1.Comparative Study on the Topological Attributes of Deficiency and Excess Pattern of Cerebral Small Vessel Disease Mild Cognitive Impairment Based on fMRI Graph Theory Network

Objective This study aimed to explore the differences in global and local brain network topological properties between deficient pattern(DP)and excess pattern(EP)of mild vascular cognitive impairment caused by subcortical small vessel disease based on graph theory network.Methods Patients were recruited prospectively and were classified with DP and EP subtype.The global small-world topological attributes and local nodes were calculated for the comparison of DP,EP,and healthy controls(CN)using the GRETNA platform.Results The three groups all had small-world attributes,but only the patients in EP had a significantly lower small world attribute δ in the range of 0.05-0.26 than the control group(P<0.05).The node efficiency and node strength indicators of multiple brain region were able to significantly distinguish the DP group from the EP group.However,there was no positive brain region in the node efficiency of the DP patients(P>0.05),and only a few brain regions showed increased node strength efficiency(P<0.05).Conclusion The results indicate that the syndrome of DP and EP have significantly different neuroimaging phenotypes,providing a basis for further research of biological classification based on Chinese Medicine syndromes.
2.Early pregnancy fasting plasma glucose levels based on pre-pregnancy body mass index as a predictor of gestational diabetes mellitus

Objective:To investigate the value and clinical significance of fasting plasma glucose (FPG) in early pregnancy (8-12 gestational weeks) as a predictor of gestational diabetes mellitus (GDM) among women with different pre-pregnancy body mass index (pre-BMI) categories.Methods:A retrospective study was conducted including 9 710 singleton pregnant women (FPG levels in early pregnancy ≤5.6 mmol/L) who underwent prenatal screening and delivery in Yuyao People's Hospital from January 2020 to December 2022. Participants were stratified based on their pre-BMI as follows: <18.5 ( n=1 406), ≥18.5 to <25.0 ( n=7 162), ≥25.0 to <30.0 ( n=978), and ≥30.0 kg/m 2 ( n=164). Within each pre-BMI category, women were further divided into four groups based on FPG levels in early pregnancy (<4.5, ≥4.5 to <4.8, ≥4.8 to <5.1, and ≥5.1 mmol/L). Univariate and multivariate logistic regression analysis were used to identify risk factors for GDM, and receiver operating characteristic (ROC) curve was applied to evaluate the efficacy of FPG in early pregnancy based on different pre-BMI in predicting GDM. Results:The overall incidence of GDM in the singleton pregnancy women with FPG levels in early pregnancy ≤5.6 mmol/L was 12.3% (1 197/9 710). For a pre-BMI of <18.5 kg/m 2, the ORs with 95% CIs for GDM within the different FPG categories (<4.5, ≥4.5 to <4.8, ≥4.8 to <5.1, and ≥5.1 mmol/L) were 0.041 (95% CI: 0.015-0.409), 1.834 (95% CI: 1.089-3.088), 6.779 (95% CI: 4.041-11.371), and 13.723 (95% CI: 5.560-33.871), respectively. For pre-BMI of ≥18.5 to <25.0 kg/m 2, the respective the ORs with 95% CIs were 0.048 (95% CI: 0.012-0.203), 2.573 (95% CI: 2.091-3.168), 9.023 (95% CI: 7.240-11.245), and 9.158 (95% CI: 6.484-12.937). For pre-BMI of ≥25.0 to <30.0 kg/m 2, the ORs with 95% CIs were 0.108 (95% CI: 0.053-0.446), 1.698 (95% CI: 1.064-2.654), 7.537 (95% CI: 5.285-13.080), and 9.994 (95% CI: 5.613-18.218). For pre-BMI of ≥30.0 kg/m 2, the ORs with 95% CIs were 0.098 (95% CI: 0.072-1.015), 2.888 (95% CI: 0.911-9.157), 13.674 (95% CI: 3.480-53.736), and 20.509 (95% CI: 6.674-63.019). The optimal cutoff value of FPG in early pregnancy for GDM prediction was 4.7 mmol/L with an area under the curve of 0.752, the risk of GDM significantly increased with FPG levels ≥4.7 mmol/L in early pregnancy across all pregnant women ( OR=17.356, 95% CI: 13.757-21.896, P<0.001). Conclusions:In the singleton pregnancy women with FPG levels in early pregnancy ≤5.6 mmol/L, FPG in early pregnancy is an independent risk factor for the occurrence of GDM; for pregnant women stratified by the same pre-BMI, the risk of developing GDM increases progressively with the rise of FPG in early pregnancy. FPG in early pregnancy has a certain value in predicting the occurrence of GDM.
3.A case of anti-neutrophil cytoplasmic antibodies-associated vasculitis with ocular masses as the main manifestation

A case of anti-neutrophil cytoplasmic antibodies (ANCA)-associated vasculitis (AAV) with ocular masses as the main manifestation was reported. The patient was a middle-aged female, the initial symptom was eye swelling, pulmonary nodules were found before eye surgery, and further examination revealed proteinuria, hematuria and renal insufficiency. Renal pathology showed ANCA-associated glomerulonephritis. The final diagnosis was eye, kidney and lung lesions caused by AAV. Treatment with glucocorticoids and cyclophosphamide resulted in improvement in eye, kidney, and pulmonary lesions. Atypical clinical manifestations of AAV may lead to delayed diagnosis, and attention should be paid to the exclusion of AAV for ocular masses of unknown cause.
4.Influential factors of psychological state and their correlation with nutritional risk in patients undergoing thoracic tumor radiotherapy

Objective:To investigate the prevalence of anxiety and depression in patients undergoing thoracic tumor radiotherapy, preliminarily evaluate the nutritional risk of patients, and analyze the possible influential factors of anxiety and depression and their correlation with nutritional risk, providing a theoretical basis for further formulating a personalized psychological nursing plan for patients undergoing thoracic tumor radiotherapy.Methods:A total of 121 patients who underwent thoracic tumor radiotherapy between April 2019 and March 2020 in Cancer Hospital of The University of Chinese Academy of Sciences were selected for this study. We evaluated patients' psychological statuses using the Hospital Anxiety and Depression Scale and patients' nutritional risk using the Nutrition Risk Screening. We performed univariate analysis, unconditional logistic regression analysis, and contingency analysis to analyze the factors that affected the psychological status of patients undergoing thoracic tumor radiotherapy and correlate patients' psychological status with nutritional risk.Results:Among 121 patients, 99 (81.8%) patients undergoing thoracic tumor radiotherapy would develop or had developed anxiety, and 42 (34.7%) patients were at the boundary of depression or had depression. Correlation analysis revealed that nutritional risk was correlated with the prevalence of anxiety and depression. Logistic regression analysis showed that surgery, first radiotherapy, and nutritional risk were the factors that affected the prevalence of anxiety. Marital status, location of tumors, surgery, first radiotherapy, and nutritional risk affected the prevalence of depression.Conclusion:Patients undergoing thoracic tumor radiotherapy have serious psychological problems. Nutritional risk is positively correlated with the prevalence of anxiety and depression. Surgery and first radiotherapy are the factors that affect the prevalence of anxiety and depression.
5.Gender disparities in epidemiological trend of HIV/AIDS disease burden among Chinese adolescents from 1990 to 2019

Objective:
To analysis gender disparities in epidemiological trend of HIV/AIDS disease burden among Chinese population aged 10-24 years from 1990 to 2019.
Methods:
The Global Burden of Disease Study (GBD 2019) results tool was used to extract the number of deaths, mortality rate, disability adjusted life years (DALY), DALY rate, years of life lost (YLL), YLL rate, years lived with disability (YLD) and YLD rate to comprehensively analyze the change range of HIV/AIDS disease burden in the Chinese population aged 10-24 years from 1990 to 2019.
Results:
In 2019, the number of deaths, mortality rate, DALY, DALY rate, YLL, YLL rate, YLD and YLD rate of Chinese population aged 10-24 years due to HIV/AIDS were 847, 0.37 per 100 000 , 62 621 person years, 27.51 per 100 000, 58 036 person years, 25.49 per 100 000, 4 584 person years, 2.01 per 100 000 respectively, with the increase of 401.18%, 691.49%, 370.48%, 648.26%, 409.53%, 710.33%, 138.87% and 279.25% , compared with the data in 1990. In term of gender, the number of deaths, mortality rate, DALY, and DALY rate of Chinese male population aged 10-14, 15-19 and 20-24 years due to HIV/AIDS in 2019 were higher than those of female, and the growth rate (compared with 1990) was also higher than that of female. In term of the age group, the increase in people aged 10-14 years was higher than that of people aged 20-24 years. From 1990 to 2019, with the increase of year and age, gender disparity between mortality and DALY rate widened.
Conclusion
Compared with data in 1990, the risk of HIV/AIDS infection in 2019 among Chinese population aged 10-24 years continued to increase, especially among young people of 10-14 years. Gender disparity is exacerbating.
6.The value of PSAD and CGPSAD combined mpMRI in the diagnosis of prostate cancer and hyperplasia in PSA gray zone

【Objective】 To evaluate the diagnostic value of prostate specific antigen density (PSAD) and central glandular prostate specific antigen density (CGPSAD) combined with multi-parameter magnetic resonance imaging (mpMRI) in the diagnosis of prostate cancer (PCa) and prostate hyperplasia (BPH) in the gray area of prostate specific antigen (PSA) (4.0-10.0 μg/L). 【Methods】 Data of 634 patients who had received prostate biopsy in our hospital were retrospectively collected. Among them, 121 patients were selected. According to the pathological results of the biopsy, they were divided into PCa group and BPH group. The three diameters of the prostate and the central gland were measured by MRI. We calculated the prostate volume (PV) and the central gland volume (PVc), and then compared age, PSA, PV, PVc, PSAD, CGPSAD, prostate images, and PI-RADS score between PCa and BPH groups. Multifactor logistic regression analysis was performed to study the independent risk factors for PCa. Receiver operating characteristic (ROC) curves of PCa diagnosis were plotted, respectively, and the area under the curve (AUC) was calculated and compared with the reference. 【Results】 There was no significant difference in total prostate specific antigen (tPSA) between the two groups (P>0.05). However, significant differences were observed in age, PV, PVc, PSAD, CGPSAD and PI-RADS score between the two groups (P<0.05). Multifactor logistic regression analysis showed that PI-RADS score was an independent risk factors for PCa (OR=4.156, P<0.001). The AUC value of PSAD, CGPSAD, PI-RADS score, PSAD combined with PI-RADS score and CGPSAD combined with PI-RADS score were 0.744, 0.771, 0.844, 0.884, and 0.903, respectively. The AUC value of CGPSAD combined PI-RADS score was the highest. 【Conclusion】 CGPSAD is better than PSAD in diagnosing PCa in the grey area of PSA. Combined with PI-RADS score of mpMRI, it can improve the diagnosis of prostate cancer and guide clinical and prostate biopsy.
7.Analysis of gene mutation in a pedigree with autosomal dominant Charcot-Marie-Tooth disease.

OBJECTIVE:
To investigate the molecular genetic mechanism of Charcot- Marie-Tooth (CMT) disease in a pedigree.
METHODS:
Genomic DNA was extracted from the peripheral blood of the family members of a pedigree with autosomal dominant CMT disease, and 65 candidate genes of the proband were screened using target exon capture and the next generation sequencing, and the suspicious genes were verified using Sanger sequencing. PolyPhen-2, PROVEAN and SIFT software were used to predict the function of the mutant genes, and PyMOL-1 software was used to simulate the mutant protein structure.
RESULTS:
A heterozygous missense mutation [c.371A>G (p.Y124C)] was detected in exon 3 of gene of the proband. This heterozygous mutation was also detected in both the proband's mother and her brother, but not in her father. Multiple sequence alignment analysis showed that tyrosine at codon 124 of GDAP1 protein was highly conserved. All the 3 prediction software predicted that the mutation was harmful. Molecular structure simulation showed a weakened interaction force between the amino acid residues at codon 124 and the surrounding amino acid residues to affect the overall stability of the protein.
CONCLUSIONS
The mutation of gene may be related to the pathogenesis of autosomal dominant AD-CMT in this pedigree. The newly discovered c.371A>G mutation (p.Y124C) expands the mutation spectrum of gene, but further study is needed to clarify the underlying pathogenesis.

The prevalence of various types of mental illness has reached 17.5%, and the current treatment of mental illness generally includes western medicine, psychotherapy and physical therapy. Due to the high disability rate, low cure rate and easy recurrence, mental illness has become a major public health problem that seriously affects human health and social stability. And it has attracted the attention of the government and society. At present, the typical western medicine for treating mental illness has serious side effects, while new types of medicine for treating mental illness has a slow efficacy. Physical therapy has an unstable effect, and the side effects of drugs for treating mental illness lack effective method. These problems are expected to be successfully solved through traditional Chinese medicine, acupuncture and acupoint stimulation. This paper puts forward the integrated traditional Chinese and western medicine for the treatment of mental illness, and strives to build a technical system for the treatment of mental illness with integrated Chinese and Western medicine through the organic combination of multidisciplinary and multi-technology.Meanwhile, it lists some classic cases of treatment in order to provide reference for those who are keen on the integrated Chinese and western medicine in the treatment of mental disease.
9.The value of MRI in differentiating diagnosis of ovarian thecoma-fibroma and adult-type ovarian granulosa cell tumor

Objective To compare the MRI features of ovarian thecoma-fibroma and adult-type ovarian granulosa cell tumor (OGCT),and to investigate the differential value of MRI.Methods 13 patients (8 cases of ovarian thecoma-fibroma,5 cases of adult-type OGCT)confirmed by pathology were collected,in which MRI findings were analyzed retrospectively.Results 13 lesions were all unilateral.The shape of more lesions appeared round or oval,and huge lesions were lobulated.Most tumors were dominant solid and well-defined borders.The length-diameter of ovarian thecoma-fibromas ranged from 23 to 268 mm,with the mean diameter of (124±84)mm;on T1WI the solid part of tumors showed equal signal in 5 cases,slight lower signal in 3 cases,on T2WI all tumors were heterogeneous signal,6 lesions appeared equal signal with obscure nebulous high signal;all of 8 lesions were enhanced slightly;4 cases were accompanied with pelvic effusion and other type tumors of the ovarian.The length-diameter of adult-type OGCT tumors ranged from 27 to 110 mm, with the mean diameter of (84±33)mm;the solid part of lesions showed slight lower signal on T1WI and high signal on T2WI,the honeycomb and fissure cystic areas were found in all tumors,4 cases were accompanied with hemorrhage;all tumors were enhanced from moderately to highly,and delay contrast enhancement;the cases were accompanied with endometrial hyperplasia in 3 cases,endometrial carcinoma in 1 case.The non-enhanced cystic areas of two groups of tumors showed lower signal on T1WI and high signal on T2WI. Conclusion There are different characteristics between ovarian thecoma-fibroma and adult-type OGCT on MRI findings,which are helpful to the differential diagnosis.
10.Use of Internet of Things platform for employee health management program in large enterprise

Objective To study the application of Internet of Things, wireless health monitor all-in-one machine, health management platform, energy consumption monitoring in employee health management. Methods Enrollment criteria were set based on employees' health examination data, 126 employees were enrolled in this study voluntarily, 97 were male, and 29 were female. The age was from 26 to 59 years, the average age was 43.7 ± 6.1 years. Using motion energy consumption monitor, wireless health monitor all-in-one machine and health management platform, employee's exercise, body weight, body mass index, fat and muscle mass, systolic blood pressure, diastolic blood pressure, cholesterol, triglyceride, low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol (HDL-C), uric acid, fasting blood glucose etc. were monitored. Data were collected for before and after 3 months intensive intervention. Results After 3 month of intensive intervention, body weight ( (74.90 ± 9.95) kg, (71.77 ± 9.57) kg), body mass index ((25.94 ± 2.65) kg/m2, (24.96 ± 2.55) kg/m2), fat mass ((21.30 ± 4.31) kg, (18.89 ± 4.23) kg), muscle mass ((49.78 ± 7.12) kg, (49.07 ± 6.97) kg), systolic pressure ((129.72 ± 11.16) mmHg(1 mmHg=0.133 kPa), (118.32 ± 10.50) mmHg), diastolic blood pressure ((89.10 ± 8.28) mmHg, (76.94 ± 7.57) mmHg), cholesterol ((5.16±0.85) mmol/L, (4.96±0.90) mmol/L), triglyceride ((1.72±0.92) mmol/L, (1.43±0.64) mmol/L), uric acid ((353.00 ± 85.33) μmol/L, (345.00 ± 73.01) μmol/L) were decreased with statistical significance (t=10.92, 11.03, 6.75, 5.56, 4.23, 3.99, 4.26, 3.46, 1.98, P<0.05); and the value of HDL-C ((1.20 ± 0.24) mmol/L, (1.28 ± 0.25) mmol/L) increased significantly (t=-4.62, P<0.05); the value of LDL-C((2.54 ± 0.52) mmol/L, (2.66±0.58) mmol/L) increased and fast blood glucose ((5.05±0.73) mmol/L, (5.02±0.79) mmol/L) decreased, but there was no significant difference(t=-3.03, 0.14 respectively, P>0.05). Conclusion Health Internet of Things can help employees to develop scientific exercise habits , to correct unhealthy diet habits, and improve health. It will provide a new option for enterprise employee health management and can be recommended for health management programs by large enterprises with domestic and abroad projects.
