1.Pharmaceutical care for a case of severe dermal toxicity induced by durvalumab

OBJECTIVE To provide references for the accurate identification and management of immune-related cutaneous adverse events (irCAEs) caused by durvalumab, and ensuring safe clinical drug use. METHODS Clinical pharmacists participated in the diagnosis and treatment process of a patient with gallbladder cancer who developed irCAEs caused by durvalumab. The clinical pharmacists systematically reviewed the patient’s past medical history and medication history, and assisted physicians in assessing the association between adverse drug reactions and administered drugs. Meanwhile, the clinical pharmacists conducted a graded assessment of the adverse reaction, proposed recommendations such as discontinuing durvalumab and adjusting the administration regimen of glucocorticoids, assisted physicians in restarting immunotherapy, and carried out medication education and other pharmaceutical care. RESULTS The occurrence of irCAEs in this patient was “highly likely” related to durvalumab and was classified as severe. The physicians adopted the clinical pharmacist’s opinion, and after symptomatic treatment, the patient’s skin symptoms improved, and discharged with medication. After the completion of glucocorticoid therapy for the patient, the physician restarted immunotherapy with tislelizumab, and no related adverse reactions occurred again in the patient. CONCLUSIONS Durvalumab can cause irCAEs such as severe skin maculopapular rash. In clinical practice, it is crucial to promptly identify and discontinue suspicious drugs, immediately implement effective symptomatic treatment measures, and actively resume immunotherapy to ensure the continuity and safety of the patient’s treatment.
2.Correlation between seasonal blood pressure variability and total burden score of cerebral small vessel disease with different severities

Objective To investigate the correlation between seasonal blood pressure (BP) variability and total burden score of cerebral small vessel disease (CSVD) with different severities. Methods The patients with CSVD who were consecutively admitted were enrolled, and according to the total burden score based on head MRI, they were divided into control group (CSVD 0 points), mild group (CSVD 1‒2 points), and moderate-to-severe group (CSVD 3‒4 points).General information was collected from all patients, as well as 24-hour ambulatory blood pressure monitoring (ABPM) during warm and cold seasons. The correlation between ABPM parameters in different seasons and the imaging burden of different severities of CSVD was analyzed. Results A total of 145 patients were enrolled, with 29 patients in the control group,64 in the mild group, and 52 in the moderate-to-severe group.Compared with the control group, the mild group and the moderate-to-severe group had significantly higher age(F=9.721,P=0.001), 24-hour systolic blood pressure (SBP) in hot season(F=6.572,P=0.002), daytime SBP in hot season(F=6.460,P=0.002), daytime diastolic blood pressure (DBP) in hot season(F=5.802,P=0.004), nighttime SBP in hot season(F=8.508,P<0.001). Compared with the control group, the moderate-to-severe group had significantly higher levels of 24-hour DBP in hot season(F=4.564,P=0.012), nighttime DBP in hot season(F=6.294,P=0.002),24-hour SBP in cold season(F=7.012,P=0.001), 24-hour DBP in cold season(F=4.527,P=0.012),daytime SBP in cold season(F=5.708,P=0.004),daytime DBP in cold season(F=3.138,P=0.046),nighttime SBP in cold season(F=9.154,P<0.001), and nighttime DBP in cold season(F=8.006,P=0.001). Compared with the control group, the mild group and the moderate-to-severe group had a significantly higher proportion of patients with abnormal BP circadian rhythm in hot season (χ2=13.059,P=0.001) and cold season (χ2=10.091,P=0.006).The ordinal logistic regression analysis showed that age (OR=1.147, 95%CI 1.084‒1.214) was an independent risk factor for CSVD, and compared with the patients with dipper-type blood pressure in hot season, the patients with non-dipper blood pressure pattern had a risk of CSVD increased by 13.282 times (OR=13.282, 95% CI 2.379‒74.159), while those with reverse-dipper blood pressure pattern had a risk of CSVD increased by 25.569 times(OR=25.569,95%CI 3.061‒213.551). Conclusion The imaging burden score of CSVD increases with the increase in age and the proportion of abnormal circadian blood pressure pattern in hot season, and both age and abnormal circadian blood pressure pattern in hot season are independent risk factors for the imaging burden of CSVD.
3.Association of vegetables and fruits intake with depressive symptoms in Yunnan multi ethnic first year junior high school students

Objective:
To explore the association of vegetables and fruits intake with depressive symptoms among multi ethnic first year junior high school students in Yunnan Province, so as to provide data support for preventing and reducing depressive symptoms among first year junior high school students.
Methods:
From October to December 2022, a cluster random sampling method was used to select 8 500 first year junior high school students from 11 ethnic minority areas in Yunnan Province (Fugong County, Longling County, Longyang District, Luchun County, Mojiang County, Nanjian County, Qiaojia County, Shuangjiang County, Tengchong City, Yuanmou County, Zhenyuan County), to investigate with a questionnaire. The Dietary Frequency Questionnaire was used to collect dietary behavior datas, and the Chinese version of Depression Anxiety Stress Scale-21 (DASS-21) was used to assess depressive symptoms. The generalized linear model was used to analyze the association of vegetable and fruit intake with depressive symptoms in students, and stratified analysis was performed according to ethnicity.
Results:
The detection rate of depressive symptoms among first year junior high school students in Yunnan Province was 29.5%. The detection rates of depressive symptoms in Han and minority first year junior high school students were 26.9% and 31.6%. After controlling for demographic variables such as gender, age, family residence and other confounding factors, the generalized linear model analysis results showed that the intake of leafy vegetables ( β= -0.07 , 95%CI =-0.12 to -0.01), flat fruits ( β=-0.06, 95%CI =-0.12 to -0.00) and hot natured fruits ( β=0.11, 95%CI = 0.04- 0.17) were associated with depressive symptoms in Han first year junior high school students (all P <0.05). The intake of melon and fruit vegetables ( β=-0.06, 95%CI =-0.11 to -0.01) and hot natured fruits ( β=0.06, 95%CI =0.01-0.12) were associated with depressive symptoms in ethnic minority first year junior high school students (both P <0.05).
Conclusions
The intake of vegetables and fruits among multi ethnic first year junior high school students in Yunnan Province is related to the risk of depressive symptoms. It is suggested to strengthen the consumption guidance and education of vegetables and fruits to prevent depressive symptoms among first year junior high school students.
4.Correlation between IL-6 , chemerin , lipid ratio and insulin resistance in obese patients with type 2 diabetes mellitus

Objective To investigate the correlation between insulin resistance and interleukin-6 (IL-6), chemerin, total cholesterol (TC)/high density lipoprotein cholesterol (HDL-C) ratio, triglyceride (TG)/HDL-C ratio, low density lipoprotein cholesterol (LDL-C)/HDL-C ratio and insulin resistance in obese patients with type 2 diabetes mellitus (T2DM), and to provide scientific basis for T2DM prevention and control. Methods A total of 355 obese T2DM patients in Songjiang Hospital Affiliated to Shanghai Jiaotong University School of Medicine were selected from January 2021 to December 2023. IL-6, chemerin and lipids were detected, and the assessment of insulin resistance was conducted through the homeostasis model assessment of insulin resistance (HOMA-IR). Results Among the 355 obese T2DM patients, there were 280 cases of insulin resistance, with the incidence rate of 78.87%. The BMI, IL-6, chemerin, TC/HDL-C, LDL-C/HDL-C, and TG/HDL-C in the insulin resistance group were higher than those in the non-insulin resistance group (P<0.05). The above insulin resistant patients were divided into 4 subgroups by means of insulin resistance, and there were significant differences in BMI, IL-6, chemerin, and TG/HDL-C among the subgroups (P<0.05). IL-6, chemerin, and TG/HDL-C were positively correlated with HOMA-IR in obese T2DM patients (P<0.05), while TC/HDL-C and LDL-C/HDL-C had no significant correlation with HOMA-IR (P>0.05). BMI, IL-6, chemerin, and TG/HDL-C were all influencing factors of insulin resistance in obese T2DM patients (P<0.05). Conclusion IL-6, chemerin and TG/HDL-C are correlated with insulin resistance in obese patients with T2DM and are influencing factors for the occurrence of insulin resistance.
5.Analysis of imaging differences between high-resolution CT and digital radiography chest films in pneumoconiosis screening

Background The early lesions of pneumoconiosis that cannot be clearly shown by digital radiography (DR) chest films can be clearly displayed in high-resolution CT (HRCT). HRCT enables systematic observation of the evolution and progression of pneumoconiosis, providing reliable evidence for diagnosis. Objective To provide reliable evidences for the early screening of pneumoconiosis, By analyzing the imaging difference between HRCT and DR chestfilms in pneumoconiosis screening. Methods Six casting workers in a casting forging company suspected of early stage of pneumoconiosis through regular occupational health examination screening were recruited , and 64 rows of spiral CT thin layer were scanned and reconstructed by high-resolution bone algorithm. Compare the imaging findings of early stage of pneumoconiosis small shadow on DR chest film and HRCT and make diagnostic conclusion. Results All six subjects had Multiple small nodules and/or irregular shadows in the lungs were found in all six patients’ DR chest films, but the density and distribution of small shadows did not meet the diagnostic criteria of pneumoconiosis. The HRCT review showed that, dispersively distributed micro nodule shadows and/or short-thread shape thin shadows in the lung in four patients, the rest two patients' HRCT images showed multiple localized pleural thickening and partial calcification and multiple nodules like pleural uplift thickening. Other lesions were also identified, including lung multiple localized pleural thickening and calcification in 3 cases, lung multiple cystic bronchiectasis in 1 case, inflammatory nodules in 2 cases, of chronic lesions in 2 case. Through comprehensive analysis, early stage of pneumoconiosis were seen in 4 patients, among which 3 patients engaged in sand mixing and sand clearing presenting early stage of silicosis; 1 patient engaged in moulding sand manufacturing presenting early stage of foundry worker pneumoconiosis. Conclusion Compared with DR chest film, HRCT can more accurately show the morphology and distribution of lesions, and has significant advantages in identifying small shadows in early stage of pneumoconiosis and other lung diseases. The diffuse small nodules in the lung is not a characteristic manifestation of pneumoconiosis, and cannot be used as the basis for the diagnosis of early stage of pneumoconiosis alone. Even with a clear occupational dust exposure history, an early stage of pneumoconiosis diagnosis can be made after excluding other lung diseases. After dust work, especially silicon dust work workers showing abnormal changes in X-ray chest film, review of the HRCT should be conducted even if the small shadow distribution does not meet the diagnostic criteria of pneumoconiosis, so as to early detection of pneumoconiosis patients and high-risk groups.
6.Pathogen spectrum of diarrheal disease surveillance in Fengxian District, Shanghai, 2013‒2023

ObjectiveTo investigate the pathogenic spectrum and epidemiological characteristics of diarrheal disease in Fengxian District of Shanghai, and to provide scientific basis for the prevention and control of diarrheal diseases. MethodsBasic information of the initial adult cases visited diarrheal disease surveillance sentinel hospital in Fengxian District, Shanghai, was collected from August 2013 to 2023, and fecal samples were collected at 1∶5 sampling intervals to isolate and identify 5 kinds of diarrheagenic Escherichia coli (DEC), Salmonella (SAL), Vibrio parahaemolyticus, Campylobacter, Vibrio cholerae, Shigella and Yersinia enterocolitica (YE). Simultaneously, nucleic acid detection was performed for 3 kinds of rotavirus, 2 kinds of norovirus, intestinal adenovirus, astrovirus and sapovirus. ResultsA total of 1 861 cases of newly diagnosed diarrheal disease were reported, with the peak in July to August. Additionally, 704 surveillance samples were detected, with a total positive detection rate of 50.57%. The detection rates of bacterial, viral and mixed infection were 25.14%, 21.02% and 4.40%, respectively. Among the pathogens detected, DEC accounted for the highest (17.61%, 124/704), followed by norovirus (16.48%, 116/704), rotavirus (6.39%, 45/704), SAL (5.97%, 42/704) and Campylobacter (3.84%, 27/704). DEC detected were mainly enteroaggregative Escherichia coli and enterotoxigenic Escherichia coli, with no detection of Vibrio cholerae, Shigella and YE. The highest total pathogen detection rate was observed from June to September, and the detection peaks of norovirus were from March to June and from October to December, whereas that of DEC was from June to October. The detection rate of rotavirus peaked from January to February, but which was not detected between 2020‒2023. The SAL positive rate peak was in September, whereas that of Campylobacter was from July to September. ConclusionThe main pathogens detected in Fengxian District from 2013‒2019 are DEC, norovirus, rotavirus, SAL and Campylobacter. Different pathogens have different detection peaks, with bacteria predominating in summer and viruses in winter and spring. Prevention and control measures should be carried out according to the epidemiological characteristics of different seasons.
7.Role of TIM3 Pathway in Immune Pathogenesis and Targeted Therapy of Myelodysplastic Syndrome

Myelodysplastic syndrome (MDS), a myeloid tumor derived from the malignant clones of hematopoietic stem cells, has an annually increasing incidence. The contemporary research direction has shifted to analyzing the synergistic effect of immune surveillance collapse and abnormal bone marrow microenvironment in the pathological process of MDS. Against this backdrop, the immune checkpoint molecule TIM3 has emerged as a key target because of its persistently high expression on the surface of important immune cells such as T and NK cells. The abnormal activation of the TIM3 pathway is the mechanism by which solid tumors and hematological malignancies achieve immune escape and is a key hub in the formation of immune exhaustion phenotypes. This work integrates the original discoveries of our team with the latest international progress, systematically demonstrating the bidirectional regulatory network of TIM3 between the malignant clone proliferation of MDS and the immunosuppressive microenvironment. Integrating the evidence from emerging clinical trials allows us to consider the clinical significance of TIM3-targeted blocking for MDS, providing a transformative path to overcome the resistance of traditional treatments and marking a new chapter in the active immune reconstitution of MDS treatment.
8.Mechanism of Huoxue Rongluo Prescription Regulating Bmal1 Gene to Promote Blood-brain Barrier Repair After Ischemic Stroke

ObjectiveTo explore the mechanism of Huoxue Rongluo prescription (HXRLP) in repairing the blood-brain barrier (BBB) after ischemic stroke (IS). MethodsMale C57BL/6 mice were randomly divided into sham operation (Sham) group, cerebral infarction model (MCAO) group, environmental circadian disruption with cerebral infarction model (ECD-MCAO) group, low-, medium-, and high-dose HXRLP (HXRLP-L, M, and H) groups (8.5, 17, 34 g·kg-1·d-1, respectively), and positive drug butylphthalide (NBP) group (0.23 mL·d-1). In the Sham group, only the exposed blood vessels were isolated without suture insertion. In the other groups, the middle cerebral artery occlusion (MCAO) model of mice was prepared. In the ECD-MCAO group, HXRLP groups, and NBP group, the environmental circadian disruption (ECD) model was prepared. The mice in the Sham group, MCAO group, and ECD-MCAO group were given the same volume of soybean oil by gavage, while those in the other groups were given the corresponding drugs by gavage. Samples were collected after 7 consecutive days of administration. The mNSS score was used to evaluate the repair effect of HXRLP on neurological deficits after IS. Hematoxylin-eosin (HE) staining was used to assess the impact of HXRLP on the pathological damage of brain tissue after IS. 2,3,5-Triphenyltetrazolium chloride (TTC) staining and cerebral blood perfusion status were used to evaluate the repair effect of HXRLP on brain tissue damage after IS. Evans blue staining and transmission electron microscopy were used to evaluate the improvement effect of HXRLP on the permeability injury of BBB after IS. Immunofluorescence (IF) staining was used to observe the expression of von Willebrand Factor (vWF), brain and muscle Arnt-like 1 (Bmal1), and Occludin in brain tissue. Western blot was used to detect the protein expression of Bmal1, Occludin, tight junction protein (Claudin-5), vascular endothelial growth factor (VEGF), and angiopoietins(Ang), and related analysis was conducted. ResultsCompared with the Sham group, the MCAO group exhibited significantly aggravated neurological deficits, cerebral infarction volume, brain pathological damage, and BBB leakage (P0.01) and significantly reduced cerebral blood perfusion (P0.01). The expression of Bmal1, vWF, vascular endothelial growth factor A (VEGFA), and Ang in brain tissue was significantly enhanced (P0.01), while the expression of Occludin and Claudin-5 was significantly weakened (P0.01). Compared with the MCAO group, the ECD-MCAO group showed significantly aggravated neurological deficits, cerebral infarction volume, and BBB leakage (P0.01), obviously worsened brain pathological damage (P0.05), significantly reduced cerebral blood perfusion (P0.01), and significantly decreased expression of Bmal1, vWF, VEGFA, Ang, Occludin, and Claudin-5 in brain tissue (P0.01). Compared with the ECD-MCAO group, the HXRLP groups of all doses presented significantly improved neurological deficits, cerebral infarction volume, brain pathological damage, and BBB leakage (P0.01), significantly increased cerebral blood perfusion (P0.01), and enhanced expression levels of Bmal1, vWF, VEGFA, Ang, Occludin, and Claudin-5 in brain tissue (P0.01). ConclusionHXRLP can regulate the clock protein Bmal1 and promote the expression of VEGFA, Ang, Occludin, and Claudin-5, thereby improving BBB damage after IS.
9.Ethical considerations and coping strategies for growth hormone therapy in children with short stature

Height, as one of the crucial indicators for assessing children’s growth and development, has consistently been a global focus. With economic development and improvements in social living standards, the clinical management needs for children with short stature have been increasingly growing. While growth hormone brings hope to children with short stature, it also triggers ethical challenges such as medical standardization, expansion of indications, equitable accessibility, and informed consent. To avoid the ethical issues related to the use of pediatric growth hormone, multidimensional and comprehensive clinical management should be implemented for children with short stature, including strictly adhering to medical standards and ethical guidelines, enhancing public awareness, and promoting the standardized development of recombinant human growth hormone (rhGH) therapy and ethics.
10.Association between Chinese visceral adiposity index and diabetes mellitus and hypertension among elderly people in Hebei Province

Objective To investigate the association between the Chinese visceral adiposity index (CVAI) and diabetes mellitus, hypertension, diabetes mellitus or hypertension, and diabetes with hypertension among elderly people in Hebei Province. Methods In 2020, a stratified multi-stage random sampling was used to conduct questionnaire survey, physical examination and laboratory detection among permanent residents of 10 monitoring sites in Hebei Province. Logistic regression was used to analyze the association between CVAI and diabetes mellitus, hypertension, diabetes mellitus or hypertension, and diabetes with hypertension. The area under the ROC curve (AUC) was used to evaluate the predictive value of CVAI for diabetes mellitus, hypertension, diabetes mellitus or hypertension, and diabetes with hypertension. Results The detection rates of diabetes mellitus, hypertension, diabetes mellitus or hypertension, and diabetes with hypertension were 19.8%, 74.6%, 78.2%, and 16.2%, respectively. Multivariate logistic regression analysis showed that compared with the lowest quartile of CVAI group Q1, the OR (95% CI) of diabetes mellitus, hypertension, diabetes mellitus or hypertension, and diabetes with hypertension in the highest quartile Q4 group were 3.55 (2.58~4.89), 2.52 (1.92~3.31), 3.09 (2.31~4.12), and 4.92 (3.40~7.12), respectively. The ROC curve results showed that CVAI had the best predictive value in the diagnosis of diabetes with hypertension, and the optimized critical values in males and females were 128.54 and 141.88, respectively. Conclusion The detection rates of diabetes mellitus and hypertension are high in the elderly population in Hebei Province. CVAI is positively associated with the risk of diabetes mellitus, hypertension, diabetes mellitus or hypertension, and diabetes with hypertension among the elderly in Hebei. CVAI has the strongest prediction ability for diabetes with hypertension.
