum CRP and PCT levels were decreased significantly compared with before treatment,the observation group after treatment,serum CRP and PCT levels were(45.19+5.79)mg/L and(1.08+0.36)μg/L was significantly lower than that before treatment and control group after treatment,the difference was statistically significant(P<0.05),the two groups after treatment of serum IL-6 and TNF-α levels were decreased significantly compared with before treatment,the observation group after treatment,serum IL-6 and TNF-α levels were(165.29+19.23)pm/mL and(1.16+0.25)pm/mL,was significantly lower than that before treatment and control group after treatment,the difference was statistically significant(P<0.05).Conclusion The inflammatory reaction in patients with ventilator-associated pneumonia ulinastatin can effectively reduce auxiliary patients,reduce inflammatory factors on lung injury,can alleviate the disease progression and to improve its prognosis.
		                        		
		                        		
		                        		
		                        	
10.Effect of RU486 on pre-adipocytes differentiation and NF-κB activation

		                        			Objective To investigate the roles of RU486 inhibiting 3T3-L1 pre-adipocytes differentiation and regulating NF-κB activation. Methods Cells were treated with RU486 with concentrations of 0.1 ~ 10μmol/L for 48 h , then the relative contents of triglyceride were analyzed by Oil-Red O staining assay on 9 th day during adipogenesis. The mRNA expressions of PPARγ2,C/EBPa, LPL and FAS were further measured by Real-time PCR. IκBα protein level was detected by Western bolt and nuclear translocation of NF-κB was observed by immunofluorescence assay. Results The relative contents of triglyceride decreased with the increasing of RU486 concentration. Compared with the control, the relative contents of triglyceride in RU486-treatment groups from 0.5 μmol/L were significantly decreased (P < 0.05 or P < 0.01). Compared with the control, PPARγ2, C/EBPa, LPL and FAS mRNA expression and IκBα protein level were significantly decreased (P < 0.01) and NF-κB nuclear translocated from cytoplasm to nucleus in Group 5 mmol/L RU486. Conclusions RU486 could down regulate IκBα protein level , activate NF-κB nuclear translocation , then down regulate PPARγ2 , C/EBPa , LPL and FAS mRNA expression and inhibit adipocytes differentiation.
