1.T-cadherin regulates Caspase-1 mediated cell pyrolysis and affects the sensitivity of endometrial cancer to radiotherapy in vitro

Objective:To investigate whether T-cadherin affects the radiotherapy sensitivity of endometrial cancer cells by regulating the Caspase-1 mediated pyrolysis pathway.Methods:Endometrial cancer and adjacent tissue samples were surgically obtained from 82 patients admitted to Hainan Western Central Hospital from October 2019 to March 2021. Immunohistochemical staining and qRT-PCR were used to detect the expression of T-cadherin in endometrial cancer and adjacent tissue samples. By transfecting pcDNA3.1-T-cadherin lentivirus to human endometrial cancer cell line Ishikawa, a stable Ishikawa cell line with high T-cadherin expression was established. Ishikawa cells were treated with 2 Gy X-ray, and the cell proliferation activity was detected after 24, 48, 72 h of culture. The cells were divided into the control group (normally cultured Ishikawa cells), irradiation group (treated with 2 Gy X-ray irradiation), pcDNA3.1-NC+irradiation group (transfected with pcDNA3.1-NC followed by 2 Gy X-ray irradiation), pcDNA3.1-T-cadherin+irradiation group (transfected with pcDNA3.1-T-cadherin followed by 2 Gy X-ray irradiation), pcDNA3.1-T-cadherin+VA765+irradiation group (transfected with pcDNA3.1-T-cadherin, plus 10 μmol/L VA765 treatment followed by 2 Gy X-ray irradiation). After corresponding treatment, cell survival rate was detected by CCK-8 assay. Cell proliferation was determined by colony formation assay. The level of lactate dehydrogenase (LDH) release was measured by LDH release test. The expression levels of Caspase-1, interleukin (IL)-1β, IL-18 and gasdermin A (GSDMA) were detected by Western blot. The expression level of Caspase-1 was detected by immunofluorescence staining. SPSS 23.0 statistical software was used for data analysis. One-way ANOVA was used for data comparison among multiple groups. LSD- t test was used for two-paired comparison. Results:The positive expression rate of T-cadherin protein in endometrial cancer tissues was 6.09%, lower than 87.80% in adjacent normal tissues ( t=58.48, P<0.01). The relative expression level of T-cadherin mRNA in endometrial cancer tissues was 1.00±0.07, significantly lower than 4.02±0.38 in adjacent normal tissues ( t=32.35, P<0.01). The cell activity of pcDNA3.1-T-cadherin++irradiation group was decreased, the number of cell clones was decreased, LDH release level was increased, the relative expression levels of Caspase-1, IL-1β, IL-18 and GSDMA proteins in cells were up-regulated, and red fluorescence intensity of Caspase-1 protein was enhanced ( P<0.01). However, the cell activity of pcDNA3.1-T-cadherin+VA765+irradiation group was increased, LDH release level was decreased, the relative expression levels of Caspase-1, IL-1β, IL-18 and GSDMA proteins were down-regulated, and the red fluorescence intensity of Caspase-1 protein was also decreased (all P<0.01). Conclusion:T-cadherin can increase the radiotherapy sensitivity of endometrial cancer cells by increasing Caspase-1 mediated pyrolysis.
2.Aging weakens Th17 cell pathogenicity and ameliorates experimental autoimmune uveitis in mice.

Aging-induced changes in the immune system are associated with a higher incidence of infection and vaccination failure. Lymph nodes, which filter the lymph to identify and fight infections, play a central role in this process. However, careful characterization of the impact of aging on lymph nodes and associated autoimmune diseases is lacking. We combined single-cell RNA sequencing (scRNA-seq) with flow cytometry to delineate the immune cell atlas of cervical draining lymph nodes (CDLNs) of both young and old mice with or without experimental autoimmune uveitis (EAU). We found extensive and complicated changes in the cellular constituents of CDLNs during aging. When confronted with autoimmune challenges, old mice developed milder EAU compared to young mice. Within this EAU process, we highlighted that the pathogenicity of T helper 17 cells (Th17) was dampened, as shown by reduced GM-CSF secretion in old mice. The mitigated secretion of GM-CSF contributed to alleviation of IL-23 secretion by antigen-presenting cells (APCs) and may, in turn, weaken APCs' effects on facilitating the pathogenicity of Th17 cells. Meanwhile, our study further unveiled that aging downregulated GM-CSF secretion through reducing both the transcript and protein levels of IL-23R in Th17 cells from CDLNs. Overall, aging altered immune cell responses, especially through toning down Th17 cells, counteracting EAU challenge in old mice.

3.Analysis of urinary iodine level and thyroid function in Tibetan adults in Lhasa

Objective To investigate the distribution of urinary iodine level and its relationship with thyroid function in Tibetan adults in Lhasa. Methods Tibetan residents living in Lhasa and its surrounding countryside were recruited by the method of multistage randomized cluster sampling. Their salt iodine, drinking water iodine, urinary iodine, and thyroid function levels were detected. According to the urinary iodine level, these subjects were divided into the iodine deficiency group ( urinary iodine<100μg/L) , the iodine enough group ( urine iodine 100-199μg/L) , the iodine adequate group ( urine iodine 200-299 μg/L ) and the iodine excessive group ( urine iodine≥300μg/L) . The differences in thyroid function among various groups were compared. Results A total of 2235 subjects were included in the study. The overall level of urinary iodine was in skewed distribution, with a median ( upper and lower quartiles) of 154 (99, 229) μg/L. The proportion of subjects with insufficient iodine intake was 25.7%, while those of the enough, adequate, and excessive groups were 41. 5%, 21. 3%, and 11. 5%, respectively. There was no significant difference in urine iodine level between males and females [152(95,219)μg/L vs 155(100,232)μg/L P>0.05]. The urinary iodine levels in residents of urban were higher than those in rural residents [157(101,232)μg/L vs 140(92,200)μg/L, P<0.05]. The urinary iodine levels in the young, middle-aged, and elderly groups were 175 ( 116,256) , 136 ( 91, 200) , and 116 ( 68, 164)μg/L respectively, showing a gradual decrease in urine iodine level with aging (P<0.05). The average salt iodine content of Tibetan adults in Lhasa was 23.16 mg/kg, and that in drinking water was 4.33μg/L. There were no significant differences in TSH levels among various iodine intake groups ( P>0.05) . The levels of thyroid peroxidase antibody ( TPOAb) and thyroglobulin antibody ( TGAb) were gradually decreased with the increase of urinary iodine level (P<0.05). Conclusion More than 50% of Tibetans are at the status of low iodine and high iodine intakes in Lhasa. Although the salt iodine content meets national standards, the drinking water iodine content is lower than that standard.
4.Pregnancy in patients on maintenance hemodialysis: seven cases report and 169 reported cases review

Objective To analyze and summarize the cases of pregnancy on maintenance hemodialysis (MHD),and review the literature.Methods Seven cases of pregnancy and childbirth in patients on MHD in the First Affiliated Hospital of Zhejiang University from Jan 2009 to Dec 2017 were analyzed,and the literature about pregnancy in patients on MHD reported in Pubmed and Web of Science database was retrieved.Both maternal and fetal outcome were studied.Results There are seven pregnant MHD patients in this center,among whom six patients went through a smooth pregnancy and one patient had intra-uterine fetal death at 14 weeks of pregnancy.The six patients had preterm labor.Among six fetuses,four grew in good health and developed well,one had physical development retardation and one had heart malformation at born.In the literature,169 cases reported pregnant patients and 182 fetuses were evaluated,of which 145 live infants were delivered,79.67% overall fetal survival rate,with gestational age of (32.94±3.34) weeks.In 164 delivered fetuses,147 were preterm labor (89.63%).The weight of live births was (1824±609) g.There were no maternal deaths.Fetal survival rate was zero in < 20 weeks of gestational age,20-24 weeks was 14.29%,25-27 weeks was 45.45%,≥28 weeks was 94.44%.The fetal survival rate was higher in pregnant woman receiving hemodialysis≥28 hours weekly compared to women receiving < 16 hours weekly (92.31% vs 52.94%,P=0.02).Conclusions There are still a very high matemal and fetal complication rate in hemodialysis patients,especially in fetus.Gestational age≥28 weeks has a high fetal survival rate.Intensive dialysis during pregnancy may benefit higher fetal survival rate.

6.The Value of Two-dimensional Speckle Tracking Echocardiographic Imaging for Assessing Left Ventricular Function at Pre-and Post-percutaneous Coronary Intervention in Patients With Chronic Total Coronary Occlusion

Objective:To explore the value of two-dimensional speckle tracking echocardiographic imaging (2D-STI) for assessing partial and global left ventricular functional changes at pre-and post-percutaneous coronary intervention (PCI) in patients with chronic total coronary occlusion.
Methods:Echocardiograph was conducted in 23 chronic total coronary occlusion patients at pre-PCI and 1 day, 3, 6 months post-PCI to examine left ventricular ejection fraction (LVEF), meanwhile 2D-STI was applied to obtain the global longitudinal strain (GLS) value of left ventricle.
Results:In all 23 patients, for LVEF, compared with pre-PCI (59.29±12.15)%, it was increased at 3 and 6 months post-PCI (60.00±12.35)%and (61.37±11.8)%respectively, all P<0.05;for GLS value, compared with pre-PCI (-12.77d wit )%, it was decreased at 1 day and 3, 6 months post-PCI (-13.23ecrea)%and (-15.67ecrea)%, (-16.97ecrea)%respectively, all P<0.05.
Conclusion:PCI could effectively improve left ventricular function in patients with chronic total coronary occlusion, 2D-STI technology may quantitatively assess those changes at the early stage.
7.Study on clinical management of HPV+/Pap-during cervical cancer screening

Objective To study the clinical management way for HPV+/papanicolaou (Pap)-during cervical cancer screening.Methods To analyze retrospectively the data from the patients who had loop electrical excision procedure (LEEP) for biopsy confirmed cervical intraepithelial neoplasia (CIN)Ⅱ in Peking University People's Hospital from Jan.2010 to Dec.2014.Results (1) For biopsy confirmed CINⅡ,HPV positive rate was 98.5% (135/137),Pap test positive [≥atypical squamous cell of undetermined significance (ASCUS)] rate was 69.3% (95/137),there was significant difference between them (x5=43.32,P<0.01).(2) For the 42 patients with HPV+/Pap-,whose cytology slides were reviewed again.Among them,the interpretations of there were 16 cases confirmed as the same before,while 26 cases were changed to abnormal (≥ASCUS).Cytology be misdiagnosed was 19.0% (26/137) at the first review.Among the 26 cases,13 (50.0%) cases were missed for the little amount of abnormal cells,8 (30.8%) cases for mild atypical morphology changed;the other 5 (19.2%) cases missed for stain problems.(3) For the cervical LEEP samples,37 cases of the pathology diagnosis were upgrade to CIN Ⅲ+,among them,2 cases of microinvasive cervical carcinoma,1 case of invasive cancer,34 cases of CIN Ⅲ;37 cases were CINⅠ or no lesion found;63 cases were still CIN Ⅱ.Four to six months later after LEEP,the cytology abnormal rate was 11.7% (16/137),and the HR-HPV positive rate was 34.3% (47/137).Conclusions Compared with cytology alone,cytology combined with HPV testing increase the sensitivity of cervical high grade lesion.For the cases of HPV+/Pap-cases,the cytology slides should be reviewed.The quality control of cervical exfoliate sample collection and interpretation should be strengthened.LEEP procedure is not only a treatment method,but also it could provide samples to confirm the diagnosis.
8.Clinical Observation of Kudiezi Injection Combined with Alprostadil in the Treatment of Posterior Circula-tion Ischemic Vertigo

OBJECTIVE:To observe the clinical efficacy and safety of Kudiezi injection combined with alprostadil in the treat-ment of posterior circulation ischemic vertigo. METHODS:Totally 180 patients with posterior circulation ischemia were randomly divided into group A,group B and group C. All patients were given aspirin,atorvastatin,nutrition nerve,antihypertensive,hypo-glycemic and other conventional treatment. On this basis,group A was given Alprostadil injection 10 μg,adding into 0.9% Sodium chloride injection 100 ml,iv,once a day;Kudiezi injection 30 ml,adding into 0.9% Sodium chloride injection 250 ml,iv,once a day. Group B was given Alprostadil injection(the same usage as group A);group C was given Kudiezi injection(the same usage as group A). The course was 14 d. The clinic data was observed,including clinical efficacy;blood flow velocity in left vertebral ar-tery(LVA),blood flow velocity in right vertebral artery(RVA),blood flow velocity of basilar artery(BA),changes of peak laten-cy(PL)and incubation period latency(IPL)before and after treatment;and the incidence of adverse reactions. RESULTS:The to-tal effective rate in group A was significantly higher than group B and C,with significant difference(P<0.05). After treatment,the blood flow velocity of LVA,RVA and BA in each group were significantly higher than before,group A was higher than group B and C;PL-Ⅰ,PL-Ⅲ,PL-Ⅴ,IPL-Ⅰ-Ⅲ and IPL-Ⅲ-Ⅴ were significantly lower than before,group A was lower than group B and C,with significant differences(P<0.05);however,there were no significant difference between group B and C(P>0.05). There were no obvious adverse reactions during treatment. CONCLUSIONS:Kudiezi injection combined with alprostadil has better effica-cy than only Kudiezi injection or alprostadil in the treatment of posterior circulation ischemic vertigo,with good safety.
9.Compliance of secondary prevention in patients with coronary artery disease post coronary revascularization

Objective To investigate the compliance status of secondary prevention in patients with coronary artery disease (CAD) following revascularization.MethodsA total of 512 patients with CAD who received procedures for coronary revascularization were enrolled in the study from January 2009 to October 2010,including 472 cases of percutaneous coronary intervention stenting,25 cases of coronary artery bypass grafting and 15 cases of stenting plus bypass.The demographic information,prophylactic drug therapies, lifestylechangesandmodifiableriskfactorsweresurveyedwithquestionnaires,anthroposomatologicalmeasurementsandlaboratorytestsinpatients3monthsaftercoronary revascularization.ResultsThe proportion of patients on statins,aspirin,β-blockers,angiotensin-converting enzymeinhibitors/angiotensinreceptorblockers(ACEIs/ARBs)andinfluenzavaccinationwere 81.4% (417/512),93.9% ( 481/512 ),82.0% ( 420/512 ),76.2% ( 390/512 ) and 3.7% ( 19/512 ) respectively.Based on the criteria recommended by the American Heart Association/American College of Cardiology (AHA/ACC)Guidelines for Secondary Prevention for Patients with Coronary and Other Atherosclerotic Vascular Disease: 2006 Update, the percentages of achieving therapeutic targets of modifiable risk factor management were as follows:glycosylated hemoglobin (90.2%,462/512 ),total cholesterol ( 68.6%,351/512 ),triglycerides ( 58.8%,301/512 ),high-density lipoprotein cholesterol ( 91.6%,469/512 ),low-density lipoprotein cholesterol ( 44.5 %,228/512 ),systolic pressure ( 75.2 %,385/512 ) and diastolic pressure (90.4%,463/512 ) respectively.And the proportions of improved lifestyle were as follows:smoking cessation/non-smoking 81.4% (417/512),diet control 78.5% ( 402/512 ),achieving weight targets 61.7% (316/512)and regular exercise 58.2% (298/512).ConclusionsThere is a relatively high percentage of standardized antiplatelet therapy and continuous statins medication in patients with coronary artery disease following revascularization. However,many significant modifiable risk factors have not been controlled optimally and lifestyle of patients needs further improvement. There is still a considerable scope for further improvement of secondary prevention in this group of patients.
10.Prognostic value by combination of angiographic and clinical characteristics in stenting patients with unprotected left main coronary artery lesion

Objective To compare the predicting values for Prognosis among Global Risk Classification (GRS),Synergy Between Percutaneous Coronary Intervention With TAXUS and Cardiac Surgery (SYNTAX) score,the European System for Cardiac Operative Risk Evaluation (EuroSCORE) in patients who received stenting because of unprotected left main coronary artery (ULMCA) lesion.Methods Totally 105 successive elderly patients with ULMCA lesion who received stenting were divided into 2 groups:with and without main adverse cardiac events (MACE).The clinical and angiographic characteristics were analyzed and then compared among GRC,SYNTAX score and EuroSCORE.Results As compared with none MACE group,MACE group had higher EuroSCORE score (2.0±2.3 vs.6.5±2.9,t=8.18,P=0.002),and more trivessel disease and left main bifurcation lesion (x2 =8.96,6.96,P =0.011,P =0.008).High risk GRC showed more MACE than medium or low risk GRC [55.9% (19/34) vs.20.5%(9/44),7.4% (2/27),x2 =19.77,P=0.001].AUC(95%CI )of GRC,SYNTAX score and EuroSCORE were [0.821 (0.730-0.912),0.586(0.462-0.709) and 0.631 (0506-0.757)],respectively.Compared with SYNTAX score and EuroSCORE,GRC was superior in the MACE predicting value (Z=3.29,2.63,P<0.01 or P<0.05).
