1.Correlation between IL-6 , chemerin , lipid ratio and insulin resistance in obese patients with type 2 diabetes mellitus

Objective To investigate the correlation between insulin resistance and interleukin-6 (IL-6), chemerin, total cholesterol (TC)/high density lipoprotein cholesterol (HDL-C) ratio, triglyceride (TG)/HDL-C ratio, low density lipoprotein cholesterol (LDL-C)/HDL-C ratio and insulin resistance in obese patients with type 2 diabetes mellitus (T2DM), and to provide scientific basis for T2DM prevention and control. Methods A total of 355 obese T2DM patients in Songjiang Hospital Affiliated to Shanghai Jiaotong University School of Medicine were selected from January 2021 to December 2023. IL-6, chemerin and lipids were detected, and the assessment of insulin resistance was conducted through the homeostasis model assessment of insulin resistance (HOMA-IR). Results Among the 355 obese T2DM patients, there were 280 cases of insulin resistance, with the incidence rate of 78.87%. The BMI, IL-6, chemerin, TC/HDL-C, LDL-C/HDL-C, and TG/HDL-C in the insulin resistance group were higher than those in the non-insulin resistance group (P<0.05). The above insulin resistant patients were divided into 4 subgroups by means of insulin resistance, and there were significant differences in BMI, IL-6, chemerin, and TG/HDL-C among the subgroups (P<0.05). IL-6, chemerin, and TG/HDL-C were positively correlated with HOMA-IR in obese T2DM patients (P<0.05), while TC/HDL-C and LDL-C/HDL-C had no significant correlation with HOMA-IR (P>0.05). BMI, IL-6, chemerin, and TG/HDL-C were all influencing factors of insulin resistance in obese T2DM patients (P<0.05). Conclusion IL-6, chemerin and TG/HDL-C are correlated with insulin resistance in obese patients with T2DM and are influencing factors for the occurrence of insulin resistance.
2.Mechanism of Huoxue Rongluo Prescription Regulating Bmal1 Gene to Promote Blood-brain Barrier Repair After Ischemic Stroke

ObjectiveTo explore the mechanism of Huoxue Rongluo prescription (HXRLP) in repairing the blood-brain barrier (BBB) after ischemic stroke (IS). MethodsMale C57BL/6 mice were randomly divided into sham operation (Sham) group, cerebral infarction model (MCAO) group, environmental circadian disruption with cerebral infarction model (ECD-MCAO) group, low-, medium-, and high-dose HXRLP (HXRLP-L, M, and H) groups (8.5, 17, 34 g·kg-1·d-1, respectively), and positive drug butylphthalide (NBP) group (0.23 mL·d-1). In the Sham group, only the exposed blood vessels were isolated without suture insertion. In the other groups, the middle cerebral artery occlusion (MCAO) model of mice was prepared. In the ECD-MCAO group, HXRLP groups, and NBP group, the environmental circadian disruption (ECD) model was prepared. The mice in the Sham group, MCAO group, and ECD-MCAO group were given the same volume of soybean oil by gavage, while those in the other groups were given the corresponding drugs by gavage. Samples were collected after 7 consecutive days of administration. The mNSS score was used to evaluate the repair effect of HXRLP on neurological deficits after IS. Hematoxylin-eosin (HE) staining was used to assess the impact of HXRLP on the pathological damage of brain tissue after IS. 2,3,5-Triphenyltetrazolium chloride (TTC) staining and cerebral blood perfusion status were used to evaluate the repair effect of HXRLP on brain tissue damage after IS. Evans blue staining and transmission electron microscopy were used to evaluate the improvement effect of HXRLP on the permeability injury of BBB after IS. Immunofluorescence (IF) staining was used to observe the expression of von Willebrand Factor (vWF), brain and muscle Arnt-like 1 (Bmal1), and Occludin in brain tissue. Western blot was used to detect the protein expression of Bmal1, Occludin, tight junction protein (Claudin-5), vascular endothelial growth factor (VEGF), and angiopoietins(Ang), and related analysis was conducted. ResultsCompared with the Sham group, the MCAO group exhibited significantly aggravated neurological deficits, cerebral infarction volume, brain pathological damage, and BBB leakage (P0.01) and significantly reduced cerebral blood perfusion (P0.01). The expression of Bmal1, vWF, vascular endothelial growth factor A (VEGFA), and Ang in brain tissue was significantly enhanced (P0.01), while the expression of Occludin and Claudin-5 was significantly weakened (P0.01). Compared with the MCAO group, the ECD-MCAO group showed significantly aggravated neurological deficits, cerebral infarction volume, and BBB leakage (P0.01), obviously worsened brain pathological damage (P0.05), significantly reduced cerebral blood perfusion (P0.01), and significantly decreased expression of Bmal1, vWF, VEGFA, Ang, Occludin, and Claudin-5 in brain tissue (P0.01). Compared with the ECD-MCAO group, the HXRLP groups of all doses presented significantly improved neurological deficits, cerebral infarction volume, brain pathological damage, and BBB leakage (P0.01), significantly increased cerebral blood perfusion (P0.01), and enhanced expression levels of Bmal1, vWF, VEGFA, Ang, Occludin, and Claudin-5 in brain tissue (P0.01). ConclusionHXRLP can regulate the clock protein Bmal1 and promote the expression of VEGFA, Ang, Occludin, and Claudin-5, thereby improving BBB damage after IS.

4.Prediction of liver failure in patients with hepatocellular carcinoma after extensive resection by functional liver imaging score

Objective To evaluate the predictive value of functional liver imaging score(FLIS)based on preoperative gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid(Gd-EOB-DTPA)enhanced MRI for post-hepatectomy liver failure(PHLF)in patients with hepatocellular carcinoma(HCC).Methods The data of HCC patients who underwent extensive hepatectomy and preoperative Gd-EOB-DTPA enhanced MRI were analyzed retrospectively.The FLIS was scored based on the three features including liver parenchyma enhancement,biliary excretion and portal vein signal enhancement in hepatobiliary phase images,and the consistency between different observers was evaluated.Logistic regression model and receiver operating characteristic(ROC)curve were used to analyze the ability of FLIS to predict the PHLF.Results PHLF occurred in 29 of 120 HCC patients(24.2%).The intraclass correlation coefficient(ICC)of FLIS evaluated by two observers was 0.944.Multivariate logistic regression analysis showed that FLIS was an independent predictor of PHLF of HCC patients[odds ratio(OR)0.520,95%confidence interval(CI)0.355-0.726;P<0.001].The area under the curve(AUC)of FLIS for predicting the PHLF was 0.709,the optimal diagnostic threshold was 4,and the corresponding sensitivity and specificity were 78.0%and 58.6%.Conclusion Preoperative FLIS can predict the PHLF of HCC patients,which may help to make more accurate treatment plans for HCC patients.
5.Application of case-based reverse teaching model based on micro-classes in Neurology teaching

Objective:To investigate the advantages of case-based reverse teaching model based on micro-classes in Neurology teaching.Method:A total of 60 undergraduates of grade 2017 majoring in psychiatry from Bengbu Medical University were selected and divided into the control group and the experimental group according to random number table method,with 30 students in each group.The control group received the traditional teaching model to study Neurology.The experimental group received the case-based reverse teaching model based on micro-classes.At the end of the course,Neurology professional knowledge and case analysis ability were evaluated,and students'satisfaction with the teaching mode were compared by questionnaire scores.Results:The scores of Neurology professional knowledge and case analysis ability and the total score in the experimental group were higher than those in the control group(P<0.05).The satisfaction score in the experimental group was also higher than that in the control group(P<0.05).Conclusion:The case-based reverse teaching mode based on micro-classes in Neurology teaching can promote students'professional knowledge learning and mastering,and improve teaching satisfaction,which is worthy of promotion.
6.Summary of the best evidence for self-management in patients with degenerative spinal deformity

Objective:To systematically retrieve, evaluate, and summarize the best evidence on self-management in patients with degenerative spinal deformity, and to provide an evidence-based basis for healthcare professionals to implement health promotion and for patients to engage in self-management.Methods:According to the "6S" evidence model, a top-down search was conducted, including relevant guideline websites, evidence-based databases, and comprehensive databases in English and Chinese, with the search period ranging from establishing the database to March 31, 2023. Two researchers independently evaluated the quality of the literature, extracted information, and summarized the evidence for the quality of the literature.Results:A total of 12 publications were included, including 1 guideline, one expert consensus, two clinical decisions, five systematic reviews, and three randomized controlled trials. The best evidence was summarized in 30 items in 4 domains: health education, exercise management, brace management and daily life management.Conclusions:This study summarized the best evidence of self-management in patients with degenerative spinal deformities, which could provide lessons and references for clinical healthcare professionals to guide patients to self-manage scientifically and rationally, intending to improve disease-related clinical outcomes and quality of life.
7.Application of BOPPPS teaching model in Neurology teaching

Objective:To explore the application effect of BOPPPS teaching model in Neurology teaching.Methods:Eighty undergraduates majoring in clinical medicine were randomly divided into the control group and the experimental group by using the random number table method.The control group received traditional teaching model and the experimental group received BOPPPS teaching model,respectively.The theoretical exam score and clinical practice score were compared between the two groups.The post-teaching anonymous electronic questionnaire was used to investigate the teaching satisfaction between the two groups.Results:The theoretical exam score and the clinical practice score of the experimental group were significantly higher than those of the control group(P<0.05).The teaching satisfaction of the experimental group was significantly higher than that of the control group(P<0.01).Conclusion:BOPPPS teaching model can improve the students'score,teaching quality,and satisfaction in Neurology teaching.

As a key enzyme in the nitrogen cycle,urease is not only a nitrogen source for the growth of organisms,but also a virulence factor found in various pathogenic bacteria.The urease produced by Helicobacter pylori(H.pylori)plays an im-portant role in the colonization and survival of H.pylori.However,H.pylori infections are closely associated with gastrointestinal diseases such as intestinal metaplasia,active gastritis,peptic ulcer and gastric cancer.With the increasing of drug-resistant strains,there is an urgent need for effective and safe new drugs,so H.pylori has become one of the most frequently studied ure-ase-producing bacteria.Urease has also received considerable attention as a potential target for antibacterial drugs.Medicinal plants with high safety have been proven to have therapeutic potential,and many natural plant extracts have become the inspira-tion and starting point of new drug development;previous studies have also found that many naturally occurring flavonoids have anti-urease activity.Therefore,this review summarizes the effects of inhibiting some plant-derived flavonoids on H.pylori urease.According to their origins,structural characteristics,and possible mechanism of action,we seek and develop plant-derived com-pounds with the specificity of anti-H.pylori,which can provide references for developing clinical drug candidates.
9.Clinical features of invasive Streptococcus pneumoniae disease in children and early warning indicators of severe case

Objective To investigate the clinical features in children with invasive pneumococcal disease (IPD) and early warning indicators of severe case.Methods The case data of 101 children inpatients with IPD isolated Streptococcus pneumoniae (Sp) at least once blood culture in this hospital from January 2007 to De-cember 2021 were collected retrospectively.The children patients were divided into the pediatric intensive care unit (PICU) group (35 cases) and general ward group (66 cases) according to whether or not entering PICU during hospitalization.The general clinical data,infection types and results of drug sensitivity test and bacteri-al strain serotype identification were compared between the two groups and analyzed.Results Compared with the general ward group,the male proportion,proportions of complicating nervous system disease and iron-defi-ciency anemia,winter incidence rate,disturbance of consciousness,convulsion,dyspnea,heart rate increase,oc-currence rate of vomiting and diarrhea,proportion of peripheral blood neutrophilic granulocytes,C reactive protein (CRP),lactic dehydrogenase (LDH) level,Hb<90 g/L,LDH>700 U/L,creatine kinase isoenzyme-MB (CK-MB) abnomality,proportions of bacterial meningitis,sepsis and severe pneumonia in the PICU group were higher,the proportion of 3-97 percentile of body weight,bronchopneumonia and no-focus blood stream infection were lower,WBC,Hb and albumin level were lower,and the differences were statistically significant (P<0.05).Eleven kinds of serotypes were identified in this study,ranking the top five in order were 6B,14,19F,23F and 19A.Pneumococcal 13-valent conjugate vaccine (PCV13) could cover 96.9% of the IPD sero-types.The multivariate logistic regression analysis showed that consciousness disturbance,convulsion,per-centage of neutrophils increase,Hb<90 g/L,LDH>700 U/L and CK-MB abnormality indicated the condition of IPD children patients was critical,which was the early-warning indicator of entering PICU (P<0.05).Con-clusion Severe IPD lacks the early specific early-warning indicator,and the cover rate of PCV13 is high.
10.Current Status of Self-transcendence among Lung Cancer Patients and Its Influencing Factors

Background and objective Different degrees of self-transcendence exist in lung cancer patients,which can stimulate patients'self-awareness and promote them to face negative events in life positively,thus improving pa-tients'quality of life and treatment outcomes.However,there are few reports on self-transcendence in lung cancer patients in China,and the related influencing factors have not yet been clarified.This study aims to investigate the current situation of self-transcendence in lung cancer patients and explore its risk factors,so as to provide a theoretical basis for clinical intervention decision-making.Methods 243 lung cancer patients who were admitted to the Department of Lung Cancer Center of West China Hospital,Sichuan University from September 2023 to February 2024 were enrolled as the study subjects;general infor-mation questionnaire,self-transcendence scale,Herth hope scale and social support scale were used for the investigation.The influencing factors related to self-transcendence of lung cancer patients were analyzed.Results The total mean score of self-transcendence in lung cancer patients was(44.73±8.94);the total mean score of hope level was(37.60±4.98),and the total mean score of social support was(41.31±7.27).Self-transcendence was positively correlated with hope level and social support(P<0.001,P<0.001).Education,hope level and social support were influencing factors of self-transcendence in lung cancer pa-tients(P<0.05,P<0.001,P<0.05).Conclusion Self-transcendence in lung cancer patients was at a low level and was influenced by hope level and social support.Healthcare professionals should pay attention to improving the hope level of lung cancer pa-tients,carrying out targeted psychological interventions,and at the same time guiding them to enhance the perception of social support,so as to promote the realization of self-transcendence in patients.
