1.Effect of bupivacaine mediated apelin on gene expression of rat cardiomyocytes

Objective:To investigate the effect of bupivacaine on Apelin/APJ pathway and subsequent gene expression in Sprague-Dawley (SD) rat cardiomyocytes.Methods:H9c2 cardiomyocytes were cultured in vitro and treated with 0-1 mmol/L bupivacaine for 6 h. The concentration of Apelin was detected by enzyme-linked immunosorbent assay (ELISA), and the expression of APJ was detected by Western blot; Fifteen adult male SD rats were randomly divided into sham group (sham group), bupivacaine 30 mg/kg group (Bupi group), bupivacaine 30 mg/kg+ [Pyr 1] apelin-13 0.15 mg/kg (Bupi-A) group. After corresponding treatment, myocardial tissue was taken to detect the expression of Apelin and APJ, and high-throughput sequencing was performed. Results:Western blot and ELISA results showed that bupivacaine down regulated the expression of Apelin and APJ in H9c2 cardiomyocytes (all P<0.05). The expression levels of Apelin and APJ in the myocardial tissue of SD rats in Bupi group were lower than those in sham group (all P<0.05). The expression of Apelin in myocardial tissue of Bupi-A group was higher than that of Bupi group ( P=0.006), but the expression of APJ had no significant difference ( P>0.05). High throughput sequencing revealed that compared with sham group, the top five differentially expressed genes that were significantly upregulated in Bupi group but significantly downregulated in Bupi-A group were ubiquinone NADH dehydrogenase Fe-S protein 3, enolase 1, aquaporin 1, ATP synthase F0 complex C subunit 1 lipid binding protein, and peroxidase 2 (all P<0.001); The top five genes that were significantly down regulated in Bupi group but significantly up-regulated in Bupi-A group were mesothelin, Rho GTPase activating protein 29, sten20 like kinase, carbonic anhydrase, and paraplatelet lysin (all P<0.001). These genes were associated with increased cardiomyocyte apoptosis, energy metabolism disorders and exercise attenuation. Conclusions:bupivacaine can reduce the expression of Apelin, leading to the changes of gene expression related to increased apoptosis, energy metabolism disorder and exercise attenuation in rat cardiomyocytes.
2.Association between blood glucose-to-lymphocyte ratio and prognosis of patients with sepsis-associated acute kidney injury.

OBJECTIVE:
To investigate the association between the glucose-to-lymphocyte ratio (GLR) and prognosis of patients with sepsis-associated acute kidney injury (SA-AKI).
METHODS:
Based on the Medical Information Mart for Intensive Care-IV (MIMIC-IV), SA-AKI patients aged ≥ 18 years were selected. According to the tertiles of GLR, the patients were divided into GLR1 group (GLR ≤ 4.97×10-9 mmol), GLR2 group (4.97×10-9 mmol < GLR < 9.75×10-9 mmol) and GLR3 group (GLR ≥ 9.75×10-9 mmol). Patients with SA-AKI were divided into survival group and death group according to whether they survived 28 days after admission. The patient's gender, age, vital signs, laboratory test results, comorbidities, sequential organ failure assessment (SOFA), acute physiology score III (APS III) score and treatment measures were extracted from the database. Kaplan-Meier survival analysis was used to make the survival curves of patients with SA-AKI at 28 days, 90 days, 180 days and 1 year. Multivariate Logistic regression analysis model was used to explore the independent risk factors of 28-day mortality in patients with SA-AKI. Receiver operator characteristic curve (ROC curve) was used to analyze the predictive efficacy of GLR for the prognosis of patients with SA-AKI.
RESULTS:
A total of 1 524 patients with SA-AKI were included, with a median age of 68.28 (58.96, 77.24) years old, including 612 females (40.16%) and 912 males (59.84%). There were 507 patients in the GLR1 group, 509 patients in the GLR2 group and 508 patients in the GLR3 group. There were 1 181 patients in the 28-day survival group and 343 patients in the death group. Grouping according to GLR tertiles showed that with the increase of GLR, the 28-day, 90-day, 180-day and 1-year mortality of SA-AKI patients gradually increased (28-day mortality were 11.64%, 22.00%, 33.86%, respectively; 90-day mortality were 15.98%, 26.72%, 40.55%, respectively; 180-day mortality were 17.16%, 28.29% and 41.73%, and the 1-year mortality were 17.95%, 29.27% and 42.72%, respectively, all P < 0.01). According to 28-day survival status, the GLR of the death group was significantly higher than that of the survival group [×10-9 mmol: 9.81 (5.75, 20.01) vs. 6.44 (3.64, 10.78), P < 0.01]. Multivariate Logistic regression analysis showed that GLR was an independent risk factor for 28-day mortality in patients with SA-AKI [when GLR was used as a continuous variable: odds ratio (OR) = 1.065, 95% confidence interval (95%CI) was 1.045-1.085, P < 0.001; when GLR was used as a categorical variable, compared with GLR1 group: GLR2 group OR = 1.782, 95%CI was 1.200-2.647, P = 0.004; GLR3 group OR = 2.727, 95%CI was 1.857-4.005, P < 0.001]. ROC curve analysis showed that the area under the ROC curve (AUC) of GLR for predicting 28-day mortality in patients with SA-AKI was 0.674, when the optimal cut-off value was 8.769×10-9 mmol, the sensitivity was 57.1% and the specificity was 67.1%. The predictive performance was improved when GLR was combined with APS III score and SOFA score, and the AUC was 0.806, the sensitivity was 74.6% and the specificity was 71.4%.
CONCLUSIONS
GLR is an independent risk factor of 28-day mortality in patients with SA-AKI, and high GLR is associated with poor prognosis in patients with SA-AKI.

Innovative and practical platform safety management is the foundation of the innovation and entrepreneurship for undergraduate students in universities and colleges, which plays a critical role in maintaining the stability of the university and society, and ensuring the safety of teachers and students. In this present paper, the complex situation and problems of innovative and practical platform safety management, such as the imperfect regulations, weak atmosphere, incomplete facilities, unprofessional management, etc., are discussed under the background of "innovation and entrepreneurship" in China. With the combination of the above problems and the actual situation of Chengdu Medical College, the constructions of the regulations, such as access regulation, certification regulation, check regulation, rewards and punishment regulation, responsibility regulation, and a series of standard operating procedures (SOP) have also been carried out. Furthermore, adequate safety education and proper safety drills have been performed during the whole process of safety management. In addition, a diverse and professional management team with putting more emphasis on the development of safety facilities has been formed. In conclusions, the practice and exploration on the safety management of undergraduate students' innovative and practical platform in this paper may provide a theoretical basis and practical value to optimize the safety management in local universities and colleges under the background of "innovation and entrepreneurship" in China.

5. Diclofenac diethylamine emulgel delivery via nano plum-blossom needle for acute gouty arthritis: study protocol for a single-center blind randomized controlled trial

Background:
Patients with acute gouty arthritis experienc severe pain and often have concomitant limited joint movement. High-dose intake of anti-inflammatory analgesics often results in adverse reactions, such as nausea, vomiting, and diarrhea, and might increase the risk of gastrointestinal bleeding. In order to reduce the adverse reactions by oral intake, local use of externally useddrugs with the same active ingredients as the oral intake is an option in clinical practice. However, externally useddrugs take effect slowly and efficacy to reduce painful feelingis unsatisfactory. Therefore, it is demanding to develop a safe and effective treatment plan. In this regard, researchers have put up a hypothesis. The skin of the human body has a dense stratum corneum, which is difficult for externally used drugs to penetrate effectively; if the keratin barrier of the skin around affected joints can be broken without further damaging the skin, externally applied drugs may safely and effectively alleviate the local symptoms of acute gouty arthritis. The nano plum blossom needle is made of a modern new material. The needle body is thin and the diameter of the needle tip is only 300 nm. The needle is used clinically to break through the keratin barrier of the human skin and promote the absorption of external medicine. Therefore, this trial was designed to preliminarily evaluate the safety and effectiveness of using the nano plum blossom needle for the introduction of diclofenac diethylamine emulgel in the treatment of acute gouty arthritis.
Methods:
A randomized controlled trial including 83 patients with acute gouty arthritis was conducted, who was assigned to three groups, the intervention group, control group 1, and control group 2 in a ratio of 2:2:1, respectively. The nano plum blossom needle was used for the introduction of diclofenac diethylamine emulgel in the intervention group; the nano plum blossom needle was used as a placebo along with the local use of diclofenac diethylamine emulgel in control group 1; and the nano plum blossom needle was used alone in control group 2. The treatments were applied once a day until the pain was completely relieved and the course of treatment lasted up to 7 days. The primary outcome measurement was the visual analog scale for evaluating the degree of pain. The secondary outcome indicators included scoring of the symptoms and signs with comprehensive consideration of the joint skin color, local tenderness, and degree of joint motion, the time and dose of emergency medication usage during trial, and adverse events.
Discussion:
This trial couldprovide preliminary evidence for evidence-based practice of using nano plum blossom needle transdermal drug delivery technology for diclofenac diethylamine emulgel for the treatment of acute gouty arthritis, and provide a reference for sample size calculation and experimental design of future clinical trials verifying the effectiveness and safety of such a technical scheme in a larger target population.

6.The inflammatory effect of exosomes derived from alveolar epithelial cells stimulated by lipopolysaccharide on the alveolar macrophages

Objective To explore the inflammatory effect of exosomes derived from alveolar epithelial cells stimulated by lipopolysaccharide (LPS) on the alveolar macrophages (NR8383). Methods The alveolar epithelial cells disposed with different treatments were co-cultured with alveolar macrophages by using a Transwell system separately. Alveolar epithelial cells (RLE-6TN) were randomly divided into 4 groups: normal group, LPS-stimulated group, exosome inhibitor group, and exosome inhibitor pretreatment + LPS stimulation group. NR8383 cultured alone was considered as a blank control. After the 12-h co-culture, the real-time PCR (qPCR) was performed to examine the mRNA relative expression of IL-6, TNF-α, and IL-1β in NR8383 cells. To further explore the role of exosomes derived from RLE-6TN on alveolar macrophages mediated inflammationary response, the experimental exosomes (exosomes derived from LPS-induced RLE-6TN) and control exosomes exosomes derived from normal RLE-6TN were extracted by gradient ultracentrifugation. Transmission electron microscopy and Western blotting analyses was performed to identify the exosomes, and qNano particle diameter analyzer was conducted to measure the particle diameter of exosomes. In vitro, NR8383 cells were divided into 3 groups which were cultured with exosomes derived from LPS-stimulated RLE-6TN at a concentration of 10 μg/mL (experimental group), exosomes derived from untreated RLE-6TN at the same concentration of 10 μg/mL (control group), and the PBS at the same volume with experimental group (PBS group), respectively for 12 h. After the treatment, the phagocytosis of NR8383 cells was observed by laser confocal microscope and the release of interleukin (IL)-1β, IL-6 and tumor necrosis factor (TNF)-α in supernatants of NR8383 was detected by enzyme-linked immunosorbent assay ELISA Results (1)In the co-culture experiment, the mRNA relative expression of pro-inflammatory cytokine in the LPS group was significantly increased compared with the blank control group (P<0.01), however comparing the exosome inhibitor pretreatment+LPS group with the LPS group, the expression of pro-inflammatory cytokine was decreased (P<0.01). (2) The extracted exosomes were observed as circular or elliptical vesicles with a diameter of 40-100 nm under the transmission electron microscopy. Western blotting analyses showed that the extracted exosomes express the protein marker, such as CD63 and CD9; After incubation with NR8383 cells for 5 h, laser scanning confocal microscope showed that the exosomes labeled with red fluorescent were uptaken by NR8383 cells. (3)After the exosomes derived from the LPS-disposed RLE-6TN and the normal RLE-6TN cells were incubated with NR8383 cells respectively. The ELISA test showed that treated the alveolar macrophages with LPS induced alveolar epithelial secreted exosomes led to a robustly increased release of pro-inflammatory cytokine (P<0.01), but there was no significant difference between the control group and PBS group (P>0.05). Conclusions Exosomes derived from LPS-disposed alveolar epithelial cells activate the alveolar macrophage-mediated inflammatory response.

Objective To evaluate the efficacy of the anterior approach to quadratus lumborum block in each abdominal and back region.Methods Twelve healthy volunteers of both sexes,aged 18-45 yr,of American Society of Anesthesiologists physical status Ⅰ or Ⅱ,with body mass index of 18-30 kg/m2,were enrolled in the study.Quadratus lumborum block was performed via the anterior approach under the guidance of ultrasound with 0.375% ropivacaine 20 ml.The block was assessed by cold stimulation (ice cake) in each abdominal and back region (the right side of the body was divided into 15 regions using the anatomical landmarks on the body surface).The positive condition in each region was recorded at 30 min after administration.Results The region in which the positive rate ≤ 5% was 0.The regions in which the positive rate >5%-20% were 1 and 13 regions.The regions in which the positive rate >20%-50% were 2,3 and 4 regions.The regions in which the positive rate >50%-70% were 6,7 and 10 regions.The regions in which the positive rate >70%-<95% were 5,14 and 15 regions.The regions in which the positive rate ≥95% were 8,9,11 and 12 regions.Conclusion The anterior approach to quadratus lumborum block is effective in the middle-lower region of the anterior abdominal wall on the blocked side.
9.Role of leader-member exchange in the relationship between abusive supervision and clinical nurse's work involvement

Objective To investigate the relationship among leader-member exchange with abusive supervision and work engagement in nurses, to improve the leadership in nursing management and the promotion of nursing work.Methods A total of 597 nurses in two Xi'an tertiary general hospitals,who were selected by convenient sampling method,were investigated by abusive supervision scale,leader-member exchange scale and work engagement scale. The data was analyzed by software SPSS 16.0.Results A total of 620 questionnaires were distributed with effective 597 questionnaire collected. The total score of abusive supervision was(18.52±6.09),leader-member exchange score(26.13±4.37),and work engagement score (47.87±11.62). With the demographic variables controlled,abusive supervision was negatively related to work engagement(r= -0.23,P<0.01),and leader-member exchange had a positive effect on work engagement in nurses(r=0.44,P<0.01).Abusive supervision exerted a negative effect on nurses' work engagement through the medium of leader-member exchange.Conclusions Abusive supervision should be avoided to improve the quality of leader-member exchange quality,which is benefit to work engagement in clinical nurses.
10.Pulmonary Benign Metastasizing Leiomyoma:A Clinicopathological Study of 5 Cases

Pulmonary benign metastasizing leiomyoma (PBML) is a rare and otfen to be misdiagnosed disease. In this study, we want to investigate the diseased of the clinicopathological characteristics, diagnosis, treatment and prognosis. hTe retrospective analysis were performed on the clinicopathological data of 5 patients with PBML. All 5 cases were female, mean age 46.8 years old, and were found with single nodule. One case was found with letf kidney metastasis. Surgical procedures in-cludes:VATS biopsy (3 cases), resection of superior lobe of letf lung (1 case), resection of superior lobe of right lung and wedge resection of middle and inferior lobe of right lung (1 case). hTe residual nodules in lung were stable in all cases with followup 3-48 mo. PBML is dominated occurring in females with history of uterine leiomyoma, preferential to metastasize to lung, and surgery is the primary therapy. Since it is hormone dependent, hormonal therapy may be suggested in these patients.
