1.Effect of Qinghua Yichang Formula (清化益肠方) on NLRP3 Inflammasome in Intestinal Tissue of Mice with Acute Radiation-Induced Intestinal Injury

ObjectiveTo explore the effect and possible molecular mechanism of Qinghua Yichang Formula (清化益肠方, QYF) in treating acute radiation-induced intestinal injury mice via NOD-like receptor thermal protein domain associated protein 3 (NLRP3). MethodsSixty C57BL/6J mice were randomly divided into control group, model group, pre-modeling medication group, post-modeling medication group, inhibitor group, and QYF plus inhibitor group, with 10 mice in each group.Except for the control group, the other five groups were irradiated with a single full dose to establish the acute radiation-induced intestinal injury mice model. The pre-modeling medication group and the QYF plus inhibitor group were continuously given 4 g/ml of QYF decoction by gavage before modeling, 0.2 ml each time, once a day for 7 days. The post-modeling medication group, pre-modeling medication group and QYF plus inhibitor group were given 4 g/ml of QYF decoction for 14 days after modeling. The control group, model group and inhibitor group were given 0.2 ml of normal saline once a day for 14 consecutive days. Two hours after irradiation, the inhibitor group and the QYF plus inhibitor group were given an intraperitoneal injection of 0.2 ml of the NLRP3 inhibitor MCC950 (concentration: 10 mg/kg), once every two days. To observe the pathological changes in intestinal tissues, hematoxylin-eosin (HE) staining was used. Western blotting and RT-qPCR were used to detect the protein and mRNA expression levels of NLRP3 in intestinal tissues. Immunohistochemistry was used to detect the expression level of NLRP3, Caspase-1 and GSDMD in intestinal tissues. The proportion of CD4+ and CD8+ T cells in the spleens of mice were detected by flow cytometry. ELISA was used to determine the levels of IFN-γ, IL-18, and IL-1β in mice serum. ResultsHE staining showed no lesions in the intestinal tissue of mice in the control group. The mice in the model group had shortened intestinal villi, thinned mucosal layers, multifocal mucosal necrosis in the lamina propria, and local neutrophil infiltration. The pathological damage of intestinal tissue of mice in each medication group was improved to varied degrees, among which the QYF plus inhibitor group showed most obvious improvement. Compared to those in the control group, the protein and mRNA expression levels of NLRP3 in the intestinal tissue of mice in the model group significantly increased, with higher NLRP3, Caspase-1, and GSDMD protein expression in the intestinal tissue, increased proportion of CD4+ T cells in spleen, decreased proportion of CD8+ T cells, and increased levels of IFN-γ, IL-18 and IL-1β in serum (P<0.05). Compared to those in the model group, the above indicators in the other medication groups were all improved (P<0.05).The NLRP3, Caspase-1, and GSDMD proteins in the pre-modeling medication group were lower than those in the post-modeling medication group (P<0.05); and the NLRP3 mRNA level in the QYF plus inhibitor group was lower than that in the inhibitor group (P<0.05). ConclusionQYF may play a role in preventing and treating acute radiation-induced intestinal injury by inhibiting the expression of NLRP3.
2.Implementation of teaching clinic in the standardized training of general practice residents: current situation and implications

Teaching clinics represent a unique form of outpatient training of resident physicians and serve as a crucial instrument and core component of standardized training of general practice residents. This article reviews the common model and innovations of teaching clinics of general practice in China, and analyzes their reported effectiveness in enhancing the capabilities of consultation of resident physicians, the teaching capabilities of general practice trainers, as well as satisfaction levels of involved participants. It outlines the challenges encountered in implementing teaching clinics, including inadequate teaching facilities and equipment, incomplete incentive system for teaching, difficulties in patient recruitment, and weaknesses in the teaching capabilities of trainers. To address these challenges, this article proposes corresponding strategies based on realistic needs, including the improvement of facilities and equipment in teaching clinics, the establishment of incentive systems for teaching clinics, the expansion of patient recruitment channels for teaching clinics, and the enhancement of training for trainers' teaching capabilities. This is envisaged to provide both theoretical bases and practical guidance for the effective execution and standardized development of teaching clinics in general practice residency training bases.

In response to the Opinions of the CPC Central Committee and the State Council on Promoting the Inheritance, Innovation, and Development of traditional Chinese medicine(TCM) and the spirit of the National Conference on TCM, Chinese Association of Chinese Medicine organized experts in Otorhinolaryngology Head and Neck Surgery of traditional Chinese and western medicine to discuss the clinical advantages of TCM and integrated traditional Chinese and western medicine in the treatment of allergic rhinitis (AR) and they reached a basic consensus. In recent years, the prevalence of AR has been on the rise, threatening the quality of life of patients and giving rise to a heavy burden to both the patients and the society. AR is resulted from immune imbalance rather than reduced immunity or hyperimmunity, and the imbalance is similar to the Yin-yang disharmony in TCM. In the treatment of this disease, western medicine features rapid onset. However, it is cost-intensive and causes severe surgical trauma, and the recurrence is common. TCM boasts diverse methods for AR, which can be used in all stages of this disease. It has advantages in controlling symptoms such as nasal congestion, runny nose, or dysosmia in the attack stage, preventing recurrence in the remission stage, and treating refractory AR or steroid-resistant AR. In particular, acupuncture enjoys a reputation in treatment of AR, which has been supported by evidence-based medicine and recommended by guidelines. While treating local symptoms of AR, TCM regulates the psychosomatic conditions, which facilitates chronic disease management and long-term follow-up. We should integrate the advantages of TCM and western medicine, give full play to the unique nonnegligible and irreplaceable advantages of TCM, formulate a comprehensive diagnosis and treatment scheme for learning and promotion, and summarize the research outcomes to promote the theoretical innovation of TCM on AR from the perspective of integrated traditional Chinese and western medicine.

H 1-antihistamines are widely used in the treatment of various allergic diseases, but there are still many challenges in the safe and rational use of H 1-antihistamines in pediatrics, and there is a lack of guidance on the prescription review of H 1-antihistamines for children.In this paper, suggestions are put forward from the indications, dosage, route of administration, pathophysiological characteristics of children with individual difference and drug interactions, so as to provide reference for clinicians and pharmacists.

Anti-seizure medications (ASMs) are the main therapy for epilepsy.There are many kinds of ASMs with complex mechanism of action, so it is difficult for pharmacists to examine prescriptions.This paper put forward some suggestions on the indications, dosage forms/routes of administration, appropriateness of usage and dosage, combined medication and drug interaction, long-term prescription review, individual differences in pathophysiology of children, and drug selection when complicated with common epilepsy, for the reference of doctors and pharmacists.

Ginsenoside Rb1, the effective constituent of ginseng, has been demonstrated to play favorable roles in improving the immunity system. However, there is little study on the osteogenesis and angiogenesis effect of Ginsenoside Rb1. Moreover, how to establish a delivery system of Ginsenoside Rb1 and its repairment ability in bone defect remains elusive. In this study, the role of Ginsenoside Rb1 in cell viability, proliferation, apoptosis, osteogenic genes expression, ALP activity of rat BMSCs were evaluated firstly. Then, micro-nano HAp granules combined with silk were prepared to establish a delivery system of Ginsenoside Rb1, and the osteogenic and angiogenic effect of Ginsenoside Rb1 loaded on micro-nano HAp/silk in rat calvarial defect models were assessed by sequential fluorescence labeling, and histology analysis, respectively. It revealed that Ginsenoside Rb1 could maintain cell viability, significantly increased ALP activity, osteogenic and angiogenic genes expression. Meanwhile, micro-nano HAp granules combined with silk were fabricated smoothly and were a delivery carrier for Ginsenoside Rb1. Significantly, Ginsenoside Rb1 loaded on micro-nano HAp/silk could facilitate osteogenesis and angiogenesis. All the outcomes hint that Ginsenoside Rb1 could reinforce the osteogenesis differentiation and angiogenesis factor's expression of BMSCs. Moreover, micro-nano HAp combined with silk could act as a carrier for Ginsenoside Rb1 to repair bone defect.

7.Effect of transcutaneous electrical acupoint stimulation on function of lung ventilation in patients undergoing laparoscopic cholecystectomy: evaluation using electrical impedance tomography

Objective:To evaluate the effect of transcutaneous electrical acupoint stimulation (TEAS) on function of lung ventilation in the patients undergoing laparoscopic cholecystectomy using electrical impedance tomography (EIT).Methods:Sixty American Society of Anesthesiologists physical status Ⅰ or Ⅱ patients of both sexes, aged 18-64 yr, with body mass index of 18.5-24.9 kg/m 2, undergoing elective laparoscopic cholecystectomy, were divided into 2 groups ( n=30 each) using a random number table method: control group (CON group) and TEAS group.EIT was carried out after entering the operating room.In group TEAS, bilateral Feishu (BL13) and Chize (LU5) acupoints were stimulated, while stimulation electrodes were only placed without electricity in group CON.The percentages of area in center of ventilation and tidal volume in the dependent areas were determined on admission to the operating room (T 1), at 5 min after tracheal intubation (T 2), at 5 min after CO 2 pneumoperitoneum (T 3), at 5 min after CO 2 was exhausted (T 4), and at 5 and 30 min after removal of the tracheal tube (T 5, 6). Radial artery blood samples were collected for blood gas analysis at T 1, T 3 and T 6, and the oxygenation index (OI) and alveolar-arterial oxygen difference (A-aDO 2) were calculated.Blood samples were taken from the peripheral vein at T 1, T 3 and T 6 for determination of the serum concentrations of Clara cell secretion protein (CC16), interleukin (IL)-6 and tumor necrosis factor (TNF)-α. Results:Compared with the baseline at T 1, the percentages of area in center of ventilation and tidal volume in dependent areas at T 2-6 and OI and A-aDO 2 at T 6 were significantly decreased, and the serum concentrations of TNF-α, IL-6 and CC16 were increased at T 3 and T 6 in two groups ( P<0.05). Compared with group CON, the percentage of tidal volume in dependent areas at T 5, 6 and OI at T 6 were significantly increased, and A-aDO 2 and serum concentrations of TNF-α, IL-6 and CC16 were decreased at T 6 in group TEAS ( P<0.05). Conclusions:TEAS can improve the function of lung ventilation in the patients undergoing laparoscopic cholecystectomy.
8.Application of white noise therapy on the alleviation of hospital procedural pain of colostomy newborns

Objective:To investigate the application value of white noise therapy on the alleviation of procedural pain of colostomy newborns.Methods:By a prospective, randomized and controlled trial, a total of 88 colostomy newborns in Hunan Children′s Hospital from January 2018 to January 2020 divided into experimental group (44 cases) and control group (44 cases) according to the random number table method. The control group received routine nursing; based on thesis, the experimental group played white noise intervention therapy on the basis of routine nursing. The intervention effect was assessed byNeonatal Infant Acute Pain Assessment Scale (NIAPAS), the first crying time and the duration of first crying, the first painful face and the duration of first painful face as well as heart rate and blood oxygen saturation.Results:The first crying time and the duration of first crying, the first painful face and the duration of first painful face were (28.05 ± 7.39) s, (46.18 ± 13.29) s, (32.89 ± 6.79) s, (52.75 ± 10.71) s in the experimental group, significantly shorter than in the control group (35.79 ± 5.81) s, (35.79 ± 5.81) s, (38.64 ± 10.53) s, (59.79 ± 13.52) s, the difference was statistically significant ( t values were 2.71-5.47, all P<0.05). During and after the procedure, the scores of NIAPAS were (6.32 ± 1.62) points, (4.18 ± 1.06) points in the experimental group, significantly lower than that in the control group (7.43 ± 1.78) points, (4.79 ± 1.34) points ( t=3.06, 2.38, both P<0.05); the heart rate were (152.82 ± 13.25) times/min and (147.84 ± 12.37) times/min in the experimental group, significantly lower than in the control group (166.11 ± 13.79) times/min and (155.77 ± 12.84) times/min ( t=4.61, 2.95, both P<0.05); the blood oxygen saturation were 0.979 8 ± 0.009 5 and 0.980 9 ± 0.012 4 in the experimental group, significantly higher than in the control group 0.969 1 ± 0.014 9, 0.972 3 ± 0.017 8, the difference was statistically significant ( t=4.01, 2.65, both P<0.05). Conclusions:White noise therapy can effectively alleviate procedural pain and stabilizing vital signs of colostomy newborns.
9.A study on sensory processing characteristics of preschool children with autism spectrum disorder

ObjectiveTo explore the sensory processing characteristics of preschool children with autism spectrum disorder (ASD), and to provide a theoretical basis for early screening and intervention training of ASD. MethodsA total of 215 preschool children with ASD and170 typically developed (TD) children were investigated with a basic situation questionnaire and sensory processing measure (SPM). The two groups were stratified according to age and gender, and the differences of scores in sensory domains were compared to analyze the sensory processing characteristics of preschool children with ASD. ResultsThe scores of social participation, vision, hearing, touch, taste and smell, body awareness, balance and motion, planning and ideas, and total sensory system in children with ASD were all higher than those in children with TD (all P<0.01). The highest degree of abnormality was found in hearing and the lowest degree in taste and smell in children with ASD. The results of Spearman correlation analysis showed that in the 4-year-old and 5-year-old children with ASD, the scores of vision (rs=-0.200, P= 0.033) and hearing (rs=-0.194, P=0.040) decreased with age. There was no correlation between the scores of other developmental quotients and age (all P>0.05). Boys and girls with ASD had higher scores in all developmental quotients than TD children (P<0.01). However, there was no significant gender difference in any developmental quotients of ASD children (all P>0.05). ConclusionSensory processing abnormalities are common in preschool children with ASD, which are different from those of TD children in multiple sensory domains. Sensory processing abnormalities may be used as an indicator for early screening of ASD, and it is necessary to conduct corresponding intervention training for sensory processing abnormalities in children with ASD.
10.Effect of transcutaneous electrical stimulation of different acupoints on pulmonary ventilation function in patients undergoing laparoscopic cholecystectomy

Objective:To evaluate the effect of transcutaneous electrical stimulation of different acupoints on pulmonary ventilation function in the patients undergoing laparoscopic cholecystectomy.Methods:Sixty American Society of Anesthesiologists physical status Ⅰ or Ⅱ patients of both sexes, aged 18-64 yr, with body mass index of 18.5-25.0 kg/m 2, undergoing elective laparoscopic cholecystectomy, were divided into 3 groups ( n=20 each) using a random number table method: control group (group C), Zusanli-Sanyinjiao group (group S 1) and Feishu-Chize group (group S 2). Pulmonary ventilation function was monitored by electrical impedance tomography after admission to the operating room, group S 1 and group S 2 underwent transcutaneous electrical stimulation of bilateral Zusanli-Sanyinjiao and Feishu-Chize acupoints at 30 min before induction of anesthesia, with disperse-dense waves, a frequency of 2/100 Hz and the wave width of 0.6 ms at 2 Hz and 0.2 ms at 100 Hz.The intensity of stimulation was the maximum current that patients could tolerate.In group C, electrodes were only placed without electrical stimulation.The percentages of area in center of ventilation (CoV) and tidal volume in the dependent areas were determined on admission to the operating room (T 1), at 5 min after tracheal intubation (T 2), at 15 min after pneumoperitoneum (T 3), and at 30 min after removal of the tracheal tube (T 4). The extubation time and development of hypoxemia and atelectasis within 48 h after tracheal extubation were recorded. Results:Compared with the baseline at T 1, the percentages of area in CoV and tidal volume in dependent areas at T 3-4 were significantly decreased in S 1 and S 2 groups ( P<0.05). Compared with group C, the percentage of tidal volume in dependent areas at T 4 was significantly increased, and the extubation time and incidence of hypoxemia and atelectasis within 48 h after tracheal extubation were decreased in S 1 and S 2 groups ( P<0.05). Compared with group S 1, the percentage of tidal volume in dependent areas at T 4 was significantly increased, and the incidence of hypoxemia within 48 h after tracheal extubation was decreased in group S 2 ( P<0.05). Conclusions:The efficacy of transcutaneous electrical stimulation of Feishu-Chize acupoints in improving pulmonary ventilation function is better than that of Zusanli-Sanyinjiao acupoints in the patients undergoing laparoscopic cholecystectomy.
