Purpose/Significance Disease description texts are analyzed to reach a deeper understanding of the current status of on-line consultations for sleep disorders and the thematic characteristics of users with sleep disorders.Method/Process Data about sleep dis-orders from"haodf.com"website is collected by using a web crawler.Furthermore,the main themes about patients'description are i-dentified by the latent Dirichlet allocation(LDA)model.Result/Conclusion The departments of sleep disorders are more dispersed,and the main treatment is drug therapy.Online consultations could improve 83.2%of patients'condition.The themes of patients's disease descriptions include medication and consultation,external environment,description of symptoms,surrogate questions and causes.It is suggested that the platform and doctors should pay attention to the prognosis of patients'medication and mental health status,and pay at-tention to the popularization of comorbidities.
2.Preparation of Oligomeric Hyaluronic Acid Modified Ellagic Acid-Loaded Liposomes and Study on in vitro Transdermal Effect and Whitening Activity

Objective To construct oligomeric hyaluronic acid(5 KDa)-modified ellagic acid-loaded liposomes(EA-HA-L)to improve the aqueous solubility,in vitro transdermal effect and whitening activity of ellagic acid.Methods Oligomeric hyaluronic acid-modified cholesterol(HA-Chol)was prepared by esterification reaction and structurally characterized by FTIR and 1H NMR;Oligomeric hyaluronic acid-modified ellagic acid-loaded liposomes were prepared by film dispersion-ultrasound method,and the prescribing process was optimized by Box-Behnken design-response surface method,and the particle sizes,the polydispersity index(PDI),zeta potential and encapsulation rate of liposomes under the optimal prescribing process were determined;the difference in solubility between EA-HA-L and free EA was evaluated;in vitro transdermal effect of liposomes were investigated using rat abdominal skin;inhibitory effect on tyrosinase and intracellular tyrosinase in mouse melanoma cells(B16-F10)was surveyed via dopa oxidation method.Results HA-Chol was synthesized and characterized;the optimized prescription process was mass ratio of 10:1 for soy phospholipids to HA-Chol,lipid-drug ratio of 40:1,hydration temperature of 30℃,hydration time of 60 min,ultrasound intensity of 35%,ultrasound time of 21 min,and the particle size of EA-HA-L produced under the optimized prescription process was(140.30±1.30)nm,PDI was(0.29±0.01),the encapsulation rate of ellagic acid was 91.16%±3.06%,and the zeta potential was(-5.67±0.09)mV;after EA was encapsulated by liposomes,the solubility of EA in water increased by about 40-fold;the cumulative transdermal amount of EA-HA-L was 46.98±2.17 μg·cm-2 in 24 h,and the intradermal retention was 66.15±0.61 μg·cm-2,which was 1.72 times higher than that of free EA(P<0.0001)and 1.23 times higher than plain liposome(EA-L)(P<0.01);and the tyrosinase inhibitory activity of EA-HA-L was higher than that of both free EA and EA-L in the EA concentration range of 50-400 μg·mL-1.Conclusion Oligomeric hyaluronic acid-modified ellagic acid-loaded liposomes with small particle size and high encapsulation rate were successfully prepared.EA-HA-L significantly improved the water solubility of EA and possessed better transdermal effect and stronger whitening activity than free EA and EA-L.

A total of 85.71% (24/28) of Yersiniosis cases were under 5?year old. The children of 3-4 age group had the highest isolation rate (1.52%) while the rate (0.18%) of 0-1 age group was the lowest (P=0.025). The Yersinia enteroclitica isolation rates of diarrheal patients with the symptoms including mucus feces, fever, white blood cell (WBC) and red blood cell (RBC) in feces were higher than the patients without these symptoms (P>0.05). The 9 of 28 Yersiniosis cases by etiology diagnosis were clinical diagnosed as bacillary dysentery. Conclusion The infants and young children under 5?year old were the main population of Yersiniosis adolescent patients under 14?year old. The typical symptoms characterized with mucus stool, fever, WBC and RBC by routine microscopic examination. The preliminary clinical diagnosis of Yersiniosis is easily confused with bacterial dysentery.

6.Comparison of radiofrequency ablation and operation for overmuch residual thyroid removal before 131I treatment in differentiated thyroid cancer

Objectives The aim of this study is to compare efficacy and safety of radiofrequency ablation versus operation for overmuch residual thyroid removal before 131I treatment in differentiated thyroid cancer.Methods Clinical data from 52 newly diagnosed differentiated thyroid cancer patients who treated with operation at Zhejiang Cancer Hospital from January 2014 to December 2016 were retrospectively reviewed.The serum TSH was less than 30 IU/ml,and 99mTc thyroid imaging and ultrasound examination showed obviously residual thyroid tissue after 3 weeks of low iodine diet and discontinuation of euthyrox.Of these patients,28 cases received radiofrequency,while 24 cases treated with operation.The operation time,the waiting time of 131I treatment and the efficacy of residual thyroid tissue clearance were compared between two groups.At the same time,the complications of the two groups were observed.Results The mean values of operation time between two groups were(22.14±7.12)min and(55.45±13.56)min,respectively,and there was a statistically significant difference(t=11.822,P<0.05).The mean waiting time of 131I treatment in patients received radiofrequency was(9.40±4.14)days,while the mean waiting time in operation group was(13.53±4.55)days,and the differences were statistically significant(t=9.144,P<0.05).In RFA group,hoarseness,temporary parathyroid injury and permanent parathyroid injury were observed in 2 cases,0 case and 1 case,respectively.While there were 2 cases,3 cases and 0 case in the operative group.There were no significant differences between the two groups(all P > 0.05).85.7%(24/28)of patients in RFA group and 91.7%(22/24)in operation group showed favorable efficacy of residual thyroid removal,and there were no significant differences between the two groups(P>0.05).Conclusion Compared with operation,postsurgical residual thyroid tissue removal in differentiated thyroid cancer by ultrasound guided radiofrequency ablation by ultrasound guided is safe and effective,and provides a new minimally invasive treatment for patients who are reluctant to undergo surgery.

8.Analysis of Folium Epimedium toxicity in combination with Radix Morindae Officinalis based on zebrafish toxicity/metabolism synchronization

Metabolic transformation in vivo is a critical approach in the study of toxicity, but real-time dynamic observation of the transformation is difficult. We proposed that zebrafish toxicity/metabolism synchronization may be used in the analysis of toxicity of Folium Epimedium (Yinyanghuo for Chinese, YYH) and the toxicity may be reduced by Radix Morindae Officinalis (Bajitian for Chinese, BJT). Healthy zebrafish embryos 1 day post fertilization (1 dpf) were exposed to different concentrations of YYH, total flavonoids of YYH (YTF), representative flavonoids (epimedin C and icariin) and their respective in combination with BJT. Death numbers of the embryos or larvals were counted during 1-5 days after dosing (2-6 dpf); embryonic micro-morphology of zebrafish (3 dpf) was observed and pictures were taken. The blank vehicle (0.4% DMSO) was used in the control group, and LC50 value of 2 to 6 dpf was calculated by SPSS16.0. A relative safe concentration was sampled every day to analyze the dynamic metabolites changes of major flavonoids of YYH. The results showed that epimedin A/B/C (EA/EB/EC) and icariin, the major flavonoids of YYH, were dynamically transformed into major metabolites of sagittatoside C (SC) and baohuoside I (BI) by zebrafish. BI was mainly derived from EA, EB and icariin. Neither original form nor their metabolite BI can cause zebrafish poisoning. SC was mainly derived from EC, and its accumulation was closely related to the toxicity of YYH, YTF and EC. After combination with BJT, the metabolism of EC was slowed down and the toxicity was alleviated. Zebrafish toxicity/metabolism synchronization revealed that the toxicity of EC of YYH was increased after metabolism into SC, which maybe the key potential poisonous factor of YYH, and BJT can reduce the toxicity by slowing down the metabolism rate of EC. The data provides new ideas and methods in the study of toxic substances in Chinese medicine and mechanism of detoxicity by combination.

The increasingly apparent liver injury problems of bone strengthening Chinese medicines have brought challenges for clinical application, and it is necessary to consider both effectiveness and safety in screening anti-osteoporosis Chinese medicines. Metabolic transformation is closely related to drug efficacy and toxicity, so it is significant to comprehensively consider metabolism-action/toxicity(M-Act/Tox) for screening anti-osteoporosis Chinese medicines. The current evaluation models and the number of compounds(including metabolites) severely restrict efficient screening in vivo. By referring to previous relevant research and domestic and abroad literature, zebrafish M-Act/Tox integrative method was put forward for efficiently screening anti-osteoporosis herb medicines, which has organically integrated zebrafish metabolism model, osteoporosis model and toxicity evaluation method. This method can break through the bottleneck and blind spots that trace compositions can't achieve efficient and integrated in vivo evaluation, and realize both efficient and comprehensive screening on anti-osteoporosis traditional medicines based on in vivo process taking both safety and effectiveness into account, which is significant to accelerate discovery of effective and safe innovative traditional Chinese medicines for osteoporosis.
10.Application of Bayesian Mixed Treatment Comparison Method in Pharmacoeconomics Evaluation

OBJECTIVE:To explore the basic principles,advantages and application prospects of Bayesian mixed treatment comparison (MTC),and provide theoretic support for making reasonable decisions of pharmacoeconomics. METHODS:The prob-lems existing in model and method that had been used in the study of pharmacoeconomics as starting point,literature research was used to sort out the domestic development and applications of MTC and study its application and development prospect. RESULTS &CONCLUSIONS:The evaluation methods of pharmacoeconomics play an important role in selection of essential medicines,medi-care insurance directory medicines,basic health and basic public health programs,the results directly affect the total effective rate of medical resources’configuration. The current research models cannot effectively solve the problem,for example,decision tree model is difficult to make evaluation of long-term treatment;Markov model and the use of multiple attribute utility theory are restricted by the model parameters;discrete event simulation is limited because it depends on the epidemiological study and clinical trial. MTC, an extension approach of the traditional Meta-analysis,is an effective new method developed recently to analyze and compare the di-rect or indirect evidence of several different clinical treatment factors in order to make a comprehensive evaluation of the various inter-ventions in the absence of evidence support. So the MTC can make up for the limitation of current evaluation methods.
