1.Comparative analysis on radiological health technical services in China in 2021 and 2022

		                        			ObjectiveTo analyze the quality and status of technical service items of radiation health technical service institutions (RHTSI) in China. Methods A total of 608 and 622 RHTSIs with radiation health technical service qualifications from 31 provinces, autonomous regions, and municipalities in the years of 2021 and 2022 were selected as the research subjects. The data of quality monitoring of radiological health technology services, comparison of radiological health testing capabilities, and investigation of the current status of technical support institutions were collected to analyze the status of technical service items which was conducted by RHTSIs from 2021 to 2022. Results A total of 622 RHTSIs in China obtained technical service institution qualifications in 2022, an increase of 14 from 608 in 2021. While a total of 404 of 622 RHTSIs conducted radiological health technology services, a decrease of 11.4% from 456 in 2021. A total of 241 804 technical service items were conducted in 2022, an increase of 39.7% from 173 064 in 2021. The median number of technical service items by non-health system RHTSIs was higher than that of the health system RHTSIs in 2021 and 2022, (203 vs 40 items, 215 vs 55 items, all P<0.01). The number of technical service institutions and technical service items conducted in different regions from high to low were the eastern, central, and western regions in 2021. The number of technical service institutions in different regions was highest in the eastern region, followed by the central and western regions, while the number of technical service items from high to low were the eastern, western, and central regions in 2022. The number of provincial, municipal, and county-level health system RHTSIs decreased by 6.5%, 26.3%, and 27.3%, respectively, in 2022 compared with 2021. The number of technical service items conducted by provincial health system RHTSIs increased by 48.6%, while those conducted by municipal and county-level health system RHTSIs decreased by 13.8% and 21.3%, respectively. Conclusion Although the number of RHTSI conducting technical services decreases in 2022 compared with 2021, the number of technical service items conducted increases. Non-medical RHTSI undertake the majority of technical service items. Within the medical institutes, the number of technical service items conducted by provincial RHTSI is higher than that of municipal and county-level RHTSI. 
		                        		
		                        		
		                        		
		                        	
2.Neoadjuvant chemoradiotherapy versus neoadjuvant chemo-immunotherapy for locally advanced esophageal squamous cell carcinoma

		                        			Objective:To compare the clinical efficacy of neoadjuvant chemoradiotherapy (nCRT) and neoadjuvant chemo-immunotherapy (nCIT) for locally advanced esophageal squamous cell carcinoma (ESCC).Methods:Clinical data of patients who received nCRT or nCIT followed by esophagectomy for locally advanced ESCC between January 2010 and December 2022 were retrospectively collected from Zhejiang Cancer Hospital, with 155 patients in the nCRT group and 470 patients in the nCIT group. Propensity score matching (PSM) was performed in the two groups. After PSM, 120 patients were allocated to the nCRT group and 192 patients to the nCIT group. The pathological response and disease recurrence were compared between the two groups after PSM. Log rank test were used to compare the survival outcomes before and after PSM. Univariate and multivariate Cox regression analyses were performed to identify the prognostic factors for locally advanced ESCC.Results:After PSM, the R0 resection rate in the nCRT group and the nCIT group was 93.3% (112/120) and 93.8% (180/192), respectively, with no statistical significance ( P=0.884). However, the pathological complete response rate in the nCRT group [36.7% (44/120)] was higher than that in the nCIT group [21.4% (41/192), P=0.003]. For patients with R0 resection, the major recurrence pattern was distant metastasis [18.8% (21/112)] in the nCRT group, while the pattern was locoregional recurrence [12.2% (22/180)] in the nCIT group. The 3-year disease-free survival rates were 52.7% and 66.1% ( P=0.022) and the 3-year overall survival rates were 59.2% and 75.5% ( P=0.002) in the nCRT and nCIT groups, respectively. Multivariate Cox regression analysis also revealed that the neoadjuvant therapy mode was an independent prognostic factor for patients with locally advanced ESCC. Compared with nCRT, nCIT could significantly prolong disease-free survival ( HR=0.58, 95% CI: 0.40-0.86) and overall survival ( HR=0.53, 95% CI: 0.35-0.79). Conclusion:These results suggest that nCIT could significantly improve disease-free survival rate and overall survival rate over nCRT in locally advanced ESCC, even with lower pathological complete response rate.

4.Association between urinary excretion of protein-bound uremic toxins and upper urinary tract calculus

		                        			Objective·To investigate the relation between urinary excretion of protein-bound uremic toxins(PBUTs)and upper urinary tract calculus.Methods·Residents aged 18?80 years in the community of Haitou,Danzhou city in Hainan Province were recruited.Basic information and diet for the last 3 d of the subjects were recorded.Their fasting sera and 24-hour urine samples were collected,and they also underwent ultrasound examination of kidneys and ureters.The subjects with upper urinary calculi detected by ultrasound or a clear history of upper urinary calculi were selected as the calculus group,and the others as the non-calculus group.The biochemical indicators related to the formation of calculus in blood and urine were detected,and the levels of PBUTs,including indoxyl sufate(IS),indole-3-acetic acid(IAA),and p-cresol sulfate(PCS)in blood and urine,as well as oxalic acid and citric acid in urine were detected by high-performance liquid chromatography.The related factors of upper urinary tract calculus formation were analyzed by multivariate Logistic regression.The correlations of urine PBUTs with urine uric acid,oxalic acid,and citric acid were analyzed by Spearman correlation test.Results·A total of 117 participants were screened out with 54 people in the calculus group and 63 people in the non-calculus group.There were no significant differences between the two groups in terms of gender,age,serum indicators,and prevalence of complications such as hypertension,diabetes,and hyperuricemia/gout.The 24-hour urine pH,calcium,uric acid,and chlorine in the calculus group were significantly higher than those in the non-calculus group(all P<0.05),while IS was significantly lower than that in the non-calculus group(P<0.05).Multivariate Logistic regression analysis showed that urinary IS(OR=0.929,95%CI 0.875?0.986,P=0.016)was related to the calculus formation independently,in addition to urinary calcium.The Spearman correlation analysis results showed that the levels of IAA(r=0.420,P=0.000)and PCS(r=0.307,P=0.001)in 24-hour urine were positively correlated with oxalic acid,PCS was positively correlated with uric acid(r=0.297,P=0.002),and IS was positively correlated with citric acid(r=0.289,P=0.002).Conclusion·In the population,a decrease in urinary excretion of IS may be an independent risk factor for the formation of upper urinary tract calculus,and PBUTs levels are correlated with levels of uric acid,oxalic acid,and citric acid.
		                        		
		                        		
		                        		
		                        	
5.Effects of Ditan Yizhi Decoction on Neuronal Morphology and FAM134B Protein Expression in Hippocampus of Vascular Dementia Rats

		                        			Objective To observe the effects of Ditan Yizhi Decoction on learning and memory ability,structure of hippocampal tissue,neuronal morphology of hippocampus,and the expression of endoplasmic reticulum autophagy-related protein FAM134B in hippocampal tissue;To explore the mechanism of its therapeutic effect on vascular dementia.Methods Totally 32 SD rats were randomly divided into sham-operation group,model group,donepezil group and Ditan Yizhi Decoction group,with 8 rats in each group.The model group,donepezil group and Ditan Yizhi Decoction group were prepared with a modified permanent ligation method of bilateral common carotid arteries to create a rat model of vascular dementia,the common carotid artery was separated in the sham-operation group,but not ligated.After modeling,the donepezil group was given donepezil hydrochloride,Ditan Yizhi Decoction group was given Ditan Yizhi Decoction,and the sham-operation group and model group were given equal volume of distilled water for gavage for 4 consecutive weeks.Morris water maze experiment was used to evaluate the learning and memory ability,HE staining and Nissl staining were used to observe the morphological changes of hippocampus,ultrastructure of hippocampal neurons was observed using transmission electron microscopy,Western blot was used to detect the protein expression of FAM134B and p-FAM134B in hippocampal tissue.Results Compared with the sham-operation group,the escape latency period was prolonged of the rats in model group,and the number of crossing the original platform and the duration of stay in the target quadrant was reduced(P<0.01),the gap between neurons in CA1 region of the hippocampus increased,the cell morphology was irregular,the boundaries were blurred,the neurons shrinked,the Nissl bodies dissolved and broke,the number decreased,the endoplasmic reticulum arrangement was scattered,mitochondria swelled and deformed,and the expressions of FAM134B and p-FAM134B protein in hippocampal tissue increased(P<0.01).Compared with the model group,the escape latency period of rats in donepezil group and Ditan Yizhi Decoction group were significantly shortened,and the number of crossing the original platform and the duration of stay in the target quadrant were increased(P<0.01),the morphology and quantity of neurons in CA1 region of the hippocampus were more regular,with a decrease in neuronal pyknosis,an increase in the number of Nissl bodies,and a reduction in dissolution and fragmentation,the swelling and deformation of the endoplasmic reticulum were restored,and the expression of FAM134B and p-FAM134B protein in hippocampal tissue increased(P<0.01).Moreover,the effects of Ditan Yizhi Decoction group were better than those of the donepezil group(P<0.01).Conclusion Ditan Yizhi Decoction can improve the learning and memory ability and the morphology of neurons in vascular dementia rats.The mechanism may related to increasing the expression and phosphorylation of FAM134B protein,thereby promoting endoplasmic reticulum autophagy.
		                        		
		                        		
		                        		
		                        	
6.Investigation and analysis of the current situation of radiation health technical services in China

		                        			Objective To investigate the current situation of radiological health technical service institutions and their technical services in China, and to provide a basis for better utilizing radiological health technical service resources and strengthening institutions’ capability building. Methods From October to December 2021, we conducted quality monitoring, ability comparison, capability building surveys, and other investigations on radiation health technical services to collect information on the qualifications and technical services of radiation health service institutions. The data were pooled and analyzed using SPSS software. Results By the end of 2021, there were 608 radiological health technical service institutions across China, with 47.0% of them located in the eastern region, and the percentage of institutions in the health system (42.6%) was less than that in the non-health system (57.4%). Institutions of grade A offered 62.3% of technical services, while institutions of grade B provided 37.7% of technical services; technical services provided by the non-health system and the health system accounted for 84.2% and 15.8%, respectively; institutions in the health system at the province, prefecture, and county levels offered 37.1%, 50.6%, and 12.3% of technical services, respectively. Conclusion In recent years, the proportion of institutions in the non-health system has increased significantly, but the proportion of institutions in the health system with radiological health qualifications is small, and the development is uneven in the numbers of institutions, technical services, and professional and technical personnel in the eastern, central, and western regions and at the province, prefecture, and county levels in China. It is necessary to further encourage and support health system institutions in strengthening capability building and applying for radiological health qualifications, and to increase construction and investment in the western region and at the district/county levels, in order to improve the overall capability and level of radiological health technical services in the country.
		                        		
		                        		
		                        		
		                        	
7.Analysis of individual dose intercomparison results for province-level health institutions during 2009—2022

		                        			Objective:To summarize and analyze the results of the individual dose intercomparison in which province-level health institutions participating, and explore the role of ability intercomparison in improving the detection capabilities of province-level health institutions.Methods:From 2009 to 2022, the results obtained by the province-level CDCs and occupational prevention and control institutions in the individual dose intercomparison countrywide for time periods of 2009—2020 and 2021—2022 were summarized. The intercomparison results were analyzed and compared for three regions of east, central and west China divided according to the method from China Health Statistics Yearbook 2022.Results:A total of 24 province-level CDCs and 14 occupational prevention and control institutions countrywide participated in the ability intercomparison from 2009—2022 for 13 times. Number of participating institutions increased from 26 in 2009 to 37 in 2022. The qualified rates of the intercomparison results among the province-level health institutions in the three regions were maintained at higher than 85% for 37 times and 100% for 28 times. The pass rate and excellent rate of the three regions were higher than the overall level at whole country level (2009-2020) and national level (2021-2022) in the same period for 9 and 5 times, respectively. The excellent rate of province-level health institutions in central region were higher than that of the other two regions in 9 intercomparisons.Conclusions:With the improvement of the intercomparison standard, the province-level health institutions in various regions have maintained a relatively stable and high detection capability. The ability intercomparison effectively improve the detection ability and the quality control of the province-level health organizations. 8.Results and analysis of national personal dose monitoring intercomparison in 2021

		                        			To improve radiation testing institutions’ ability and levels of personal dose monitoring for external
exposure and standardize monitoring work.
		                        		
		                        			Methods:
		                        			According to Testing Criteria of Personnel Dosimetry Performance
for External Exposure (GBZ 207—2016), the intercomparison programme was made, the 2021 class-based personal dose
monitoring intercomparison were organized by the National Institute for Radiological Protection, Chinese Center for Disease
Control and Prevention and provincial monitoring institutions.
		                        		
		                        			Results:
		                        			A total of 150 institutions participated in the 2021
national intercomparison of personal dose monitoring capability. Of them, 143 (95.3%) passed the assessment, including 51
excellent institutions (excellent rate, 34.0%), and 7 (4.7%) failed to pass the assessments.
		                        		
		                        			Conclusion
		                        			The 2021 national
personal dosimetry performance intercomparison showed great improvements in the pass rate and excellent rate compared
with previous national assessments. However, still seven institutions failed to pass this assessment, including four institu-
tions with class A qualifications. The testing institutions need to find out the reasons to increase their testing ability. This
class-based intercomparison improved the point-to-plane relationship between the organizing and testing institutions, help-
ing the organizing institution focus on improving the testing ability of provincial testing institutions and class A radiation

9.Research progress in estimation and monitoring of individual exposure dose based on mobile phones

		                        			In case of accidental exposure, a rapid assessment of radiation dose is of great significance for taking timely countermeasures. Finding an alternative portable emergency dosimeter is crucial for people who have been exposed to accidental radiation but are not equipped with a personal dosimeter. In addition, the real-time monitoring of radiation dose level in the normal environment can meet the needs of the public to know their health status in time. Thus, the portability and intelligence of mobile phones attract researchers to carry out related work. 10.Results and analysis of monitoring ability assessment of external exposure personal dose in 2020

		                        			Objective In order to improve the personal dose monitoring ability of radiation health technical institutions, and standardize the detection methods and procedures. Methods Develop adetailed assessment plan according to GBZ 207—2016 “Testing criteria of personnel dosimetry performance for external exposure”, organize a national assessment of personal dose monitoring capabilities in 2020, analyze the problems arising from the assessment, and strengthen supervision and training in the future. Results A total of 457 institutions participated in this assessment, of which 421 were qualified (including 74 excellent institutions, with an excellent rate of 16.2%), and 36 were unqualified, accounting for 92.1% and 7.9% respectively. Conclusion The number of institutions that participated in the assessment this year reaches a record high, with a passing rate 4.08% higher than that in 2019. The overall monitoring capacity of the country has been greatly improved. However, some institutions still have problems such as irregular symbol writing, unupdated detection standards, and large deviations in testing data. It is necessary to improve the ability of loboratory detection and data analysis, standardize the operation process, and ensure the quality of assessment.
