1.Teaching discussion on improving post competence of medical imaging major students based on non-standardized assessment teaching mode

With the great development of medical education reform,clinical competency has become the requirements and goal of talent training in Medical Imaging in medical universities within and beyond China.The aim of this article is to review the role of non-standardized assessment at raising students'clinical competency in the course of Diagnostic Medical Imaging in our university.To achieve the goal of reinforcing student's thought of medical imaging and their clinical competency,building up high-quality educational evaluation system,strengthening the teaching faculty,facilitating the combination of theory teaching and practice teaching,improving the practice ability and the awareness of services after their formal induction,and therefore cultiva-ting effective and practical medical imaging talents for our nation.
2.Qualitative diagnosis analysis with MRI of huge ovary and adnexal tumors

Objective To analyze MRI manifestations of huge tumors in ovary and adnexal,to improve qualitative diagnosis and differential diagnosis ability.Methods Thirty-one female patients with huge tumor in pelvic cavity confirmed by surgical pathology underwent preoperative MRI plain scan,DWI and enhancement scan.MRI features were retrospective analysis and compared with pathological results.MRI manifestations included the tumor size,signal intensity,ADC value of the cystic component,ADC value of the solid component and enhanced index.Then,statistical analysis was carried out.Results The mean size of the benign ovary and adnexal tumors was (103.04±32.75)mm,and that of the malignant was (139.29 ±49.20)mm.There was no statistical difference between the two groups (P>0.05).The number of cystic lesions in the benign group was significantly higher than in the malignant group,while the number of the cystic-solid and solid lesions in the benign groups were obviously lower than in the malignant group.The signal intensity of the lesions was statistically different between the two groups (P<0.05).The enhanced index in the malignant group (53.15±13.78)was significantly higher than in the benign group (3 5.3 9 ±8.1 2)(P<0.05).Moreover,the AUC was 0.8 6 1 ,indicating that the diagnostic effect was good.There were no statistical differences in the tumor size,ADC value of the solid component and the cystic component (P>0.05).Conclusion Benign ovary and adnexal tumors are mostly cystic,while malignance are mostly solid-cystic and solid.Enhanced index is helpful for identifying benign and malignant ovary and adnexal tumors.ADC value of cystic and solid component are not useful to identifying benign and malignant tumors.MRI is powerful in diagnosing huge tumor in female genital system and help to qualitative diagnosis.

In 2010, the Asian Society of Cardiovascular Imaging (ASCI) provided recommendations for cardiac CT and MRI, and this document reflects an update of the 2010 ASCI appropriate use criteria (AUC). In 2016, the ASCI formed a new working group for revision of AUC for noninvasive cardiac imaging. A major change that we made in this document is the rating of various noninvasive tests (exercise electrocardiogram, echocardiography, positron emission tomography, single-photon emission computed tomography, radionuclide imaging, cardiac magnetic resonance, and cardiac computed tomography/angiography), compared side by side for their applications in various clinical scenarios. Ninety-five clinical scenarios were developed from eight selected pre-existing guidelines and classified into four sections as follows: 1) detection of coronary artery disease, symptomatic or asymptomatic; 2) cardiac evaluation in various clinical scenarios; 3) use of imaging modality according to prior testing; and 4) evaluation of cardiac structure and function. The clinical scenarios were scored by a separate rating committee on a scale of 1–9 to designate appropriate use, uncertain use, or inappropriate use according to a modified Delphi method. Overall, the AUC ratings for CT were higher than those of previous guidelines. These new AUC provide guidance for clinicians choosing among available testing modalities for various cardiac diseases and are also unique, given that most previous AUC for noninvasive imaging include only one imaging technique. As cardiac imaging is multimodal in nature, we believe that these AUC will be more useful for clinical decision making.

4.Analysis of the effects of the combination of PBL and multimedia network teaching model in the preclinical course of image diagnostics

Objective To investigate the effect of the courses of image diagnostics on clinical pro-bation period via the combination of PBL and multimedia network teaching model. Methods 126 under-graduates from the department of medical image (Grade 2011 and 2012) were selected as teaching subjects, and PBL combined with multimedia network teaching was adopted in the teaching of the course of medical imaging diagnosis. After class we implemented a questionnaire survey, and let the students write learning experience. Statistical analysis was conducted by using SSPS 13.0 to reflect the teaching participation in the students' questionnaire and a. summary analysis of the learning experience of the students was also made. Results 123 valid questionnaires were collected. 94.31%(116 people) of students were active participants in these courses, 90.25%(111 people) of the students thought they could actively communicate in teaching, and 43% (110 people) of the students thought they got fully prepared before class. 123 articles in accor-dance with the requirements of experience were received; most students took positive attitude to the active influence the teaching methods had brought; some student pointed out that the teaching method had the disadvantages such as too much time to prepare, too much information, and so on. Conclusions The com-bination of PBL and multimedia network teaching has preliminary results. It is a novel model that is available for teaching image diagnostics on clinical probation period.
5.The evaluation of cardiac function in coronary heart disease with stress myocardial perfusion imaging using 320-slice spiral CT

Objective To explore the application value of 320-slice spiral CT myocardial perfusion technique in the evaluation of cardiac function in patients with coronary artery disease.Methods 24 patients with known reversible myocardial ischemia with coro-nary heart disease and 13 cases of normal were detected by 320-slice spiral CT myocardial perfusion scan in resting and after dobu-tamine-induced stress,measured the values of cardiac function indexes(ESV、EDV、SV、EF)in resting and after dobutamine-induced stress respectively,the results were analyzed by paired t test and independent sample t test,when the P value is less than 0.05,con-sidered statistically significant or correlation.Results The values of cardiac function indexes (ESV,EDV,SV)between the resting and after dobutamine-induced stress were significantly different except EF value in normal group.But only the EF value was signifi-cantly different in CHD,the rest values of cardiac function indexes(ESV,EDV,SV)no significant difference.Each corresponding value of cardiac function indexes in resting and after dobutamine-induced stress was significantly different between the CHD and the normal.Conclusion The dobutamine-induced stress myocardial perfusion with 320-slice spiral CT can judge the impaired degree of cardiac function in patients with myocardial ischemia,provide the basis for comprehensive evaluation of coronary artery disease and its prognosis.
6.Analysis of carrying status and drug susceptibility of Streptococcus agalactiae in genital tract of pregnant women

Objective To understand the carrying status and drug resistance of streptococcus agalactiae in the genital tract of pregnant women.Methods The vaginal discharge from pregnant women was collected for conducting the bacterial culture.The iso-lated streptococcus agalactiae was performed the identification and the drugs susceptibility test by the VITEK 2 fully automatice bacterial identification analyzer.Results Among 1 042 samples,streptococcus agalactiae was isolated in 6 cases with the separation rate of 5.4%.The drug sensitivity test showed the Streptococcus agalactiae was highly sensitive to penicillin,cefazolin,vancomycin, linezolid and nitrofurantoin.The intermediate strains or drug resistant strains were not found.The sensitive rate to levofloxacin, moxifloxacin and clindamycin,erythromycin,tetracycline were 84.0%,84.0%,71.4%,39.3% and 8.9% respectively.Conclusion The carrying rate of streptococcus agalactiae in pregnant women is not high in our hospital.Penicillin and cefazolin should be as the drug of first choice for therapy and the clincal and laboratory nould payattention to detection of streptococcus agalactiae in pregnant women.
7.Value of low-dose volume lung perfusion imaging with 320-detector CT in differential diagnosis of SPNs

Objective To evaluate the diagnostic value of low-dose volume perfusion imaging with 320-detector CT in differentia-ting solitary pulmonary nodules (SPNs).Methods 130 patients with 130 SPNs (79 malignant tumors,1 6 active inflammations,1 6 benign nodules and 1 9 chronic inflammations)underwent contrast-enhanced dynamic volume perfusion CT prospectively.50ml of contrast medium with iodine concentration of 370 mg I/mL was injected at a flow rate of 5 mL/s,and the scan began at 0 second or 4 seconds after the bolus injection.20 or 21 intermittent low-dose volume acquisitions were made with 2 s intervals,and the total ac-quisition time was 38 s or 40 s.Post-procession was performed using a perfusion software on the CT (Body Perfusion,Dual-input single compartment mode).The perfusion parameters (PF,BF,PPI),T-DC and TPF were measured or calculated,and then as-sessed.Results PF and PPI of active inflammation nodules showed significant higher value than those of the other three kinds of nodules,and there were significant differences in PF and PPI between active inflammation nodules and other kinds of nodules.The value of BF in malignant nodules was higher than that in the other three kinds of nodules,however no statistic significant difference in PPI was found between malignant and chronic inflammation nodules.The value of TPF in malignant and active inflammation nod-ules was higher than that in benign and chronic nodules,and there were statistic significant differences between malignant and active nodules and chronic ones.Six types of T-DC were displayed on SPNs:malignant,benign and chronic nodules showed a variety of T-DC types,and active inflammatory nodules showed only one type-typeⅠ.Conclusion Low-dose volume perfusion technique with 320-detector row CT can provide abundant information about blood flow of SPNs,and it may be regarded as an important technique in the differential diagnosis of SPNs.
8.Discussion of teaching reform in medical imaging based on the optimized integration of imaging diagnostics course

9.Survival outcome of women with synchronous cancers of endometrium and ovary: a 10 year retrospective cohort study.

OBJECTIVE: Synchronous occurrence of endometrial and ovarian tumors is uncommon, and they affect less than 10% of women with endometrial or ovarian cancers. The aim of this study is to describe the epidemiological and clinical factors; and survival outcomes of women with these cancers. METHODS: This is a retrospective cohort study in a large tertiary institution in Singapore. The sample consists of women with endometrial and epithelial ovarian cancers followed up over a period of 10 years from 2000 to 2009. The epidemiological and clinical factors include age at diagnosis, histology types, grade and stage of disease. RESULTS: A total of 75 patients with synchronous ovarian and endometrial cancers were identified. However, only 46 patients met the inclusion criteria. The median follow-up was 74 months. The incidence rate for synchronous cancer is 8.7% of all epithelial ovarian cancers and 4.9% of all endometrial cancers diagnosed over this time frame. Mean age at diagnosis was 47.3 years old. The most common presenting symptom was abnormal uterine bleeding (36.9%) and 73.9% had endometrioid histology for both endometrial and ovarian cancers. The majority of the women (78%) presented were at early stages of 1 and 2. There were 6 (13.6%) cases of recurrence and the 5 year cumulative survival rate was at 84%. CONCLUSION: In our cohort, we found that majority of women afflicted with synchronous cancer of the endometrium and ovary were younger at age of diagnosis, had early stage of cancer and good survival.

10.Correlation study of multi-slice spiral CT perfusion imaging in liver cirrhosis and portal morphology

Objective To discuss correlation of hemodynamic changes and portal vein diameter with multi-slice spiral CT peffusion imaging in liver cirrhosis. Method 31 cases liver cirrhosis were enrolled in this study. The first porta hepatis were selected for target lay of CT perfusion scan. Liver perfusion parameters were obtained by color perfusion map method. Right to left diameter and occipitofrontal diame-ter of portal vein were measured. 30 cases of normal persons were used as control group. Result Hepatic arterial perfusion (HAP) in liver 0.05). Hepatic perfusion index (HPI) were (19.13±3.33)% and (20.61±8.56)%, which had no statistically significant difference with the other two groups (P>0.05). Conclusion Multi-spiral CT perfnsion imaging is an effectively noninvasive method to evaluate the hemodynamic changes of liver cirrhosis. Occipitofrontal diameter of portal vein with liver cirrhosis can reflect the state of liver hemodynamics.
