1.Mechanism of catalpol regulating Th17 cell differentiation by interfering PKM2/LDHA expression

OBJECTIVE To investigate the mechanism of catalpol affecting the differentiation of helper T cell 17 (Th17) by interfering the expressions of pyruvate kinase M2 (PKM2) and lactate dehydrogenase A (LDHA). METHODS The naive CD4+ T cells were selected from the spleen of C57BL/6 mice, and were differentiated into Th17 cells by adding directional differentiation stimulants for 72 hours. At the same time, the cells were treated with 0 (directed control), 20, 40 and 80 μg/mL catalpol. The flow cytometry was used to detect the proportion of Th17 cell differentiation in cells; the colorimetric method was adopted to detect the levels of pyruvate and lactate in cell culture supernatant; mRNA expressions of retinoid-related orphan nuclear receptor gamma t (RORγt), PKM2 and LDHA were detected by qRT-PCR method; Western blot was used to detect the expression levels of PKM2, LDHA, signal transducer and activator of transcription 3 (STAT3), and phosphorylated STAT3 (p-STAT3) proteins in cells. RESULTS Compared with the directed control group, after 72 hours of treatment with 20, 40, 80 μg/mL catalpol, the differentiation ratio of Th17 cells were decreased by 6.74%, 8.41%, 9.24%, and the levels of pyruvate and lactate in the cell culture supernatant, the mRNA expressions of PKM2, LDHA and RORγt as well as the protein expressions of PKM2 and LDHA and the phosphorylation of STAT3 were significantly reduced (P<0.05). CONCLUSIONS Catalpol can reduce the glycolysis level by down-regulating the expressions of PKM2 and LDHA, thereby inhibiting the differentiation of Th17 cells.
2.Comparison of occupational exposure limits in China with threshold limit values announced by American Conference of Governmental Industrial Hygienists

Background The threshold limit values (TLVs) established and regularly updated by the American Conference of Governmental Industrial Hygienists (ACGIH) are widely adopted and referenced globally, serving as a crucial reference for China's occupational exposure limits (OELs). It is necessary to track it regularly and compare it with China's OELs. Objective To compare the OELs stipulated in Occupational exposure limits for hazardous agents in the workplace—Part 1: Chemical hazardous agents (GBZ 2.1—2019) and the ACGIH TLVs (2024) and to provide references for subsequent formulation and revision of OELs in China. Methods The OELs specified in GBZ 2.1—2019 and the TLVs issued by ACGIH were used to establish a database using Microsoft Excel 2019 software. Cross verification was conducted through matching Chemical Abstracts Service Registry Numbers (CAS Rn) and both Chinese and English names to ensure accuracy. Then, comparisons and analyses were carried out based on the type of limit values, which were matched as follows: permissible concentration-time weighted average (PC-TWA) with threshold limit value-time weighted average (TLV-TWA), permissible concentration-short term exposure limit (PC-STEL) with threshold limit value-short term exposure limit (TLV-STEL), and maximum allowable concentration (MAC) with threshold limit value-ceiling (TLV-C). Comparisons included types, quantities, and sizes of limits. Results The GBZ 2.1—2019 OELs and the ACGIH TLVs (2024) were generally consistent in terms of types and definitions, but there were differences in the number and size of the limits. In terms of the number of limits, GBZ 2.1—2019 specified 365 OELs for 358 chemical hazardous agents, while ACGIH TLVs (2024) included 316 corresponding limits. Among these, 148 (46.9%) limits were consistent, 38 (12.0%) were basically consistent, and 130 (41.1%) were inconsistent. In terms of the size of the limits, out of the 130 inconsistent limits, 51 OELs were lower than the corresponding TLVs, 67 OELs were higher than the corresponding TLVs, and 12 were under different limit types. For some chemical hazardous agents, their OELs were significantly lower or higher than their TLVs. Conclusion Some of the OELs for chemical hazardous agents specified in GBZ 2.1—2019 are significantly lower or higher than the TLVs. For these chemical hazardous factors, it is recommended to prioritize their inclusion in research projects and to complete the revisions as soon as possible based on the latest scientific evidence.
3.Modified Xiaoyao powder for postpartum depression: A systematic review and meta-analysis

Objective:
To evaluate the effectiveness and safety of modified Xiaoyao powder for postpartum depression (PPD) by conducting a systematic review of randomized controlled trials (RCTs).
Methods:
The Chinese National Knowledge Infrastructure Databases (CNKI), the Chinese Scientific Journals Database (VIP), Wanfang, Google Scholar, the SinoMed, Embase, Cochrane Library, and PubMed databases were searched from their inception to April 25, 2023. The Cochrane Risk of Bias tool was used to assess the quality of the trials. We applied the risk ratio to present dichotomous data and the mean difference to present continuous data. Data with similar characteristics were pooled for meta-analysis and heterogeneity was assessed using I2.
Results:
This review included 35 trials involving 2848 participants. The quality of the included studies was low (unclear randomization processes and insufficient reporting of blinding). Participants treated with modified Xiaoyao powder plus Western medicine showed lower Hamilton Depression Scale (HAMD) depression score than those who used Western medicine alone (mean difference = −2.15; 95% confidence interval:−2.52 to 1.78; P < .00001), and higher effective rate (relative risk = 1.19; 95% confidence interval: 1.15 to 1.24; P < .00001), When comparing modified Xiaoyao alone with Western medicine, the HAMD depression score remained low, however, the efficacy rate was higher in the modified Xiaoyao group. Regarding adverse events, the modified Xiaoyao group reported weight gain, nausea, and diarrhea, but no severe adverse events were reported.
Conclusion
Modified Xiaoyao may help relieve depression in PPD when used alone or in combination with Western medicine, with minor side effects. Therefore, future high-quality, large-sample size RCTs are warranted.
4.Evidence Graph Analysis of Postoperative Pain Sensitization Induced by Perioperative Sleep Deprivation

To describe and evaluate the clinical studies of postoperative pain sensitization caused by sleep deprivation through the evidence map system, understand the distribution of evidence in this field, and provide reference for subsequent evidence research. A computer-based search of PubMed, EMBASE, Cochrane library, Web of Science, CNKI, Wanfang Data, VIP and Chinese Biomedical Literature Database from inception to August 2023 was conducted to obtain intervention studies, observational studies and systematic reviews/Meta-analysis of postoperative pain sensitization caused by sleep deprivation. The research characteristics and methodological quality were analyzed and evaluated. The Cochrane Handbook for Systematic Reviews, the Newcastle-Ottawa Scale (NOS) and the AMSTAR-2 scale were used to evaluate the quality of the included studies, and the evidence was comprehensively analyzed and displayed by means of bubble chart, table and text. A total of 35 observational studies (31 cohort studies and 4 case-control studies), 15 randomized controlled trials and 4 systematic reviews/Meta-analyses were included. The number of publications increased rapidly after 2018 and peaked in 2022, and clinical studies in this field mainly focused on cohort studies, with fewer randomized controlled trials and systematic reviews/Meta-analysis studies. The results of the evidence map showed that in terms of quality, 22 studies were 'high quality', 24 studies were 'medium quality', and 8 studies were 'low quality'.Thirty studies showed that sleep deprivation could induce postoperative pain sensitization. Only 2 studies suggested that sleep disorders were not significantly associated with postoperative pain sensitization, and ten studies were uncertain whether sleep deprivation could induce postoperative pain sensitization. Overall evidence shows that sleep deprivation can induce postoperative pain sensitization, but the evaluation dimensions are limited and the methodological quality of the included literature needs to be improved. More high-quality, large-sample and standardized clinical studies should be carried out in the future to provide better scientific basis for clinical work.
5.Effects of three sterilization methods on the magnetic flux of magnetic surgical devices and analysis of sterilization cost

Objective To analyze the effects of three sterilization methods,namely,pressure steam,low-temperature plasma and ethylene oxide,on the magnetic flux of magnetic surgical devices and their sterilization costs.Methods A total of 234 magnetic surgical devices of different specifications and models(magnetic rings)were randomly divided into Group A,Group B and Group C after the paired number was labelled,and each group consisted of 78 pieces(39 pairs).After packaging each pair of devices according to sterilization specifications,Group A was sterilized by pressure steam,Group B was sterilized by low-temperature plasma,and Group C was sterilized by ethylene oxide.We measured the magnetic flux of three sets of magnetic rings before and after sterilization,and comparatively analyzed the sterilization cost and sterilization time of the single package.Results There was no statistically significant difference in the impact of the three sterilization methods on the magnetic flux of the magnetic surgical devices(P>0.05),but there was a significant difference in the magnetic flux before and after sterilization for each sterilization method(P<0.001);the sterilization cost was(1.96±0.16)yuan for Group A,(23.17±0.32)yuan for Group B,and(8.16±0.18)yuan for Group C,showing statistically significant differences among the three groups(P<0.01).The sterilization time was(65.21±3.36)min for Group A,(45.46±1.39)min for Group B,and(1020.38±12.21)min for Group C,with statistically significant differences among the three groups(P<0.01).Conclusion None of the three sterilization methods affects the magnetic flux of the magnetic surgical devices.Pressure steam method shows the lowest cost of single package,low-temperature plasma method shows the highest cost of single package,while ethylene oxide method shows the highest sterilization time.Pressure steam should be the preferred sterilization method for magnetic surgical devices.
6.Values and approaches of integrating red culture with Party building in public hospitals

Red culture,an integral part of socialism with Chinese characteristics,plays a significant role in addressing contemporary ideological challenges and guiding comprehensive development of public hospitals.However,public hospitals cur-rently face several challenges,including insufficient integration of Party leadership in various hospital operations,inadequate de-velopment of Party functionaries,underestimation of the importance of Party building,and a lack of innovation.To address these issues,it is essential to leverage the leadership of the Party committee,strengthen the construction of Party-building workforce within hospitals,and enhance the influence of red culture through diverse promotional strategies.Additionally,public hospitals should extract core values from red culture that align with their unique characteristics,enrich their red culture resource libraries,and employ experiential education models to bolster the sense of mission and responsibility among medical staff.These measures aim to foster a deep integration of red culture with Party-building efforts in public hospitals.
7.Influence of Postoperative Diet Type and Regimen on Hospital Comfort and Rehabilitation of Lung Cancer Patients

Background and objective A reasonable and standardized dietary plan and procedure can help patients recovering quickly from lung cancer surgery.The aim of this study is to optimize the diet plan and procedure mainly based on medium chain triglyceride(MCT)diet and explore its clinical advantages for postoperative lung cancer patients.Methods From October 2023 to December 2023,a total of 156 patients were collected,who underwent lung cancer surgery in Lung Cancer Cen-ter,West China Hospital of Sichuan University.The patients were randomized into MCT group(76 cases)and routine diet(RD)group(80 cases).Clinical symptoms,biochemical index,postoperative hospitalization time and cost,dietary satisfaction and hospitalization comfort between the two groups were analyzed.Results The mean anus exhausting time in MCT group[24.00(9.75,36.97)h]was significantly shorter than that in RD group[28.50(24.00,48.00)h](P<0.001).And the incidence of dizzi-ness(18.42%),nausea and vomiting(6.58%)in MCT group were remarkably lower than those in RD group(51.25%,31.25%)(P<0.001).Hospitalization comfort score in MCT group[(16.74±1.70)]was significantly higher than that in RD group[(14.83±2.34)](P=0.016).Meanwhile,the average hospitalization cost in MCT group[(39,701.82±8105.47)¥]showed an obvious decrease compared with RD group[(44,511.79±9593.19)¥](P=0.007).Conclusion Optimizing the dietary plan and procedure mainly based on MCT diet for postoperative lung cancer patients can help the recovery of gastrointestinal function and improve hospitalization comfort,which promoted overall postoperative rehabilitation of patients with lung cancer surgery.
8.Mechanism of Juanxiao decoction regulating type 3 innate lymphoid cells in treatment of obese asthmatic mice

Objective To explore the mechanism of Juanxiao decoction in regulating type 3 innate lymphoid cells(ILC3s)in treating obese asthma.Methods Sixty male BALB/c mice were randomly divided into a normal group,model group(high-fat diet+OVA),Juanxiao decoction groups(low,middle,and high doses of 8.5,17,and 34 g/kg,respectively),and dexamethasone group(1 mg/kg)with 10 mice in each group.Except for the normal group,the other groups were fed a high-fat diet for 12 weeks,and OVA sensitization by inhalation of an atomized OVA solution was used to establish the obese asthma model.From the first inhalation,the low-,medium-,and high-dose groups of Juanxiao decoction and the dexamethasone group were administered corresponding drugs by gavage,whereas normal and model groups were administered equal amounts of saline by gavage for 7 days.The state of mice and changes in typical symptoms of obese asthma were observed.At 24 hours after the last challenge,a fully automated biochemical analyzer was used to assess four blood lipids and count inflammatory cells in alveolar lavage fluid(BALF).Hematoxylin-eosin staining was used to observe morphological changes in lung tissue and abdominal fat.Enzyme-linked immunosorbent assays were used to measure the immunoglobulin E in BALF and serum,and interleukin(IL)-1β,IL-13,and mouse thymus activation regulating chemokine(CCL17)in lung tissue.IL-17A+ILC3 and IL-22+ILC3 in lung tissue and peripheral blood were analyzed by flow cytometry.Western blot was used to detect expression of P-STAT3 protein in lung tissue.Results Compared with the normal group,model group mice showed infiltration of airway inflammatory cells and thickening of airway walls.However,compared with the model group,lung inflammation in dexamethasone and Juanxiao decoction groups was improved,especially in middle-and high-dose groups.Compared with the normal group,IL-1β,IL-17A+ILC3,IL-13,and CCL17 in lung tissue of the model group were significantly increased(P<0.05),whereas the proportion of IL-22+ILC3 and expression of P-STAT3 were significantly decreased(P<0.01,P<0.05).Compared with the model group,IL-1β,IL-17A+ILC3,IL-13,and CCL17 in lung tissue were significantly decreased and the proportion of IL-22+ILC3 and expression of P-STAT3 were significantly increased in middle-and high-dose Juanxiao decoction groups(P<0.05,P<0.01,P<0.001).Conclusions Juanxiao decoction improves the inflammatory environment of obese asthmatic mice and alleviates lung inflammatory and allergic reactions.Its mechanism may be related to regulating secretion of cytokines by ILC3s.
9.Establishment of suckling rabbits model for monkeypox virus infection

OBJECTIVE To establish a model of monkeypox virus(MPXV)infection in suckling rabbits and analyze its pathological changes.METHODS Ten-day-old suckling rabbits were divided into two groups:the MPXV group(intranasally inoculated with 100 μL of MPXV at 106 PFU·mL-1)and the normal control group(intranasally inoculated with 100 μL of PBS solution),with 10 rabbits in each group.After infection,such indicators as body weight,survival rate,and mental state of the suckling rabbits were monitored daily for 15 days.On days 3,9,and 15 post-infection,samples of the heart,liver,spleen,lung,kidney,brain,small intestine,and peripheral blood were collected from each group for RT-qPCR to detect viral the load.Histopathological analysis using HE staining was performed on samples collected on day 9 post-infection.RESULTS Suckling rabbits infected with MPXV exhibited weight loss,reduced survival rates,and such symptoms as lethargy and chilliness,which were similar to the clinical manifes-tations observed in humans infected with MPXV.Viral load results indicated the presence of the virus in the heart,liver,spleen,lung,kidney,brain,small intestine,and peripheral blood,with varying degrees of pathological changes in each tissue and organ.CONCLUSION Intranasal inoculation can help estab-lish a model of MPXV infection in suckling rabbits.
10.Effect of Maxing Shigantang and Supplemented Guominjian Decoction on Symptoms and Inflammatory Cytokine Levels in Induced Sputum of Children with Cough Variant Asthma

ObjectiveTo investigate effect of Maxing Shigantang and supplemented Guominjian decoction on symptoms and levels of inflammatory cytokines in induced sputum of children with cough variant asthma (CVA). MethodA total of 118 CVA children who were treated in our hospital from January 2020 to January 2021 were enrolled and randomized into the control group and the observation group with the random number table method. Control group received routine western medicine and the observation group was treated by routine western medicine, Maxing Shigantang, and supplemented Guominjian decoction. In the one-month follow-up, the scores of cough and accompanying symptoms, levels of inflammatory cytokines [interleukin-10 (IL-10), interleukin-5 (IL-5), tumor necrosis factor-α (TNF-α), neutrophil, eosinophil] in induced sputum, pulmonary function parameters [forced vital capacity (FVC), forced expiratory volume in one second (FEV1), FEV1/FVC], and treatment outcomes were compared between the two groups. Moreover, the frequency of acute asthma attacks during the three-month follow-up was also compared. ResultNo cases dropped out from this study. After treatment, the scores of cough and accompanying symptoms were decreased in both groups (P<0.05) and were lower in observation group than in control group (P<0.05). After treatment, FVC, FEV1, and FEV1/FVC were raised in both groups and were higher in observation group than in control group (P<0.05). The increase in the level of IL-10 along with the decrease in levels of IL-5, TNF-α, neutrophil, and eosinophil in induced sputum was found in both groups after treatment (P<0.05), and observation group had higher level of IL-10 and lower levels of IL-5, TNF-α, neutrophil, and eosinophil than the control group (P<0.05). The effective rate was 86.44% (51/59) in observation group, which was higher than the 69.49% (41/59) in control group (χ2=4.933, P<0.05). No serious adverse reaction occurred in either group. The frequency of acute asthma attacks during the three-month follow-up was (1.09±0.18) in observation group, which was lower than the (2.83±049) in the control group (P<0.05). ConclusionRoutine western medicine combined with Maxing Shigantang and supplemented Guominjian decoction can effectively and safely alleviate the airway inflammatory responses, control the clinical symptoms, improve pulmonary function, and reduce the frequency of acute recurrence in the treatment of CVA children.
