1.Rapid health technology assessment of efficacy,safety and cost-effectiveness of vonoprazan in the treatment of gastroesophageal reflux disease

		                        			OBJECTIVE To evaluate the efficacy, safety and cost-effectiveness of vonoprazan (VPZ) in the treatment of gastroesophageal reflux disease (GERD) by rapid health technology assessment, and provide evidence for clinical decision-making. METHODS English and Chinese databases including PubMed, Medline, Cochrane Library, CNKI, VIP, and Wanfang data as well as the official websites of domestic and international health technology assessment agencies were searched by computer from the database establishment to August 2024. After literature screening, data extraction and quality evaluation of included studies were conducted by two researchers, the results were described and analyzed qualitatively. RESULTS A total of 21 pieces of literature were included, involving 15 systematic reviews/meta-analyses and 6 pharmacoeconomic studies. In terms of efficacy, compared with the control regimen (different doses of VPZ, placebo, other positive controls or combination therapy), VPZ (mainly 20 mg/d VPZ) significantly improved the total response rate, mucosal healing rate at 2nd week, symptom remission rate, and maintenance rate at 12th and 24th week after mucosal healing (P<0.05); when the endoscopic Los Angeles classification was C/D, the effective rate of VPZ was significantly higher than that of the control regimen (P<0.05). In terms of safety, there was no significant difference in the incidence of adverse events between VPZ and the control regimen for GERD treatment (P>0.05), but the risk of elevated serum gastrin and abnormal liver function caused by long-term use of VPZ was higher than that of the control regimen (P<0.05). In terms of cost-effectiveness, compared with rabeprazole, lansoprazole and esomeprazole, VPZ had a cost- effectiveness/cost-utility advantage. 2.Rapid health technology assessment of the efficacy,safety and cost-effectiveness of vericiguat in the treatment of heart failure

		                        			OBJECTIVE To evaluate the efficacy, safety and cost-effectiveness of vericiguat in the treatment of heart failure (HF) by rapid health technology evaluation method, and provide reference for the selection and decision-making of clinical treatment plans. METHODS Chinese and foreign databases such as CNKI, PubMed and related health technology evaluation websites were searched by computer. Relevant researchers independently screened literature, extracted data, and comprehensively analyzed the results of the included literature based on literature quality evaluation. RESULTS A total of 17 pieces of literature were included, involving 12 systematic reviews/meta-analyses and 5 pharmacoeconomic studies. The effectiveness analysis showed: for HF patients, compared with placebo, vericiguat (10 mg/d) significantly improved the EuroQol five dimensions questionnaire (EQ-5D) index and decreased the rate of hospitalization due to HF (P<0.05). For heart failure with reduced ejection fraction (HFrEF) patients, vericiguat reduced the incidence of hospitalization due to HF compared with sodium-glucose cotransporter 2 inhibitor (SGLT2i)(P<0.05); compared with angiotensin-converting enzyme inhibitor, vericiguat significantly reduced the occurrence risk of composite endpoints of cardiovascular death or hospitalization due to HF(P<0.05). For HFrEF patients with chronic kidney disease, vericiguat had a tendency to reduce the occurrence risk of composite endpoints of cardiovascular death or hospitalization due to HF compared with neurohormone inhibitors. Safety analysis showed: vericiguat did not increase drug-related adverse reactions compared to placebo (P>0.05). Economic analysis showed: domestic studies indicated that vericiguat had a higher incremental cost-effectiveness ratio. CONCLUSIONS Vericiguat has good safety and efficacy in the treatment of HF but does not possess an economic advantage in the Chinese population.
		                        		
		                        		
		                        		
		                        	
3.Study on the influencing factors and risk prediction model for proteinuria in patients with malignant tumors induced by apatinib

		                        			OBJECTIVE To study the influencing factors for proteinuria in patients with malignant tumors treated with apatinib, then establish and evaluate a risk prediction model based on it. METHODS A total of 120 patients with malignant tumors treated with apatinib in our hospital from January 2020 to December 2022 were selected as the training set, and the clinical data was collected. Univariate analysis and multivariate Logistic regression analysis were used to identify independent risk factors for proteinuria associated with apatinib and then construct a risk prediction model. The predictive value of the model was evaluated by using the receiver operator characteristic (ROC) curve. A total of 34 patients with malignant tumors treated with apatinib from January to December 2023 in our hospital were selected as the validation set, and their clinical data were obtained to cross-validate the accuracy of the prediction model. RESULTS The incidence of proteinuria in the training set of 120 patients was 26.67%. The proportions of patients with smoking history, combined hypertension, apatinib daily dose of ≥500 mg, and alanine aminotransferase level were significantly higher in proteinuria group than those in non-proteinuria group. At the same time,the neutrophilic granulocyte count was significantly lower than that in non-proteinuria group (P<0.05). Patients with smoking history and combined hypertension were the independent risk factors for apatinib-induced proteinuria (odds ratios were 5.005 and 5.342, respectively; with 95% confidence intervals of 1.806- 13.872 and 1.227-9.602, respectively; P<0.05). The binary Logistic regression model equation for the probability (P) of apatinib- induced proteinuria is expressed as LogitP=1.610XMH+1.233XSH-1.483 (MH for combined hypertension, SH for the smoking history), with a model accuracy of 80.0%. ROC curve analysis demonstrated the area under the ROC curve of 0.771, the maximum Youden’s index of 0.474, and the optimal cut-off value for LogitP was 0.159 9, with a sensitivity of 90.6% and specificity of 56.8%. Cross-validation results indicated an overall prediction accuracy of 88.24% for the 34 patients. CONCLUSIONS Combined hypertension and smoking history are independent risk factors for apatinib-induced proteinuria. The constructed risk prediction model has moderate predictive value and can be used to predict the risk of proteinuria in patients with malignant tumors induced by apatinib.
		                        		
		                        		
		                        		
		                        	
4.Enzyme production mechanism of anaerobic fungus Orpinomyces sp. YF3 in yak rumen induced by different carbon source.

		                        			In order to investigate the enzyme production mechanism of yak rumen-derived anaerobic fungus Orpinomyces sp. YF3 under the induction of different carbon sources, anaerobic culture tubes were used for in vitro fermentation. 8 g/L of glucose (Glu), filter paper (Flp) and avicel (Avi) were respectively added to 10 mL of basic culture medium as the sole carbon source. The activity of fiber-degrading enzyme and the concentration of volatile fatty acid in the fermentation liquid were detected, and the enzyme producing mechanism of Orpinomyces sp. YF3 was explored by transcriptomics. It was found that, in glucose-induced fermentation solution, the activities of carboxymethyl cellulase, microcrystalline cellulase, filter paper enzyme, xylanase and the proportion of acetate were significantly increased (P < 0.05), the proportion of propionate, butyrate, isobutyrate were significantly decreased (P < 0.05). The results of transcriptome analysis showed that there were 5 949 differentially expressed genes (DEGs) between the Glu group and the Flp group, 10 970 DEGs between the Glu group and the Avi group, and 6 057 DEGs between the Flp group and the Avi group. It was found that the DEGs associated with fiber degrading enzymes were significantly up-regulated in the Glu group. Gene ontology (GO) function enrichment analysis identified that DEGs were mainly associated with the xylan catabolic process, hemicellulose metabolic process, β-glucan metabolic process, cellulase activity, endo-1,4-β-xylanase activity, cell wall polysaccharide metabolic process, carbohydrate catabolic process, glucan catabolic process and carbohydrate metabolic process. Moreover, the differentially expressed pathways associated with fiber degrading enzymes enriched by Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway analysis were mainly starch and sucrose metabolic pathways and other glycan degradation pathways. In conclusion, Orpinomyces sp. YF3 with glucose as carbon source substrate significantly increased the activity of cellulose degrading enzyme and the proportion of acetate, decreased the proportion of propionate, butyrate and isobutyrate. Furthermore, the degradation ability and energy utilization efficiency of fungus in the presence of glucose were improved by means of regulating the expression of cellulose degrading enzyme gene and participating in starch and sucrose metabolism pathway, and other glycan degradation pathways, which provides a theoretical basis for the application of Orpinomyces sp. 		                        			Objective:To explore the mediating effect of meaning in life between family care and subjective well-being of family caregivers of community disabled elderly, and provide reference for improving caregivers' mental health.Methods:From August 2019 to August 2020, A total of 526 family caregivers of disabled elderly from several communities in Guangzhou were household surveyed by general information questionnaire, family APGRA index, Chinese meaning in life questionnaire and general well-being schedule.SPSS 22.0 software was used for descriptive analysis, correlation analysis and mediating effect analysis.AMOS 22.0 software was used for structural equation model construction, and Bootstrap method was used for mediation effect test.Results:The score of subjective well-being, family care, meaning in life of family caregivers of disabled elderly were (69.41±19.17), (6.42±2.56), (42.96±7.61), respectively.The family care was positively correlated with meaning in life and subjective well-being ( r=0.275, 0.289, both P<0.01) and a positive correlation was found between meaning in life and subjective well-being ( r=0.345, P<0.01). Meaning in life had a partial mediating effect between family care and subjective well-being.The mediating effect value was 4.304, accounting for 30.6% of the total effect value. 7.Prevalence of comorbid chronic obstructive pulmonary disease and osteoporosis among residents aged 40 years and older in Zhangjiagang City

		                        			Objective :
		                        			To investigate the prevalence of comorbid chronic obstructive pulmonary disease (COPD) and osteoporosis (OP) and its influencing factors among residents aged 40 years and older in Zhangjiagang City, Jiangsu Province, so as to provide insights into prevention of comorbid COPD and OP. 
		                        		
		                        			Methods:
		                        			Permanent residents aged 40 years and older were sampled using a multi-stage stratified cluster random sampling method in Zhangjiagang City in 2019, and their pulmonary functions and bone mineral density were measured. The prevalence of comorbid COPD with OP was estimated. The demographic features, smoking, diet, exercises and physical examinations were collected, and factors affecting the prevalence of comorbid COPD with OP were identified using a logistic regression model.
		                        		
		                        			Results:
		                        			Totally 3 140 subjects were enrolled, including 1 315 men (41.88%) and 1 825 women (58.12%), and a mean age of (62.89±9.13) years. A total of 137 subjects were detected with comorbid COPD and OP, with prevalence of 4.36%. The patients with COPD alone included 19 cases with mild, 133 cases with moderate, 87 cases with severe and 26 cases with very severe COPD, and the patients with comorbid COPD and OP included 10 cases with mild, 56 cases with moderate, 56 cases with severe and 15 cases with very severe COPD. The grade of pulmonary functions was higher in patients with comorbid COPD and OP than in patients with COPD alone (Z=-12.304, P<0.001). Multivariable logistic regression analysis identified ages of 65 years and older (OR=2.703, 95%CI: 1.862-3.923), women (OR=2.897, 95%CI: 1.915-4.384) and physical labor (OR=1.540, 95%CI: 1.083-2.189) as factors affecting the development of comorbid COPD and OP.
		                        		
		                        			Conclusion
		                        			The prevalence of comorbid COPD and OP was 4.36% among residents at ages of 40 years and older in Zhangjiagang City, and moderate and severe degree was the predominant grade of pulmonary functions. 8.Correlation between post-traumatic growth and meaning in life among family caregivers of the disabled elderly in the community

		                        			Objective:To explore the status of post-traumatic growth and meaning in life among family caregivers of the disabled elderly in the community, and to analyze the correlation between the two.Methods:From August 2019 to August 2020, convenience sampling was used to select 560 family caregivers of the disabled elderly in the community of Guangzhou as the research object. The General Information Questionnaire, Posttraumatic Growth Inventory (PTGI) and the Chinese Meaning in Life Questionnaire (C-MLQ) were used for the investigation. Pearson correlation and hierarchical regression were used to analyze the correlation between post-traumatic growth and meaning in life in family caregivers of the disabled elderly. A total of 560 questionnaires were distributed, and 526 valid questionnaires were recovered after excluding incomplete questionnaires and answering questionnaires according to a certain rule.Results:The total scores of PTGI and C-MLQ of 526 family caregivers of the disabled elderly were (49.43±20.43) and (42.96±7.61) , respectively. The total score of PTGI and the scores of each dimension were positively correlated with the total score of C-MLQ and the two dimensions of meaning experience and meaning pursuit ( P<0.01) . The hierarchical regression showed that, after controlling for general data, both meaning experience and meaning pursuit could positively predict post-traumatic growth. Conclusions:Post-traumatic growth of family caregivers of disabled elderly is at low level. Family caregivers' meaning in life has a positive effect on post-traumatic growth. Community workers should strengthen the meaning in life education, help caregivers to effectively cope with traumatic events, improve the level of post-traumatic growth, and promote mental health.
		                        		
		                        		
		                        		
		                        	
9.Age cut-off value of the Barthel Index in the assessment of children's activities of daily living

		                        			Objective:To determine the best age cutoff value of Barthel Index in the assessment of children's activities of daily living based on the scores of children's activities of daily living at different ages and the expert consensus.Methods:From December 5, 2019 to March 31, 2020, 186 children with elective surgery who were admitted to the Stomatology Department of Anhui Provincial Children's Hospital were selected as the study subject by convenience sampling. The General Information Questionnaire and Barthel Index were used to evaluate the activities of daily living of children.According to children's age, children were divided into infant group 1 (12-17 months) ( n=25), infant group 2 (18-35 months) ( n=49), preschool group (36-71 months) ( n=54), and school age group (≥ 6 years) ( n=54). The differences of children's activities of daily living in different age groups were compared. A total of 186 questionnaires were distributed, and 182 valid questionnaires were recovered, with avalid recovery rate of 97.85% (182/186). From January 7 to 14, 2020, 8 nursing experts and 2 medical experts were selected by purposive sampling.After two rounds of face-to-face expert meetings, consensus was reached on the best age cutoff value for children's activities of daily living evaluation according to the children's neuropsychiatric development progress. Results:Among children in infant group 1, 96.00% (24/25) were severely dependent and 4.00% (1/25) were moderately dependent.Among children in infant group 2, 61.22% (30/49) were severely dependent, 36.73% (18/49) were moderately dependent, and 2.04% (1/49) were lightly dependent. Among children in preschool group, 20.37% (11/54) were moderately dependent, 77.78% (42/54) were lightly dependent, and 1.85% (1/54) were completely self-care. In the school age group, 27.78% (15/54) of the children were lightly dependent and 72.22% (39/54) were completely self-care. The scores of activities of daily living in different age groups were statistically significant ( P<0.01). Through face-to-face expert meeting, the best age cutoff value for children's activities of daily living assessment was determined to be ≥ 3 years old. Conclusions:Based on the scores of children of different ages and the expert consensus, it is recommended that children ≥ 3 years old use the Barthel Index to evaluate their activities of daily living as one of the bases for establishing the nursing grade.
